Complications of operative treatment of clavicle fractures in a Level I Trauma Center.

Traditionally, the belief is that the clavicle heals readily and with nonoperative treatment. Since evidence suggests that many clavicles benefit from operative care, the aim of this study was to find the incidence of complications after operative treatment of clavicle fractures. This retrospective study includes 56 patients (41 male and 15 female) who were treated operatively for clavicle fractures between 2010 and 2012. The indications were displacement more than 20 mm, comminution, tenting of the skin, floating shoulder, and open fractures. Seventy-five percent of the fractures occurred in

[Hip fractures].

Hip fractures are ranked among the frequent injuries. These fractures have been often coupled with high energy trauma in children and in patients with normal bone structure, low energy trauma and osteoporotic fracture (fragility fracture) is typical in elder patients. Hip fractures are divided into five groups: femoral head fracture, femoral neck fracture, pertrochanteric, intertrochateric and subtrochanteric fracture. Surgical treatment is indicated in all patients unless contraindications are present. Long bed rest has been accompanied by a high risk of development of thromboembolic disease,

Extraarticular hand fractures in adults: a review of new developments.

UNLABELLED: This report cites new developments in the treatment of extra-articular hand fractures in adults. Recent reports confirm that small amounts of metacarpal shortening or dorsal angulation cause minimal functional impairment. Unilateral excision of the lateral band and oblique fibers of the extensor apparatus of the metacarpophalangeal joint facilitates proximal phalangeal fracture exposure and may improve functional recovery. Results using open mini screw fixation of oblique extra-articular metacarpal and phalangeal fractures may be comparable to those of percutaneous Kirschner wire f

[Operative treatment of complex acetabular fractures through the modified extensile iliofemoral approach].

Fourty-nine patients with 50 complex acetabular fractures were treated with open reduction and internal fixation from August 1993 to February 1996 in a prospectively documented study. The fractures were fixed with small fragment implants through a modified extensile iliofemoral approach (Maryland approach). Thirty-seven patients were polytraumatized with an ISS of 20 pts. on average. In 10 patients the ISS was > 25 pts. Good or anatomic initial fracture reduction with a fragment stepoff K 2 mm could be achieved in 48 fracturers (96 %). Thirty-six patients with 37 fractures could be evaluated a

[Complex trauma of the knee joint. Diagnosis--management--therapeutic principles].

The complex nature of combined fractures and soft tissue injuries of the distal femur and proximal tibia needs special attention and specific management. Distal femoral and proximal tibial fractures in young patients are usually caused by high-energy trauma. They are complicated by a high rate of systemic and local injuries to cartilage, ligaments and skin. This small but important group with severe injuries needs a detailed treatment algorithm, because despite the treating surgeon's skill, enthusiasm and wishful thinking, these injuries frequently lead to unsatisfactory results. The combinati

The effect of the Tscherne injury pattern on the outcome of operatively treated Lisfranc fracture dislocations.

BACKGROUND: Lisfranc fracture dislocations cause severe tarso-metatarsal malalignment. The research question of this study was to evaluate the severity of the soft tissue injury on the final clinical outcome and compare that with the effect of various determinants on the disability in daily living activities after open reduction and internal fixation of a Lisfranc injury. METHODS: This study consisted of a retrospective analysis of patients with Lisfranc fracture dislocations who were treated by open reduction and internal fixation beween 2004 and 2009. Evaluation focused mainly on the severi

Management of Bilateral Condylar Fracture With ORIF Using the Puppet Technique in the Adolescence: A Case Report.

INTRODUCTION: Condyle fracture may result in functional and cosmetic issues. Malocclusion becomes one of the adverse effects of the fracture that impairs the quality of life. It is crucial for maintaining the mandibular condyle because it is an essential component of jaw movement. The critical decision for the treatment choice in early adolescence has an impact on accurate reduction and rigid fixation. Improper methods of therapy might have major consequences for facial growth and development, with ankylosis as a frequent occurrence. IMPORTANCE: To report a case of bilateral condylar fracture

The effect of ding's screw and tension band wiring for treatment of olecranon fractures: a finite element study.

BACKGROUND: Tension band wiring (TBW) is a common surgical intervention for olecranon fractures. However, high rate of complications such as loss of reduction, skin irritation, and migration of the K-wires were reported up to 80%. Ding's screw tension band wiring (DSTBW) is a new TBW technique that has shown positive results in the treatment of other fracture types. The objective of this study was to evaluate the stability of DSTBW in the treatment of olecranon fractures by finite element analysis. METHOD: We used Ding's screw tension band fixation (DSTBW) and K-wire tension band fixation (TB

Elastic stable intramedullary nailing versus submuscular plating in pediatric diaphyseal femur fractures: a randomized controlled trial among children in South India.

Elastic stable intramedullary nailing (ESIN) is the current preferred method for treating diaphyseal femur fractures in children. Introduction of the submuscular locked plate (SMP) fixation construct has opened the debate on treatment options for pediatric diaphyseal femur fractures in the older children and adolescents. A randomized controlled trial (RCT) protocol was designed to compare ESIN and SMP for diaphyseal femur fractures in children. An open-labelled RCT comparing SMP with ESIN was conducted from January 2013 to June 2016, for children aged 6-15 years with closed, acute femoral diap

Minimally invasive plate osteosynthesis for complex comminuted bone fractures in the Fraser's type II floating knee: a case report.

OBJECTIVE: Floating knee type IIC, according to Fraser's classification, is an uncommon severe injury that typically occurs in polytrauma. In such cases, intra-articular fracture and the high degree of comminution and deformity of the mid-distal femur make fixation challenging. The purpose of this study was to demonstrate that minimally invasive plate osteosynthesis (MIPO) technology can simplify these complex problems and improve patient prognosis. CASE PRESENTATION: A 38-year-old man injured his left leg in a car accident, causing pain, swelling, deformity, and limited mobility on his left

The common comorbidities leading to poor clinical outcomes after the surgical treatment of ankle fracture-dislocations.

BACKGROUND: The ankle fracture-dislocations are a significant traumatic incident for the bone and the soft tissue surrounding the ankle. Bone stabilization, joint immobilization, anatomic reduction and intervention for soft tissue protection should be performed as early as possible. The present study aims to determine the frequency of major comorbidities that can be seen after surgery in patients with ankle fracture-dislocations and the relationship between the trauma mechanism and clinical status with these comorbidities. METHODS: Thirty-eight patients (25 males, 13 females) who underwent su

First dorsal compartment musculotendinous avulsion accompanied by close radial styloid fracture: Case report.

INTRODUCTION: Radial styloid fracture and concomitant first dorsal compartment proximal musculotendinous avulsion is extremely rare injury. This togetherness is difficult to diagnose fully on routine physical examination. PRESENTATION OF CASE: In this study, we present a thirty nine year old male patient who suffered musculotendinous avulsion injury of the extensor pollicis brevis (EPB) tendon and abduc- tor pollicis longus (APL) tendon that is rarely accompanied by a closed, non-displaced radial styloid fracture developed following an in-car traffic accident. Diagnosis of avulsion was made w

History of internal fixation with plates (part 2): new developments after World War II; compressing plates and locked plates.

The first techniques of operative fracture with plates were developed in the 19th century. In fact, at the beginning these methods consisted of an open reduction of the fracture usually followed by a very unstable fixation. As a consequence, the fracture had to be opened with a real risk of (sometimes lethal) infection, and due to unstable fixation, protection with a cast was often necessary. During the period between World Wars I and II, plates for fracture fixation developed with great variety. It became increasingly recognised that, because a fracture of a long bone normally heals with mini

Percutaneous fixation of first metacarpal base fractures using locked K-wires: a series of 14 cases.

The treatment of choice for first metacarpal base fractures is surgical. Open fixation is stable but causes tendinous adhesions. Percutaneous fixation is minimally invasive but is often followed by secondary displacement. Herein, we describe an alternative approach that combines advantages of both techniques through increasing stability of the Iselin technique by externally connecting the K-wires. Our series included 13 men of mean age 28 years. There were 13 fractures, 6 of which were extra-articular; there were 7 Bennett fractures, 5 of which had a large fracture fragment. After reduction, t

Vertical cross contamination of trichloroethylene in a borehole in fractured sandstone.

Boreholes drilled through contaminated zones in fractured rock create the potential for vertical movement of contaminated ground water between fractures. The usual assumption is that purging eliminates cross contamination; however, the results of a field study conducted in a trichloroethylene (TCE) plume in fractured sandstone with a mean matrix porosity of 13% demonstrates that matrix-diffusion effects can be strong and persistent. A deep borehole was drilled to 110 m below ground surface (mbgs) near a shallow bedrock well containing high TCE concentrations. The borehole was cored continuousl

Evaluation of postural stability in patients screened for osteoporosis: A retrospective study of 1086 cases.

BACKGROUND: Although the interactions between balance, falls and fracture risk have been demonstrated in various aspects, the clinical focus of fracture risk evaluation has been on bone properties, e.g., bone mineral density (BMD). Accordingly, the role of balance parameters in the setting of osteoporosis assessment remained to be further explored. RESEARCH QUESTION: Is postural stability assessed by Romberg quiet stance posturography influenced by BMD or prior fragility fractures in a cohort of more than 1000 patients screened for osteoporosis? METHODS: Patients who completed both Romberg t
