Medication treatment for depression in children and adolescents.

Depression in children and adolescents is relatively common and associated with significant morbidity and mortality-thus, it is strongly deserving of treatment. To date, there have been a number of randomized, controlled trials of both tricyclic antidepressants (TCAs) and selective serotonin reuptake inhibitors (SSRIs) in the acute treatment of depression in youths. Surprisingly, the available data do not demonstrate TCA superiority over placebo for this disorder in this age group. There is, however, evidence of SSRI superiority to placebo, and longer-term treatment with SSRIs may help prevent

Suicide in children and adolescents at a province in Turkey.

Despite the well-documented increases in the rate of completed suicide among children, accurate knowledge of the characteristics of these suicides is limited. We examined all suicides by children and adolescents in Diyarbakir during 1999 and 2001. All of the cases were analyzed regarding the age, sex, method and location of suicide, time of year, and predictive factors. There were 56 children aged 11 through 19 years. Adolescents between 15 and 19 years of age constituted the overwhelming number (87%) of all the childhood suicides. Female children and adolescents predominated among the suicide

Prevention of drowning in infants, children, and adolescents.

Drowning is a leading cause of injury-related death in children. In 2000, more than 1400 US children younger than 20 years drowned. Most (91%) of these deaths were unintentional and were not related to boating. For each drowning death, it is estimated that at least 1 to 4 children suffer a serious nonfatal submersion event, many of which leave children with permanent disabilities. Environmental strategies, such as installation of 4-sided fences around swimming pools, and behavioral strategies, such as increased supervision of children while around water, are needed to prevent these tragedies.

Headache and facial pain in children and adolescents.

Headaches commonly affect children and adolescents. Proper diagnosis and management is dependent on thorough history taking and a comprehensive physical and neurological examination. Additional diagnostic testing is indicated in some cases. The second edition of the headache classification system by the International Headache Society has recently become available. The classification system is primarily based on adults, but we discussed the subtle distinctions made regarding children. In addition to the primary headache types of migraine, tension-type, and cluster headaches, we discussed select

An update on the prevention of influenza in children and adolescents.

UNLABELLED: Influenza virus types A and B cause yearly outbreaks of respiratory tract infections in all age groups including children and adolescents. Complications, such as high fever, febrile convulsions, secondary bacterial infections and myositis frequently lead to hospitalisation. Safe and effective split, subunit and virosome vaccines are available from 6 months of age onwards. Most European countries do have guidelines for the use of influenza vaccines and current strategies primarily aim at decreasing the burden of influenza disease in certain, heterogeneously defined high risk groups.

Bulimia and impulsive behaviour in middle adolescence.

Managing epilepsy during pregnancy is to balance the maternal and fetal risks associated with uncontrolled seizures against the potential teratogenic effects of antiepileptic drugs (AEDs). A rational approach requires knowledge of such risks as well as an understanding of the effects of pregnancy on seizure control and of gestational effects on AED disposition. Uncontrolled tonic-clonic seizures are potentially hazardous to the mother and, although strict evidence is lacking, are generally also assumed to be more harmful to the fetus than are AEDs. However, infants who have been exposed to AED

A longitudinal investigation of anxiety sensitivity in adolescence.

This investigation sought to expand existing knowledge of anxiety sensitivity in a sample of high school students (N = 2,365) assessed over 4 years. The stability of anxiety sensitivity levels across assessment periods was examined, and cluster analyses were used to identify different developmental pathways in levels of anxiety sensitivity. Groups of adolescents with stable low, stable high, and escalating anxiety sensitivity levels were identified. Adolescents with stable high or escalating anxiety sensitivity were significantly more likely to report experiencing a panic attack than individua

Transient orthostatic hypotension is common in adolescents.

OBJECTIVES: We hypothesized that transient hypotension associated with the symptom of lightheadedness with standing is a common phenomenon in healthy adolescents but may be mistaken for orthostatic intolerance. STUDY DESIGN: We studied 23 healthy adolescents, combining upright tilt to 70 degrees and to 35 degrees (in 12 patients) with continuous heart rate and blood pressure (BP) measurements. We measured peripheral blood flow and venous pressure in the arms and legs by using venous plethysmography. RESULTS: Healthy subjects with normal vasoconstrictive responses to sustained upright tilt un

Longitudinal study of melanocytic nevi in adolescents.

BACKGROUND: Although melanocytic nevi are the strongest known risk factors for melanoma, their etiology is not well understood. OBJECTIVES: This study was performed to assess the roles of constitutional pigmentary factors and sun exposure in the development of new nevi in adolescents. METHODS: A cohort of 111 school children, aged 12 and 13 years at baseline, were followed up for 5 years. Whole-body counts of nevi of all sizes and detailed sun exposure histories were updated each year. Psychologic and physiologic effects of dieting in adolescents.

Obesity in adolescents has increased by 75% in the past three decades. Cross-sectional and prospective surveys have shown that a large percentage of adolescents, particularly females and even those of normal weight, diet at some time. While moderate changes in diet and exercise have been shown to be safe, significant psychologic and physiologic consequences may occur with extreme or unhealthy dieting practices. Moderate dieting has been shown to be associated with negative self-esteem in some adolescents. The very act of starting any diet increases the risk of eating disorders in adolescent gi

Pelvic inflammatory disease in adolescents: a review.

Pelvic inflammatory disease is relatively common in sexually active adolescents. Most cases are only mildly symptomatic, and criteria for diagnosis should not be stringent. Although chlamydial or gonorrheal infections or both are common in pelvic inflammatory disease, other aerobic and anaerobic organisms are often also present and may be the only causative agents. Commonly used initial therapy, e.g., ceftriaxone and doxycycline, treats some but not all of these organisms. Patients should be seen within 48 to 72 hours after initiating therapy; lack of improvement suggests noncompliance with an

[Injuries of the alar ligaments in children and adolescents].

Cervical spine trauma most commonly involves the lower parts in adults. In children lesions of the cervical spine can predominantly be found in the region of C1/C2 including ligament injuries at this level. However such injuries are difficult to detect and only few data are available concerning therapy and prognosis of atlantoxial ligament lesions. We report on two children suffering from isolated rupture of the alar ligaments. Both injuries were proven by magnetic resonance imaging which is recommended as the resource of choice for the evaluation of the cervical spine soft tissues in children
