Eating disorder not otherwise specified in an inpatient unit: the impact of altering the DSM-IV criteria for anorexia and bulimia nervosa.

OBJECTIVE: To evaluate (1) the Eating Disorder Not Otherwise Specified (EDNOS) prevalence in an eating disorder inpatient unit; (2) the impact of altering the diagnostic criteria for anorexia nervosa and bulimia nervosa on the prevalence of EDNOS. METHOD: One hundred and eighty six eating disorder patients consecutively hospitalised were included in the study. The prevalence of anorexia nervosa, bulimia nervosa and EDNOS was evaluated with the Eating Disorder Examination (EDE). The EDNOS prevalence was recalculated after the alteration of three diagnostic criteria for anorexia nervosa and one

Which elements in the treatment of eating disorders are necessary 'ingredients' in the recovery process?--A comparison between the patient's and therapist's view.

BACKGROUND: Little is known about which therapeutic 'ingredients' in the treatment of eating disorders (anorexia nervosa (AN), bulimia nervosa (BN) and binge eating disorder (BED)) are needed for recovery. Remarkably, most studies on this topic have neglected the patient's view. METHOD: In this study, a large sample of eating disorder patients (n = 132) was invited to evaluate which elements in the treatment they consider to be helpful and effective in their recovery process. These results were compared to the view of 49 eating disorder experts. RESULTS: Following the patient's view, 'improv

Treating disturbances in the relationship between mothers with bulimic eating disorders and their infants: a randomized, controlled trial of video feedback.

OBJECTIVE: Maternal eating disorders interfere with parenting, adversely affecting mother-infant interaction and infant outcome. This trial tested whether video-feedback treatment specifically targeting mother-child interaction would be superior to counseling in improving mother-child interaction, especially mealtime conflict, and infant weight and autonomy. METHOD: The participants were 80 mothers with bulimia nervosa or similar eating disorder who were attending routine baby clinics and whose infants were 4-6 months old. They were randomly assigned to video-feedback interactional treatment

Position of the American Dietetic Association: Nutrition intervention in the treatment of anorexia nervosa, bulimia nervosa, and other eating disorders.

It is the position of the American Dietetic Association that nutrition intervention, including nutritional counseling, by a registered dietitian (RD) is an essential component of the team treatment of patients with anorexia nervosa, bulimia nervosa, and other eating disorders during assessment and treatment across the continuum of care. Diagnostic criteria for eating disorders provide important guidelines for identification and treatment. However, it is thought that a continuum of disordered eating may exist that ranges from persistent dieting to subthreshold conditions and then to defined eat

Cognitive impairment and its association with homocysteine plasma levels in females with eating disorders - findings from the HEaD-study.

Higher plasma homocysteine levels have been found in females with anorexia nervosa. Furthermore, elevated homocysteine levels are associated with cognitive decline in dementia and healthy elderly people. Aim of this prospective study was to investigate a possible association between homocysteine serum levels and Clinically well known cognitive deficits in females with eating disorders. We found that moderately elevated plasma homocysteine levels were associated with normal short- and long-term verbal memory while normal plasma homocysteine levels were associated with poorer memory performance

Reconceptualizing causative factors and intervention strategies in the eating disorders: a shift from body image to self-concept impairments.

In this report, we argue that impairments in self-concept development function as a cognitive vulnerability that contributes to the formation of the eating disorders (ED) of anorexia nervosa (AN) and bulimia nervosa (BN). More specifically we argue that impairments in development of the total collection of identities that comprise the self-concept contribute to body image disturbances which in turn, motivate the eating and body-weight attitudes and behaviors that characterize the disorders. First, we review current understandings of the role of body image disturbances in the ED and discuss lim

Self-harm and substance use in a community sample of Black and White women with binge eating disorder or bulimia nervosa.

OBJECTIVE: This study evaluated rates of self-harm and substance use in women with either bulimia nervosa (BN) or binge eating disorder (BED) and assessed whether differences in self-harm and substance use are related to sexual or physical abuse. METHOD: Alcohol abuse, self-harm, and use or abuse of various illicit drugs were evaluated in a sample of 53 women with BN and 162 women with BED. RESULTS: Self-harm and substance use generally did not differentiate BED and BN cases, but rates of self-harm and substance use were elevated among women with a history of sexual or physical abuse relativ

Prospective study of outcome in bulimics as a function of Axis-II comorbidity: long-term responses on eating and psychiatric symptoms.

We assessed prognostic implications of categorical personality disorder (PD) diagnoses at selected points during and following treatment for bulimic syndromes. Seventy-six bulimic cases were organized into Borderline PD, Other PD, or No PD groups, and then assessed at pretreatment, after 3 months of therapy, and at termination (after a modal 8 months). Results at 3- and 12-month posttreatment follow-ups (available in 43 and 38 cases, respectively) provided a reflection of status after therapy termination. On comorbid symptoms, borderline/ nonborderline differences were very striking: Borderlin

Should an athlete eat straight after training?--A study of intestinal transit time and its relationship to prior exercise.

The mouth-to-caecum transit time of food was measured using the rise in breath hydrogen after a standard breakfast of baked beans on two occasions in seven healthy volunteers. The first occasion was after resting and the second after moderate exercise on a bicycle ergometer. There was no significant difference between the transit times with or without prior exercise. It is concluded that moderate exercise taken before food does not interfere with transit time and therefore should not in that way interfere with nutrition.

Pursuit of thinness and onset of eating disorder symptoms in a community sample of adolescent girls: a three-year prospective analysis.

Community-based prospective studies are needed to shed light on mechanisms that may influence development of eating disorders and identify variables that could serve as potential targets for prevention efforts. In this paper we examine level of weight preoccupation and other variables prospectively associated with age of onset of eating disorder symptoms over a 3-year interval in a community sample (N = 939) of young adolescent girls. 3.6% (32/887) experienced onset of symptoms over the interval. Only one factor, a measure of Weight Concerns, was significantly associated with onset (p < .001).

Comparative study of the prevalence of abnormal eating attitudes among Arab female students of both London and Cairo universities.

Two matched samples of Arab female undergraduate students attending London and Cairo Universities were recruited to determine the relative prevalence of abnormal eating attitudes and the effect of exposure to Western culture upon this prevalence. A positive response was reported on the Eating Attitudes Test (EAT-40) in 22% of the students in the London group and 12% in the Cairo group, indicating that abnormal attitudes occur in this non-Western population. Six cases among the London sample fulfilled diagnostic criteria for bulimia nervosa, but no cases of either anorexia or bulimia were ident

Effects of cold, eating, efferent nerve stimulation and angiotensin on heart rate in sheep before and after autonomic blockade.

1. Effects of the natural stimuli of cold exposure and feeding on heart rate were tested in conscious sheep in which blockade of the cardiac efferent nerves was achieved by administration of propranolol and atropine. Effects of direct nerve stimulation, isoproterenol and angiotensin on heart rate before and after autonomic blockade were studied in acute preparations anaesthetized with pentobarbitone sodium.2. Propranolol reduced the extent of cardioacceleration induced both by exposure to cold and by eating. Heart rate in propranolol treated animals exposed to cold did not exceed about 120 bea
