[Anatomic relationship between the pronator quadratus and brachioradialis muscle insertion: implications for repair of the muscle after volar plate fixation of distal radius fractures].

BACKGROUND: In open reduction and volar plate fixation of distal radius fractures, the pronator quadratus (PQ) muscle is usually detached at the radial edge of the distal radius. Repair of the muscle is reasonable for coverage of the plate and to maintain normal pronation of the forearm. However, repair of the muscle is not always satisfactory. In this study, the topographic relationship of both the PQ and the brachioradialis muscle (BR) insertions are investigated with regard to optimising the repair of the PQ in open reduction and volar plate fixation of distal radius fractures. Total hip replacement versus open reduction and internal fixation of displaced femoral neck fractures: a randomized long-term follow-up study.

BACKGROUND: Clinical trials with short and intermediate-term follow-up have demonstrated superior results for total hip replacement as compared with internal fixation with regard to hip function and the need for secondary surgery in elderly patients with a displaced intracapsular femoral neck fracture. The aim of the present study was to compare the results of total hip replacement with those of internal fixation over a long-term follow-up period of seventeen years. METHODS: We enrolled 100 patients who had sustained a femoral neck fracture in a single-center, randomized controlled trial;all

Outcome of total elbow replacement for distal humeral fractures in the elderly: a comparison of primary surgery and surgery after failed internal fixation or conservative treatment.

We analysed the outcome of the Coonrad-Morrey total elbow replacement used for fracture of the distal humerus in elderly patients with no evidence of inflammatory arthritis and compared the results for early versus delayed treatment. We studied a total of 32 patients with 15 in the early treatment group and 17 in the delayed treatment group. The mean follow-up was 56.1 months (18 to 88). The percentage of excellent to good results based on the Mayo elbow performance score was not significantly different, 84% in the early group and 79% in the delayed group. Subjective satisfaction was 92% in bo

A randomised clinical study comparing palmar and dorsal fixed-angle plates for the internal fixation of AO C-type fractures of the distal radius in the elderly.

Current surgical treatments for distal radial fractures include dorsal and palmar plate fixation. We report results of a randomised study comparing these methods for AO C1-3 fractures. The emphasis was placed on the early postoperative functional recovery within the first 6 months as this interval is of decisive importance for elderly patients. Thirty patients with unilateral AO C1-3 fractures were enroled, 15 were treated with a palmar plate and 15 received a dorsal Pi-plate. Results were assessed 6 weeks, 3 months and 6 months postoperatively focusing on functional recovery. The palmar plate

[Intramedullary fixation of humerus shaft fractures. An analysis of complications of 2 implants with special reference to outcome after management with the unreamed humerus interlocking nail].

INTRODUCTION: Operative treatment of humeral shaft fractures has gained a new impetus with the development of intramedullary interlocking systems for the humerus. Because of the anatomical structure of the humeral marrow cavity, a regular jamming of nail into bone is not to be expected, so that the stability of these systems needs to be achieved through an interlocking mechanism. Among the commercially available interlocking nailing systems the unreamed humeral nail (UHN) and the Seidel-nail (SHN) can be regarded as the standard implants. DESIGN: In a retrospective study comparing two groups

Clinical Outcomes of Single Intramedullary K-Wire Fixation with Figure-of-Eight Suture Augmentation for Ulnar Styloid Base and Distal Ulnar Metaphyseal Fractures.

Background/Objectives: The optimal fixation method for ulnar styloid base fractures remains controversial. Conventional dual K-wire fixation may be technically challenging in certain fracture configurations, whereas hook plate fixation provides rigid fixation but requires additional implant placement and surgical exposure. The purpose of this study was to evaluate the clinical and radiographic outcomes of single intramedullary K-wire fixation combined with figure-of-eight suture augmentation for ulnar styloid base and distal ulnar metaphyseal fractures. Methods: We retrospectively reviewed 60

Is Fixation Strategy Associated With Complication Risk in Ogden Type IV Tibial Tubercle Avulsion Fractures? A Multicenter Study of the Tibial Tubercle Study Database.

BACKGROUND: The optimal surgical treatment of Ogden Type IV tibial tubercle avulsion fractures (TTAFs) remains controversial, given this injury pattern's rarity and its multiplanar involvement of the proximal tibial physis. Small, single-center studies suggest type IV fractures have higher complication rates and more variable fixation constructs than other TTAF types. The purpose of this study is to delineate fixation constructs among surgically managed type IV TTAFs and identify factors associated with postoperative complications. METHODS: A retrospective, multicenter cohort study was conduc

Humeral Fractures in Metabolically Compromised Bone: Outcomes of Structural Strut Onlay Allograft with Open Reduction and Internal Fixation - A Case Series of Three Patients.

INTRODUCTION: The "terrible tetrad" of the elbow - simultaneous posterior ulno-humeral dislocation, comminuted radial head fracture with dislocation, coronoid process fracture, and proximal ulnar fracture - is an injury pattern that simultaneously fulfills criteria for both the terrible triad and a Monteggia-type fracture-dislocation. Fewer than fifteen cases have been documented worldwide. We present two cases managed by different surgeons at different institutions and propose the formal adoption of the term "terrible tetrad." CASE REPORT: Case 1: A 50-year-old male painter sustained a right

Comparative clinical outcomes of suprapatellar intramedullary nailing vs. minimally invasive plate osteosynthesis fixation for distal tibial fractures: a retrospective cohort study.

INTRODUCTION: Distal tibial fractures are challenging injuries due to limited soft tissue coverage and complex biomechanics. Suprapatellar intramedullary nailing (IMN) and minimally invasive plate osteosynthesis (MIPO) are widely used; however, their comparative effectiveness remains controversial. This study compared perioperative parameters, functional outcomes, radiographic results, and complications to inform surgical decision-making. METHODS: This retrospective cohort study included 70 patients with distal one-third tibial fractures treated with suprapatellar IMN (n = 35) or MIPO (n = 35

Outcomes of immediate versus delayed weight-bearing after cannulated screw fixation for valgus-impacted and nondisplaced femoral neck fractures in older adults.

BACKGROUND: Valgus-impacted and nondisplaced femoral neck fractures in older adults are commonly treated with cannulated screw fixation. However, the optimal timing for initiating postoperative weight-bearing remains controversial. This study aimed to compare immediate weight-bearing (IWB) as tolerated versus delayed weight-bearing (DWB) to determine whether IWB could improve patient outcomes without increasing the risk of surgical complications. METHODS: A total of 136 patients aged 65 years or older with valgus-impacted or nondisplaced femoral neck fractures treated using three cannulated s
