Pregnancy-associated pulmonary embolism during the peripartum period: An 8-year experience at a single center.

OBJECTIVE: The purpose of this study was to estimate the incidence, timing of onset, risk factors, and mortality rate of pregnancy-associated pulmonary embolism (PAPE). METHODS: We analyzed PAPE cases that occurred between January 2005 and December 2012 at Cheil General Hospital & Women's Healthcare Center. Those cases that were not confirmed by computed tomography scan or were confirmed as amniotic fuid embolisms were excluded. We analyzed various risk factors such as previous surgery, mode of delivery, maternal age, and obesity in PAPE. RESULTS: There were 57,092 deliveries over 8 years. O

[Pregnancy outcomes in a patient-heterozygous carrier of R506Q mutation of factor V (Leiden)].

The aim was to review currently available evidence on the association between thrombophilia and adverse pregnancy outcomes. Maternal thrombophilia has recently been identified as a major cause of thromboembolism, placental thrombosis and adverse pregnancy outcome including severe preeclampsia, placental abruption, intrauterine growth retardation, recurrent pregnancy loss, and stillbirth. The relatively high prevalence of thrombophilia defects in the general population and the association with adverse maternal and fetal outcomes have prompted obstetricians to focus their interest on this area.

Modes of delivery of first and second twins as a function of their presentation. Study of 614 consecutive patients from 1992 to 2000.

OBJECTIVES: To study modes of delivery and neonatal morbidity of twins as a function of their presentation. STUDY DESIGN: The study related to 614 consecutive patients who gave birth to twins in the maternity ward of the Hôpital Robert Debré from 1992 to 2000. Group A (n=529) included patients who gave birth after 33 weeks of gestation and group B (n=85) before 33 weeks. The parameters studied were the mode of delivery, the need for intubation at birth, 5-min Apgar score <9, transfer to intensive care, death in the neonatal period. The chi-square test was used for statistical analysis. RESUL

Two different regimens of preinduction ripening of the uterine cervix with prostaglandin E2: a randomized clinical study.

A randomized clinical study was designed to test the relative efficacy of preinduction cervical ripening with 0.25 mg prostaglandin E2 (PGE2), repeated if necessary (group 1) compared to a single maturation with 0.50 mg PGE2 (group 2). In group 1 (n = 42), the ripening process was repeated every day until spontaneous onset of labor occurred or induction with oxytocin was decided (for improved Bishop score above 5, or maternal or fetal distress). In group 2 (n = 42) the patients who had not labored 12 h after the maturation procedure had labor induced with oxytocin, irrespective of their cervic

Induction of Labor at 40 Weeks of Gestation after Dissemination of the ARRIVE Trial.

OBJECTIVE: The ARRIVE trial showed that induction of labor in low-risk nulliparous individuals at 39 weeks reduced cesarean delivery. We sought to determine whether publication of the ARRIVE trial was associated with changes in the rate of induction of labor at 40 weeks, and if so, whether similar changes were seen in the rate of cesarean delivery. STUDY DESIGN: This was a repeated cross-sectional analysis using US natality data from February 2017 to August 2019. Low-risk nulliparous individuals delivering a non-anomalous, cephalic, singleton infant were included; exclusion criteria included

Isthmocele: from Detection to Treatment: A Narrative Review.

Isthmocele, also known as cesarean scar defect, is an increasingly recognized complication of cesarean delivery, often associated with abnormal uterine bleeding, pelvic pain, and secondary infertility. This narrative review aims to provide a comprehensive overview of the condition, including its pathogenesis, risk factors, diagnostic methods, and current treatment options. The most significant risk factor for isthmocele formation is the number of previous cesarean sections, although other contributing elements include surgical technique, uterine position, and individual wound-healing capacity.

Delivery mode-associated gut microbiota in the first 3 months of life in a country with high obesity rates: A descriptive study.

Delivery methods during childbirth and their related gut microbiota profiles have important impacts on health later in life, they can contribute to the development of diseases such as obesity, whose highest prevalence rate is found among the Mexican child population. Coincidentally, Mexico has one of the highest global average annual rate increase in cesarean births (C-section). Since Mexico leads the world in childhood obesity, studying the relationship between childbirth delivery methods and gut microbiota profiles in this vulnerable population may be used to identify early risk factors for

Obstetrical, pregnancy and socio-economic predictors for new-onset severe postpartum psychiatric disorders in primiparous women.

BACKGROUND: Childbirth is a potent trigger for the onset of psychiatric illness in women including postpartum depression (PPD) and postpartum psychosis (PP). Medical complications occurring during pregnancy and/or childbirth have been linked to postpartum psychiatric illness and sociodemographic factors. We evaluated if pregnancy and obstetrical predictors have similar effects on different types of postpartum psychiatric disorders. METHOD: A population-based cohort study using Danish registers was conducted in 392 458 primiparous women with a singleton delivery between 1995 and 2012 and no pr

Gestational age, mode of birth and breastmilk feeding all influence acute early childhood gastroenteritis: a record-linkage cohort study.

BACKGROUND: Acute gastroenteritis (AGE) is a leading cause of infectious morbidity in childhood. Clinical studies have implicated caesarean section, early birth and formula feeding in modifying normal gut microbiota development and immune system homeostasis in early life. Rates of early birth and cesarean delivery are also increasing worldwide. This study aimed to investigate the independent and combined associations of the mode and timing of birth and breastmilk feeding with AGE hospitalisations in early childhood. METHODS: Population-based record-linkage study of 893,360 singleton livebirth

Cephalic version by postural management for breech presentation.

BACKGROUND: Babies with breech presentation (bottom first) are at increased risk of complications during birth, and are often delivered by caesarean section. The chance of breech presentation persisting at the time of delivery, and the risk of caesarean section, can be reduced by external cephalic version (ECV - turning the baby by manual manipulation through the mother's abdomen). It is also possible that maternal posture may influence fetal position. Many postural techniques have been used to promote cephalic version. OBJECTIVES: The objective of this review was to assess the effects of pos
