Obesity: pathophysiology and intervention.

Obesity presents a major health hazard of the 21st century. It promotes co-morbid diseases such as heart disease, type 2 diabetes, obstructive sleep apnea, certain types of cancer, and osteoarthritis. Excessive energy intake, physical inactivity, and genetic susceptibility are main causal factors for obesity, while gene mutations, endocrine disorders, medication, or psychiatric illnesses may be underlying causes in some cases. The development and maintenance of obesity may involve central pathophysiological mechanisms such as impaired brain circuit regulation and neuroendocrine hormone dysfunc

Adipose tissue and breast cancer progression: a link between metabolism and cancer.

INTRODUCTION/AIMS: Obesity, an excess accumulation of adipose tissue occurring in mammalians when caloric intake exceeds energy expenditure, is associated with an increased incidence, morbidity and mortality from several types of neoplastic diseases including postmenopausal breast cancer. METHODS AND RESULTS: Several investigators have recently studied the role of human white adipose tissue (WAT) progenitors in preclinical models of breast cancer. WAT progenitors were found to promote breast cancer local growth, angiogenesis, EMT, migration and metastatic spreading. Breast cancer patients wit

[Nutritional abnormalities in chronic obstructive pulmonary disease].

Nutritional abnormalities are associated with chronic obstructive pulmonary disease with a frequency ranging from 2 to 50%, depending on the geographical area and the study design. Diagnostic tools include anthropometry, bioelectrical impedance, dual energy radioabsortiometry and deuterium dilution, being the body mass and the lean mass indices the most frequently used parameters. While the most important consequences of nutritional abnormalities are muscle dysfunction and exercise limitation, factors implicated include an imbalance between caloric intake and consumption, and between anabolic

Malnutrition and the heart.

Earlier concepts that the heart is spared in malnutrition have been shown to be incorrect. Inadequate intake of protein and energy results in proportional loss of skeletal and myocardial muscle. As myocardial mass decreases, so does the ability to generate cardiac output; however, various compensatory factors come into play. Nutritional supplementation for malnourished patients reverses the compensatory factors and may increase the short-term potential for heart failure. Severe cardiac debility results in poor nutrition, which may in turn produce unsuspected but clinically significant myocardi

The physiology of leptin in avians: A review.

Leptin, discovered in 1994 in ob/ob and db/db mutant mice, is a key regulator of energy balance, appetite, and multiple physiological systems including immunity and reproduction. While its function in mammals is well-established, leptin's presence and role in birds remain controversial due to differences in gene sequence, tissue expression, and signaling mechanisms. Avian leptin exhibits a GC-rich sequence, predominates in autocrine and paracrine signaling, and maintains conserved receptor-binding residues, supporting functional activity despite low systemic levels. Experimental studies show l

Neglected and Underutilized Species (NUS) of Crops: A Solution for Future Nutritional Challenges?

This article addresses the current state of global food insecurity and the rising burden of chronic diseases-both of which are nutrition-related crises-highlighting the urgent need to diversify the global dietary matrix. Currently, a mere four crops supply 60% of human energy intake, while macroeconomic barriers prevent over a third of the global population from affording a healthy diet. In this context, neglected and underutilized species (NUS) emerge as a highly sustainable and resilient alternative, given that certain species possess exceptional nutritional profiles and high adaptability to

Bone health in patients with cancer: a SEOM-SEIOMM consensus review of risk factors, assessment strategies, and management approaches.

Anticancer therapies frequently compromise skeletal health and increase fracture risk. This narrative review synthesizes information and provides guidance on assessment, prevention, and management of therapy-related bone loss in adults with cancer. Major contributors to bone loss in this population include aromatase inhibitors, androgen deprivation therapy, systemic glucocorticoids, chemotherapy, and selected targeted or immune agents. Recommended assessments include clinical evaluation, laboratory tests (calcium, vitamin D, and renal function), and bone mineral density by dual-energy X-ray ab

Heat stress associated depression and allied affliction: Therapeutic roles of nutrition, micronutrients, and phytochemicals-a review.

Heat stress is a critical environmental factor disrupting systemic homeostasis and impairing mental health, with increasing evidence linking prolonged thermal exposure to depression and related neuropsychological disorders. Chronic heat stress induces neuroendocrine dysregulation via hyperactivation of the hypothalamic-pituitary-adrenal (HPA) axis, alongside oxidative stress, neuroinflammation, mitochondrial dysfunction, and altered neurotransmitter signaling, collectively elevating risks of mood disturbances, anxiety, cognitive decline, and sleep disorders. Emerging evidence highlights nutrit

Reprogramming of host energy metabolism mediated by the TNF-iNOS-HIF-1α axis plays a key role in host resistance to Plasmodium infection.

TNF has a dual effect in Plasmodium infection, bolstering the host's immune defense while also inducing sickness behavior. Here, we confirm that TNF signaling hampers physical activity, food intake, and energy expenditure while enhancing glucose uptake by the liver and spleen, as well as controlling parasitemia in Plasmodium chabaudi (Pc)-infected mice. We also report that TNF is required for expression of inducible nitric oxide synthase (iNOS), stabilization of hypoxia-inducible factor 1α (HIF-1α), expression of glucose transporter GLUT1, and enhanced glycolysis in monocytic cells from Pc-inf

Diabetes mellitus-Progress and opportunities in the evolving epidemic.

Diabetes, a complex multisystem metabolic disorder characterized by hyperglycemia, leads to complications that reduce quality of life and increase mortality. Diabetes pathophysiology includes dysfunction of beta cells, adipose tissue, skeletal muscle, and liver. Type 1 diabetes (T1D) results from immune-mediated beta cell destruction. The more prevalent type 2 diabetes (T2D) is a heterogeneous disorder characterized by varying degrees of beta cell dysfunction in concert with insulin resistance. The strong association between obesity and T2D involves pathways regulated by the central nervous sy

Severe lactation ketoacidosis presenting as a respiratory complaint.

Ketoacidosis, a type of high anion gap metabolic acidosis, results from 1 of 3 etiologies: diabetic ketoacidosis, alcoholic ketoacidosis, or starvation ketoacidosis (SKA). In rare instances, young and otherwise healthy lactating women have been found to develop lactation ketoacidosis, a form of SKA, when the high energy requirements of breastfeeding are not met with adequate carbohydrate intake. We present the case of a 29-year-old woman who presented to our emergency department with respiratory distress and headache and was found to have severe lactation ketoacidosis. The patient was treated

SIRT1 accelerates the progression of activity-based anorexia.

Food consumption is fundamental for life, and eating disorders often result in devastating or life-threatening conditions. Anorexia nervosa (AN) is characterized by a persistent restriction of energy intake, leading to lowered body weight, constant fear of gaining weight, and psychological disturbances of body perception. Herein, we demonstrate that SIRT1 inhibition, both genetically and pharmacologically, delays the onset and progression of AN behaviors in activity-based anorexia (ABA) models, while SIRT1 activation accelerates ABA phenotypes. Mechanistically, we suggest that SIRT1 promotes p

How maternal malnutrition affects linear growth and development in the offspring.

Maternal malnutrition is common in the developing world and has detrimental effects on both the mother and infant. Pre-pregnancy nutritional status and weight gain during pregnancy are positively related to fetal growth and development. Internationally, there is no agreement on the method of diagnosis or treatment of moderate or severe malnutrition during pregnancy. Establishing clear guidelines for diagnosis and treatment will be essential in elevating the problem. Possible anthropometric measurements used to detect and monitor maternal malnutrition include pre-pregnancy BMI, weight gain, and

[Nutritional support in acute kidney injury].

Optimal nutritional requirements and nutrient intake composition for patients with acute kidney injury is still a partially unresolved issue. Targeting nutritional support to the actual protein and energy needs improves the clinical outcome of critically ill patients. So far, very few data are currently available on this topic in acute kidney injury. In this specific clinical condition, the risk for under- and overfeeding may be increased by factors interfering on nutrient need estimation, such as rapidly changing body weight due to fluid balance variations, nutrient losses and hidden calorie

Estrogen: a master regulator of bioenergetic systems in the brain and body.

Estrogen is a fundamental regulator of the metabolic system of the female brain and body. Within the brain, estrogen regulates glucose transport, aerobic glycolysis, and mitochondrial function to generate ATP. In the body, estrogen protects against adiposity, insulin resistance, and type II diabetes, and regulates energy intake and expenditure. During menopause, decline in circulating estrogen is coincident with decline in brain bioenergetics and shift towards a metabolically compromised phenotype. Compensatory bioenergetic adaptations, or lack thereof, to estrogen loss could determine risk of

alpha-Lipoic acid prevents diabetes mellitus in diabetes-prone obese rats.

Several lines of evidence have suggested that triglyceride accumulation in skeletal muscle and pancreatic islets is causally related to type 2 diabetes mellitus. We recently showed that alpha-lipoic acid (ALA), a potent antioxidant and cofactor of mitochondrial respiratory enzymes, reduces body weight of rodents by suppressing food intake and increasing energy expenditure. We sought to determine if ALA can prevent the development of diabetes mellitus in obese Otsuka Long-Evans Tokushima Fatty (OLETF) rats. Most (78%) untreated OLETF rats showed glycosuria at 40 weeks of age, but this was compl

Sibutramine sensitivity assay revealed a unique phenotype of bombesin BB3 receptor-deficient mice.

Sibutramine sensitivity assay in genetically obese (bombesin BB3 receptor (BRS-3)-deficient mice, KK-Ay mice, db/db mice and Zucker obese rat) and wild-type animals was examined. The sensitivity of Sibutramine (10 mg/kg, p.o.) in BRS-3-deficient mice was retained as well as normal animals; however, it was decreased in KK-Ay, db/db mice and Zucker obese rat. The suppression values of food intake in BRS-3-deficient, KK-Ay, db/db mice and Zucker obese rat were 49.8+/-5.8%, 16.1+/-4.7%, 0.1+/-2.8% and -2.0+/-2.2% (mean +/- S.E.), respectively. Posttransplant bone disease: a case illustrating dramatic improvements in bone density with vitamin D replacement therapy.

Although bisphosponates are proposed as first-line treatment for posttransplant bone disease they are not optimal in all situations. A kidney transplant recipient developed hypercalcemia from mobilization of extraskeletal calcium. He had low serum parathyroid hormone and vitamin D; high calcium excretion; and normal calcium intake. Bone biopsy revealed severe osteomalacia. Bisphosphonates, used in the early treatment of acute hypercalcemia, were not indicated to treat osteomalacia. However, over several months serum calcium declined sufficiently to allow treatment of the bone disease with oral
