Stabilizing Plasmodium falciparum proteins for small molecule drug discovery.

Early-stage drug discovery relies on the availability of stable protein for reliable biophysical characterization of ligand binding. However, many Plasmodium falciparum proteins are challenging to produce in heterologous systems, which limits their experimental utility. To address this, we tested whether ProteinMPNN-guided sequence design could generate stabilized surrogate constructs that retain wild-type-like structure and binding thermodynamics. Designs were generated with constraints to maintain conserved and binding-site residues for three therapeutically relevant targets: PfBDP1-BRD, PfB

Fast and accurate identification of emerging viral reassortment from genome sequences.

Segmented virus genomes, such as those of influenza A viruses (IAVs), consist of multiple segments. This structure enables the generation of novel strains through reassortment, where segments from different strains combine, greatly increasing the genetic diversity of segmented viruses. Reassortment can confer new biological properties to viruses, such as increased transmission rates, as evidenced by several influenza pandemics throughout history. Consequently, it is crucial to monitor reassortment events, particularly emerging ones. The vast availability of viral genome data provides an opport

From General-Purpose to Disease-Specific Features: Aligning LLM Embeddings on a Disease-Specific Biomedical Knowledge Graph for Drug Repurposing.

Identifying new therapeutic uses for existing drugs is a major challenge in biomedicine, especially for complex neurodegenerative conditions such as Alzheimer disease and related dementias (ADRD), where treatment options remain limited and relevant data are often sparse, heterogeneous, and difficult to integrate. Although general-purpose Large Language Model (LLM) embeddings encode rich semantic information, they often lack the task-specific biomedical context needed for inference tasks such as computational drug repurposing. We introduce Contextualizing LLM Embeddings via Attention-based gRap

Cardiac Rhythm and Conduction Abnormalities in Aircrew.

INTRODUCTION: Cardiac rhythm and conduction abnormalities are common findings in aircrew and are a frequent cause of a loss of flying privileges. Given the great variety of electrophysiological disorders, risk assessment is challenging for the aviation medical examiner. For aeromedical assessment, important aspects of an electrophysiological evaluation are the presence or absence of symptoms and prognosis of the arrhythmia. In young fit people with increased vagal tone, some abnormal electrocardiogram findings are acceptable and do not require further investigation. CASE SERIES: This article

Freeform vs. Aspheric Spectacle Lenses: A Comprehensive Review of Optical Performance, Clinical Outcomes, and Patient Considerations.

Spectacle lens design has evolved from spherical lenses to aspheric lenses (ASL) and freeform lenses (FFL) to improve peripheral optics and wearer comfort. In this review, we compare ASLs and FFLs across optical performance, visual function, patient-reported outcomes, and practical considerations. We synthesized evidence from more than 90 peer-reviewed studies, technical reports, and authoritative textbooks. Databases included PubMed, Scopus, Web of Science, and specialist optometry sources. Eligibility focused on comparative ASL vs. FFL studies and high-quality reports. Data were organized in

Pre-treatment T-cell Transcriptional Signatures Predict Immunotherapy Outcomes in Melanoma.

Immune checkpoint inhibitors (ICIs) have improved outcomes for patients with melanoma and are now the standard of care for high-risk and advanced disease. However, long-term benefits are observed in only around 25% of patients, with significant risk for immune-related adverse events, highlighting the need for predictive biomarkers. To develop a minimally invasive, pre-treatment biomarker strategy, we profiled functional and subset-specific transcripts in peripheral blood T lymphocytes (PBTLs) and applied machine learning to identify predictive signatures. Patients were enrolled prior to receiv

Evaluation of the performance and temporal variability of large language models in patient education regarding pneumothorax: a seven-day analysis.

This study investigates the readability, clinical reliability, and temporal consistency of artificial intelligence (AI) chatbots regarding pneumothorax information. A question bank comprising 40 patient-centered queries was deployed across three large language models (ChatGPT, Gemini, Copilot), stratified by two access tiers and two prompting strategies (zero-shot versus the optimized PROMPORT strategy). Queries were replicated longitudinally on Days 1, 3, and 7 under strict session-control protocols. Text accessibility was quantified using five automated readability indices, while two indepen

Biomimetic and Locally Active Drug Delivery Systems for the Oral Biofilm and Periodontal Pocket: Formulation Strategies, Mechanisms and Translational Perspectives.

Periodontitis and dental caries remain among the most prevalent chronic diseases, and their local treatment is limited less by the choice of active agent than by the difficulty of sustaining therapeutic concentrations against salivary and crevicular clearance, the mucosal barrier and the biofilm matrix. This narrative review (PubMed/MEDLINE, Scopus, Web of Science; 2010-2026; 118 sources) maps five mechanistic classes-mucoadhesive, in situ gelling, stimuli-responsive, nano-/microparticulate and biomimetic-alongside marketed sustained-release products onto the specific barrier each addresses an

metadeconfoundR: Covariate analysis of high-dimensional cross-sectional omics data.

MOTIVATION: Identifying disease biomarkers from large molecular datasets is complicated by correlated and confounded signals like comorbidities and treatment regimens, batch effects, and cohort biases. These effects bias statistical inference and clinical conclusions. Robust methodologies are fundamental for reliable biomarker discovery. RESULTS: metadeconfoundR is an R package for conservative biomarker discovery in (multi-)omics case-control datasets. It has a scalable two-step confounder-aware statistical framework for retaining only associations with independent support. It identifies cov

Longitudinal alignments and syntheses of multimodal clinical data for personalized medicine with the PULSE framework.

Multimodal models capable of imputing diverse data used in single-cell biology studies provide potential foundational opportunities in clinical practice. However, patient data uniquely comprise longitudinal mosaic measurements that reflect underlying physiological dynamics and exhibit temporal covariation, demanding a specialized approach. Here we present Patient Unified Longitudinal Signal Engine (PULSE), a longitudinal self-supervised framework that explicitly encodes personalized past states (historical paired modalities) to reconstruct full profiles from subsequent unpaired measurements, t

Parameterization of the PA Endonuclease Bimetallic Center Reveals the Dynamics of Clinically Relevant Mutations.

Influenza A virus continues to impose a major global health and economic burden through seasonal epidemics and occasional pandemics, highlighting the critical need for continued antiviral development. As the latest addition to anti-influenza therapy, baloxavir marboxil (BXM) targets the highly conserved PA N-terminal endonuclease domain (PAN), blocking the cap-snatching process essential for viral transcription initiation. However, the rapid emergence of resistance mutations significantly reduces BXM susceptibility and compromises its clinical efficacy. Understanding the dynamics underlying re

Deep Learning for Automated Tic Detection and Prediction in Tourette Syndrome Using Electromyography and Video.

Tourette syndrome is characterized by involuntary motor and vocal tics, which are challenging to monitor objectively. This study evaluated convolutional neural networks (CNNs) and Transformer models on EMG and video data ('Rest', 'Tic', and 'Voluntary Movement') for detection and prediction. We hypothesized that EMG windows <5s would better capture tics and improve detection and prediction accuracy. 685 EMG recordings (n = 16 participants) were each segmented into 1-, 5-, and 10-second windows. Each model was trained on clinical EMG and validated on held-out data, using corresponding expert-an

Scalable phosphotyrosine enrichment with SH2 superbinder enables deep profiling of EGF responses.

Phosphotyrosine signaling plays a critical role in many biological processes, from cell proliferation to immune response. Despite its importance, proteomic studies of tyrosine phosphorylation have been limited in scale and throughput due to the need for specialized enrichment with costly reagents and labor-intensive protocols. To address these challenges, we developed R2HaPpY, a phosphotyrosine enrichment method that combines highly simplified phosphotyrosine superbinder reagent preparation and automated high-throughput enrichment. Our new reagent binds phosphotyrosine peptides at higher effic

GEDO: topology-based inference of gene module activity in Sjögren's disease.

Inferring gene module activity is key to understanding transcriptomic dysregulation in case-control studies. Most existing gene set analysis methods focus on differences between groups without considering the complex geometry of the data. We introduce GEDO, a graph and topology-based method that infers gene module activity through a transition score, quantifying the shift from healthy controls to diseased individuals. When applied to bulk RNA-seq data from Sjögren's disease patients and healthy controls (PRECISESADS cohort), and a breast cancer dataset from The Cancer Genome Atlas, GEDO was be

Data-driven modeling of spatiotemporal dynamics using multimodal imaging data.

Understanding how biological systems evolve across space and time remains a fundamental challenge, particularly when dynamic processes vary substantially across individuals. We present a personalized graph-based dynamical modeling framework for characterizing spatiotemporal biological dynamics from longitudinal multimodal imaging data. The framework constructs individualized brain graphs from MRI and PET measurements and learns patient-specific dynamical parameters governing regional structural and molecular changes. Applied to 1,891 participants from the Alzheimer's Disease Neuroimaging Initi

Rheological properties of the human and porcine oral mucosa.

PURPOSE: The objective of this study was to characterize the viscoelastic shear properties of gingival connective tissue under physiological conditions in humans and pigs. Porcine tissue is commonly used as a preclinical model due to its anatomical and mechanical similarity to human gingiva, which supports translational studies for gingival tissue engineering. MATERIALS AND METHODS: Human (n = 17) and porcine (n = 15) hard palate oral mucosa samples were subjected to histological characterization and shear rheological testing. Storage (G') and loss (G″) moduli were measured at 1 Hz for the ep

Cruzain Inhibitors for Chagas Disease: Anticorrelated Optimisation Landscapes and the Multiparametric Path to Clinical Candidates.

Over three decades, cruzain, the principal cysteine protease of Trypanosoma cruzi and a validated drug target for Chagas disease, has accumulated 215 inhibitors with IC50 ≤ 1 μM, more than 30 crystal structures, and no clinical candidates. This review argues that the resulting translational paradox reflects not target invalidity, but a persistent strategic mismatch between how cruzain inhibitors have been optimised and what intracellular efficacy requires. Across peptidyl vinyl sulfones, non-peptidic scaffolds, and thiosemicarbazones, enzymatic potency has been systematically prioritised over

Indirect state-level estimation of sexual minority adolescent populations by sex, age, and race/ethnicity using random forests.

PURPOSE: Estimating population sizes of adolescents who identify as lesbian, gay, or bisexual (LGB) is important for addressing health needs and disparities. Most states have Youth Risk Behavior Surveys (YRBS) among high school students, but not all include an item about sexual identity. This study's aim was to estimate the percentages of students identifying as LGB stratified by sex, age, and race and ethnicity where state data is incomplete. States where 2021 YRBS data are not available are outside the scope of this study. METHODS: We developed two random forests trained separately for each
