[Vascular injuries as a complication of fractures, dislocations and surgical interventions].

This is a presentation of 34 patients with traumatic or intraoperative vascular injuries treated at the Rural Clinic am Plattenwald during the timeperiod from 1978 until 1988. Limb amputation was performed in three cases. Two patients died. Our goal is to show the importance in early recognition and treatment of such injuries which requires an efficient interdisciplinary cooperation between orthopedic and vascular surgeons, as well as the radiologist. The employment of angiography should be generous.

Prevalence and Patterns of Multiple Injuries Affecting a Single Permanent Tooth (Combination Injuries): A Systematic Review and Meta-Analysis.

BACKGROUND: Combination injuries or multiple traumatic dental injuries (TDI) affecting a single permanent tooth are clinically significant but remain underrecognized. Although the global prevalence of TDI is well documented, the epidemiology of combination injuries has not been clearly established. This systematic review and meta-analysis aimed to assess the prevalence and patterns of combination injuries in traumatized permanent teeth. METHODS: A review protocol was registered in PROSPERO (CRD1245967). Comprehensive searches of PubMed, EMBASE, Web of Science, and Scopus were conducted withou

Osteoporotic vertebral compression fracture in patients with childhood-onset lupus nephritis: prevalence and risk factors.

UNLABELLED: Vertebral fractures (VF) can occur due to childhood-onset lupus nephritis (cLN), which needs long-term use of immunosuppressants that affect bone health. This study found VF in 26.7% of cLN patients. Risk factors included low lumbar spine BMD Z-scores and high disease activity (SLEDAI-2K ≥ 20). Bone-strengthening and disease management strategies are important. PURPOSE: Pediatric vertebral fracture (VF) may occur under conditions that lead to decreased bone mass. Childhood-onset lupus nephritis (cLN) requires treatment with glucocorticoids and immunosuppressants. This study aimed

Beyond the short-term relief: Outcomes of geriatric rib fracture patients receiving paravertebral nerve blocks and epidural analgesia.

BACKGROUND: Adequate pain control is a critical component of rib fracture management. Our study aimed to evaluate the in-hospital and post-discharge outcomes of geriatric rib fracture patients who received paravertebral nerve block (PVNB) versus epidural analgesia (EA) on a national level. METHODS: We performed a 5-year (2011-15) retrospective analysis of the Nationwide Readmission database. We included all the geriatric (≥65 years) blunt trauma patients with rib fractures who received a paravertebral nerve block (PVNB) or Epidural analgesia (EA) for chest injuries. We excluded patients who w

Reversal of direct oral anticoagulants in adult hip fracture patients. A systematic review and meta-analysis.

BACKGROUND: Increasing numbers of patients are taking Direct Oral Anticoagulants at the time of hip fracture. Evidence is limited on how and if the effects of DOAC's should be reversed before surgical fixation. Wide variations in practice exist. We conducted a systematic review to investigate outcomes for three reversal strategies. These were: "watch and wait" (also referred to as "time-reversal"), plasma product reversal and reversal with specific antidotes. METHODS: A systematic search was conducted using multiple databases. Results were obtained for studies directly comparing different DOA

Risk of fracture of the acromion depends on size and orientation of acromial bone tunnels when performing acromioclavicular reconstruction.

PURPOSE: Current techniques for anatomic repair of the dislocated acromioclavicular (AC) joint aim on reconstruction of the AC ligaments and utilize tunnels drilled through the acromion . This improves the stability of the reconstruction but might also increase the risk of fractures at the acromion. The purpose of this study was to evaluate the fracture risk for the acromion after transacromial tunnel placement for anatomic AC joint stabilization procedure. It was hypothesized that the risk of fracture of the acromion is correlated to size and orientation of bone tunnels commonly used for anat

Altered distributions of bone tissue mineral and collagen properties in women with fragility fractures.

Heterogeneity of bone tissue properties is emerging as a potential indicator of altered bone quality in pathologic tissue. The objective of this study was to compare the distributions of tissue properties in women with and without histories of fragility fractures using Fourier transform infrared (FTIR) imaging. We extended a prior study that examined the relationship of the mean FTIR properties to fracture risk by analyzing in detail the widths and the tails of the distributions of FTIR properties in biopsies from fracture and non-fracture cohorts. The mineral and matrix properties of cortical

MRI Correlation of Radial Head Fractures and Forearm Injuries.

Background: Radial head fractures can be associated with soft tissue injuries of the forearm and wrist. The Essex-Lopresti injury can lead to significant morbidity, especially if the diagnosis is not made acutely. Better identification of such injuries is needed to allow optimal surgical treatment and prevent long-term sequelae. We used magnetic resonance imaging (MRI) to correlate the degree of soft tissue injuries with radial head fractures. Methods: Sixteen pairs of forearms with an associated radial head fracture in 15 patients prospectively underwent an MRI within 2 weeks of their injury.

Teriparatide treatment complicated by malignant myeloma.

Recombinant human parathyroid hormone (1-34) (rhPTH 1-34), teriparatide (Forsteo in Europe), is a new compound that has been introduced and shown to be successful in the treatment of osteoporosis. The mechanisms of action include a pulsative influence on the RANKL/OPG system resulting in osteoblast activation and increased bone formation by teriparatide. In malignant myeloma there is an imbalance between osteoclast and osteoblast activity with involvement of the RANKL/OPG system among others. We report a case with monoclonal gammopathy of uncertain significance (MGUS) who developed malignant m

[Treatment of the combined pelvic and thoracic trauma in the emergency room].

Priorities in the diagnosis and treatment of a multiple trauma with injuries to the thorax and pelvis are usually determined by the pattern and the severity of the injury sustained. In this case a haemodynamically unstable patient with severest pelvic trauma and a moderate thoracic trauma developed progressive haemodynamic instability during an intervention to the pelvis, resulting in a lethal outcome for the patient. The cause only became obvious when computed tomography of the thorax (CTT) was performed which was able to demonstrate venous pulmonary bleeding compressing the left atrium. In h
