Pelvic inflammatory disease and its sequelae in adolescents.

Pelvic Inflammatory Disease (PID) is the most common serious complication of sexually transmitted diseases (STDs). Each year over one million women in the United States experience an episode of PID, with approximately 16-20% of cases occurring in teenagers. Acute PID increases a woman's risk for recurrent PID, chronic pelvic pain, infertility, and ectopic pregnancy. Recent reports indicating that PID rates are rising and are highest among adolescent females aged 15-19 underscore the need to remain current on the clinical as well as the epidemiologic aspects of PID. We present such an update in

Follicular proctocolitis and neuromatous hyperplasia with lymphogranuloma venereum.

Lymphogranuloma venereum (LGV), a sexually transmitted disease, is caused by certain immunotypes of Chlamydia trachomatis. Proctitis due to LGV may be histologically indistinguishable from Crohn's disease of the rectum, thereby creating a diagnostic and therapeutic dilemma. The pathologic features found at autopsy at The Johns Hopkins Hospital in the 28 patients in whom LGV had been diagnosed clinically were reviewed, with the clinical features, to determine whether any of those features could be used to distinguish between LGV and Crohn's colitis. The results showed that although many of the

Harnessing the Vaginal Microbiome: A New Frontier in the Prevention of Sexually Transmitted Infections.

BACKGROUND: The vaginal microbiome plays a vital role in maintaining reproductive and urogenital health in females. In healthy women, it is dominated by Lactobacillus species, including L. crispatus, L. gasseri, L. iners, and L. jensenii. These species provide strong protection against sexually transmitted infections (STIs) and other gynecological diseases. They produce lactic acid, which helps maintain a low vaginal pH, supporting epithelial integrity, modulating the immune system, and preventing colonization by bacterial pathogens. Dysbiosis, or an imbalance in microbial communities, is link

First detection of JC polyomavirus in vaginal secretions from nonpregnant women: Exploring sexual transmission as a potential route of infection.

OBJECTIVES: JC polyomavirus (JCPyV) is a ubiquitous human virus responsible for asymptomatic lifelong persistent infection. Profound immunosuppression, or prolonged treatment with specific immunomodulatory molecules, may result in the development of Progressive Multifocal Leukoencephalopathy (PML). The main mode of JCPyV transmission remains uncertain. This study aimed to evaluate the presence of JCPyV DNA in vaginal secretions to encourage further research on sexual transmission. METHODS: JCPyV DNA presence was assessed in 293 vaginal swabs of sexually active nonpregnant women through real-t

Comparative analysis of the vaginal bacteriome and virome in healthy women living in high-altitude and sea-level areas.

The vaginal microbiota plays an important role in the health of the female reproductive tract and is closely associated with various pregnancy outcomes and sexually transmitted diseases. Plenty of internal and external factors have strong influence on the changes in a woman's vaginal microbiome. However, the effect of a high-altitude on female vaginal microbiota has not been described. In this study, we characterized the vaginal bacteriome and virome of 13 and 34 healthy women living in high-altitude and sea-level areas, using whole-metagenome shotgun sequencing of their vaginal mucus samples.

Does Lactobacillus Exert a Protective Effect on the Development of Cervical and Endometrial Cancer in Women?

Cervical cancer is a significant health problem with increasing occurrence and mortality. This infection-associated tumour is caused by the human papillomavirus (HPV). HPV infection is cleared by the immune system within 6-18 months in most patients; however, persistent high-risk HPV (hrHPV) infections can lead to the development of cervical cancer. Virus persistence is promoted by immunodeficiency, Chlamydia trachomatis infection, smoking, and age, as well as the imbalance of cervicovaginal microbiota and inflammation. The abundance of bacteria in the vagina favours the maintenance of a dynam

The predictive value of macaque models of preexposure prophylaxis for HIV prevention.

PURPOSE OF REVIEW: We review macaque models for preexposure prophylaxis (PrEP) for HIV prevention and highlight their role in advancing currently approved and novel PrEP agents. RECENT FINDINGS: The development of the repeat low dose simian HIV (SHIV) challenge models represented a significant advancement in preclinical PrEP modeling that has allowed the investigation of PrEP under conditions that better mimic HIV exposures in humans. These models incorporate relevant drug pharmacology to inform drug correlates of PrEP protection. Models of rectal, vaginal, and penile infection are now availa

Sarcoidosis of the spermatic cord - case report and literature review.

BACKGROUND: Sarcoidosis is a multi-system disease characterized by the formation of non-caseating granulomas in various organs. The lungs remain the most frequently affected organ, whereas lesions in the genitourinary system affect around 0.2% of patients. The primary site found in the spermatic cord is extremely rare. CASE PRESENTATION: We present a patient's case where the spermatic cord involvement was the first manifestation of sarcoidosis. For several months, a number of tests had been performed, which showed, among others, non-caseating granulomas in pathomorphological material, bilater

Age- and Sex-Related Differences in Morbidities of Sexually Transmitted Diseases in Children.

Sexually transmitted diseases (STDs) are causes of public health burden globally. The purpose of this study is to document age-specific and sex-related changes in the morbidity of four representative STDs in children. Japanese national surveillance data from 1999 to 2017 on morbidities of the following four STDs were analyzed by age and sex: Neisseria gonorrhoeae (NG), Chlamydia trachomatis (CT), condylomata acuminate (CA), and genital herpes simplex virus (GHSV). The morbidities of males and females in each age group were compared through the male-to-female morbidity (MFM) ratios. The MFM rat

The microbiome of HPV-positive tonsil squamous cell carcinoma and neck metastasis.

BACKGROUND: Oropharyngeal squamous cell carcinoma (OPSCC) has now surpassed cervical cancer as the most common site of HPV-related cancer in the United States. HPV-positive OPSCCs behave differently from HPV-negative tumors and often present with early lymph node involvement. The bacterial microbiome of HPV-associated OPSCC may contribute to carcinogenesis, and certain bacteria may influence the spread of cancer from the primary site to regional lymphatics. OBJECTIVE: To determine the bacterial microbiome in patients with HPV-associated, early tonsil SCC and compare them to benign tonsil spec

High Willingness to Participate in Partner Notification among Women Attending Reproductive Health and STI Clinics in Shenzhen, China: A Cross-Sectional Study.

Genital Chlamydia trachomatis (CT) is one of the most common sexually transmitted infections (STI) worldwide. We explored the factors associated with willingness to participate in partner notification (PN) among women attending reproductive health and STI clinics in Shenzhen, China. An anonymous questionnaire was used to collect the sociodemographic characteristics, STI histories, and willingness to participate in routine CT screening and partner notification. In total, 87.31% (n = 10,780) of participants were willing to notify their sex partner(s) if they were diagnosed with a CT infection. W

Sensitive Detection of Nucleic Acids Using Subzyme Feedback Cascades.

The development of Subzymes demonstrates how the catalytic activity of DNAzymes can be controlled for detecting nucleic acids; however, Subzymes alone lack the sensitivity required to detect low target concentrations. To improve sensitivity, we developed a feedback system using a pair of cross-catalytic Subzymes. These were individually tethered to microparticles (MP) and separated by a porous membrane rendering them unable to interact. In the presence of a target, active PlexZymes® cleave a first Subzyme, which separates a first DNAzyme from its MP, allowing the DNAzyme to migrate through the

Sexually transmitted infections in the military environment.

Sexually transmitted infections (STIs) have always been a major health issue affecting military personnel in all types of services and in all armed forces around the world, especially during deployments and in operational settings. Although the research shows that STIs are still reported in the military, the epidemiological risk for contracting a sexually transmitted infection is much lower nowadays than it was in the past. It is important, however, that service members are routinely screened for sexually transmitted diseases. Because of a high prevalence of STIs in the general population as w

Endometriosis III and IV as a risk factor for tubal obstruction in infertile women.

OBJECTIVE: A previous study carried out among infertile women with tubal obstruction identified a relative risk of 2.5 for Chlamydia trachomatis seropositivity. However, endometriosis may also be associated with increased risk. This study aimed to evaluate the risk of tubal obstruction associated with endometriosis III/IV among women submitted to assisted reproductive procedures. METHODS: A case-control study was performed among 144 women with and without tubal obstruction. We calculated the odds ratio with 95% CI regarding the association of endometriosis III/IV and tubal obstruction. Calcul

Chlamydia in adolescent/adult reproductive management trial study (CHARM): Clinical core protocol.

INTRODUCTION: Chlamydia trachomatis (CT) is a common sexually transmitted pathogen with significant reproductive health implications. Data are mounting that the bacterial communities that reside within the vagina, collectively termed the vaginal microbiota, aid in defense against sexually transmitted infections. Disruptions in the vaginal microbiota, such as during episodes of bacterial vaginosis, may increase susceptibility to infection. Herein, we describe the clinical core protocol for a NIH NIAID Cooperative Research Center titled Eco-Pathogenomic of Chlamydial Reproductive Tract Infection

Prevalence and impact of sexually transmitted infections in pregnant women in central Ethiopia.

Human immunodeficiency virus (HIV) continues to be a major global public health issue and omnipresent sexually transmitted infections (STIs) increase the risk of HIV acquisition. Moreover, STIs and HIV in pregnant women can harm the unborn child. In this study, we systematically investigated the prevalence of HIV, relevant STIs and vaginal group B streptococcus colonization among pregnant women presenting at Asella Teaching Hospital in central Ethiopia and their effect on perinatal mortality. A follow-up was performed six weeks after delivery. A total of 580 women were included, of which 26.6%

Mycoplasma genitalium macrolide resistance in Stockholm, Sweden.

OBJECTIVES: Mycoplasma genitalium is an important cause of STI in men and women. Worldwide evidence suggests a reduction in efficacy of azithromycin treatment due to the prevalence of macrolide resistant M. genitalium. The aim of this study was to describe the prevalence of macrolide resistance in patients with a positive test for M. genitalium within our setting. METHODS: Two STI clinics in Stockholm offered tests for M. genitalium as part of a routine care pathway. Positive specimens were analysed for macrolide resistance mediating mutations by sequencing. RESULTS: During the study period,
