Diagnostic and management difficulties in a case of multiple intracranial juvenile xanthogranuloma.

INTRODUCTION: Juvenile xanthogranuloma (JXG) preferentially occurs in childhood, and its characteristics have been thought to be benign and with slow growth. JXG is classified as an inflammatory disease, which forms multiple lesions in the patients' head, neck, and other organs and typically skin. JXG is rare, and few case reports have been published in the past, particularly with multiple intracranial lesions, which as in our case, is an extremely rare finding. CASE: Patient is a 4-year-old boy who presented with polydypsia and polyuria for 1 year. He had been followed up only under l-deamin

Life-threatening acute subdural haematoma after combined spinal-epidural anaesthesia in labour.

BACKGROUND AND OBJECTIVES: Only few reports in literature have pointed out to the possibility of a cranial subdural haematoma formation associated with dural puncture during spinal or epidural analgesia. We herein describe such a rare case who was diagnosed to have acute subdural haematoma after combined spinal-epidural anaesthesia used in labour. CASE REPORT: A 34-year-old, primigravid women with a gestation of 38 weeks underwent caesarean section under combined spinal-epidural anaesthesia and gave birth to a healthy boy. Thirty-two hours after delivery, her moderate headache progressed to a

Simpson grade: an opportunity to reassess the need for complete resection of meningiomas.

BACKGROUND: The relevance of the Simpson grading system as a predictor of meningioma progression or recurrence in modern neurosurgical practice has recently been called into question. The aim of our study was to compare the risk of progression/recurrence of tumours that had been treated with different Simpson grade resections in a contemporary population of benign (WHO grade I) meningioma patients. METHOD: One hundred eighty-three patients with histologically confirmed WHO grade I meningioma were retrospectively analysed. All patients underwent first-time craniotomy as their initial therapy b

Growing skull fracture stages and treatment strategy.

OBJECT: A growing skull fracture (GSF) is a rare but significant late complication of skull fractures, usually occurring during infancy and early childhood. Delayed diagnosis and improper treatment could exacerbate this disease. The aim of this study was to introduce a new hypothesis about, describe the stages of, and discuss the treatment strategy for GSF. METHODS: The authors performed a retrospective review of 27 patients with GSF, who were grouped according to 3 different GSF stages. RESULTS: Over a period of 20 years, 27 patients with GSF (16 males and 11 females) were treated in the au

Stereotactic radiosurgery to treat presumed Rathke's cleft cysts.

OBJECTIVE: Stereotactic radiosurgery (SRS) has been developed as a clinical treatment method for certain brain tumours that does not need craniotomy. As a more accurate radiation technique, SRS can deliver a relatively large dose of stereotactic radiation to a conformal target with less normal brain tissue irradiated in a single treatment. The aim of this study is to evaluate the therapeutic effects of SRS for Rathke's cleft cysts (RCCs). METHODS: SRS was performed using a rotating gamma ray unit in seven selected patients with symptomatic RCCs diagnosed upon typical MR imaging combined with

Pearl in interhemispheric fissure: A rare phenomenon.

Epidermoid cysts (Keratin pearls) are benign congenital lesions, found commonly in cerebello-pontine angle, suprasellar cistern, sylvian cistern, pineal region, but they are very rare in interhemispheric fissure. Approaching these lesions are challenging to neurosurgeons because of narrow and deep fissure with surrounding vital structures. The present study constitutes an analysis of interhemispheric epidermoid managed at our hospital in last 10 years (Jan 2001-Dec 2010). Total 187 cases of intracranial epidermoid operated in our institute; eight of them were interhemispheric epidermoid making

Recurrence of a cerebral arteriovenous malformation following complete surgical resection: A case report and review of the literature.

BACKGROUND: Angiography-confirmed complete resection of an arteriovenous malformation (AVM) has traditionally been considered curative. However, recurrence of AVM following angiographically proven complete resection does exist, especially in children. This rare occurrence has been reported 29 times in the English language literature. Although recurrence may be asymptomatic, many reported cases result in epilepsy or intracranial hemorrhage anywhere from 0.5 to 9 years following complete resection. We report a rare case of AVM recurrence that became symptomatic 16 years after complete resection.

Craniofacial resection of midline anterior skull base malignancies: a reassessment of outcomes in the modern era.

OBJECTIVE: Craniofacial resection has been considered the gold standard in the management of malignancies involving the anterior skull base, where the goal of surgery is negative margins with minimal morbidity. In recent years, there has been growing enthusiasm for purely endoscopic techniques for craniofacial malignancies. Given recent advancements in open surgical approaches, there is a need to review the technique of open craniofacial resection in the modern surgical era. METHODS: We review our experience of open craniofacial resection of midline anterior cranial fossa malignancies in the

Distress in patients with newly diagnosed brain tumours.

OBJECTIVE: Patients with intracranial tumours often suffer from clinically relevant psychological distress. However, levels of distress and contributing factors have not been systematically evaluated for the early course of the disease. Using the National Comprehensive Cancer Network's Distress Thermometer (DT), we evaluated the extent and sources of distress within a population of patients with intracranial neoplasms. METHODS: One hundred and fifty-nine patients were included who underwent craniotomy for newly diagnosed intracranial tumours at our department. All patients completed the DT qu

Microvascular free tissue transfer in the reconstruction of scalp and lateral temporal bone defects.

OBJECTIVES: Defects of the scalp and lateral temporal bone (LTB) represent a unique challenge to the reconstructive surgeon. Simple reconstructive methods such as skin grafts, locoregional flaps, or tissue expanders are often not feasible owing to a myriad of reasons. Vascularized free tissue transfer coverage offers distinct advantages in managing these defects. MATERIALS AND METHODS: A retrospective case series was performed on all patients at the University of Washington Medical Center who had scalp or LTB defects reconstructed with free tissue transfer from May 1996 to July 2009. Cases we

Motor cortex electrical stimulation applied to patients with complex regional pain syndrome.

Motor cortex stimulation (MCS) is useful to treat patients with neuropathic pain syndromes, unresponsive to medical treatment. Complex regional pain syndrome (CRPS) is a segmentary disease treated successfully by spinal cord stimulation (SCS). However, CRPS often affects large body segments difficult to cover by SCS. This study analyzed the MCS efficacy in patients with CRPS affecting them. Five patients with CRPS of different etiologies underwent a small craniotomy for unilateral 20-grid-contact implantation on MC, guided by craniometric landmarks. Neurophysiological and clinical tests were p

Upper extremity amputations after motor vehicle rollovers.

BACKGROUND: The upper extremity is vulnerable to injury during a rollover motor vehicle crash (MVC). There is some concern that positioning one's arm on a vehicle door/window eliminates the benefit of maintaining containment within a protective structure. Mangled extremities with associated vascular injuries have an amputation rate exceeding 40%. The primary goal was to describe the care process and outcome of patients requiring an emergent upper extremity amputation after a rollover MVC. METHODS: All patients requiring an upper extremity amputation after a rollover MVC (2000-2008) were inclu
