[Management of chemotherapy in hemodialysis patients].

BACKGROUND: The increased incidence of cancer in dialysis patients has been discussed since the mid-70s. Consequently, oncologists, nephrologists and pharmacists are increasingly facing challenging situations of cytotoxic drug handling in dialysis patients. In dialysis patients, two main issues must be considered. First, renal function of hemodialysis (HD) patients is no longer functional. Therefore, these patients may necessitate drug dosage reduction, namely drug prescription, must be cautiously checked before administration with appropriate dosage adjustment whenever necessary to ensure eff

Predicting inadequate spirometry technique and the use of FEV1/FEV3 as an alternative to FEV1/FVC for patients with mild cognitive impairment.

INTRODUCTION AND OBJECTIVES: Some patients cannot perform forced vital capacity (FVC). We conducted a study to answer three questions: Can the ability to perform components of spirometry be predicted by the Mini Mental State Examination (MMSE)? What proportion of subjects can perform forced expiratory volume in 3 s (FEV3) but not FVC? Does the forced expiratory volume in 1 s (FEV1)/FEV3 ratio concord with FEV1/FVC ratio in patients with airflow obstruction? METHODS: We conducted a prospective observational study of 267 patients with a mean age of 79 years, including subjects with indicators o

HDL-cholesterol and physical performance: results from the ageing and longevity study in the sirente geographic area (ilSIRENTE Study).

BACKGROUND: High-density lipoprotein (HDL) cholesterol has been hypothesised to be a reliable marker of frailty and poor prognosis among the oldest elderly. We evaluate the relationship of HDL-cholesterol with measures of physical performance, muscle strength, and functional status in older persons aged 80years or older. METHODS: Data are from baseline evaluation of the ageing and longevity study in the Sirente geographic area (ilSIRENTE study) (n = 364). Physical performance was assessed using the physical performance battery score [short physical performance battery (SPPB)], which is based

Methods for analysing recurrent events in health care data. Examples from admissions in Ebeltoft Health Promotion Project.

BACKGROUND: Evaluation of health care contacts from first events alone often misses large amounts of potentially important data and may produce different results than evaluation of all data including recurrent events. OBJECTIVE: We aim to bring the different methodological approaches for analysing longitudinal health care data to the attention of researchers in primary care. METHODS: We used hospital admission data from the Ebeltoft Health Promotion Project, a randomized trial in primary care examining the effect of preventive health checks. Comparisons included three randomized groups: an i

Effects of exercise on body composition and functional capacity of the elderly.

The capacity of older men and women to adapt to regularly performed exercise has been demonstrated by many laboratories. Aerobic exercise results in improvements in functional capacity and reduced risk of developing type II diabetes in the elderly. High-intensity resistance training (above 60% of the 1 repetition maximum) has been demonstrated to cause large increases in strength in the elderly. In addition, resistance training results in significant increases in muscle size in elderly men and women. Resistance training has also been shown to significantly increase energy requirements and insu

Clinical profile of elderly urinary incontinence in Singapore: a community-based study.

The prevalence of urinary incontinence in the elderly aged 65 years and above was found to be 4.6% (42 out of a total of 919 respondents), in a community-based study in Singapore. Urinary incontinence was defined as leakage of urine on at least two occasions in the previous one month. The clinical profile of 30 out of the 42 (71%) subjects were studied. There was equal distribution of male and female subjects. Eighteen (60%) of the subjects were below 75 years of age. Twenty-five (83.3%) subjects had severe incontinence. Twenty-four (80%) had the incontinence for more than one year. Despite th

Endoscopic sphincterotomy for common bile duct calculi in patients with gall bladder in situ considered unfit for surgery.

Endoscopic sphincterotomy (ES) was attempted in 106 patients with common bile duct (CBD) calculi and gall bladders present, who were considered unfit for surgery on the grounds of age and frailty alone (35%) and/or the presence of major medical problems (65%). Endoscopic sphincterotomy was successful in 105 patients (99%). Early ES related complications occurred in 21 patients (19.8%). Twelve hospital deaths occurred (11.3%), although this was due to biliary causes in only five (4.7%) and one of these was moribund on admission. Complications were more frequent in those in whom initial ES did n

Compliance with early postoperative ambulation and Its associated barriers in hepatobiliary surgery patients within an enhanced recovery after surgery framework.

BACKGROUND: Early postoperative ambulation is a core element of the Enhanced Recovery After Surgery (ERAS) pathway for hepatobiliary surgery, yet real-world compliance has been reported to be variable and the patient-level barriers in this surgical population remain incompletely characterised. We therefore retrospectively reviewed the routinely collected ERAS quality-control archive of our hepatobiliary surgical unit to describe the actual compliance status and to identify factors associated with non-compliance. METHODS: We retrospectively extracted the records of 320 consecutive adult patien

Perioperative inflammation in gynecologic oncology: nursing strategies for enhanced recovery and complication prevention.

Major gynecologic oncology surgery produces an acute inflammatory and neuroendocrine stress response superimposed on cancer-associated inflammation, frailty, malnutrition, anemia, and treatment-related physiologic vulnerability. Although this response is necessary for tissue repair, excessive or poorly resolved activation can contribute to pain, postoperative nausea and vomiting, ileus, insulin resistance, endothelial dysfunction, infection, venous thromboembolism, functional decline, and delayed return to oncologic treatment. Enhanced recovery after surgery (ERAS) pathways reduce avoidable st

Immune checkpoint inhibitor combinations across clinically distinct metastatic renal cell carcinoma populations with limited or heterogeneous trial evidence: A systematic review, evidence map, and meta-analysis.

Immune checkpoint inhibitor (ICI) combinations are standard for metastatic renal cell carcinoma (mRCC), but evidence is uneven across distinct populations. We searched PubMed, Embase and Cochrane CENTRAL through June 3, 2026, and synthesized original studies reporting efficacy or safety in non-clear cell RCC, brain, bone or liver metastases, adults aged 75 years or older, poor performance status, trial ineligibility, frailty, renal impairment or hemodialysis, and sarcomatoid differentiation. Independent studies were the analytical unit. Proportions were pooled only when at least three studies

Can Changes in Sit-To-Stand Performance Throughout A 30-S Time Interval Be Used as A Marker of Performance Fatigability in Middle-Aged To Older Adults?

Performance fatigability during the 30-s sit-to-stand (STS) test is not well characterized despite its potential to detect early functional decline. Therefore, this study aimed to quantify temporal changes in power, trunk flexion and movement subphase durations during the 30sSTS, and to examine differences by age and sex. 93 middle-aged adults (50 males and 43 females; mean age 60.5 ± 3.0 years) and 102 older adults (48 males and 54 females; mean age 71.5 ± 5.0 years) performed a 30sSTS. Inertial measurement units (IMUs) mounted over the L4/L5 vertebral level were used to capture sit-to-stand

Development and Validation of a Risk Prediction Model for Depression in Elderly Chinese Patients on Maintenance Hemodialysis: A Multicenter Study.

BACKGROUND: Depression is the most prevalent psychological condition among elderly patients on maintenance hemodialysis, significantly impairing quality of life and increasing readmission and mortality risks. METHODS: This study recruited 871 elderly patients undergoing hemodialysis from nine tertiary hospitals in China between November 2023 and February 2024. Depression symptoms were assessed using the Geriatric Depression Scale (GDS-15), with a score of ≥ 8 indicating clinically significant depressive symptoms. Participants were divided into depressed (n = 333) and nondepressed (n = 538) gr

Heterogeneous grip strength trajectories and clinical determinants in middle-aged and older chinese patients with cardiovascular disease: a group-based trajectory modelling analysis of the CHARLS.

BACKGROUND: Grip strength (GS) is a potent biomarker in cardiovascular disease (CVD). However, evidence is scarce regarding the long-term dynamic changes of GS and their multifactorial determinants. This study aimed to identify distinct GS trajectories and their determinants among middle-aged and older Chinese adults with CVD. METHODS: We analyzed data from 2,189 patients with CVD across three waves (2011, 2013, 2015) of the China Health and Retirement Longitudinal Study (CHARLS). GS was measured using a standardized protocol. Sex-stratified group-based trajectory model (GBTM) was employed to
