Divergence insufficiency revisited: natural history of idiopathic cases and neurologic associations.

OBJECTIVES: To determine the natural history of primary divergence insufficiency and to identify clinical features that distinguish patients with this neurologically isolated form of divergence insufficiency from those harboring neurologic disorders. METHODS: Retrospective survey of patients with divergence insufficiency. Patients were categorized into 2 groups, primary (ie, neurologically isolated based on clinical criteria) and secondary (ie, associated with a neurologic or systemic disorder). Long-term follow-up and clinical features of the 2 groups were compared. RESULTS: Of the 20 patie

[Wegener's granulomatosis with meningeal involvement: clinic-radiological correlations].

INTRODUCTION: Wegener's granulomatosis is a systemic vasculitis which, in its classical form, is characterized by involvement of the superior and inferior respiratory tract and the kidneys. The vasculitis may be multisystemic. Ophthalmic and neurological involvement are common (22% and 54% of those affected respectively). When considering involvement of the nervous system, the commonest finding is peripheral neuropathy, particularly in the form of multiple mononeuritis. Meningeal involvement is exceptional. CLINICAL CASE AND CONCLUSIONS: We present a case of Wegener's granulomatosis with meni

Concurrent trigeminal, abducens, and facial nerve palsies presenting as false localizing signs: case report.

Multiple cranial nerve dysfunction presenting as false localizing signs is rare. We report a 20-year-old woman who had concurrent trigeminal sensory disturbance, abducens, and peripheral facial nerve palsies in association with a contralateral acoustic neuroma. After surgery, the trigeminal nerve disturbance and the abducens nerve palsy completely recovered, but the peripheral facial nerve palsy persisted. The nature of tumor, the presence of brain stem distortion, the anatomic variation of posterior fossa, and the relationships of cranial nerves and nearby blood vessels, which are factors inf

Convergence insufficiency and failure of accommodation following midfacial trauma.

A prospective evaluation of patients who had sustained midfacial trauma was carried out in order to determine the prevalence of impaired convergence and accommodation and to establish the risk factors for such defects. Of 52 patients in this study, 11 suffered accommodation and/or convergence disturbances. They included 6 males (17% of the male population) and 5 females (29% of the female population). Nine of these 11 patients sustained their injuries due to alleged assaults (24.3% of all assaulted patients) and 2 following simple falls (25% of all fall victims). Five patients complained of do

Recurrent Dedifferentiated Clival Chondrosarcoma Presenting 32 Years after Initial Treatment: A Case Report.

Background Skull base chondrosarcomas are rare and locally aggressive tumors with heterogeneous biology accounting for 0.1-0.2% of all intracranial tumors. Dedifferentiated chondrosarcoma is an exceptionally uncommon and aggressive variant, seldom seen at the skull base. We report a recurrent clival chondrosarcoma with a small, dedifferentiated component, and an unusually long latency after initial treatment. Case Report A 66-year-old woman was referred with a recurrent progressive clival lesion, initially suspected to be a chordoma. She had undergone subtotal resection followed by single-frac

A Case of Drowning with Acute Kidney Injury and Raised Intracranial Tension.

BACKGROUND: Drowning is the third leading cause of unintentional injury and death worldwide, accounting for 7% of all injury-related deaths. People who are at higher risk include those who have free accessibility to water and younger children lacking supervision. The common complications of drowning are mainly due to hypoxia. It mainly affects the respiratory, cardiac, and central nervous system. Rarely, drowning may cause acute kidney injury (AKI) secondary to rhabdomyolysis. Here, we present a case of near drowning with AKI, rhabdomyolysis, and elevated intracranial tension (ICT) with a fals
