Mutation and recombination in pathogen evolution: Relevance, methods and controversies.

Mutation and recombination drive the evolution of most pathogens by generating the genetic variants upon which selection operates. Those variants can, for example, confer resistance to host immune systems and drug therapies or lead to epidemic outbreaks. Given their importance, diverse evolutionary studies have investigated the abundance and consequences of mutation and recombination in pathogen populations. However, some controversies persist regarding the contribution of each evolutionary force to the development of particular phenotypic observations (e.g., drug resistance). In this study, w

Exploiting Genetic Interference for Antiviral Therapy.

Rapidly evolving viruses are a major threat to human health. Such viruses are often highly pathogenic (e.g., influenza virus, HIV, Ebola virus) and routinely circumvent therapeutic intervention through mutational escape. Error-prone genome replication generates heterogeneous viral populations that rapidly adapt to new selection pressures, leading to resistance that emerges with treatment. However, population heterogeneity bears a cost: when multiple viral variants replicate within a cell, they can potentially interfere with each other, lowering viral fitness. This genetic interference can be e

A unified approach towards Trypanosoma brucei functional genomics using Gibson assembly.

Trypanosoma brucei is the causative agent of human African trypanosomiasis and nagana in cattle. Recent advances in high throughput phenotypic and interaction screens have identified a wealth of novel candidate proteins for diverse functions such as drug resistance, life cycle progression, and cytoskeletal biogenesis. Characterization of these proteins will allow a more mechanistic understanding of the biology of this important pathogen and could identify novel drug targets. However, methods for rapidly validating and prioritizing these potential targets are still being developed. While gene t

Dopamine receptor partial agonists and addiction.

Many drugs abused by humans acutely facilitate, either directly or indirectly, dopamine neurotransmission in the mesolimbic pathway. As a consequence dopamine receptor agonists and antagonists have been widely investigated as putative pharmacological therapies for addiction. This general strategy, however, has had only limited success due in part to poor treatment adherence and efficacy and the significant adverse effects of dopaminergic medications. In this perspective, we discuss the potential therapeutic use of dopamine receptor partial agonists in addiction, developed initially as antipsyc

Update on interventional cardiology.

This article provides a detailed review of the most important studies on interventional cardiology reported in publications or presentations during the year 2012. With regard to coronary interventions, ST-elevation myocardial infarction is extensively addressed in studies focusing on the relevance of reducing the reperfusion time and the utility of various devices and pharmacological strategies in primary angioplasty. Multiple comparative studies involving different generations of drug-eluting stents are available and indicate a favorable progression in terms of safety and efficacy. The risk o

[Present status and future of hypnotic drug treatment for insomnia].

Pharmacological treatments of insomnia have become safer since the first benzodiazepine receptor agonist (BzRA) hypnotic was introduced in the 1960's. Though BzRAs could hardly cause a fatal condition even in cases of overdosing, they had inherited the arguments on addiction and withdrawal from the prior studies of barbiturate hypnotics that indicated they are strongly addictive. In the 2000s, it was repeatedly demonstrated that insomnia as well as sleep deprivation underlie the development and deterioration of comorbid diseases such as hypertension, cardiovascular diseases, diabetes and depre

Zonisamide in the treatment of epilepsy.

INTRODUCTION: Epilepsy affects approximately 3 million people in the USA and up to 2% of the worldwide population. The yearly direct medical cost of epilepsy in the USA alone is estimated to be $9.5 billion. Epilepsy affects both children and adults and can significantly impair quality of life. Zonisamide is a second-generation antiepileptic drug (AED) that has broad-spectrum efficacy, a favorable side-effect profile and simpler dosing than earlier drugs. AREAS COVERED: The history of the development of zonisamide is reviewed in this paper. The data available demonstrating zonisamide's mechan

HIV genes diversify in B cells.

Mutation of the human immunodeficiency virus by host cells inhibits viral dissemination by creating non-functional variants. However, viral mutation does not always eliminate the ability of the virus to disseminate and, in fact, is thought to promote persistence by generating functional mutants that evade immunity or drugs. How and where HIV mutates is not known. Accordingly, where and to what extent variants emerge may be determined by the cell type with optimal mutation apparatus as well as by the properties of the viral genomic sequence itself. Here we considered that HIV, which can infect

Continuous intra-articular patient-controlled analgesia in a cancer patient with a pathological hip fracture. A case report.

Pain management is one of the major goals to achieve in treating terminal patients. Managing severe pain with drug therapy by using a combination of narcotics, non-narcotics, and adjuvant drugs provides the best results for most patients. In patients with fractures due to bone metastases there is a relative indication for operative treatment if the patient is expected to survive the operation and to recover in the postoperative period. In this case report a terminal 48 year-old female with a pathological femoral neck fracture is presented. No operative treatment was offered because of her bad

Switching from a PI-based to a PI-sparing regimen for management of metabolic or clinical fat redistribution.

Concerns regarding metabolic perturbations occurring during protease inhibitor (PI)-based regimens have led to investigation of switching from a PI-based to a non-nucleoside reverse transcriptase inhibitor- or abacavir-based regimen. There appear to be considerable benefits to switching from a PI-based regimen to one of these PI-sparing regimens. In particular, patients appear generally pleased with the improved administration characteristics of the new regimens, and improvements in quality of life have been reported. However, resolution of the metabolic abnormalities that may arise during PI

Side effects of NSAIDs and dosing recommendations for ketorolac.

We reviewed the pathogenesis of the most important side effects of non-steroidal antiinflammatory drugs (NSAIDs). Short-term treatments for postoperative pain are not generally associated with gastric damage. Pharmacoepidemiological studies have shown that for ketorolac the risk of gastrointestinal bleeding was only important in older patients and when doses > 90 mg/day were used. Although NSAIDs cause platelet dysfunction resulting in prolonged bleeding time, these drugs do not affect clot formation. Therefore, NSAIDs should be used with caution in surgical procedures involving considerable d

Amineptine (Survector 100) in the treatment of depression in Brazil.

Amineptine was studied in open, simple, and comparative trials in several Latin American countries, primarily in Brazil, Uruguay, and Chile. These countries developed studies with this antidepressant in an individualized way in different centers as well as multicenter studies under single coordination. The author analyzes the results of two of these studies. Generally, amineptine showed a positive result in depression in which the inhibition symptom is predominant. This drug also showed a good tolerance: it may be prescribed to elderly patients with depression and to people who suffer from dis

Relative efficacy of heparin and related glycosaminoglycans as antithrombotic drugs.

In a standardized animal model, unfractionated heparin (UFH) prevents venous thrombogenesis at a dose of 80 micrograms/kg. Oligosaccharide fragments of heparin, with very high anti-Xa activity both in vitro and in ex vivo plasma samples were less effective than UFH in preventing thrombosis. A decasaccharide fragment was virtually inactive in impairing thrombosis at this dose, although a 20-22 monosaccharide fragment showed some impairment. Dermatan sulfate, which has no anti-factor Xa activity, partially impairs both thrombin generation and stasis thrombosis. However, dermatan sulfate could no

Erythroderma: review of 82 cases.

We reviewed 82 cases of erythroderma, including clinical, laboratory, and biopsy material. The diseases most commonly associated with erythroderma were drug eruptions (34%), preexisting dermatoses (30%), and lymphoreticular neoplasms (20%), especially cutaneous T cell lymphomas (CTCL). The prognosis was related to the associated process; all deaths occurred in the drug or lymphoma groups. The clinical presentation of erythroderma was fairly uniform, with malaise (34%), pruritus (36%), and a subjective chilly sensation (34%) the most common symptoms. Keratoderma of the palms and soles (37%), mi

Prazosin update. A review of its pharmacological properties and therapeutic use in hypertension and congestive heart failure.

Prazosin is an orally active post-synaptic selective alpha 1-adrenoreceptor antagonist that has been widely used in treating hypertension and congestive heart failure (CHF). Its role in the treatment of hypertension has previously been reviewed in this journal. Subsequent reports confirm its efficacy in treating mild to severe hypertension as a single agent or, more frequently, in combination with another antihypertensive agent and/or a diuretic. Recent studies of the metabolic effect of prazosin indicate that the drug may have a favourable effect on plasma lipids in hypertensive patients. Its

[Primary pulmonary hypertension in childhood: familial aspects].

Primary pulmonary hypertension (PPH) is a rare, progressive and usually fatal disease. Its pathogenesis remains obscure. Probably multiple genetic and environmental factors play a role in the occurrence of PPH. PPH sometimes runs in families. We have studied two families in which some members were affected: in family A two sibs, their father and probably their paternal grandmother, in family B two sibs. When PPH is present in more than one generation (family A), an autosomal dominant mode of inheritance would be likely, whereas, when the disease is present in only one generation (family B), an

Ticarcillin: a review of its pharmacological properties and therapeutic efficacy.

Ticarcillin is a semisynthetic penicillin for parenteral administration. The antibacterial activity of ticarcillin is similar to that of carbenicillin except that it is two to four times more active in vitro against Pseudomonas aeruginosa, generally less active against Gram-positive cocci and more active against most Gram-negative bacilli. As the pharmacokinetics of ticarcillin and carbenicillin are also similar, ticarcillin should theoretically be clinically effective when administered at a lower dosage than carbenicillin. There is some evidence that ticarcillin is comparable in efficacy with

In vivo or in vitro modulating effects of vincristine on the generation of allogeneic cytotoxic lymphocytes in vitro.

Cytotoxic lymphocytes carrying the H-2(i5) haplotype were generated in vitro against alloantigens specified by the IC-S-G-D regions of H-2. Responder lymphocytes were collected from donors B10.A(5R) non-treated or treated with 3 or 0.6 mg/Kg, ip of Vincristinee (VCR) and incubated with irradiated B10 spleen cells. High doses of VCR produced depression of the cytotoxic response (CR), whereas 0.6 mg/Kg of the drug produced substantial increase of the response. Further experiments were conducted adding VCR in vitro to the tissue culture during the generation of cytotoxic lymphocytes. Depression o

A Knowledge Graph-Driven Multimodal Framework for Drug-Disease Association Prediction.

Drug repurposing represents a cost-effective strategy to identify novel therapeutic applications for existing pharmaceuticals, circumventing the protracted timelines of traditional drug discovery. While knowledge graph (KG) based methods excel at integrating heterogeneous biomedical data, they often struggle to harmonize high-level domain knowledge with fine-grained molecular mechanisms. We propose KGDDA, a multimodal framework designed for drug-disease association prediction that synergistically integrates KGs with medical ontologies. By leveraging an attention-driven fusion mechanism, KGDDA
