Exposure to air pollution and cardiovascular risk in young children - a pilot project.

OBJECTIVE: To examine relationships between traffic-related air pollution (TRAP) and markers of pre-clinical cardiovascular risk in young children. STUDY DESIGN: We studied a cohort of healthy children ages 2-5 recruited from pediatric primary care sites (n = 122). We obtained child weight, height, blood pressure and hair nicotine levels. A blood sample was obtained for biomarkers of systemic inflammation, oxidation, and prevalence of circulating endothelial progenitor cells. This manuscript represents a secondary analysis. TRAP exposure (particulate levels, nitrogen dioxide, nitrogen oxides,

Investigating social determinants of child health and their implications in reducing pediatric traumatic injury: A framework and 17-year retrospective case-control study protocol.

INTRODUCTION: Traumatic physical injuries are the number one cause of hospitalization and death among children in Canada. The majority of these injuries are preventable. The burden from injury can be reduced through prevention programs tailored to at-risk groups, however, existing research does not provide a strong explanation of how social factors influence a child's risk of injury. We propose a theoretical framework to better understand social factors and injury in children and will examine the association between these social factors and physical traumatic injury in children using large pop

"I feel so bad but have nothing to do." Exploring Ugandan caregivers' experiences of parenting a child with severe malaria and subsequent repeated uncomplicated malaria.

BACKGROUND: Severe malaria in children is often associated with long-term behavioural and cognitive problems. A sizeable minority of children go on to experience repeated malaria due to the high transmission and infection rates in the region. The purpose of this study was to explore caregivers' experiences of parenting a child with a history of severe malaria followed by repeated episodes of uncomplicated malaria in comparison to healthy community children. METHODS: Thirty-one caregivers were enrolled in the study. These included caregivers of children previously exposed to severe malaria and

Impact and safety of endoscopic ultrasound guided fine needle aspiration on patients with cirrhosis and pyrexia of unknown origin in India.

BACKGROUND AND AIMS: Etiologic diagnosis of pyrexia of unknown origin is important in patients with cirrhosis for optimal management and to prevent flare up of infectious disease after liver transplantation. However, there is very limited literature available on this subject. The present study aimed to examine the safety and impact of endoscopic ultrasound (EUS) guided fine needle aspiration (FNA) in patients with cirrhosis. METHODS: The study was conducted between January 2014 and January 2016 at a tertiary care center. A total of 50 (47 lymph nodes, 3 adrenal) EUS guided FNAs were performed

A survey of parentally reported sleep health disorders in Estonian 8-9 year old children.

BACKGROUND: Pediatric sleep research is rather new in Estonia. There has not been a comprehensive study of age specific sleep disorders in Estonian children. The aim of this study was to investigate sleep disorders in a sample of Estonian second grade children. We hypothesized that:: Children with low BMI are as susceptible to SDB as are children with high BMI. Under weight children are susceptible to residual SDB after adenotonsillectomy. Parasomnias present with SDB in children.• Excessive day time sleepiness is a significant symptom which leads parents to suspect sleep disorders in their chi

TP53 p.R337H is a conditional cancer-predisposing mutation: further evidence from a homozygous patient.

BACKGROUND: Adrenocortical carcinomas (ACCs) are among the most common childhood cancers occurring in infants affected with the Li-Fraumeni and Li- Fraumeni-like (LFS/LFL) syndromes, which are caused by dominant germline mutations in the TP53 gene. In Brazil, a particular mutation, occurring in the tetramerisation domain of the gene, p.R337H, is exceedingly common due to a founder effect and is strongly associated with ACC. In this report, we describe the phenotype and long-term clinical follow-up of a female child diagnosed with ACC and homozygous for the TP53 p.R337H founder mutation. CASE

Traumatic brain injury in the neonate, child and adolescent human: an overview of pathology.

In the middle of the last century it had been thought that a good recovery of function and behavior would occur after traumatic brain injury (TBI) in very young human beings. A recent major change in thinking states that early childhood TBI may result in a severe compromise of normal brain growth and development such that TBI, rather, may compromise later normal development resulting in a need for very long term patient care and management. The mechanisms of injury and pathology within the injured brain are reviewed and compared between when injury occurs at or close to the time of birth, in a

Late antenatal care initiation and its associated factors among pregnant women attending antenatal care at public hospitals of Kellem Wollega zone, Western Ethiopia: a facility-based cross-sectional study.

BACKGROUND: Late antenatal care initiation refers to the beginning of antenatal care for the first time during pregnancy after 12 weeks of gestation. Beginning antenatal care services after the first trimester misses opportunities to offer preventive measures, screening, and effective interventions early in the pregnancy. There is limited evidence on the magnitude of late antenatal care initiation in the Kellem Wollega Zone, Western Ethiopia. OBJECTIVE: The objective of this study was to assess the magnitude of late antenatal care initiation and its associated factors among pregnant women att

Retention-in-care in the PMTCT cascade: definitions matter! Analyses from the INSPIRE projects in Malawi, Nigeria and Zimbabwe.

INTRODUCTION: Definitions of retention-in-care in Prevention of Mother-to-Child Transmission of HIV (PMTCT) vary substantially between studies and programmes. Some definitions are based on visits missed/made, others on a minimum total number of visits, or attendance at a final clinic visit at a specific time. An agreed definition could contribute to developing evidence-based interventions for improving retention-in-care. In this paper, we estimated retention-in-care rates according to different definitions, and we quantified and visualized the degree of agreement between definitions. METHODS:

Perceptions, careseeking, and experiences pertaining to non-cephalic births in rural Sarlahi District, Nepal: a qualitative study.

BACKGROUND: In low-resource settings, a significant proportion of fetal, neonatal, and maternal deaths can be attributed to intrapartum-related complications. Certain risk factors, such as non-cephalic presentation, have a particularly high risk of complications. This qualitative study describes experiences around non-cephalic births and highlights existing perceptions and care-seeking behavior specific to non-cephalic presentation in rural Sarlahi District, Nepal. METHODS: We conducted in-depth interviews with 34 individuals, including women who recently gave birth to a non-cephalic infant a

Household health care-seeking costs: experiences from a randomized, controlled trial of community-based malaria and pneumonia treatment among under-fives in eastern Uganda.

BACKGROUND: Home and community-based combined treatment of malaria and pneumonia has been promoted in Uganda since mid 2011. The combined treatment is justified given the considerable overlap between the symptoms of malaria and pneumonia among infants. There is limited evidence about the extent to which community-based care reduces healthcare-seeking costs at the household level in rural and urban settings. This paper assesses the rural-urban differences in direct and indirect costs of seeking care from formal health facilities compared to community medicine distributors (CMDs). METHODS: Exit

Juvenile Idiopathic Arthritis for the Pediatric Orthopedic Surgeon.

Juvenile idiopathic arthritis includes conditions characterized by joint inflammation of unknown etiology lasting longer than 6 weeks in patients younger than 16 years. Diagnosis and medical management are complex and best coordinated by a pediatric rheumatologist. The mainstay of therapy is anti-inflammatory and biologic medications to control pain and joint inflammation. Orthopedic surgical treatment may be indicated for deformity, limb length inequality, or end-stage arthritis. Evaluation of the cervical spine and appropriate medication management in consultation with a patient's rheumatolo

Exploring Women's Perspectives on Receiving AI-Enabled Digital Support for Infant Feeding: Multimethods Cross-Sectional Study.

BACKGROUND: Infant feeding practices, including breastfeeding, are known to benefit maternal and child health outcomes. Therefore, parent access to evidence-based infant feeding advice is critical. In recent years, there has been increased use of digital health technologies to support infant feeding. Despite its potential, using AI to complement existing health care and connect families to timely infant feeding support remains relatively unexplored. OBJECTIVE: This study aims to explore women's perceptions of using AI-enabled infant feeding support within mobile health (mHealth) interventions

Maternal and Perinatal Outcomes Associated with Maternal HIV Infection in a Tertiary Hospital in Eastern Cape Province, South Africa.

Background: Despite substantial progress in prevention of mother-to-child transmission (PMTCT) programmes and widespread access to antiretroviral therapy (ART), maternal HIV infection remains associated with adverse maternal and neonatal outcomes in many high HIV-burden settings. This study compared maternal and perinatal outcomes among women living with HIV and HIV-negative women delivering at a tertiary referral hospital in the Eastern Cape Province, South Africa. Methods: A retrospective comparative cohort study was conducted using routinely collected clinical records of 600 women (300 HIV-

Acute Stress and Need for Psychological Follow-Up Care in Parents of Children Treated at the Paediatric Intensive Care Unit.

BACKGROUND: Admission of a child to a paediatric intensive care unit (PICU) is a highly stressful experience for parents and may trigger acute stress disorder (ASD), posing a risk for long-term post-traumatic stress disorder (PTSD) and depression. Despite this vulnerability, structured follow-up care is rarely offered. AIM: This study examined (1) acute stress, post-traumatic adjustment and the perceived need for psychological follow-up care among parents of PICU patients, (2) variables associated with ASD symptoms and the need for follow-up care, and (3) dyadic associations between mothers'

Early care in childhood and psychological burden among East and West German adults.

BACKGROUND: The consequences of external childcare for children are controversially discussed. Many claim that early extrafamilial care is harmful to the child. This article aimed to study the relationship between external childcare at preschool age and psychological burden in adulthood. Given that extrafamilial care followed different norms and regulations depending on the location in East or West Germany during their division, the question was also pursued whether the association between early childcare and psychological burden differed between those regions. METHODS: The analyses are based

Completion and predictors of maternity continuum of care among women in the post-partum period in Gedeb district, southern Ethiopia: A community based cross-sectional study.

INTRODUCTION: Maternal and neonatal complications related to pregnancy and childbirth pose a significant risk of morbidity and mortality to both the mother and the child. Despite its benefits in reducing maternal and neonatal mortality and morbidity associated with pregnancy and childbirth, the majority of Ethiopian mothers were dropped from the maternal continuum of care. Furthermore, there is a dearth of data regarding the status of the maternal continuum of care and its underlying factors in southern Ethiopia. OBJECTIVE: This study aimed to assess the completion of the maternity continuum

Association between antenatal care follow-up and institutional delivery service utilization: analysis of 2016 Ethiopia demographic and health survey.

BACKGROUND: Globally, the magnitude of maternal mortality is the major public health problem. Nearly all (99%) of maternal deaths occur in low- and middle-income countries. Of which 66% occur in sub-Saharan Africa. Institutional delivery under the hygienic environment with the necessary skills and equipment promotes to identify and treat complications, infections, and the death of the mother and baby. In Ethiopia, the utilization of maternal health services is very low. For instance, 62% of women had antenatal care utilization during pregnancy while only 26% of women utilize institutions for d
