Acquired pulmonary artery branch stenosis caused by pulmonary thromboembolism.

We present a case of acquired pulmonary artery branch stenosis caused by pulmonary thromboembolism. The patient presented with symptoms mimicking aortic dissection. An emergent transesophageal echocardiogram showed a high gradient across the right pulmonary artery along with mobile thrombi. The vital importance making an accurate diagnosis and ruling out aortic dissection is emphasized in view of the need for urgent heparinization or thrombolysis for pulmonary thromboembolism as this is contraindicated in aortic dissection.

Preventative valve-sparing aortic root replacement and pregnancy outcome in Marfan syndrome.

In Marfan syndrome, with dilatation of the aortic root secondary to an underlying connective tissue defect, pregnancy can cause hemodynamic stress leading to the development of an aortic aneurysm and even a fatal aortic dissection. In the presence of existing aortic root enlargement and a family history of aortic dissection, preventative elective surgery is suggested. Aortic root replacement with or without a valve-sparing procedure is superior to total aortic root replacement with prosthetic valve/tube graft. It provides excellent survival with low rates of aortic - valve related complication

Intraoperative coronary artery dissection in fibromuscular dysplasia.

A 61-year-old woman with bicuspid aortic stenosis, an ascending aortic aneurysm, and a remote history of renal fibromuscular dysplasia underwent aortic root replacement complicated by extensive dissection of the left circumflex artery extending retrograde into the left anterior descending artery. This was managed by coronary artery bypass grafting, left ventricular support, and percutaneous coronary intervention for propagation of the dissection. This case highlights the prevalence, diagnosis, and management of intraoperative coronary dissection secondary to fibromuscular dysplasia.

Fluoroquinolone Use Preceding Visceral Artery Dissection: A Case Series.

Fluoroquinolones (FQ), commonly prescribed antibiotics, may trigger aortic and carotid dissections. We report three successive cases of visceral artery dissection: one patient with celiac trunk dissection and two with dissection of the superior mesenteric artery. These events occurred up to 4 months after 7 to 14 days of FQ treatment (2 cases of ofloxacin, 1 of norfloxacin). There was no other apparent cause of dissection. These dissections were isolated, apart from a minimal aortic dissection separate from the visceral arterial dissection in one case. A case series cannot certify the relation

Aortic events in a nationwide Marfan syndrome cohort.

BACKGROUND: Marfan syndrome is associated with morbidity and mortality due to aortic dilatation and dissection. Preventive aortic root replacement has been the standard treatment in Marfan syndrome patients with aortic dilatation. In this study, we present aortic event data from a nationwide Marfan syndrome cohort. METHOD: The nationwide cohort of Danish Marfan syndrome patients was established from the Danish National Patient Registry and the Cause of Death Register, where we retrieved information about aortic surgery and dissections. We associated aortic events with age, sex, and Marfan syn

<Editors' Choice> Thoracic endovascular aortic repair and spinal cord injury.

We previously reported that spinal cord injury following thoracic endovascular aortic repair for a thoracic aortic aneurysm is a micro embolism caused by a vulnerable mural thrombus. Conversely, patients who underwent thoracic endovascular aortic repair for aortic dissection develop spinal cord injury less frequently due to fewer mural thrombi. Paying attention to preserving blood flow toward the spinal cord, namely collateral circulation and steal phenomenon, prevents spinal cord injury following thoracic endovascular aortic repair for aortic dissection.

Composite graft replacement of the aortic root: long-term results, incidence of reoperations.

BACKGROUND: Long-term results after composite graft replacement of the aortic root may depend on the insertion technique. METHODS: 181 consecutive patients (mean age 53 years; 153 men) operated on between 1983 and 1993 were studied. Indications for surgery were annuloaortic ectasia (n=98), acute aortic dissection (n = 46), other indications (n = 12), and various indications after previous aortic valve replacement (n = 25). Mean follow-up was 28 months. The open-button technique was performed in 74 patients (41 %) and the Bentall inclusion technique in 107 patients (59%), with a Cabrol shunt t

Quantification of Mesenteric Blood Flow by Magnetic Resonance Flow Imaging in Patients with Arterial Dissection.

Mesenteric ischemia is an infrequent, but potentially dangerous, complication of aortic or mesenteric artery dissection. Severe mesenteric ischemia presenting as a malperfusion syndrome, caused by aortic dissection or isolated mesenteric dissection, should be treated immediately by central aortic repair or angioplasty. However, treatment of non-critical, symptomatic mesenteric ischemia without narrowing true lumen of the aorta or localized mesenteric artery dissection was difficult. We treated two distinctly different cases of non-critical, symptomatic mesenteric ischemia superior mesenteric a

Duplication of aortic wall seen by echocardiography.

In four patients, echocardiography showed duplication of an aortic wall echo. An aortic dissection was present in only one. Other causes for the echocardiographic pattern were abscess in the interventricular septum, mitral stenosis, and dilatation of the noncoronary sinus of Valsalva. Division of the echo from the mitral ring into two separate posterior aortic wall echoes was seen only in the patient with dissecting aneurysm; it is suggested that this appearance adds to the specificity of the finding of aortic wall duplication by echocardiography in the diagnosis of aortic dissection.

The role of imaging in acute aortic syndromes.

The classic entity of life-threatening aortic dissection represents one pathology of a spectrum of acute conditions coined the acute aortic syndrome comprising dissection, intramural haematoma, penetrating atherosclerotic ulcer, and contained aortic rupture of any cause. The common denominator is disruption of the aortic media layers associated with severe pain and a variety of other symptoms. Any clinical suspicion of acute aortic syndrome should prompt immediate action and confirmatory non-invasive imaging; with respect to sensitivity and specificity for acute aortic pathology modern contras

A large French family with TGFBR2 pathogenic variant: illustration of variability.

AIMS: To report aortic events in a large family carrying a variant in the TGFBR2 gene. METHODS: Since 1990 up to 2024, we have conducted a longitudinal clinical study of a large single family comprising 63 members across four generations who carry the same TGFBR2 pathogenic variant. We assessed the incidence of aortic events and prophylactic surgery, as well as life expectancy. RESULTS: Over a follow-up, 21 patients died (33% of the population), of whom ten were related to a dissection (48%). Eight patients underwent prophylactic aortic root surgery (13%). Over the generations, there is an i

[Various choices of reconstructing the left subclavicular artery to extend the proximal landing zone thoracic endovascular aortic repair].

Thoracic endovascular aortic repair (TEVAR) has become the preferred treatment for Stanford type B aortic dissection. Covered the left subclavian artery (LSA) directly may cause corresponding complications in Stanford type B aortic dissection with unfavourable proximal landing zone. TEVAR can be successfully implemented by reconstructing LSA to expand the proximal landing zone. Currently, the methods of reconstructing LSA mainly include hybridization technology (carotid-subclavian artery transposition), chimney technology (including branch stent technology) and fenestration (or slot technology

Aortic Pathology in Systemic Lupus Erythematosus: A Case Report and Review of Literature.

Patients with autoimmune disease, and specifically lupus erythematosus, have an increased risk of developing aortic pathology, including aneurysm and dissections. We report a case of a 49-year-old woman with lupus erythematosus treated surgically for an abdominal aortic aneurysm in 2009, who developed type B aortic dissection 1 year later, treated by endovascular means. Dissection furtherly underwent aneurysm evolution, and the patient finally was brought to vascular attention with a fatal thoracic aneurysm rupture. A review of literature of cases of aortic aneurysm and dissection in patients

We summarize our experience out of 133 operations involving the aortic arch which were performed in 130 patients throughout the last 13 years. Aortic pathology was aneurysmal disease in 57 cases, acute aortic dissection in 44, and chronic aortic dissection in 29 patients. Operative strategies included partial replacement or repair of the aortic arch in 80 cases and subtotal or total replacement of the transverse arch in 53 patients. In 19 cases presenting with aneurysms of the descending thoracic aorta, implantation of an elephant trunk prothesis was performed simultaneously. The operations we

Cross-Entropy Learning for Aortic Pathology Classification of Artificial Multi-Sensor Impedance Cardiography Signals.

An aortic dissection, a particular aortic pathology, occurs when blood pushes through a tear between the layers of the aorta and forms a so-called false lumen. Aortic dissection has a low incidence compared to other diseases, but a relatively high mortality that increases with disease progression. An early identification and treatment increases patients' chances of survival. State-of-the-art medical imaging techniques have several disadvantages; therefore, we propose the detection of aortic dissections through their signatures in impedance cardiography signals. These signatures arise due to pa

Elective replacement of the ascending aorta: is the 5.5-cm threshold appropriate? The insidious, small aorta.

OBJECTIVES: As thoracic aortic aneurysm disease continues to cause significant morbidity and mortality in the general population, the cardiovascular community continues the search for the golden threshold of elective surgical replacement of the ascending aorta. METHODS: Thoracic aortic aneurysm is a common disease, classified within the 20 most common causes of death in patients over 65 years old. Once aortic complications like dissection or rupture occur, they can prove fatal. Prophylactic surgical replacement of the ascending aorta remains the mainstay of treatment to prevent these complica
