Incidence of typhoid bacteremia in infants and young children in southern coastal Pakistan.

INTRODUCTION: The burden of typhoid fever in preschool children is not well recognized. The purpose of this study was to estimate the incidence of typhoid bacteremia in Pakistani children <5 years of age, with a focus on children younger than 2 years of age. This will help to inform prevention policies in highly endemic countries. METHODS: Household surveillance from February 1, 2007 to May 12, 2008, was carried out by community health workers in 2 low-income, coastal communities of Karachi. Workers referred each sick child <5 years old to the local clinic. Blood for culture was obtained from

Psychiatric disorders in children with autism spectrum disorders: prevalence, comorbidity, and associated factors in a population-derived sample.

OBJECTIVE: Autism spectrum disorders are now recognized to occur in up to 1% of the population and to be a major public health concern because of their early onset, lifelong persistence, and high levels of associated impairment. Little is known about the associated psychiatric disorders that may contribute to impairment. We identify the rates and type of psychiatric comorbidity associated with ASDs and explore the associations with variables identified as risk factors for child psychiatric disorders. METHOD: A subgroup of 112 ten- to 14-year old children from a population-derived cohort was a

[Health promotion in the school--fiction or central responsibility of school medicine?].

The causes of the most common diseases are now well known. This knowledge makes it possible to prevent them. Child mortality has been drastically reduced during the present century due to the eradication of infectious diseases. This target was reached by means of general immunization programmes and health education of the entire population. Other diseases were eliminated by changes of nutrition habits (gout) and by active substitution of vitamins (rickets) or by administration of minerals (caries, goitre). Diseases that are responsible for man not attaining his genetic life-span can only be er

Differences in growth trajectories in breastfed HIV-exposed uninfected and HIV-unexposed infants in Kenya: An observational cohort study.

BACKGROUND: Children who are HIV-exposed and uninfected (CHEU) are at increased risk for poor growth compared to children who are HIV-unexposed (CHU). There are limited data on growth among CHEU in the era of preferred dolutegravir-based antiretroviral therapy (ART) for pregnant and breastfeeding women living with HIV (WLWH). We aimed to compare child growth outcomes in the first two years of life between breastfed CHEU and CHU, and to examine maternal HIV factors associated with growth in CHEU. METHODS AND FINDINGS: We enrolled pregnant women in Kenya and followed them with their child to ag

The Initial Creation of a Family Well-being Index Using the National Survey of Children's Health.

OBJECTIVE: The well-being of families is critical for health, but a family well-being metric has not yet been developed for use in federal surveys, making it impossible to assess national trends in family well-being. The National Survey for Children's Health (NSCH) includes items relating to different dimensions of family well-being. The purpose of this study was to conduct exploratory research to create an index of family well-being using items from the NSCH to examine trends in family well-being among families with children in the United States. METHODS: Using the 2016 and 2021 NSCH, we cre

The health of homeless children revisited.

To the extent that representative data are available for specific health conditions (eg, under-immunization, asthma prevalence), the authors' data suggest that the gap between the health status of homeless children and housed children in minority, low-income families is narrowing. Studies of the health status of homeless children allow a window into the health status of medically underserved children whose needs may not be readily documented because of their lack of access to the health care system. Although prevalence rates of most of the health conditions discussed in this article exceeded n

Learning from neurodivergent students about their mental health and wellbeing during transition to secondary school: A systematic review.

The impact of transition from primary to secondary education for neurodivergent children is an area where research, though emerging, has not yet produced consistent or conclusive findings. By adopting a neurodiversity-informed framework, we conducted a systematic review of empirical studies examining the child-reported impact of this transition on psychosocial and mental health outcomes amongst neurodivergent children such as autistic, children with developmental language disorder, and children with Down syndrome (DS). Six databases (PubMed, PsycInfo, Scopus, Web of Science, ASSIA, and Google

Comparison of WHO and Centers for Disease Control and Prevention Growth References for Classifying Height and Weight Status in Young Children in the United States-Affiliated Pacific.

BACKGROUND: Multiple international growth reference systems exist for classifying child undernutrition and excess weight, yet different references yield systematically different prevalence estimates. This has important implications for surveillance in Pacific Islander populations, where both the Centers for Disease Control and Prevention (CDC) and WHO references are used across jurisdictions. OBJECTIVES: This study aimed to quantify classification discrepancies between WHO Growth Standards (ages 2-5 y), WHO Growth Reference (ages 6-8 y), and CDC Growth Charts (ages 2-8 y) in Pacific Islander

The ChUSE trial - improving access to psychological therapies for children with distressing sensory experiences in NHS CAMHS: a randomised controlled feasibility trial protocol.

INTRODUCTION: Distressing sensory experiences (DSEs) are commonly reported by children and young people accessing Child and Adolescent Mental Health Services (CAMHS) in the UK. Despite their prevalence, evidence-based tailored interventions for this population are scarce. The ChUSE trial aims to address this gap by evaluating the feasibility and acceptability of a novel brief talking therapy for children aged 8-16 years and an accompanying parent coaching programme. METHODS AND ANALYSIS: A single-blind, two-arm randomised controlled feasibility trial will be conducted across three National He

Applying a human-centered design approach to increase immunization coverage and reduce the number of zero-dose children in Tshopo Province, Democratic Republic of the Congo: A costing analysis.

The Democratic Republic of the Congo (DRC) is among the countries with the highest number of zero-dose and under-immunized children in the world. Tshopo Province, located in the North-Central area of the country, is particularly impacted, with alarmingly low immunization rates and a proportion of zero-dose children higher than 60%. Innovative and targeted interventions are required to reach children missed by the immunization program. In collaboration with the DRC Ministry of Health, PATH implemented a human-centered design (HCD) project to co-create and test solutions that could help the comm

Germline duplication of MYCN predisposes to childhood embryonal tumours.

BACKGROUND: Neuroblastoma and Wilms tumour (WT) are common childhood embryonal malignancies. Germline 2p24 duplication has been reported in several cases of neuroblastoma and WT, either as part of a larger 2p duplication or as a microduplication involving just 2p24.3. Although the larger duplications involve many genes, including ALK, the microduplications have been localised to a region including MYCN and DDX1. METHODS: We analysed Whole Genome Sequence data from adults and children sequenced for various indications. We utilised a workflow to extract structural and copy number variants, filt

Sex differences in internalising and externalising symptom patterns, pathway and persistence in adolescents receiving psychiatric care: a 2-year follow-up of the MILESTONE European cohort.

BACKGROUND: Sex differences in adolescent mental disorders are well documented, but less is known about how these differences evolve during the transition to adulthood, particularly during the shift from Child and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS), a critical period marked by vulnerability to discontinuity of care. OBJECTIVE: To examine sex-specific differences in psychiatric symptom profiles and clinical trajectories during the CAMHS-AMHS transition using data from the European MILESTONE project, a 2-year longitudinal study. METHODS: A cohort o

Maternal and early-life longitudinal cytokine profiles in a South African birth cohort: the impact of HIV.

INTRODUCTION: During pregnancy, exposure to maternal HIV and a disrupted cytokine environment may impact foetal immune development and health outcomes through cytokine-mediated mechanisms. We evaluated (i) peripheral blood cytokine differences in pregnant women with and without HIV, (ii) longitudinal trajectories in HIV-exposed uninfected (HEU) and HIV-unexposed uninfected (HUU) children, and (iii) cytokine groupings. Additionally, we explored the impact of the timing of maternal antiretroviral treatment (ART) initiation on cytokine levels and if cytokines predicted repeated hospitalisations d

Social capital and uptake of primary healthcare during protracted displacement: Evidence from four refugee settlements in northern Uganda.

The world's refugee populations primarily reside in low- and middle-income countries, where they face protracted displacement and limited financial and human capital. Social capital is among the few assets they can leverage for resilience. Despite its importance, few studies have examined how social capital influences the uptake of healthcare services during protracted displacement. This study examines the associations of refugee with access to maternal, newborn and child health (MNCH) services in northern Uganda's long-term settlements. We conducted cross-sectional probability household surve
