Platelet-membrane-coated nanoparticles enable safe and targeted thrombolysis with preserved neurovascular integrity.

Recombinant tissue-type plasminogen activator (rtPA) is the only FDA-approved thrombolytic agent for acute ischemic stroke, yet its clinical utility is constrained by a narrow therapeutic time window, rapid clearance, and hemorrhagic transformation risk. Here, we developed a biomimetic nanothrombolytic comprising rtPA-conjugated, platelet-membrane-coated PLGA nanoparticles loaded with perfluorohexane (PNP-rtPA) to improve thrombus specificity and vascular safety. PNP-rtPA preserved rtPA enzymatic activity, achieved efficient fibrin clot lysis in vitro, and markedly enhanced thrombolysis in an

Cerebral Schistosoma japonicum infection confirmed by histopathology and PCR: Diagnostic and therapeutic challenges in multiple parasitic coinfections.

Cerebral schistosomiasis, a rare manifestation of schistosomiasis, is challenging to diagnose due to non-specificity in the clinical and radiologic findings and cross-reactivity in serologic testing with other helminths. Coinfection with multiple parasites further complicates diagnosis and treatment. A previously healthy 32-year-old Filipino man presented with a seizure. Brain magnetic resonance imaging (MRI) revealed multiple clustered enhancing nodules with surrounding vasogenic edema in the left frontal lobe. Serologic testing was positive for anti-Schistosoma japonicum, Strongyloides, and

Histopathological and molecular mechanisms of multi-organ damage in heat stroke: Endothelial failure, immune crosstalk, and regulated cell death pathways.

Heat stroke is a life-threatening condition characterized by severe hyperthermia accompanied by central nervous system dysfunction and progressive multi-organ injury. Although rapid cooling remains the cornerstone of clinical management, accumulating evidence indicates that normalization of core body temperature alone does not fully prevent delayed organ damage. Experimental and clinical studies increasingly demonstrate that heat stroke-induced pathology extends beyond acute thermal injury and involves sustained endothelial dysfunction, coagulation imbalance, sterile inflammation, mitochondria

Gatm ablation disrupts spermatogenesis by impairing ribosome biogenesis and coupling defective steroid biosynthesis to immunoglobulin silencing.

BACKGROUND: Glycine amidinotransferase (GATM) catalyzes the rate-limiting step in creatine biosynthesis, a pathway essential for cellular energy buffering in tissues with high metabolic demands. While its roles in muscle and brain are well established, the function of GATM in testicular development and spermatogenesis remains largely unexplored. METHODS: We generated a constitutive Gatm knockout mouse model using CRISPR/Cas9 technology. Comprehensive phenotypic characterization was performed through histological analysis, transmission electron microscopy (TEM), and transcriptomic profiling of

Propofol and Glioblastoma Biology: A Narrative Review of Perioperative Clinical Evidence and PI3K/AKT-Wnt-miRNA, GABA-A, and Immune-Checkpoint Pathways.

Glioblastoma multiforme (GBM) is the most prevalent and aggressive primary brain tumor, with a median survival of approximately 15 months despite current multimodal therapies. Recent clinical observations have suggested that the intravenous anesthetic propofol may be used to examine whether anesthetic agents can affect glioblastoma behavior. Several retrospective studies reported that, in surgical settings, propofol-based total intravenous anesthesia (TIVA) is associated with reduced operative time, faster recovery, lower intraoperative blood loss, and decreased postoperative complications suc

Case Report: Leiomyosarcoma of the inferior vena cava presenting as a right atrial mass: the value of multimodality imaging.

Leiomyosarcoma of the inferior vena cava (IVC) is a rare malignant tumor that may extend into the right atrium and mimic a primary cardiac neoplasm. Because clinical presentation is often non-specific and depends on tumor location and extent, diagnosis may be delayed, particularly in acute settings with suboptimal imaging conditions. We report the case of a 33-year-old woman presenting with progressive dyspnea, ascites and peripheral edema consistent with right-sided heart failure. Initial contrast-enhanced computed tomography, performed to exclude pulmonary embolism, suggested a right atrial

Low-Field Magnetic Resonance Imaging in Critical Care of Stroke.

Neurological deterioration is a frequent and clinically significant challenge in patients with acute stroke admitted to neurocritical care units, where timely neuroimaging is essential but access to advanced imaging may be limited by patient instability and logistical constraints. Low-field magnetic resonance imaging (LF-MRI) has recently emerged as a novel approach to extend MRI capability into critical care environments. This topic review synthesizes current evidence on the feasibility, safety, and evolving clinical applications of LF-MRI in neurocritical care stroke. Existing studies demons

High-altitude hypoxia and blood-brain barrier injury: cellular mechanisms and clinical targets.

High-altitude regions, characterized by unique geographical and environmental conditions, are endowed with striking natural landscapes and abundant resources, attracting large numbers of people for exploration, habitation, and work. However, high-altitude hypoxia remains a persistent environmental challenge that threatens human health. Hypoxia is a principal environmental stressor that induces structural and functional injury to the blood-brain barrier (BBB). Its mechanisms involve disruption of tight junctions in the paracellular pathway, enhanced transcellular permeability, dysregulation of

Echinocystic acid alleviates cerebral ischemia-reperfusion injury through PPARγ-dependent modulation of microglial polarization and pyroptotic signaling.

Cerebral ischemia-reperfusion injury (CIRI) worsens outcomes after ischemic stroke, yet effective pharmacological interventions remain limited. Echinocystic acid (EA), a pentacyclic triterpenoid isolated from the fruits of Gleditsia sinensis Lam., has shown neuroprotective activity in CIRI, but its direct molecular target and underlying mechanism remain unclear. In this study, using network pharmacology, molecular docking, surface plasmon resonance, and subsequent validation in middle cerebral artery occlusion (MCAO) mice and oxygen-glucose deprivation/reoxygenation (OGD/R)-treated BV2 cells,

Fatal cerebral gas embolism during endoluminal vacuum therapy for esophageal leak.

BACKGROUND: Endoluminal vacuum therapy (EndoVAC/EVT) is widely used for managing esophageal anastomotic leaks, with high closure rates and a favorable safety profile compared with stent placement or surgical revision. Serious neurological complications associated with endoVAC treatment appear to be exceptionally rare. CASE PRESENTATION: We present the case of a 60-year-old patient who underwent open Ivor Lewis esophagectomy for pT2N0 esophageal adenocarcinoma. On postoperative day (POD) 8, fever and leukocytosis developed, and imaging confirmed an intrathoracic anastomotic leak. EVT was initi

Effects of intramuscular administration of Mg-Zn alloys on tissue and blood parameters in rats.

Magnesium-zinc (Mg-Zn) alloys are widely investigated as potential biodegradable biomaterials, but their systemic safety and tissue-level effects remain incompletely understood. This study aimed to evaluate the biochemical and histopathological responses to intramuscular implantation of Mg-Zn alloys in rats. Thirty-two male Wistar albino rats were randomly assigned to four groups: Control, Mg, MgZn1, and MgZn3, and sacrificed at the 4th and 7th weeks. Blood samples were collected for lipid profile, liver and kidney function tests, and protein analysis. Histopathological examinations of muscle,

Chronic hypertension and perfusion deficits conjointly affect disease outcome after tPA treatment in a rodent model of thromboembolic stroke.

Futile recanalization hampers prognoses for ischemic stroke patients despite successful recanalization therapy. Allegedly, hypertension and reperfusion deficits contribute, but a better understanding is needed of how they interact and mediate disease outcome. We reassessed data from spontaneously hypertensive and normotensive Wistar-Kyoto rats (male, n = 6-7/group) that were subjected to two-hour embolic middle cerebral artery occlusion and thrombolysis in preclinical trials. Serial MRI allowed lesion monitoring and parcellation of regions-of-interest that represented infarcted (core) or recov

Emergence of hemorrhagic septicemia caused by MLST type ST64 Pasteurella multocida in a European fallow deer population in Hungary.

A sudden mass mortality involving 44 fallow deer (Dama dama) occurred in September 2020 at a game management area near Budapest, Hungary. The die-off affected both sexes and all age classes equally. On postmortem examination of 2 subadults and 1 fawn, we found average body condition, diffuse subcutaneous edema, swelling of peripheral lymph nodes, marked hemorrhages in lymph nodes along the esophagus and trachea, and left ventricular dilation. We isolated Pasteurella-like colonies from spleen, liver, small intestine, lung, and brain samples of 7 animals, and identified them as P. multocida sero

MRI findings in six dogs with ependymoma of the brain and spinal cord.

There are few published descriptions of the MRI appearance of canine intracranial or spinal cord ependymoma. In this multicenter, retrospective, secondary analysis, case series study, three veterinary radiologists independently reviewed and recorded imaging characteristics of MRI studies in six dogs with histopathologically confirmed ependymoma (three intracranial and three spinal cord cases). A consensus was reached when there was disagreement on specific features. All intracranial ependymomas had forebrain location, heterogeneous signal intensity in T1-weighted (T1W) and T2-weighted (T2W) im

Inhibition of the TRPM4 channel by 9-PH attenuates hippocampal endothelial injury induced by sleep deprivation and the subsequent neuroinflammation and neurological dysfunction.

Sleep deprivation (SD) is a significant public health concern and a recognized risk factor for cognitive impairment. While both vascular dysfunction and neuroinflammation are implicated, the mechanistic link between them remains elusive. Here, we identify a pathogenic positive feedback loop centered on endothelial transient receptor potential melastatin 4 (TRPM4) as a core driver of SD-induced pathology. We demonstrate that SD triggers an early and specific upregulation of TRPM4 in hippocampal endothelial cells, which in turn induces blood-brain barrier (BBB) disruption, characterized by tight

No Harmful Effect of Endovascular Treatment before Decompressive Surgery-Implications for Handling Patients with Space-Occupying Brain Infarction.

This study explored short- and mid-term functional outcomes in patients undergoing decompressive hemicraniectomy (DHC) due to space-occupying cerebral infarction and asked whether there is a potentially harmful effect of a priorly performed endovascular treatment (EVT). Medical records were screened for patients requiring DHC due to space-occupying cerebral infarction between January 2016 and July 2021. Functional outcomes at hospital discharge and at 3 months were assessed by the modified Rankin Scale (mRS). Out of 65 patients with DHC, 39 underwent EVT before DHC. Both groups, i.e., EVT + DH

Oseltamivir phosphate (Tamiflu) alters neurobehavior of zebrafish larvae by inducing mitochondrial dysfunction.

Antiviral drugs are widely used, yet their potential risks during early development, particularly within the central nervous system, remain contentious. Oseltamivir phosphate (OSE), a commonly prescribed antiviral, is increasingly detected in various environments. However, its toxicity to organisms and the underlying mechanisms are not well understood. In this study, we employed the zebrafish model to evaluate the developmental neurotoxic effects of OSE at environmentally and therapeutically relevant doses, through high-throughput behavioral analysis, in vivo two-photon imaging, transcriptomic

Heatwaves and human sleep: Stress response versus adaptation.

The World Meteorological Organization considers a heatwave as "a period of statistically unusual hot weather persisting for a number of days and nights". Accompanying the ongoing global climate change, sharp heatwave bouts occur worldwide, growing in frequency and intensity, and beginning earlier in the season. Heatwaves exacerbate the risk of heat-related illnesses, hence human morbidity and mortality, particularly in vulnerable elderly and children. Heat-related illnesses present a continuum from normothermic (prickly heat, heat edema, heat cramps, heat tetany) to hyperthermic syndromes (fro
