Minimally invasive vs. open radical cholecystectomy for gallbladder cancer: 30-day NSQIP outcomes analysis.

BACKGROUND: Radical cholecystectomy (RC) is the standard procedure for gallbladder cancer (GBC) tumors stage TIb or higher. Advances in minimally invasive surgery (MIS) offer the potential for improved outcomes compared to the traditional open surgery. While small series have demonstrated the feasibility of an MIS approach, a detailed comparison of outcomes between the two approaches is lacking. METHODS: The National Surgeon Quality Improvement Database (2010-2021) was queried to identify patients undergoing RC for GBC. Patients with T1b or higher disease were included. Thirty-day all-cause m

The challenge of cholecystectomy after successful choledocholithiasis extraction in elderly patients with comorbidities.

INTRODUCTION: Traditionally, cholecystectomy is recommended for patients who have had choledochal stone extraction with endoscopic retrograde cholangiopancreatography (ERCP) at the same session or as soon as possible. However, it is thought that ERCP reduces the need for cholecystectomy, especially in elderly patients. In this study, the prognosis of elderly patients who had biliary tract stones removed with ERCP was examined. MATERIALS AND METHODS: A total of 2251 ERCP procedures performed in our clinic between January 2014 and January 2016 were retrospectively reviewed. The demographic data

Complete excision and management of large hyaline-vascular pelvic Castleman's disease with coexisting cholecystolithiasis: A rare case report.

INTRODUCTION: Castleman's disease (CD) refers to scarce lymphovascular hyperplasia categorized as unicentric (UCD) or multicentric (MCD). Retroperitoneal UCD is uncommon (6.7 %) and frequently grows undetected, making diagnosis challenging. The hypervascular nature of hyaline-vascular variant and the average larger size of UCD may result in complete resection difficulty. We present a case of large pelvic UCD completely resected without previous prophylactic procedures or massive intraoperative bleeding with coexisting cholecystolithiasis, which follows the SCARE 2023 guidelines. CASE PRESENTA

Electroacupuncture Regulates Mitochondria-Endoplasmic Reticulum interactions via Fn1 in a Parkinson's Disease Model: Transcriptomic Evidence.

AIMS: Parkinson's disease (PD) is characterized by dopaminergic neuron degeneration and disruption to mitochondria-associated endoplasmic reticulum membranes (MAMs). This study explores whether electroacupuncture (EA) can alleviate 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) induced PD symptoms and investigates the underlying mechanisms using RNA sequencing (RNA-seq). METHODS: A PD mouse model was established using MPTP, followed by EA treatment at governing vessel 20 (GV20) and gallbladder meridian 34 (GB34) acupoints, with sham EA treatments as a control. Behavioral assays, immunohi

Cystic Duct Stump Calculus Mimicking Functional Dyspepsia: A Rare Cause of Post-cholecystectomy Syndrome.

Persistent gastrointestinal symptoms after cholecystectomy are not uncommon and are frequently attributed to functional disorders such as dyspepsia or gastritis. However, rare structural causes (such as retained calculi within the cystic duct stump or a remnant gallbladder) can mimic benign gastrointestinal conditions, particularly in the absence of classical biliary signs. We present the case of a 47-year-old female with a one-year history of postprandial epigastric discomfort, bloating, and dyspepsia. She had undergone an open cholecystectomy five years earlier for gallstone disease. Despite

Efficacy and predictors of immune checkpoint inhibitors in patients with gallbladder cancer.

Immune checkpoint inhibitors (ICIs) have shown promising efficacy in multiple cancers including biliary tract cancers (BTCs). However, the data focusing on the efficacy of ICIs in patients with gallbladder cancer (GBC) is still limited. In this study, we aim to assess the efficacy of ICIs in GBC and explore the clinicopathologic and molecular markers associated with ICI benefit. We retrospective analyzed 69 GBC patients who had received ICI therapy between January 2016 and December 2020. Tumor samples were obtained for genomic sequencing and immunohistochemical analysis. The median progression

Functional role of the biofilm regulator CsgD in Salmonella enterica sv. Typhi.

INTRODUCTION: Typhoid fever is an infectious disease primarily caused by Salmonella enterica sv. Typhi (S. Typhi), a bacterium that causes as many as 20 million infections and 600,000 deaths annually. Asymptomatic chronic carriers of S. Typhi play a major role in the transmission of typhoid fever, as they intermittently shed the bacteria and can unknowingly infect humans in close proximity. An estimated 90% of chronic carriers have gallstones; biofilm formation on gallstones is a primary factor in the establishment and maintenance of gallbladder carriage. CsgD is a central biofilm regulator in

An Integrative Pancreatic Cancer Risk Prediction Model in the UK Biobank.

Pancreatic cancer (PaCa) is a lethal cancer with an increasing incidence, highlighting the need for early prevention strategies. There is a lack of a comprehensive PaCa predictive model derived from large prospective cohorts. Therefore, we have developed an integrated PaCa risk prediction model for PaCa using data from the UK Biobank, incorporating lifestyle-related, genetic-related, and medical history-related variables for application in healthcare settings. We used a machine learning-based random forest approach and a traditional multivariable logistic regression method to develop a PaCa pr

Gemcitabine, Cisplatin, and Nab-Paclitaxel as a First-Line Therapy for Advanced Biliary Tract Cancers.

INTRODUCTION: Locally advanced, inoperable, or metastatic gallbladder cancers (GBC) are treated with either gemcitabine-platinum combinations or gemcitabine alone based on physician discretion. However, the combination of gemcitabine, cisplatin, and nab-paclitaxel (GCNP) has shown increased response rates and prolonged survival in a phase II trial of biliary tract patients. MATERIALS AND METHODS: Consecutive series of patients diagnosed with locally advanced (liver infiltration > 5 cm, large nodes at porta, abutting duodenum), inoperable, and metastatic biliary tract patients between January

Inverse probability of treatment weighting analysis of laparoscopic versus open Sg4b-5 bi-segmentectomy in patients with gallbladder cancer.

Sg4b-5 anatomical bi-segmentectomy with regional lymphadenectomy (Sg4b5) is a surgical option for gallbladder cancer (GBC) treatment. The laparoscopic approach to this challenging operation is still controversial. Aim of this study was to compare short- and long-term outcomes of laparoscopic versus open Sg4b5 in a single institution series of patients. All consecutive patients who underwent Sg4b5 for GBC from January 2000 to September 2021 were retrospectively reviewed. Inverse probability of treatment weighting (IPTW) analysis was performed. 75 patients were analyzed, 18 in the laparoscopic a

Effects of Biliary Phospholipids on Cholesterol Crystallization and Growth in Gallstone Formation.

The prevalence of cholesterol gallstone disease is increasing, primarily due to the global epidemic of obesity associated with insulin resistance, and this trend leads to a considerable healthcare, financial, and social burden worldwide. Although phospholipids play an essential role in maintaining cholesterol solubility in bile through both mixed micelles and vesicles, little attention has been paid to the impact of biliary phospholipids on the pathogenesis of cholesterol gallstone formation. A reduction or deficiency of biliary phospholipids results in a distinctly abnormal metastable physica

Identification of gallbladder cancer by direct near-infrared measurement of deuterated chloroform-extracted organic phase from human bile.

A simple near-infrared (NIR) spectroscopic scheme enabling direct measurement of organic phase extracted from human bile with no spectral interference from the extraction solvent was demonstrated for identification of gallbladder (GB) cancer. This scheme is used to recognize the different lipid contents in bile samples from GB cancer patients using NIR spectroscopy for disease identification. To this end, the extraction solvent should provide an absorption-free NIR region to observe peaks of related metabolite. For this purpose, deuterated chloroform (CDCl3) is uniquely suited as an extraction

Optimizing Patient Pathways in Advanced Biliary Tract Cancers: Recent Advances and a French Perspective.

Biliary tract cancers (BTCs) are a heterogeneous group of tumors that are rare in Western countries and have a poor prognosis. Three subgroups are defined by their anatomical location (intrahepatic cholangiocarcinoma, extrahepatic cholangiocarcinoma and gallbladder carcinoma) and exhibit distinct clinical, molecular, and epidemiologic characteristics. Most patients are diagnosed at an advanced disease stage and are not eligible for curative-intent resection. In addition to first- and second-line chemotherapies (CisGem and FOLFOX, respectively), biologic therapies are now available that target
