Identifying biomechanical and neurophysiological risk factors for postoperative neurologic deterioration in OPLL surgery: A study using ROC curve and path analysis.

OBJECTIVE: Ossification of the posterior longitudinal ligament (OPLL) can lead to compressive myelopathy, which requires surgical intervention. This study aimed to evaluate biomechanical and neurophysiological predictors of postoperative motor (PMD) and sensory deterioration (PSD) in cervical OPLL surgery, with the goal of improving patient prognosis and surgical outcomes. METHODS: A retrospective cohort study was conducted on 111 patients with cervical OPLL who underwent surgery with intraoperative neurophysiological monitoring during 5 years in a single institute. The axial size of OPLL, in

Faces emotional expressions: from perceptive to motor areas in aged and young subjects.

The role of age in perception and production of facial expressions is still unclear. Therefore, this work compared, in aged and young subjects, the effects of passive viewing of faces expressing different emotions on perceptive brain regions, such as occipital and temporal cortical areas and on the primary motor cortex (M1) innervating lower face muscles. Seventeen young (24.41 ± 0.71 yr) and seventeen aged (63.82 ± 0.99 yr) subjects underwent recording of event-related potentials (ERP), of motor potentials evoked by transcranial magnetic stimulation of face M1 in the depressor anguli oris mus

Experimental muscle hyperalgesia modulates sensorimotor cortical excitability, which is partially altered by unaccustomed exercise.

Impaired corticomotor function is reported in patients with lateral epicondylalgia, but the causal link to pain or musculotendinous overloading is unclear. In this study, sensorimotor cortical changes were investigated using a model of persistent pain combined with an overloading condition. In 24 healthy subjects, the effect of nerve growth factor (NGF)-induced pain, combined with delayed-onset muscle soreness (DOMS), was examined on pain perception, pressure pain sensitivity, maximal force, and sensorimotor cortical excitability. Two groups (NGF alone and NGF + DOMS) received injections of NG

Preservation of the long insular artery to prevent postoperative motor deficits after resection of insulo-opercular glioma: technical case reports.

Resection of insulo-opercular gliomas carries the risk of postoperative hemiparesis caused by ischemia of the corona radiata resulting from injury to the long insular arteries. However, intraoperative identification of these perforating arteries is challenging. We attempted intra-operative motor evoked potential (MEP) monitoring under temporary occlusion of the suspected long insular artery arising from the opercular portion of middle cerebral artery in two patients with insulo-opercular gliomas. Temporary occlusion of the artery caused decrease in MEP amplitude, which recovered after release

Separating neural and vascular effects of caffeine using simultaneous EEG-FMRI: differential effects of caffeine on cognitive and sensorimotor brain responses.

The effects of caffeine are mediated through its non-selective antagonistic effects on adenosine A(1) and A(2A) adenosine receptors resulting in increased neuronal activity but also vasoconstriction in the brain. Caffeine, therefore, can modify BOLD FMRI signal responses through both its neural and its vascular effects depending on receptor distributions in different brain regions. In this study we aim to distinguish neural and vascular influences of a single dose of caffeine in measurements of task-related brain activity using simultaneous EEG-FMRI. We chose to compare low-level visual and mo

Cortical and spinal abnormalities in psychogenic dystonia.

OBJECTIVE: The pathophysiology of psychogenic dystonia has not been examined, but a growing body of literature suggests that abnormal sensory input from repetitive movements can lead to plastic cortical changes. Reduced cortical and spinal inhibition is well documented in organic dystonia. We tested the hypothesis that aberrant sensory input associated with abnormal posture may cause similar abnormalities by testing patients with psychogenic dystonia. METHODS: We assessed cortical and spinal inhibitory circuits and cortical activity associated with voluntary movement in 10 patients with clini

Open-door laminoplasty for cervical stenotic myelopathy: surgical technique and neurophysiological monitoring.

OBJECT: A modified technique of open-door laminoplasty for cervical stenotic myelopathy (CSM) is described, and the role of evoked potential monitoring in selecting patients for surgery and evaluating results is discussed. METHODS: Between October 1992 and October 1996, 33 patients with CSM underwent open-door laminoplasty. After surgery, in 27 patients (81.8%) different levels of clinical improvement were demonstrated, and in five of them (15%) full recovery was observed. The Japanese Orthopaedic Association score increased from 5 to 12 (mean 9.8) preoperatively to 8 to 14 (mean 11.6) postop

Intraoperative monitoring of auditory function: experimental observations and new applications.

The three different methods of intraoperative monitoring, namely, auditory brainstem response evaluation (ABR), electrocochleography (ECoG), and direct eighth nerve compound action potentials monitoring are reviewed. Both ECoG and direct eighth nerve monitoring surpass the ABR in their ability to provide fast, almost instantaneous, large, reproducible potentials in response to click stimuli. Changes in the amplitude and the latency of these potentials alert the surgeon of an impending eighth nerve injury. Human clinical observations revealed instances of persistence of the ECoG despite a total

Remote Intraoperative Monitoring of Cochlear Implants.

Intraoperative measurements are valuable tools in cochlear implant (CI) surgery, enabling the determination of implant functionality while the patient is still in the operating room. Standard measurements include impedances for stimulating and non-stimulating contacts, triggering of the stapedial reflex with or without estimating the threshold, and assessing cochlear nerve function by measuring electrically evoked compound action potentials. Additional measurements can also be performed. For example, electrocochleography can be used to observe and help preserve residual hearing in patients. Us

Attentional resource allocation in the early stages of motor skill learning.

The relationship between attentional resource allocation and early motor skill learning remains unclear. This study examined how proficiency in drone operation influences attention and which factors contribute to individual differences. Twenty-three participants with no prior drone experience performed a simulator task consisting of 12 three-minute trials, divided into three phases of four trials each. We estimated attentional resource allocation using auditory-evoked potentials (AEPs) elicited by task-irrelevant auditory probes. Performance improved significantly across phases (ps < 0.001), a

Perturbational signature of the somato-cognitive action network in human primary motor cortex.

INTRODUCTION: Functional-MRI evidence recently identified inter-effector regions in the primary motor cortex that show preferential connectivity with the action mode network (AMN), constituting the somato-cognitive action network (SCAN). Direct causal evidence for the functional engagement of these regions remains limited. AIM: Here, we investigated the perturbational neural response of one inter-effector region using transcranial magnetic stimulation combined with electroencephalography (TMS-EEG). METHODS: Sixteen healthy participants underwent recording of transcranial evoked potentials fr

NOTCH2NLC GGC repeat expansions cause retinal neurodegeneration in neuronal intranuclear inclusion disease mouse model.

Neuronal intranuclear inclusion disease (NIID) patients frequently exhibit ocular abnormalities, yet the pathogenic mechanisms remain unclear. Using a transgenic mouse model ubiquitously expressing NOTCH2NLC with 98GGC repeats, we revealed that polyglycine aggregates, translated from the expanded GGC repeats, predominantly localize in PAX6- and RBPMS-positive cells, accompanied by retinal neurodegeneration and thinning especially in the inner retinal layers. Functional assessment through visual evoked potentials demonstrated significantly reduced amplitudes and prolonged latencies in NIID mice

The Modulatory Effects of Anesthetics and Analgesics on Neurophysiological Monitoring and Underlying Mechanisms.

Intraoperative Neurophysiological Monitoring (IONM) is an indispensable surgical tool that offers invaluable insights into neurological function across a spectrum of anatomical areas. By comprehensively assessing the integrity of the brain, brainstem, spinal cord, cranial nerves, and peripheral nerves, IONM plays a pivotal role in guiding surgical decision-making and optimizing patient outcomes, particularly in the context of high-risk procedures. Intraoperative drugs, especially anesthetics and/or analgesics, differentially modulate neurophysiological monitoring, which remarkably affects the

Remission of myasthenia gravis following infection with SARS-CoV-2: A case report.

This case report describes an unprecedented remission of ocular Myasthenia Gravis (MG) in a 3-year-old girl following a SARS-CoV-2 infection. Both MG and SARS-CoV-2 influence the cholinergic system, and this case underscores the importance of considering multiple factors in autoimmune disorders and the potential impact of viral infections on their course. The patient initially presented with ptosis and limited eye movement. Her diagnosis of seronegative MG was confirmed with abnormal visual evoked potentials and positive response to pyridostigmine. Her ptosis and limited extraocular muscle mov

Revisiting Coactivation ("Froment's Maneuver") in Parkinson's Disease: A Physiological Approach.

BACKGROUND: In the 1920s, Jules Froment extensively studied conditions that might exacerbate rigidity in persons with Parkinson's disease (PD). OBJECTIVE: This cross-sectional, controlled study aimed at investigating the physiological basis, in particular motor cortical contribution to enhanced rigidity during contralateral extremity movements. METHODS: Motor-evoked potentials (MEPs) were recorded in 42 patients with PD and 42 age-matched healthy control subjects. Resting and active motor thresholds, MEP latency and amplitude, contralateral (cCSP) and ipsilateral cortical silent period (iCSP

Methotrexate-induced neurotoxicity: Diagnostic challenges and the role of neurophysiological testing.

Methotrexate (MTX) is widely used in the treatment of acute lymphocytic leukemia (ALL) and other onco-hematological conditions. Although subacute MTX-related neurotoxicity is relatively rare, it can present with dramatic, fluctuating neurological deficits that mimic other serious conditions. Diagnosis may be complicated by the frequent presence of asymptomatic white matter abnormalities on MRI, commonly attributed to chronic MTX toxicity. We report an 18-year-old ALL patient who developed severe, fluctuating neurological symptoms 11 days after the third intrathecal administration of MTX. MRI s
