RATIONALE: Castleman disease (CD) is a rare lymphoproliferative disorder characterized by nonmalignant lymph node enlargement, often associated with systemic symptoms. It is classified into unicentric disease (involving a single enlarged lymph node) and multicentric disease (affecting multiple lymph node stations). In some cases of idiopathic multicentric Castleman disease (iMCD), elevated levels of various serum inflammatory markers are observed, and histologically, the lymph node enlargement resembles that caused by autoimmune diseases, making diagnosis challenging.

Nicolau syndrome is a rare complication of the parenteral application of various drugs. It is characterized by the appearance of pain, followed by edema, erythema, and then a necrotic plaque. Carbamazepine and other anticonvulsants are commoner cause of severe adverse cutaneous drug reactions such as erythema multiforme, toxic epidermal necrolysis (TEN), and Stevens-Johnson syndrome (SJS). We report a case of SJS rapidly progressing to TEN with a combination of haloperidol and carbamazepine in a patient with bipolar affective disorder. The pathophysiological mechanism underlying this reaction is discussed.

Yersinia pseudotuberculosis belongs to the family Enterobacteriaceae and is known to cause enterocolitis, terminal ileitis, pseudoappendicitis, erythema nodosum, reactive polyarthritis, and, occasionally, bloodstream infections.
