Iatrogenic injury to the facial nerve is certainly the most apparent if not the most feared complication faced by the otologic surgeon. The temporal bone with its tightly packed sensory structures is no place for the "occasional operator". A thorough knowledge of temporal bone anatomy is of course required. On occasion, as in other areas of life, one encounters the unexpected and must be prepared to "adjust" to the situation at hand. An instructional case involving an anomalous facial nerve is presented. A review of the developmental anatomy of the facial nerve, preoperative diagnostic imaging

Lesion of the meniscus is the most frequently occurring lesion in the region of the knee joint. Opinions are still divided as to the effects of partial, subtotal or total meniscectomy in provoking and/or promoting arthrosis. In this study, 70 patients with isolated injury of the meniscus were followed up on an average 11 years after meniscectomy. The subjective and clinical findings are juxtaposed with the x-ray findings. It was found that no arthrosis or no increase of the already preoperatively existing degree of arthrosis was seen in only 21.4% of the patients. Correlations are set up betwe

Functional hand splints have been in use in a number of spinal injury units in the USA since the early 1950s. The splints are designed to provide a pinch-grip either by harnessing wrist dorsiflexion or by external power. Such devices are little used in the United Kingdom. This paper describes the results of late provision of 62 such splints in a Disabled Living Unit. A proportion of tetraplegic patients found such splints of considerable functional value. It is estimated that some 30--60 patients each year would benefit from them if appropriate facilities for early fitting were available.

Blunt trauma to the renal pelvis is exceedingly rare and such trauma of the ureter is only slightly more common. A tetrad of radiologic findings should suggest the diagnosis when there is a high index of suspicion. It would seem that early surgical treatment would be the preferred program but in the face of serious associated trauma there is little lost by waiting. Nineteen days elapsed between injury and repair in this case. That duration may be excessive, but as shown in this case the result was excellent and the patient's retroperitoneum was never infected. We do not advocate a purposeful d

OBJECTIVE: To report our experience in the reconstruction of soft tissue defects in the hand with a free anterolateral thigh deep fascia flap and describe the clinical outcomes.
METHODS: This study was a retrospective trial. From November 2016 to January 2020, six patients (four men and two women) with soft tissue defects in the hand were included in this study. The average age of the patients was 33.7 ± 12.7 years (range, 20 to 50 years). All patients underwent reconstructions with free anterolateral thigh deep fascia flaps. Relevant clinical characteristics were recorded prior to surgery. T

Traumatic upper extremity amputation is a life-altering event, and recovery of function depends on proper surgical management and postoperative rehabilitation. Many injuries require revision amputation and postoperative prosthesis fitting. Care should be taken to preserve maximal length of the limb and motion of the remaining joints. Skin grafting or free tissue transfer may be necessary for coverage to allow preservation of length. Early prosthetic fitting within 30 days of surgery should be performed so the amputee can start rehabilitation while the wound is healing and the stump is maturing

BACKGROUND: Forearm amputation in different zones via a similar mechanism in a group of patients of similar age is associated with different possibilities of functional recovery. The degree of postoperative recovery of function is invariably partial compared to pre-amputation function; this kind of trauma inevitably leads to disability. The patients adapt to their new circumstances to a different extent. That is why a quality of life evaluation should be included in the assessment of replantation outcomes. The aim of the study was to evaluate functional outcomes in patients after replantation

The traumatic boutonnière (or buttonhole) deformity develops after unsuccessful primary treatment of a lesion of the extensor tendon at the level of the proximal interphalangeal joint. Knowledge of the mechanisms leading to this deformity is fundamental for choosing and executing reconstructive procedures. The most frequently used methods are illustrated in this article. Because none of these procedures has been shown to be successful in all situations, we recommend a staged reconstructive approach. Even then, this deformity often results in incomplete reconstruction. Therefore, for these lesi

Closed treatment has provided good results in uncomplicated cases of mallet finger; however, surgical fixation is recommended when there is involvement of more than one third of the base of the distal phalanx. Various techniques have been described for this purpose. The goal of this report is to present a simple method of K-wire fixation and show our results with this procedure.

In the course of a ten months study on 508 patients with CTS a trauma as a possible causal factor was observed in 22 cases. In none of these cases did the other hand show normal electroneurographic parameters. The mean values of distal motor latency were among 5.5 ms (3.7-9.2 ms) on the injured side and 4.6 (3.5-6.7 ms) on the other. The relatively seldom occurrence of CTS in connection with a trauma requires therefore strict criteria for the evaluation of the casual context. A traumatic etiology can only be recognised if a close temporal relation exists (beginning of the symptomatology during

Complex regional pain syndrome (CRPS) is a chronic condition commonly consisting of allodynia, paresthesias, skin and temperature changes, and motor dysfunction. CRPS is relatively rare and classically develops in a limb after sustaining a traumatic or surgical insult. To date, CRPS developing in a previously uninjured limb after prolonged immobilization has not yet been reported. We describe a 45-year-old man with no known preexisting injury or neuropathy to his upper extremities who developed CRPS of his right wrist and hand after prolonged colorectal surgery.

BACKGROUND: Sporotrichosis is a fungal disease caused by the Sporothrix schenckii species complex. It is usually acquired by trauma with plants. Lymphatic-cutaneous is the most common clinical manifestation.
CASE REPORT: The present case describes the development of an epidermoid carcinoma as a late complication on a long evolution sporotrichosis scar. During a period of 50 years, the patient had lesions in the right hand. Sporotrichosis was diagnosed and treated with potassium iodide. Eleven years after the healing of the lesions, a squamous cell carcinoma on the scar of this mycosis was dia

Tumor lysis syndrome (TLS) is a metabolic disorder that is generally associated with a malignancy leading to hyperuricemia, hyperphosphatemia, and acute kidney injury. On the other hand, we sometimes encounter these phenomena in nonmalignant disease, which has been referred to as tumor lysis-like syndrome in some studies. We herein experienced a case in which tumor lysis-like syndrome occurred in the course of therapy for eosinophilic disease of the lung, a nonmalignant disease. Even in nonmalignant disease, massive cell lysis induced by therapy can cause phenomena such as TLS or tumor lysis-l

The ulnar tunnel syndrome occurs usually from ganglions, lipoma, cysts, chronic repetitive trauma, bicycling, and the activities that require either prolonged wrist hyper extension or continued pressure on the hypothenar eminence. The ulnar tunnel syndrome after flexor tendon repair is a rare complication. We report on a 24-year-old man with ulnar tunnel syndrome after a flexor tendon repair secondary to girder cut injury. The patient was managed conservatively for his hypertrophic scar for 9 weeks. After 9 weeks, the patient presented with no clawing and complete closure of the hand.

Compared with flexor digitorum profundus (FDP) tendon injury, the isolated rupture of flexor digitorum superficialis (FDS) tendon in a finger is a rare report in the literature. The pathogenesis is still unclear if we consider only tendon rupture without congenital anomalies or other diseases. The diagnosis is clinical, but it can be confirmed by ecography or magnetic resonance imaging (MRI). We report the case of a flexor superficialis (FS) tendon closed rupture of the ring finger of right hand in a 57-year-old patient. The diagnosis was confirmed by MRI. No surgical repair was used, given th

As a hand therapist, we don't really treat a lacerated tendon. The physician is the one that handles the repair. We actually are treating all the secondary problems that occur as a result of the injury and the repair. To quote Ken Flowers (JHT past editor), " Scar Wars" can end up being one of our biggest challenges. This author has demonstrated how adhesions can affect motion after a repair of tendons in the first extensor compartment. He has also designed a splint to help with "battling" this dilemma.-Peggy Fillion, OTR, CHT, Practice Forum Editor.

Fractures of the proximal radius are among the most common fractures and account for 9% to 25% of the elbow fractures in children. However, bilateral radial neck fractures are extremely rare injuries in children and have been reported only once before in present English literature. We report a case of bilateral radial neck fractures in a 10-year-old girl resulting from a fall on outstretched hands. Careful physical examination and proper radiographs are necessary for the diagnosis. Early recognition, proper management, and physical therapy led to complete recovery and full functional movement
