Statin use is independently associated with smaller infarct volume in nonlacunar MCA territory stroke.

BACKGROUND: Studies have shown an association between HMG-CoA reductase inhibitors (statins) and improved stroke outcomes, possibly secondary to neuroprotective properties. OBJECTIVE: To assess whether patients taking statins prior to ischemic stroke have smaller infarcts on magnetic resonance imaging (MRI), adjusting for other relevant clinical factors. DESIGN: We retrospectively reviewed the Cleveland Clinic Foundation (CCF) Neurology Inpatient Database from June 2002 through June 2004. Demographics, medications, stroke subtype, diffusion-weighted imaging (DWI) infarct volume, admission NI

Glioblastoma multiforme with atypical diffusion-weighted MR findings.

The aim of this study is to review the diffusion-weighted MRI findings of glioblastomas, to investigate those with atypical characteristics and to emphasise the reasons responsible for the atypical features on diffusion-weighted MR images. 48 cases of histologically proven glioblastomas were included in this study. In addition to conventional sequences of routine tumour protocol, diffusion-weighted MRI with spin-echo echo-planar sequence was performed. The cystic-necrotic components of the lesions, according to the conventional sequences, were determined on the diffusion-weighted MR images and

[Stereotactic mapping for radiosurgical treatment of vestibular schwannomas].

RATIONALE: As an exclusively image-guided surgery method, radiosurgery requires special attention in the choice of imaging modalities and acquisition parameters must be set with extreme care. METHODS: Quality control for resolution and accuracy of computed tomography (CT) scanners must be performed. Magnetic resonance imaging (MRI) distortions should be limited through magnetic field homogeneity adjustment (shimming) and acquisition parameters optimization. These inaccuracies should then be quantified through systematic combination of MRI and CT in the radiosurgery planning system. MRI pulse

[Intracerebral hematoma associated with reduced apparent diffusion coefficient mimicking acute stroke].

We report the case of a 67 year old patient admitted at our institution for acute onset of left hemiplegia. MRI was done 2 h 30 after symptom onset. Diffusion weighted images showed a hyperintense lesion in the right basal ganglia region with restricted apparent diffusion coefficient (ADC=428. 10(-6) mm(2)/s), a 50% decrease in value compared to the normal left side, consistent with acute ischemia. The lesion was hyperintense and moderately heterogeneous at FLAIR imaging, best seen on echo planar T2W images, with hypointense rim consistent with magnetic susceptibility artifact. The appearance

Extensive cortical damage in a case of Creutzfeldt-Jacob disease: clinicoradiological correlations.

MRI demonstrated extensive cortical involvement in a patient with pathologically proven Creutzfeldt-Jacob disease. The whole brain was atrophic; some of the supratentorial cortex, putamen and caudate nucleus gave high signal on T2-weighted images; the changes were more extensive on diffusion-weighted images (DWI). Comparison of the history, and the sites of atrophy and signal change suggested that the latter predominates in regions with long-lasting damage and prevalent gliosis, while high signal on DWI indicate current neuronal loss. This case widens the range of MRI findings in patients with

Perfusion MRI abnormalities in the absence of diffusion changes in a case of moyamoya-like syndrome in neurofibromatosis type 1.

We report on a 12-year-old boy with neurofibromatosis type 1 who suffered a transient ischemic attack. Angiography revealed occlusion of intracranial arteries, moyamoya vessels and leptomeningeal collaterals. The conventional T2-weighted and the diffusion-weighted MRI images demonstrated no pathology. Dynamic first-pass postgadolinium T2* perfusion-weighted MRI depicted altered hemodynamics in the vascular territory of the left middle cerebral artery, which defined this region as ischemic tissue at risk. The patient suffered a repeat transient ischemic attack 5 days later.

Cystic ovarian lesions in SSFP diffusion imaging.

PURPOSE: MR assessments of ovarian cystic lesions are usually based on morphological features, signal intensities and enhancement with contrast media. This study was performed to evaluate the usefulness of the steady-state free precession (SSFP) diffusion imaging of cystic ovarian lesions for analyzing cystic contents. MATERIALS AND METHODS: Sixty-one ovarian cystic lesions in 37 patients were examined. The diffusion-related coefficient (DRC) and the ratio of the relative apparent diffusion coefficient of the lesion to that of subcutaneous fat tissue (rADC(L)/rADC(F)) were calculated from SSF

Diffuse midline glioma, H3 K27-altered, forming a suprasellar mass: A case report.

Diffuse midline glioma, H3 K27-altered (DMG), is an extremely aggressive tumor that mainly arises in midline structures such as the thalamus, brainstem, and spinal cord in children and young adults. We report a rare adult case of DMG presenting as a suprasellar mass. A 56-year-old man presented with progressive visual impairment over 2 months. Magnetic resonance imaging (MRI) revealed a lobulated mass centered on the optic chiasm and extending from the suprasellar region to the anterior aspect of the midbrain, with involvement of both optic tracts and the hypothalamus. Subtotal resection was p

Superior divisional vestibular paresis in anterior inferior cerebellar artery infarction.

A 74-year-old woman with sudden vertigo and dysarthria showed spontaneous nystagmus beating rightward, upward and clockwise. She also exhibited dysfunction of the left utricle and superior and horizontal canals, which was documented by head impulse and heave tests, measurements of ocular torsion and subjective visual vertical tilt, bithermal caloric tests, and rotation axis analysis of the spontaneous nystagmus. In contrast, functions of the left posterior canal, saccule, and cochlea were remained intact, as evidenced by normal head impulse test, audiometry and vestibular evoked myogenic poten

Disruption of the white matter structural network and its correlation with baseline progression rate in patients with sporadic amyotrophic lateral sclerosis.

OBJECTIVE: There is increasing evidence that amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease impacting large-scale brain networks. However, it is still unclear which structural networks are associated with the disease and whether the network connectomics are associated with disease progression. This study was aimed to characterize the network abnormalities in ALS and to identify the network-based biomarkers that predict the ALS baseline progression rate. METHODS: Magnetic resonance imaging was performed on 73 patients with sporadic ALS and 100 healthy participan

Drip-and-Ship Thrombolytic Therapy for Acute Ischemic Stroke.

BACKGROUND: Neuroendovascular therapy is a common treatment for patients with acute ischemic stroke of the anterior circulation who fail to respond to recombinant tissue plasminogen activator. However, although most hospitals can provide recombinant tissue plasminogen activator therapy, many cannot perform neuroendovascular therapy. Thus, use of a drip-and-ship treatment-liaison system allowing recombinant tissue plasminogen activator-treated patients to be transferred to facilities offering neuroendovascular therapy is important. METHODS: We retrospectively analyzed 16 drip-and-ship patients

Diffusion-weighted MRI in evaluation of transient ischemic attack.

Diffusion-weighted magnetic resonance imaging (DWI) is a sensitive diagnostic tool for detecting recent ischemic lesions in patients with transient ischemic attacks (TIAs), but the interpretation of the presence or absence of DWI abnormalities in TIA patients still remains controversial. To elucidate the pathophysiology underlying those lesions, we analyzed DWI abnormalities in patients with recent TIAs. Based on 45 consecutive patients with TIAs who underwent DWI within 10 days of onset, demographic data and clinical manifestations were analyzed in relation to the DWI abnormalities. According

Increased hippocampal head diffusivity predicts impaired episodic memory performance in early Alzheimer's disease.

Recent neuroanatomical and functional neuroimaging studies indicate that the anterior part of the hippocampus, rather than the whole structure, may be specifically involved in episodic memory. In the present work, we examined whether anterior structural measurements are superior to other regional or global measurements in mapping functionally relevant degenerative alterations of the hippocampus in Alzheimer's disease (AD). Twenty patients with early AD (MMSE 25.7+/-1.7) and 18 healthy controls were studied using magnetic resonance and diffusion-tensor imaging. Using a regions-of-interest analy
