Bone scintigraphy, which is one of the most frequently performed tests in oncological patients, may reveal many non-oncological disorders of bone metabolism. As an example, presented is a case of a patient with a history of lung cancer, in whom a bone scan showed an abnormal accumulation of osteotropic radiopharmaceutical in the femur. An additional single photon emission computed tomography/computed tomography (SPECT/CT) acquisition allowed for the rare diagnosis of myositis ossificans.

The ideal size of intramedullary device to fix corrective osteotomy of proximal femur in abnormal bone in children and small patients may not be easily available. We report the successful use of Rush rod in combination with multiple Kirschner wires to fix the corrective osteotomy of coxa vara and shepherd crook deformity in two patients with osteogenesis imperfecta and fibrous dysplasia. The union was achieved on time, neck shaft angle and rotation were maintained.

Hydatid disease is an infrequent parasitic infestation caused by cestode, most commonly, Echinococcus granulosus. Bone involvement is distinctly uncommon. We would like to share our experience of a rare case of hydatid disease of femur in a 24-year-old male who presented with nonunion of subtrochanteric fracture. Histopathology showed typical lamellated wall and dagger-shaped hooklets. In view of its rarity, hydatid disease often remains an unsuspected infection of the bone.

Although the major mechanism of complex regional pain syndrome (CRPS) involves dysfunctional central or sympathetic nervous system activation, the peripheral nervous system also contributes significantly to its clinical manifestations. Pulsed radiofrequency (PRF) is a recently developed treatment option for neuropathic pain syndromes. Here, we report a case of CRPS Type II after a femur fracture and sciatic nerve injury, in which the pain was treated successfully with ultrasonography-guided selective sciatic nerve PRF application.

Fibular Hemimelia is a rare congenital disorder of deficiency to complete absence of fibula. It has been associated with post axial skeletal anomalies like shortening of femur, absence of lateral rays of foot and syndactyle, polydactyle etc. We report this case because of associated amniotic constriction bands over both the forearm, suggesting amniotic bands may be one of the causal factors of Fibular Hemimelia due to insult to the growing limb bud.

We describe 2 cases of intrauterine gangrene involving the lower extremity in 2 unrelated neonates. Both cases were complicated by prematurity, and 1 case was complicated by an intrauterine distal femur fracture and twin-twin transfusion syndrome. Both cases resulted in profound ischemic necrosis from the knee to the foot, requiring knee disarticulation. In both cases, a follow-up period of 7 years is now completed, and no further medical or surgical complications have arisen.

Fibrous tethers are a rare cause of unilateral limb deformity. A review of the literature revealed 9 cases previously described, all located in the distal femur. We report 3 additional cases of fibrous tethers, 2 of which were located in anatomical areas not previously described. The fibrous tethers were excised in all cases, and an osteotomy was performed in 2 of the cases. The clinical, radiological, and histopathologic findings are discussed for each case.

Interruption of the blood flow may occur in intra-osseous arteries within the femoral head. We report a 72-year-old woman who developed osteonecrosis of the femoral head 11 months after surgery involving massive cementing of a segmental distal femoral rotating hinge prosthesis to treat nonunion of the distal femur. The bone cement filled the cavity up to the femoral neck and the superolateral portion of the femoral head, blocking the bone marrow.

We present a young man who developed fat embolism syndrome following a fractured femoral shaft. By intermittently measuring oxygen saturation with a pulse oximeter and varying the inspired partial pressure of oxygen we were able to quantify the development of shunt and ventilation/perfusion (V/Q) mismatch over the course of his illness. Shunt and low V/Q gradually improved in the week following admission but deteriorated following general anaesthesia for nailing of the femur.

A 50-year-old man had metastatic pheochromocytoma to the left frontal bone. The primary adrenal tumor was removed 12 years previously. Bone scanning and diagnostic I-131 and I-123 MIBG imaging showed metastatic lesions in the right femur and midthoracic spine. However, post-therapy I-131 MIBG scanning showed extensive and widespread metastatic disease. Post-therapeutic I-131 MIBG whole-body scanning was necessary to more fully assess the extent of metastatic pheochromocytoma.

Necrosis of fatty bone marrow is an unusual complication of several pancreatic disorders. We describe a patient with polyarthritis, sterile subcutaneous abscess and osteolysis arising during the course of alcoholic chronic pancreatitis. MR images of one knee showed multiple foci of abnormal signal intensity within the marrow of the distal femur and proximal tibia, consistent with intraosseous fat necrosis. CT scans showed significant changes in the cancellous bone in these areas compatible with metaphyseal osteonecrosis.

The radiographic findings of a 15-year-old Brazilian male with diagnosis of multiple myeloma are described. He presented with claudication and recent onset of tender painful swelling of the right mid leg. Radiographs showed wide spread "soap bubble" lesions of the skull, long bones, spine, pelvis, ribs, shoulders, and clavicles. The diagnosis was confirmed by the presence of a plasmacytoma on tissue biopsy (femur), serum IgG gammopathy and Bence-Jones proteinuria.

A 28-year-old woman developed osteoporosis following seven years of neuroleptic use. She presented with amenorrhea and profuse galactorrhea of four years' duration. Dual photon absorptionometry demonstrated reduced bone mineral density in the femur and spine. Serum calcium, phosphorus and alkaline phosphatase were normal. The patient was started on bromocriptine, and her bone density, serum prolactin, dehydroepiandrosterone sulfate, and free and total testosterone improved. No deterioration in her psychiatric condition occurred.

Femoral head resection with valgus subtrochanteric osteotomy was performed on six hips in five nonambulatory adolescent patients with painful, chronically dislocated hips due to spastic paralysis. This procedure was successful because it led to pain relief, ease of perineal care, and facility of seating. Complications, such as proximal migration of the remaining femur, recurrence of adduction deformity, hip stiffness, and excessive heterotopic bone formation, common to other procedures used for this condition, have not occurred.

The authors present a series of 26 cases of ipsilateral fractures of the shafts of the femur and tibia treated by internal fixation. Twenty have been followed up for more than 2 years. Thirteen healed without incident and achieved excellent function. Six others regained a similar level of mobility after complications had been treated. The early surgical stabilisation of such fractures is advocated, allowing recognition of simultaneous injuries to ligaments of the knee.

Three-dimensional finite element analyses were carried out on idealisations of two situations where bone remodelling has been reported clinically viz. the malaligned femur and a maxillary central incisor under orthodontic loading. The mechanical responses, i.e. stress, strain and change of curvature, in various directions, are presented. They are compared with the biological response, either bone resorption or bone deposition. The results are discussed in relation to three current theories of bone remodelling.

Malignant transformation of a fibrous dysplasia into an osteosarcoma is very rare. We report the case of an 84-year-old man with telangiectatic osteosarcoma of the upper femur arising in a previous fibrous dysplasia also known as liposclerosing myxofibrous tumor. The tumor was expressing the epithelial membrane antigen. This is the first described case of a malignant transformation into an osteosarcoma arising in a liposclerosing myxofibrous tumor. We discuss the main differential diagnosis with a review.

Subacute osteomyelitis (Brodie's abscess) is essentially a problem of diagnosis, and there may be considerable difficulty in distinguishing it from other benign and malignant bone lesions. Though reported in the metaphyseal region of the femur, Brodie's abscess is rarer in the femoral neck. The authors present a case of Brodie's abscess in the femoral neck, which clinico-radiologically simulated an osteoid osteoma. Retrospectively, the presence of a cortical sinus tract should have aroused suspicion.

We present the clinical and radiological results of two patients with valgus deformity of the knee who underwent an open supracondylar osteotomy of the femur. A biocoral wedge was inserted in the lateral aspect of the osteotomy to maintain the correction without internal fixation. At 15 and 13 months, respectively, after the operation, tomograms showed correction of the deformity, sound healing of the osteotomy, partial absorption of the coral with complete incorporation of the graft within the bone and no complications.

Despite many reports of injuries to surgeons during operative procedures, there is no record of an eye injury caused by a foreign body. Orthopaedic surgeons are particularly vulnerable to such injury. An instance in which a penetrating eye injury occurred while hammering a rasp into the femur during a hip replacement is described. There is a potential oblique trajectory for a foreign body to reach the eye from the operative field despite the use of a visor for eye protection.
