Evaluation of the Importance of Stopping Elderly Accidents, Deaths, and Injuries (STEADI)-Based Factors in Wearable Fall Risk Assessment: Secondary Data Analysis.

BACKGROUND: Falls among older adults are a growing and costly public health problem that often leads to mobility decline and loss of independence. Although clinical frameworks such as the Centers for Disease Control and Prevention's (CDC) Stopping Elderly Accidents, Deaths, and Injuries (STEADI) initiative recommend multifactor screening (gait, balance, strength, fear of falling, and fall history), most wearable fall risk assessment systems rely on a small set of risk factors (typically gait), which creates a gap between clinical practice and automated wearable assessment. OBJECTIVE: This stu

Comparing Cognitive-Motor Interference across Younger, Middle-Aged, and Older Adults during Dual-Task Walking.

INTRODUCTION: Walking while listening or talking is a functionally relevant cognitive-motor dual-task (DT) activity that may challenge attentional capacity. DT activity alters gait and increases the risk of falls. The aging process may magnify challenges in DT performance. This study compared cognitive-motor interference (CMI) and DT performance across younger, middle-aged, and older adults during walking under varying memory task types. METHODS: A cross-sectional sample of 32 younger (mean age = 25.2, range 22-32 years old), 18 middle-aged (mean age = 56.2, range 48-64 years old), and 19 old

A Yoga Strengthening Program Designed to Minimize the Knee Adduction Moment for Women with Knee Osteoarthritis: A Proof-Of-Principle Cohort Study.

UNLABELLED: People with knee osteoarthritis may benefit from exercise prescriptions that minimize knee loads in the frontal plane. The primary objective of this study was to determine whether a novel 12-week strengthening program designed to minimize exposure to the knee adduction moment (KAM) could improve symptoms and knee strength in women with symptomatic knee osteoarthritis. A secondary objective was to determine whether the program could improve mobility and fitness, and decrease peak KAM during gait. The tertiary objective was to evaluate the biomechanical characteristics of this yoga p

The Heidelberg Functional Foot Model-Application to Cavovarus and Equinovarus Feet.

Multisegment foot models have become increasingly important in biomechanical research and clinical gait analysis but often face limitations in defining joint positions. Often, they rely on simplified methods, such as using the midpoint between two markers to represent a joint, which lacks functional verification. In contrast, phenomenological angles, such as the medial arch angle, bypass joint center calculations, and offer sensitive, radiologically aligned indicators of foot mechanics. The Heidelberg functional foot model (HFFM) integrates functionally verified joint positions in combination

Step width modification to change rearfoot eversion and medial longitudinal arch angle during walking and running in individuals with pronated feet.

BACKGROUND: Individuals with pronated feet often experience altered foot biomechanics, leading to increased risk of lower limb injuries. Step width modification has been proposed as a potential intervention to improve foot alignment during gait. RESEARCH QUESTION: Does modifying step width influence rearfoot eversion and medial longitudinal arch angle (MLAA) in individuals with pronated feet during walking and running? METHODS: Twenty individuals with pronated feet underwent analysis during walking and running on treadmill, maintaining increased or decreased step width using real-time visual

[Pregnancy-associated femoral nerve affection].

BACKGROUND: Common neuropathies during pregnancy are carpal tunnel syndrome or peripheral facial paralysis. However, there is little information about femoral nerve affection during pregnancy. CASE REPORT: We report on a female at 30 weeks of gestation, complaining of pain in hips and thighs and gait difficulty. Pregnancy was normally developed. Neurological examination showed a bilateral weakness in both quadriceps and iliopsoas muscles combined with soreness in hips and thighs, without deficits of sensibility. Laboratory findings and results of a lumbar punction were normal. Pelvic ultrasou

Design of a brain-machine interface for reducing false activations of a lower-limb exoskeleton based on error related potential.

BACKGROUND AND OBJECTIVE: Brain-Machine Interfaces (BMIs) based on a motor imagination paradigm provide an intuitive approach for the exoskeleton control during gait. However, their clinical applicability remains difficulted by accuracy limitations and sensitivity to false activations. A proposed improvement involves integrating the BMI with methods based on detecting Error Related Potentials (ErrP) to self-tune erroneous commands and enhance not only the system accuracy, but also its usability. The aim of the current research is to characterize the ErrP at the beginning of the gait with a low

Levodopa influence on turning dynamics in people with Parkinson's disease.

Turning behaviors are affected in people with Parkinson's disease (PwPD) leading to falls and Freezing of Gait (FOG). Levodopa therapy is commonly recommended for PwPD to help alleviate motor symptoms. Knowledge of turning dynamics with levodopa therapy is limited and could improve clinical care. We enrolled 44 PwPD already on levodopa therapy and analyzed their turns on an instrumented gait mat. Participants made 180-degree turns in their OFF-state (> 8 h off levodopa) then ON-state (1 h after levodopa). Thirteen turn measures were assessed for changes between the OFF- to ON-state. Compared t

Concurrent validity of Protokinetics movement analysis software for estimated centre of mass displacement and velocity during walking.

BACKGROUND: The possibilities of out-of-lab gait analysis are ever increasing, with a recent development from Protokinetics movement analysis software (PKMAS) which, among other parameters, proposes an estimation of centre of mass (COMe) displacement. This measure can be useful in both clinical and sports performance environments. RESEARCH QUESTION: Does PKMAS provide an accurate estimation of the COM displacement and velocity? METHODS: Fifteen participants completed 10 walking trials over the Zeno™ Walkway while simultaneously being equipped with a full-body marker set for VICON analysis. T

Ability of a multi-segment foot model to measure kinematic differences in cavus, neutrally aligned, asymptomatic planus, and symptomatic planus foot types.

BACKGROUND: Multi-segment foot models (MFMs) provide a better understanding of the intricate biomechanics of the foot, yet it is unclear if they accurately differentiate foot type function during locomotion. RESEARCH QUESTION: We employed an MFM to detect subtle kinematic differences between foot types, including: pes cavus, neutrally aligned, and asymptomatic and symptomatic pes planus. The study investigates how variable the results of this MFM are and if it can detect kinematic differences between pathologic and non-pathologic foot types during the stance phase of gait. METHODS: Independe

"Brain-muscle loop" in the fragility of older persons: from pathophysiology to new organizing models.

BACKGROUND: The imperative action of the geriatric medicine is to prevent disability in older persons. Many epidemiological studies have been conducted in the last decades for improving knowledge of the aging process and their interactions with age-related diseases, especially for the identification of the relationship between sarcopenia and loss of mobility. Factors influencing muscle integrity can be classified into six main physiologic subsystems, but the central nervous system certainly plays a crucial role for maintaining muscle integrity in older persons. Recent data show that the reduce

Dynamic contact mechanics of the medial meniscus as a function of radial tear, repair, and partial meniscectomy.

BACKGROUND: The menisci are integral to normal knee function. The purpose of this study was to measure the contact pressures transmitted to the medial tibial plateau under physiological loads as a function of the percentage of the meniscus involved by the radial tear or repair. Our hypotheses were that (1) there is a threshold size of radial tears above which contact mechanics are adversely affected, and (2) partial meniscectomy results in increased contact pressure compared with that found after meniscal repair. METHODS: A knee simulator was used to apply physiological multidirectional dynam

Effect of stiffness-optimized ankle foot orthoses on joint work in adults with neuromuscular diseases is related to severity of push-off deficits.

BACKGROUND: People with plantar flexor weakness generate less ankle push-off work during walking, resulting in inefficient proximal joint compensations. To increase push-off work, spring-like ankle foot orthoses (AFOs) can be provided. However, whether and in which patients AFOs increase push-off work and reduce compensatory hip and knee work is unknown. METHODS: In 18 people with bilateral plantar flexor weakness, we performed a 3D gait analysis at comfortable walking speed with shoes-only and with AFOs of which the stiffness was optimized. To account for walking speed differences between co

Combined training improves functional capacity, fatigue, and quality of life in individuals with multiple sclerosis: a systematic review and meta-analysis.

INTRODUCTION: Multiple Sclerosis (MS) severely affects strength, coordination, gait, and balance, leading to significant challenges for individuals. While exercise is known to benefit MS management, most studies focus on single modalities, leaving a gap in understanding the effects of combined training (CT) interventions. This systematic review aims to address this gap by examining the impact of diverse CT interventions on muscle strength, balance, gait speed, endurance, fatigue, and quality of life in people with MS (PwMS). The findings will provide valuable insights for enhancing rehabilitat
