Localized Cutaneous Nodular Amyloidosis in a Patient with Sjögren's Syndrome.

Primary localized cutaneous nodular amyloidosis (PLCNA) is included in the primary forms of cutaneous amyloidosis along with macular and lichenoid amyloidosis. It is a rare disease attributed to plasma cell proliferation and deposition of immunoglobulin light chains in the skin. We present the case of a 75-year-old woman with a personal history of Sjogren's syndrome (SjS), who consulted for asymptomatic yellowish, waxy nodules on the left leg. Dermoscopy of the lesions showed a smooth, structureless, yellowish surface with hemorrhagic areas and few telangiectatic vessels. Histopathology reveal

Targeting spectrin redox switches to regulate the mechanoproperties of red blood cells.

The mechanical properties of red blood cells (RBCs) are fundamental for their physiological role as gas transporters. RBC flexibility and elasticity allow them to survive the hemodynamic changes in the different regions of the vascular tree, to dynamically contribute to the flow thereby decreasing vascular resistance, and to deform during the passage through narrower vessels. RBC mechanoproperties are conferred mainly by the structural characteristics of their cytoskeleton, which consists predominantly of a spectrin scaffold connected to the membrane via nodes of actin, ankyrin and adducin. The role of mechanical stimulation in the enhancement of bone healing.

The biomechanical environment plays a dominant role in the process of fracture repair. Mechanical signals control biological activities at the fracture site, regulate the formation and proliferation of different cell types, and are responsible for the formation of connective tissues and the consolidation of the fractured bone. The mechanobiology at the fracture site can be easily manipulated by the design and configuration of the fracture fixation construct and by the loading of the extremity (weight-bearing prescription). Depending on the choice of fracture fixation, the healing response can

Systemic sclerosis.

Systemic sclerosis is a complex autoimmune connective tissue disease which carries a significant burden of disease-related morbidity including potentially life-threatening complications. Systemic sclerosis can affect all the major organs and therefore, although the disease is uncommon, many hospital-based specialists are involved in patient care. Vascular disease (e.g. Raynaud's phenomenon) is an almost universal symptom in patients with systemic sclerosis and is often the earliest manifestation of the disease. Systemic sclerosis not uncommonly can overlap with other rheumatological conditions

Rapamycin for refractory discoid lupus erythematosus.

Generalized discoid lupus erythematosus can pose a therapeutic challenge for dermatologists. Current treatment emphasizes photoprotection, topical and systemic steroids, and steroid-sparing immunosuppressive agents if necessary. Rapamycin, also known as sirolimus, selectively inhibits mammalian target of rapamycin, a regulatory kinase responsible for multiple signal transduction pathways. Mammalian target of rapamycin inhibition reduces cell division, lymphocyte proliferation, cytokine release, and downstream pathways unique from other classes of immunomodulatory drugs. Herein, we present a ca

Systemic Light Chain Amyloidosis Mimicking Rheumatic Disorders.

Secondary amyloidosis can complicate chronic inflammatory autoimmune diseases. However, the clinical findings of primary amyloidosis may mimic those of primary rheumatologic disorders. We present the case of a 53-year-old woman who presented with dystrophic nail changes, dry eyes, bilateral carpal tunnel syndrome, Raynaud's phenomenon, and high titer positive nucleolar pattern antinuclear antibody. She was initially misdiagnosed as having Undifferentiated Connective Tissue Disease (UCTD). On further workup, she was eventually diagnosed with lambda light chain systemic amyloidosis by abdominal

A new mechanochemical model: coupled Ginzburg-Landau and Swift-Hohenberg equations in biological patterns of marine animals.

In this work the skin coating of some vertebrate marine animals is modeled considering only dermis, epidermis and basal layers. The biological process takes into account: cellular diffusion of the epidermis, diffusion inhibition and long-range spatial interaction (nonlocal effect on diffusive dispersal) for cells of dermal tissue. The chemical and physical interactions between dermis and epidermis are represented by coupling quadratic terms and nonlinear terms additional. The model presents an interesting property associated with their gradient form: a connection between some physical, chemica

Pulmonary hypertension in chronic interstitial lung diseases.

Pulmonary hypertension (PH) is a common complication of interstitial lung diseases (ILDs), particularly in idiopathic pulmonary fibrosis and ILD associated with connective tissue disease. However, other lung diseases, such as combined pulmonary fibrosis and emphysema syndrome, pulmonary Langerhans cell histiocytosis, and lymphangioleiomyomatosis, may also include PH in their clinical manifestations. In all of these diseases, PH is associated with reduced exercise capacity and poor prognosis. The degree of PH in ILDs is typically mild-to-moderate. However, some of these patients may develop a d

XPD mutations in trichothiodystrophy hamper collagen VI expression and reveal a role of TFIIH in transcription derepression.

Mutations in the XPD subunit of the transcription/DNA repair factor (TFIIH) give rise to trichothiodystrophy (TTD), a rare hereditary multisystem disorder with skin abnormalities. Here, we show that TTD primary dermal fibroblasts contain low amounts of collagen type VI alpha1 subunit (COL6A1), a fundamental component of soft connective tissues. We demonstrate that COL6A1 expression is downregulated by the sterol regulatory element-binding protein-1 (SREBP-1) whose removal from the promoter is a key step in COL6A1 transcription upregulation in response to cell confluence. We provide evidence fo

18F-FDG-PET imaging of a benign neurofibroma masquerading as metastatic oesophageal carcinoma.

We present the case of a 65-year-old male with squamous cell carcinoma of the thoracic oesophagus on a background of neurofibromatosis type 1. On computed tomography, he was noted to have a large left-sided superior mediastinal mass. Initially, this mass was thought to be metastatic lymphadenopathy; however, it did not display fluorodeoxyglucose uptake on positron emission tomography. Subsequent biopsy confirmed the mass to be a neurofibroma and the patient commenced definitive chemo-radiation. Positron emission tomography had a major impact on management since the presumed lymph node disease

Connective tissue growth factor (CTGF) expression and outcome in adult patients with acute lymphoblastic leukemia.

We compared the gene expression profile of adult acute lymphoblastic leukemia (ALL) to normal hematopoietic and non-ALL samples using oligonucleotide arrays. Connective tissue growth factor (CTGF) was the highest overexpressed gene in B-cell ALL compared with the other groups, and displayed heterogeneous expression, suggesting it might have prognostic relevance. CTGF expression was examined by quantitative reverse transcriptase-polymerase chain reaction (ORT-PCR) on 79 adult ALL specimens. CTGF expression levels were significantly increased in ALL cases with B-lineage (P < .001), unfavorable c

Marjolin's ulcer revisited--basal cell carcinoma arising from grenade fragments? Case report and review of the literature.

BACKGROUND: Marjolin's ulcer is a rare and often aggressive cutaneous malignancy arising in previously traumatized or chronically inflamed skin. CASE REPORT: A 79-year-old World War II veteran developed basal cell carcinoma (BCC) at the site of a war wound. The tumour developed in relation to several metal grenade fragments. With a disease-free interval of 61 years between injury and onset of complications the patient had one of the longest latency periods of tumour development described so far. RESULTS: Review of the literature reveals only five cases of relation between grenade fragments a

Effects of quercetin on liver damage in rats with carbon tetrachloride-induced cirrhosis.

Flavonoids are reported to exhibit a wide variety of biological effects, including antioxidant and free radical-scavenging activities. Evidence of oxidative reactions is often associated with various chronic disease processes characterized by accumulation of connective tissue. This study was aimed to investigate the protective effects of chronic administration of the flavonoid quercetin (150 micromol/kg body wt/day intraperitoneally) in rats with carbon tetrachloride-induced fibrosis. In animals rendered cirrhotic by administration of carbon tetrachloride for 16 weeks, cell necrosis, fibrosis,

Cyr61 and CTGF are molecular markers of bladder wall remodeling after outlet obstruction.

Cysteine-rich protein (Cyr61) and connective tissue growth factor (CTGF) are key immediate early growth factors with functions in cell proliferation, differentiation, and extracellular matrix synthesis. Studies were performed to assess the gene expression profile of Cyr61 and CTGF in rat urinary bladder during growth in response to partial outlet obstruction. The mRNA levels of Cyr61 as determined by ribonuclease protection assay increased sharply after 1 day and remained elevated throughout the time period of the obstruction. This correlates well with increased bladder weight. The CTGF mRNA l

The use of stress proteins as a biomarker of sub-lethal toxicity: induction of heat shock protein 70 by 2-isobutyl piperidine and transition metals at sub-lethal concentrations.

The stress response is a highly conserved reaction to various physical, chemical and biological stimuli. The ubiquity of the response occurring across taxonomic classes has identified heat shock proteins as potential biomarkers. In this study using the neutral red assay, silver stained one-dimensional SDS-PAGE, Western blotting and ELISA, the use of heat shock proteins as biomarkers of sub-lethal toxicity was examined. Hsp70 was induced in the mouse connective tissue cell line (L929) at sub-lethal concentrations for three transition metals (cadmium, mercury and copper) and for 2-isobutyl piper

Multifocal osteonecrosis following chemotherapy and short-term corticosteroid therapy in a patient with small-cell bronchogenic carcinoma.

This report describes the development of multiple-site, biopsy-proven osteonecrosis in a patient with small-cell bronchogenic carcinoma who had received chemotherapy and short-term administration of corticosteroid. Multifocal osteonecrosis has a wide variety of etiologies, but is most often encountered in the clinical setting of corticosteroid administration, connective tissue disorders, transplantation, hemoglobinopathies and dysbarism. In the oncology patient, chemotherapy, corticosteroids and bone marrow transplantation (with associated preparation therapy) have all been implicated as possi

The Immunometabolic Bridge: Targeting Metainflammation to Prevent the Transition from Obesity to Cardiovascular Disease.

Obesity and cardiovascular disease are linked by more than excess adiposity. They are connected through a chronic, low-grade inflammatory state that determines who progresses from risk to overt disease. We propose that metainflammation, arising from adipose tissue dysfunction and metabolic surplus, constitutes a key immunometabolic bridge between preclinical obesity and clinical cardiovascular disease. A central tenet of this framework is that metainflammation acts as a contributory driver of this transition; however, its pathogenic potential is realized only within a permissive metabolic envi

Integrative Transcriptional and Chromatin Analyses Reveal Enhancer-Mediated Regulatory Programs Driving Breast Cancer Metastasis.

Breast cancer metastasis remains the leading cause of disease-related mortality, yet the regulatory mechanisms enabling tumor cells to adapt to distant tissue environments remain poorly defined. Here, we generated a multi-omic resource integrating transcriptomic (RNA-seq) and chromatin accessibility (ATAC-seq) profiles from estrogen receptor-positive primary breast tumors and matched liver and lung metastases obtained from the same patients, including diagnostic and postmortem specimens. This design enabled direct intra-patient comparisons of primary and metastatic regulatory landscapes. We id

Endoplasmic reticulum stress and maladaptive repair in fibrotic interstitial lung diseases: A human-evidence-based narrative review.

Fibrotic interstitial lung diseases (ILDs) are characterized by progressive extracellular-matrix accumulation but arise from biologically diverse injuries. This narrative review examines endoplasmic reticulum (ER) stress as a context-dependent regulator of maladaptive repair, prioritizing evidence from human genetics, tissue, single-cell and spatial studies, patient-derived systems, biomarkers, and clinical trials. In selected familial surfactant disorders, pathogenic variants provide direct evidence that impaired proteostasis can cause fibrotic lung disease. In sporadic idiopathic pulmonary f
