Bipolarity in children.

Childhood and adolescent bipolar disorder have been less studied than adult onset bipolar illness. However, case reports of mania in childhood can be found as early as the mid 19th century. Historically, several factors have made the accurate diagnosis of bipolar disorder in childhood difficult: clinical bias against the diagnosis of mania in children; low base rate of disorder; symptom overlap between bipolar disorder and other more prevalent childhood-onset psychiatric disorders; and developmental constraints and variability in clinical presentation. The epidemiology of juvenile-onset bipola

A descriptive profile of the depressed child.

We have identified a number of basic descriptive features common to the prepubertal child (aged 6-12) who meets criteria for an objectively defined mood disorder in the inpatient psychiatric setting. Clinical and structured diagnostic interviews conducted with both the child and the parent revealed that DSM-III criteria for major adult mood disorders were easily met by the depressed children. The symptom profile for the child is similar to the adult profile: feelings of hopelessness, sadness, and despair; depressed mood; vegetative symptoms of low energy, appetite loss, and disturbed sleep; an

The Intricacies of the Histopathology of Eosinophilic Esophagitis: A Critical Review.

Eosinophilic esophagitis is a chronic, immune-mediated disorder that afflicts both adults and children. Dysphagia and food impaction are common symptoms in adults, whereas children experience failure to thrive, vomiting, and food aversion. On endoscopy, mucosal exudates, plaques, and furrows correlate with histologic inflammation, whereas rings and strictures are possibly related to a remodeled hyperplastic muscularis mucosae. The overreliance on intramucosal eosinophil counts for diagnosis and for the evaluation of response to therapy is a misguided oversimplification. A judicious analysis of

Esophageal perforation: An uncommon initial manifestation of eosinophilic esophagitis.

EoE-Perforation: Eosinophilic esophagitis (EoE) is commonly observed in children and young adults. Common manifestations of EoE include dysphagia and food impaction in adolescents and adults, whereas children present with failure to thrive, regurgitation, or heartburn and abdominal pain. We describe two patients presenting with esophageal perforation and EoE. Diagnosing perforation promptly is critical to minimize and/or to avoid the multitude of complications resulting from esophageal perforation and to treat EoE because if left untreated, this condition may result in the recurrence of perfor

Standardized magnetic resonance imaging acquisition and reporting in pediatric multiple sclerosis.

Magnetic resonance (MR) imaging is one of the most important paraclinical tools for the diagnosis of multiple sclerosis (MS), and monitoring of disease progression and treatment response. This article provides clinicians and neuroradiologists caring for children with demyelinating disorders with a suggested standard MR imaging acquisition and reporting protocol, and defines a standard lexicon for lesion features typical of MS in children. As there is considerable overlap between the MR imaging features of pediatric- and adult-onset MS, the recommendations provided herein may be of relevance to

Losing a child to spinal muscular atrophy.

Spinal muscular atrophy (SMA) is a genetic disease that affects mostly children but also some adults. There are Types I, II, and III that affect children, and Type IV that affects adults. There is no cure or treatment. SMA sufferers lack a protein (SMN) that controls muscles, therefore muscles weaken, and in the most severe of cases death occurs. Research is ongoing to find a cure for SMA. This personal account by the mother of Matthew who died aged three months of spinal muscular atrophy aims to inform nurses about this life-threatening condition.

[Pulse therapy in the treatment of optic neuritis in multiple sclerosis].

Corticosteroids are the preparations in the treatment of optic neuritis at multiple sclerosis. The doses and methods of introduction of corticosteroids are chosen individually for the adults and children. A maximum effect is registered in a majority of cases of optic neuritis, when methylprednisolone is prescribed with the pulse-dose introduction, as compared to parabulbar injections or administration of corticosteroids per os. The recovery of vision at relapsing optic neuritis sets on faster and is of a more prolonged nature as compared to other variants of the introduction of corticosteroids

CT and MR findings in parameningeal leukaemic masses.

This study presents the clinical and imaging findings in seven patients with parameningeal leukaemic masses: four adults with granulocytic sarcoma and three children with soft tissue masses from acute lymphoblastic leukaemia. Computed tomography and magnetic resonance imaging were helpful in detecting these masses and in showing their extent. Magnetic resonance was particularly valuable in demonstrating abnormal bone marrow in the adult patients with granulocytic sarcoma. Computed tomography was valuable in demonstrating leukaemic relapse in the children when examination of CSF or bone marrow

[Obstructive dyspnea of oropharyngeal origin].

Obstructive dyspnoea of oropharyngeal origin is a recently recognized entity with numerous clinical forms depending on age, severity and symptoms. The authors insist on the two facets of this pathology that are most frequently seen and therefore of special interest to practitioners: the oropharyngeal obstructive syndrome due to enlarged tonsils in children, and the pharyngeal obstruction responsible for snoring and for the sleep apnoea syndrome in adults. In children, tonsillectomy is necessary to avoid complications which are sometimes severe. The various clinical features of snoring and slee

Individual and household factors associated with tungiasis in a marginalized population in Karamoja, northeastern Uganda.

BACKGROUND: Tungiasis is a neglected tropical skin disease caused by adult female fleas which burrow into the skin of people and animals, causing considerable pain and itching. The distribution of the disease is heterogeneous, with Napak district, Karamoja sub-region of northeastern Uganda having the highest disease burden recorded globally. We aimed to determine the factors associated with this high prevalence to inform future intervention strategies. METHODS: We conducted a cross-sectional study to identify factors associated with infection of individual children and adults, and a nested ca

CT-Guided transgluteal drainage of deep pelvic abscesses in children: selective use as an alternative to transrectal drainage.

OBJECTIVE: The transgluteal approach to abscess drainage through the greater sciatic foramen has been described in adults, but this route has not been as extensively studied in children. We performed CT-guided transgluteal percutaneous abscess drainage in seven children and assessed the results of drainage and catheter tolerance. CONCLUSION: Transgluteal catheters are well tolerated by children, and the transgluteal route is an effective approach to selected pelvic abscesses in children.

Understanding kinesiophobia in pediatric bone tumors: investigating its presence and predictive factors.

UNLABELLED: Primary malignant bone tumors are significant health concerns in children. These tumors, often accompanied by pain, fatigue, and reduced physical function, can lead to the development of kinesiophobia, a fear of movement that can further complicate rehabilitation. Although factors associated with kinesiophobia have been examined in various adult cancer populations, there is limited research on kinesiophobia and its predictors in children with bone tumors. This study aims to investigate the factors contributing to kinesiophobia in pediatric bone tumor patients. This prospective cros

Anesthesia in children with mastocytosis--a case based review.

INTRODUCTION: Mastocytosis is a rare heterogeneous disease of bone marrow origin which arises as a consequence of abnormal growth and/or accumulation of clonal mast cells in one or more organs. Sixty-five percent of patients with mastocytosis are children in whom it usually regresses around puberty. Adult patients with mastocytosis have been identified as at high risk of widespread mast cell degranulation in the perioperative period, this finding has not been reported in pediatric patients. This information has been repeated in mastocytosis websites where it has the potential to cause dispropo

Lithium-related adverse drug reactions in children: A case-control study comparing lithium to second-generation antipsychotics in VigiBase.

AIMS: This study aimed to improve the understanding of lithium's safety profile by analysing the frequency and distribution of reported adverse drug reactions (ADRs) compared to second-generation antipsychotics (SGAs). METHODS: Individual case safety reports from VigiBase (1968-October 2023) involving individuals older than 2 years, with lithium or SGAs as a suspected or interacting drug, were included. Medical Dictionary for Regulatory Activities (MedDRA) was used for ADRs classification. Proportional reporting ratios (PRRs) with 95% confidence intervals were calculated across different age

Effects of gap onset time and gap duration on the deviant-related negativity in typically developing school-age children.

OBJECTIVES: This study examined whether automatic change detection to within-stimulus gaps, indexed by the deviant-related response (DRN), varies with gap duration and the temporal position of the gap within a long-duration sound, and whether these effects differ between children and adults. METHODS: Fifteen typically developing children (8-12 years) and fifteen adults (23-37 years) completed passive EEG recordings using a multi-deviant sequence in which standards and deviants alternated. All stimuli were 500-ms broadband-noise bursts. Deviants contained a silent gap of 10 or 30 ms inserted a

Childhood Food Insecurity Trajectories and Adult Weight and Self-Reported Health.

INTRODUCTION: Research has demonstrated that food insecurity during childhood is associated with worse physical and mental health in childhood. However, little is known about how food insecurity during childhood impacts health outcomes in young adulthood. METHODS: This study analyzed data from the Future of Families and Child Wellbeing Study (2024), a longitudinal birth cohort study of children born in 1998-2000. Childhood food insecurity trajectory groups from age 3 to 15 years were identified using group-based trajectory modeling. Associations between childhood food insecurity trajectory gr
