Allogeneic stem-cell transplantation for lymphoproliferative disorders using BEAM-CAMPATH (+/- fludarabine) conditioning combined with post-transplant donor-lymphocyte infusion.

BACKGROUND: We report our updated experience of allogeneic transplantation in lympho-proliferative disorders using a reduced-intensity conditioning regimen combining BEAM (plus fludarabine in three cases) with pre-transplant CAMPATH. Post-transplant donor lymphocytes have been infused for persisting disease or relapse, and both chimerism and minimal residual disease have been monitored utilizing molecular techniques. METHODS: Thirty patients with median age 47.6 years underwent allogeneic transplantation for relapsed or high-risk lymphoproliferative disease using HLA-identical (sibling n = 25

Unmanipulated haploidentical hematopoietic stem cell transplantation is an excellent option for children and young adult relapsed/refractory Philadelphia chromosome-negative B-cell acute lymphoblastic leukemia after CAR-T-cell therapy.

Although chimeric antigen receptor T-cell (CAR-T) therapy produces a high complete remission rate among patients with relapsed/refractory B-cell acute lymphoblastic leukemia, relapse remains an urgent issue. It is uncertain whether consolidative haploidentical-allogeneic hematopoietic stem cell transplantation (haplo-HSCT) is suitable for achieving sustainable remission. Therefore, we aimed to assess the efficacy and safety of bridging CAR-T therapy to haplo-HSCT. Fifty-two patients with relapsed/refractory Philadelphia chromosome-negative B-cell acute lymphoblastic leukemia who underwent hapl

How to Combine the Two Landmark Treatment Methods-Allogeneic Hematopoietic Stem Cell Transplantation and Chimeric Antigen Receptor T Cell Therapy Together to Cure High-Risk B Cell Acute Lymphoblastic Leukemia?

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) has made tremendous progress in the last few decades and is increasingly being used worldwide. The success of haploidentical HSCT has made it possible to have "a donor for everyone". Patients who received transplantation in remission may have a favorable outcome, while those who were transplanted in advanced stages of disease have a poor prognosis. Although chimeric antigen receptor T (CAR-T) cell therapy is currently a milestone in the immunotherapy of relapsed or refractory (R/R) B cell acute lymphoblastic leukemia (B-ALL) and ha

Impact of alemtuzumab pharmacokinetics on T-cell dynamics, graft-versus-host disease and viral reactivation in patients receiving allogeneic stem cell transplantation with an alemtuzumab-based T-cell-depleted graft.

Administration of alemtuzumab (targeting the CD52 antigen) to the patient (in-vivo) or to the graft (in-vitro) before allogeneic stem cell transplantation (alloSCT) decreases the incidence of graft-versus-host disease (GvHD). Effectiveness of this treatment relies on depletion of donor T cells. Currently, no data are available on alemtuzumab pharmacokinetics and pharmacodynamics in patients who received combined in-vivo and in-vitro alemtuzumab-based T-cell depletion. In this prospective study, we analyzed alemtuzumab pharmacokinetics and its effect on the circulating T cells in 36 patients wh

Cyclosporiasis in Solid Organ and Hematopoietic Cell Transplant Recipients: Diagnosis, Treatment, and Prevention.

BACKGROUND: Cyclospora cayetanensis is a foodborne coccidian protozoan that causes prolonged, often relapsing watery diarrhea. Once considered largely a travel-associated infection, it has become a recurring cause of large, domestically acquired, produce-associated outbreaks in temperate countries. The 2026 US season was the largest on record and prompted a CDC Health Advisory, placing substantial numbers of solid organ transplant (SOT) and hematopoietic cell transplant (HCT) recipients at risk of exposure. AIM: To provide a practical, transplant-specific approach to the diagnosis, treatment,

Epstein-Barr virus-specific T-cells in pediatric liver transplant recipients.

To determine Epstein-Barr virus (EBV)-specific T-cell responses after pediatric liver transplantation (LT), we monitored EBV viral load and IFNγ-producing specific T-cells reacting to BZLF1, EBNA1, and LMP2 antigens from peripheral immune cells before LT until 48 weeks after LT. Among forty children, seven (17.5%) developed EBV infection. Two were subsequently diagnosed with post-transplant lymphoproliferative disorders. The infected children exhibited fewer EBNA1-specific CD4 + T-cells and CD8 + T-cells before LT (P = 0.01 and P = 0.02, respectively) than non-infected children. At 32 weeks af

Psychological Prehabilitation for People Undergoing Autologous Stem Cell Transplant: A Qualitative Study.

BACKGROUND: Autologous stem cell transplant (AuSCT) improves survival in haematological cancers, yet often involves a lengthy recovery associated with a myriad of psychological and physical challenges. Psychological prehabilitation is recommended to improve functional outcomes by addressing psychological concerns and promoting optimal mental and physical health prior to AuSCT. Despite this, little is known about the implementation and experiences of psychology-specific prehabilitation for AuSCT. AIMS: The current study explores the perspectives and experiences of AuSCT recipients and healthca

Enhancing islet transplantation outcomes in T1DM: The promise of mesenchymal stem cells and exosomes.

Islet transplantation represents a promising therapeutic strategy for type 1 diabetes mellitus (T1DM), yet its clinical application is constrained by hypoxia, oxidative stress, and immune rejection during islet isolation, culture, and post-transplantation. In recent years, mesenchymal stem cells (MSCs) have demonstrated the ability to enhance islet survival and function through immunomodulation, anti-inflammatory effects, and paracrine signaling. Among the secreted components, exosomes have emerged as key mediators of MSC function, capable of delivering nucleic acids, proteins, and lipids to t

Clear bagging workflow implementation for post-hematopoietic stem cell transplant vaccinations.

PURPOSE: To describe development and implementation of a "clear bagging" workflow to improve coordination of vaccinations after hematopoietic stem cell transplantation (HSCT) while maintaining clinic-based administration and reimbursement. SUMMARY: Vaccines administered during clinic visits are often billed through medical insurance using a buy-and-bill model, which can result in patient claim denials and subsequent institutional write-offs, particularly for Medicare beneficiaries. To address this, health systems have adopted alternative distribution models such as clear bagging, whereby an i

Innovative approaches targeting innate immune cells to promote organ transplant tolerance.

Achieving long-term allograft survival while minimizing systemic immunosuppression (IS) remains a critical unmet need in transplantation. While adaptive immunity has traditionally been the primary focus of IS therapy, innate immune cells-that include neutrophils, monocytes, macrophages, dendritic cells, myeloid-derived suppressor cells, innate lymphoid cells (ILCs), natural killer (NK) cells, and gamma delta (γδ) T cells act as key upstream orchestrators of allograft rejection and tolerance. Recent advances in single-cell RNA sequencing and spatial transcriptomics have unveiled the profound he

Precision monitoring of kidney transplant health via cell-free DNA and RNA.

PURPOSE OF REVIEW: Cell-free nucleic acids (cfNAs) in plasma and urine have emerged as noninvasive biomarkers for monitoring kidney transplant health. This review summarizes recent advances in the development of cell-free DNA (cfDNA) and cell-free RNA (cfRNA) assays for immune and infection-related complications, and discusses their potential to enable precision monitoring of allograft health. RECENT FINDINGS: Large prospective multicenter studies have established donor-derived cfDNA as a robust biomarker of acute allograft rejection, with increasing evidence supporting the use of cfDNA for s

Predictive factors for poor mobilization in autologous stem cell transplant: a multivariate model.

INTRODUCTION: Inadequate stem cell mobilization remains a major challenge in autologous stem cell transplantation. Although multiple risk factors for poor mobilization have been suggested, their clinical utility is limited. This study aimed to identify predictors of poor mobilization and develop a clinically applicable predictive model. MATERIAL AND METHODS: A retrospective analysis of patients who underwent autologous stem cell transplantation was conducted. Poor mobilization was defined as a total CD34+ cell collection of less than 2 × 10⁶ cells/kg or the requirement for extended apheresis

Mitochondria transplantation preserves retinal ganglion cells and promotes CNS axonal regeneration.

Mitochondrial dysfunction is a central driver of retinal ganglion cell (RGC) loss in glaucoma and other forms of optic neuropathies, leading to irreversible blindness. Here, we demonstrate that replenishing the mitochondrial pool through exogenous mitochondrial transplantation ("mitotherapy") in adult mice not only preserves neuronal survival but also promotes regenerative competence in the central nervous system (CNS). In aging or injured RGCs, we identified profound deficits in mitochondrial biogenesis, fission-fusion balance, and mitophagy. Transplantation of functional mitochondria in in v

Long-term remission of neuromyelitis optica with allogeneic hematopoietic stem cell transplant.

BACKGROUND: Neuromyelitis optica (NMO) is a rare, aggressive, neurological autoimmune disorder. Despite several immunosuppressive drugs having been recently approved, some patients still experience frequent relapses, leading to disability accrual. METHODS: We report the long-term follow-up of the two patients treated with allogeneic hematopoietic cell transplantation (alloHCT) for treatment-refractory anti-aquaporin-4 immunoglobulin G (AQP4-IgG)-positive NMO 16 and 15 years ago, respectively. Patients were followed with serial clinical, imaging, and immunological assessments. FINDINGS: Both

Allogeneic Stem Cell Transplant for Cerebral Adrenoleukodystrophy: The Changing Role of Alternative Donors.

BACKGROUND: Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a key therapy for stabilizing neurological function and improving survival in cerebral adrenoleukodystrophy (CALD). Identifying a suitable matched related donor (MRD) or unrelated donor remains challenging, especially in countries with limited donor registry representation. These barriers have increased the use of alternative donors, such as haploidentical or umbilical cord blood grafts. Early transplantation is associated with better outcomes. METHODS: We describe outcomes of seven consecutive children with rapidly

Management of inherited fibrinogen disorder during chemotherapy and allogeneic stem cell transplant.

Inherited fibrinogen disorders (IFDs) are rare, and limited evidence-based management guidelines exist, especially regarding haematological malignancies and haematopoietic stem cell transplantion (HSCT). This case highlights the successful multidisciplinary management of an adult patient with acute Philadelphia-negative B-cell lymphoblastic leukaemia (B-ALL) and IFD from induction through HSCT. A patient in his 20s with an IFD is diagnosed with B-ALL and receives induction chemotherapy, including dose-reduced pegaspargase (PEG-Asp), blinatumomab consolidative therapy and HSCT for post-inductio

Clinicopathological Associations of Plasma Cell-Rich Rejection in Pediatric Liver Transplant Recipients.

INTRODUCTION: Liver transplantation is the standard treatment for pediatric patients with cirrhosis, acute liver failure, metabolic liver disease, and malignancy. Plasma cell-rich rejection is a poorly characterized form of late-onset rejection. We aimed to evaluate the incidence and clinicopathological factors associated with plasma cell-rich rejection in pediatric liver transplant recipients in Johannesburg, South Africa. METHODS: We conducted a retrospective review of 91 pediatric liver transplant recipients (≤ 18 years) who underwent transplantation between January 1, 2015, and December 3

Clinicopathological and Molecular Characteristics of Plasma Cell-Rich Rejection in Renal Transplant Biopsies.

BACKGROUND: Plasma cell-rich rejection (PCRR) is an ill-defined type of renal allograft rejection currently considered a subtype of T cell-mediated rejection. Our study analyzed the clinical course, Banff lesion scores, and mRNA expression of PCRR compared with archetypical types of (late) rejection to better understand and define this presentation in kidney transplants. METHODS: We retrospectively scored the last known biopsy in 263 renal transplant recipients, including 42 PCRR cases. Banff lesion scores and clinical outcomes were assessed.
