EEG analysis of male to female transsexuals: discriminant function and source analysis.

A small series of consecutive, unmedicated male to female transsexuals were studied before hormonal treatment and prior to reassignment surgery. Quantitative EEG and LORETA source localization in the Eyes Open and Eyes Closed conditions were carried out, and they were compared to sinistral/ambilateral male and female controls, as the transsexual group was overwhelmingly sinistral. Discriminant function analysis and source localization showed that the transsexual group was similar to female heterosexual controls, with increased sources in the right hemisphere in the fast frequencies.

Psychogenic seizures: a focused clinical review for the emergency medicine practitioner.

Psychogenic seizures sometimes mimic neurogenic seizures and may occur concomitantly. Unlike neurogenic seizures, psychogenic seizures do not have abnormal electrical discharges on electroencephalogram, and their presence generally reflects an underlying psychiatric disorder. No single clinical observation can definitively distinguish psychogenic from neurogenic seizures. Evidence in support of the diagnosis includes asynchronous extremity movements, rapid turning of the head from side to side, forward pelvic thrusting, out-of-phase limb movements, presence of a light reflex, and geotropic eye

Migraine triggered by epileptic discharges in a Rasmussen's encephalitis patient after surgery.

After 2years from disconnective surgery for Rasmussen's encephalitis, a child, 9years old, began to present severe migraine attacks, lateralized to the operated side, lasting 1-2days. Video/EEG recordings during two different migraine attacks, with an interval of 6months from each other, showed, in both recordings, subsequent ictal discharges over the affected and disconnected hemisphere. Migraine and ictal discharges in both occasion disappeared with diazepam i.v. The EEGs performed during migraine-free period, on the contrary, showed very rare and mild subclinical ictal discharges. Although

Pitfalls in diagnosing psychogenic nonepileptic seizures in a sexually abused child.

A six-year-old girl living in a residential care facility because of previous sexual abuse at home showed intractable seizures with epileptic discharges on interictal EEG. The features of the attacks were sudden impaired consciousness, only in daytime while eating, studying, and walking, with no motor acts and continuing for several minutes to one hour. She could not recall the attacks, and anticonvulsants showed no effects. She gradually became irritated and antisocial, exhibiting impulsive and hyperactive behavior. Interictal EEG repeatedly showed diffuse irregular spikes and wave complexes.

Bathing epilepsy: report of two Caucasian cases.

Bathing epilepsy, also known as water-immersion epilepsy, refers to a rare form of benign reflex epilepsy in which seizures are precipitated by the normal process of domestic bathing. This condition has been confused with true hot water epilepsy, even though bathing in water at normal temperature is the trigger. Focal seizures predominate with a staring gaze, pallor and generalised features followed by prolonged postictal somnolence. A variable percentage of patients may also show unprovoked seizures. The prognosis is usually favourable, and modifying bathing habits may prevent further seizure

[Clinical and neurophysiological parameters in patients with discirculatory encephalopathy].

We studied clinical parameters and characteristics of the nonspecific brain activity, reflecting interhemispheric interactions, in patients with discirculatory encephalopathy (DEP) who had arterial hypertension. Forty-six patients with DEP (25 - with DEP, II stage, 21 - with DEP, III stage) without epileptic changes on EEG were examined. The control group included 44 healthy people. EEG was assessed using the visual analysis according to E.A. Zhimunskaya, spectral analysis programs with mapping, degree of interhemispheric asymmetry of brain biopotential power. The more severe was the disease,

Effect of dietary calcium deficiency on the cardiac function of broiler chickens based on electro- and echocardiography.

To determine the effects of dietary calcium deficiency on the heart function of broiler chickens based on electro- and echocardiography, chicks were reared for 42 days and fed rations with different amounts of calcium. At 28 and 42 days of age, electrocardiographic and echocardiographic parameters were assessed. There were significant reductions of R wave amplitude (leads II and aVR) in the Ca-deficient group II at 42 day of age as compared to the control. S wave amplitudes were decreased in most leads but the decrease was significant (P < 0.05) only at 28 days (lead aVL, Ca-deficient group I)

Olfactory hallucinations as a manifestation of hidden rhinosinusitis.

The exact anatomical origin of olfactory epileptic seizures is controversial. Partial seizures can be an uncommon manifestation of brain tumours involving the medial temporal lobe and the orbitofrontal structures or as a marker of mesial temporal sclerosis. We present a patient suffering from olfactory hallucinations associated with inflammatory tissue in the anterior ethmoidal cells, in the right fronto-ethmoidal recess and in the maxillary sinus. Our report supports the opinion that the involvement of the olfactory bulb is crucial for developing so-called uncinate fits and is independent of

[Re-evaluation of using acupuncture needle as sphenoidal electrode in temporal lobe epilepsy].

In 1965, Feng of the Peking Union Hospital published an article entitled "Innovation in electroencephalography: the use of acupuncture needles as sphenoidal electrodes". It was a preliminary report, but surprisingly EEG records in the figures did not show definite spikes or sharp waves in the sphenoidal leads. In 1983, Feng and his colleagues reported a summary of 2,000 cases studied with acupuncture needle sphenoidal electrodes. This time, spikes or sharp waves were shown in EEG recordings. However, cases studied were several "paroxysmal disorders", including psychomotor seizure (155 cases),

A hemispherotomy for intractable startle epilepsy characterized by infantile hemiplegia and drop attacks.

Startle epilepsy is provoked by unexpected sensory stimuli, mainly auditory, and reveals subsequent tonic posturing of the limbs. We present a case of intractable startle epilepsy with infantile hemiplegia and discuss the indications for a hemispherotomy.

[Specificity of epileptic seizures in the elderly: A proposed electro-clinical scale].

Diagnosis of epileptic seizure may be difficult in older patients because seizure manifestations are often unusual: confusion, paresis... and because there are multiple differential diagnoses (syncope, transient ischemic attack, transient global amnesia...). To promote and facilitate the diagnosis of seizures in the elderly, neurologists and gerontologists must work together and focus their strategy on two points: firstly, the knowledge of the specific presentation of seizures in elderly patients, and secondly, the adoption of a reasoning based on seizures and not epileptic syndromes. A multid

The role of the thalamus in sleep, pineal melatonin production, and circadian rhythm sleep disorders.

The thalamus has a strong nonphotic influence on sleep, circadian rhythmicity, pineal melatonin production, and secretion. The opening of the sleep gate for nonrapid eye movement sleep is a thalamic function but it is assisted by melatonin which acts by promoting spindle formation. Thus, melatonin has a modulatory influence on sleep onset and maintenance. A remarkable similarity exists between spindle behavior, circadian rhythmicity, and pineal melatonin production throughout life. Together, the thalamic and chronobiological control of sleep leads to a new and improved understanding of the pat

[Narcolepsy with cataplexy in the child: clinical evaluation and therapeutical management].

Narcolepsy with cataplexy is a rare but long lasting and disabling disorder where onset often occurs during childhood. Excessive daytime sleepiness, irresistible sleep attacks, partial or complete cataplexies leading to abrupt falls without loss of consciousness are the major symptoms of the disorder together with hypnagogic hallucinations and sleep paralysis. Narcolepsy cases with childhood onset are often severe. School and social life are affected in children suffering from this condition. Due to the permanence of the disorder, long-term treatment is often required targeting sleepiness. Ant

Characteristics and outcomes of patients with seizures according to the time of onset in relation to stroke.

BACKGROUND/AIM: Although most late-onset seizures (LS) appear within 2 years after stroke, some of them occur later and their characteristics are unknown. The aim of this study was to compare the characteristics of patients with very-late-onset seizures (VLS) to those with early-onset seizures (ES) and those with LS. PATIENTS: The study group consisted of 204 patients with stroke-related seizures (29 ES, 128 LS and 47 VLS). RESULTS: Intracranial haemorrhage was a more frequent cause of ES than of LS and no cause at all of VLS. On the other hand, 25% of the VLS were related to lacunar strokes

Pediatric resuscitation.

PURPOSE OF REVIEW: The new International Liaison Committee on Resuscitation documents, published in 2005, include important changes in pediatric resuscitation. Some issues that were left pending have since been supplemented with new studies. Their impact will be discussed here. RECENT FINDINGS: Studies on oxygen use for neonatal resuscitation have consistently found room air to be superior to 100% oxygen. Prospective studies indicate that intubation by first responders in preclinical resuscitation of children is dangerous and should probably be avoided. New studies point to a better neurologi

[Status epilepticus].

Status epilepticus is a neurological emergency that requires prompt care. The diagnosis and treatment must be continuous from the first minutes to its resolution. The most frequent cause in epileptic patients is drug change or non-compliance and in the non-epileptic patients are cerebrovascular diseases, head trauma, drug toxicity and metabolic disturbances. Status epilepticus can be convulsive or non-convulsive and continuous electroencephalographic monitoring is useful for diagnosis and to evaluate response to treatment. Benzodiazepines and phenytoin or fosphenytoin are first-line and second

Familial subacute sclerosing panencephalitis associated with short latency.

The familial recurrence of subacute sclerosing panencephalitis is rare. The study of such cases and a comparison of intrafamilial with sporadic cases of subacute sclerosing panencephalitis may shed light on important pathogenetic factors. We report on the occurrence of subacute sclerosing panencephalitis in two brothers from rural India, who contracted measles infection simultaneously at ages 3 and 11 years. They developed subacute sclerosing panencephalitis 21 and 37 months later, respectively. A diagnosis of subacute sclerosing panencephalitis was based on history, electroencephalographic ch
