Alcohol and Substance Use After Bariatric Surgery: Nutritional Risks and Clinical Implications in Long-Term Postoperative Care.

Metabolic and bariatric surgery (MBS) has evolved into a highly effective neurohormonal intervention for severe obesity; however, it introduces unique long-term vulnerabilities, particularly regarding alcohol (AUD) and substance use disorders (SUD). This review synthesizes the epidemiological, pharmacokinetic, and neurobiological drivers of postoperative substance misuse. Procedures like Roux-en-Y gastric bypass (RYGB) radically alter ethanol metabolism, eliminating first-pass metabolism and accelerating gastric emptying, while simultaneously recalibrating reward pathways, creating a "reward g

Methylparaben promotes colorectal tumor growth through nuclear receptor-mediated transcriptional reprogramming and inflammatory signaling: Evidence from integrated transcriptomics and network toxicology.

Methylparaben (MP) is a widely used preservative with potential endocrine-disrupting activity, but its role in colorectal cancer (CRC) remains unclear. This study integrated in vitro and in vivo experiments with transcriptomics, network toxicology, and molecular simulations to investigate MP's effects on CRC progression. MP enhanced CRC cell proliferation, migration, and epithelial-mesenchymal transition while suppressing apoptosis in vitro. In CT26 tumor-bearing mice, dietary MP accelerated tumor growth and induced a pro-inflammatory microenvironment. Transcriptomic and network toxicology ana

SmCPK20 phosphorylates SmWRKY2 to regulate jasmonate-mediated tanshinone synthesis in Salvia miltiorrhiza.

Jasmonates (JAs) are vital to plant defense and stress adaptation. In Salvia miltiorrhiza, JA significantly promotes tanshinone accumulation, a class of bioactive diterpenoids. Although transcriptional regulation of JA signaling is well characterized, the post-translational mechanisms governing JA-mediated tanshinone biosynthesis remain poorly understood. Here, we identify SmWRKY2 as a critical regulator of JA-induced tanshinone accumulation, which interacts with SmCPK20. SmCPK20 positively regulates JA-induced tanshinone accumulation. JA signaling activates SmCPK20, promoting phosphorylation

Tumour endothelial cell reprogramming orchestrates angiocrine signalling to drive chemoresistance in breast cancer.

Despite its established role in breast cancer treatment, Doxorubicin treatment remains subject to adaptive resistance mechanisms that extend beyond cancer cell intrinsic alterations ultimately reducing therapy efficacy. Our study in a MMTV-PyMT-driven mouse breast cancer model reveals that prolonged Doxorubicin (Dox) exposure triggers significant reprogramming of the tumour vasculature, substantially altering the angiocrine landscape and shaping treatment outcomes. Notably, tumours that initially respond, but later revert, display an endothelial cell subclustering with activation of proliferat

Complex hand wounds and pressure garments: learning from an iatrogenic injury case study.

This article examines issues in the management of hypertrophic scarring following plastic and reconstructive surgery for an iatrogenic extravasation injury to the hand, requiring a made-to-measure glove (pressure garment). A detailed case review was undertaken alongside a critical evaluation of clinical practice, national guidelines, and published evidence. Delayed or inappropriate access to customised garments can compromise scar outcomes, limit functional recovery, and negatively affect patient wellbeing. Wider consequences include long-term physical, psychological, and social effects, as we

From Genes to Lives: Integrating the Complexities of Primary Ovarian Insufficiency.

Primary ovarian insufficiency (POI) affects up to 3% of reproductive-aged women and is a critical yet underrecognized contributor to infertility and systemic accelerated aging. While most cases remain idiopathic, advances in genomics increasingly reveal a genetic basis, implicating pathways that govern DNA repair, meiosis, chromosomal stability, and folliculogenesis. This review synthesizes the multifactorial etiology of POI, integrating genetic contributions with emerging evidence on epigenetic dysregulation, mitochondrial dysfunction, and environmental influences such as toxins and lifestyle

A bioenergetic understanding of "aerobic glycolysis".

"Aerobic glycolysis" is a widely used term whose current meaning has drifted from its original usage in a way that has created confusion and inaccuracy. This drift has weakened "aerobic glycolysis" as a hypothesis-testing framework, despite the critical importance of glycolysis in understanding cellular bioenergetic behavior. Here, we examine the historical and contemporary uses of "aerobic glycolysis" and the related "Warburg effect". We argue that "aerobic glycolysis" as originally investigated was essentially a bioenergetic phenomenon. We review the bioenergetic model of glycolysis and mito

The dual role of TRPC channel-mediated dysregulation of calcium signaling in regulating mitophagy in early brain injury after subarachnoid hemorrhage.

Subarachnoid hemorrhage (SAH) induces early brain injury (EBI) through mechanisms involving mitochondrial dysfunction and dysregulated calcium signaling. Transient receptor potential canonical (TRPC) channels are critical mediators of calcium homeostasis and have emerged as key players in SAH pathophysiology. This review explores the interconnected roles of TRPC channel-mediated calcium dyshomeostasis and mitophagy in EBI. We summarize how mitochondrial damage post-SAH triggers mitophagy via ubiquitin-dependent and -independent pathways, a process with dual effects on neuronal survival. We the

Myelin Alteration in Neurodevelopmental Disorders: Integrating Clinical and Preclinical Evidence.

Myelination is a fundamental neurobiological process that enables rapid and efficient neural signal transmission, playing a key role in shaping brain connectivity and function-particularly during early development. Given the high energetic cost and tight developmental timing of myelination, disruptions in myelin formation or maintenance can increase vulnerability to genetic and environmental factors during critical periods. As such, impaired myelination and oligodendrocyte dysfunction may contribute to the pathogenesis of various neurodevelopmental disorders (NDDs). This review synthesizes cur

Mitochondrial Resilience: Unraveling the Triadic Interplay of Phosphocreatine, Cyclophilin D, and STAT3 in Heart Failure.

Heart failure remains a major global health burden, with mitochondrial dysfunction recognized as a key contributor to its onset and progression. This review highlights three critical regulators of mitochondrial integrity phosphocreatine (PCr), cyclophilin D (CypD), and signal transducer and activator of transcription 3 (STAT3) and their coordinated roles in cardiac function. PCr is vital for sustaining myocardial energy balance, particularly under metabolic stress. CypD controls the mitochondrial permeability transition pore, regulating cell death pathways that contribute to cardiac injury. Be

Tumour-associated dsRNA accumulation and innate immune activation: mechanistic basis and therapeutic strategies.

Endogenous double-stranded RNA (dsRNA) links disrupted RNA homoeostasis in cancer to innate immunity. Tumour-associated dsRNA arises from complementary repeat transcripts, mitochondrial transcriptional imbalance, defective RNA processing and cellular stress. Its immunogenicity depends on ligand structure, editing, persistence, localisation, sensor access and pathway competence. ADAR1 and RNA-binding or decay factors buffer self-dsRNA, allowing tumour cells to tolerate potentially immunogenic RNA. Disruption of this control engages RIG-I-MAVS, MDA5-MAVS, TLR3-TRIF, PKR-eIF2α, and OAS-RNase L pa

An NT5E loss-of-function variant permits tissue inflammation and hypertension in systemic lupus erythematosus.

Adenosine, generated by CD73/NT5E, is a critical endogenous regulator of immune-mediated inflammation. Genetic variants in NT5E have been linked to vascular and inflammatory phenotypes. Systemic lupus erythematosus (SLE) is a chronic autoimmune disease marked by persistent inflammation. Using a systems biology approach, we examined immunologic and genetic alterations in adenosine signaling in SLE. Whereas gene signatures associated with extracellular adenosine production were elevated in lupus-affected tissues, adenosine-mediated immunosuppressive pathways were broadly downregulated and correl

Decoding the dark genome reveals its organisation into modular disease networks.

The biological functions and disease relevance of the 'dark genome'-over one-third of all protein-coding genes-remain largely unknown. Here, we use integrative network and functional analyses to construct a systems-level map of dark gene contributions to human genetic diseases. We identify 16 hub dark genes, including R3HDM2 and RPUSD4, that are central to disease networks and are overwhelmingly enriched for roles in mitochondrial protein synthesis. These hubs form modular networks connecting major inflammatory conditions like psoriasis and tuberculosis, driven by specific transcription factor

Perirhinal cortex gains control of hippocampal seizures via broad downstream circuits in male mouse models.

The perirhinal cortex (PRh) has extensive interconnections with limbic structures, including the fasciola cinerea (FC), a recently identified node and therapeutic target in temporal lobe epilepsy. However, the neural circuit basis between PRh and FC in seizure progression remains unclear. Here we show that the PRh provides the principal excitatory input to the FC and plays an important role in seizure modulation in male mouse epilepsy models. PRh-FC projections are strongly activated during seizures, and their inhibition delays seizure progression. Moreover, PRh excitatory neurons bidirectiona

Commensal enteric virome regulates intestinal carbohydrate digestion and absorption.

The enteric microbiome and nutrient sensing within the small intestine play critical roles in maintaining host metabolic homeostasis. Although various bacteria and some fungi have established functions in nutrient metabolism, the role of the enteric virome remains poorly understood. Here, we demonstrate that the enteric virome significantly influences carbohydrate digestion and absorption independently of the bacteriome. Furthermore, the virome elicits distinct responses across different intestinal cell types. Specifically, it activates programs for carbohydrate digestion and absorption in int

Robust and interpretable unit level causal inference in neural networks for pediatric myopia.

Understanding causal mechanisms in deep learning is essential for clinical adoption, where interpretability and reliability are critical. Most existing AI systems act as black boxes, limiting transparency in medicine. We propose a causal inference framework integrated into neural networks to assess the influence of individual features on predictions. Using a prospective pediatric ophthalmology cohort of over 3000 children with longitudinal follow-up, our method estimates direct and indirect causal effects through intervention. Applied to myopia progression in children, our approach not only ac

Multifaceted roles for persulfide species in redox chemical biology.

Reactive sulfur species (RSS) are increasingly recognized as important bioactive agents across phyla. Application of newly developed chemical tools, detection methods and multi-omics techniques has uncovered new and specific roles of endogenous RSS and revealed that a number of biological actions previously attributed to reactive oxygen species or reactive nitrogen species can also be mediated by RSS. This Review describes the versatile chemical biology of RSS with focus on persulfide formation on cysteine residues. We examine their pro-oxidant and antioxidant capacities, involvement in redox
