Strong decline in herpes simplex virus antibodies over time among young homosexual men is associated with changing sexual behavior.

The objective of this study was to evaluate whether the change in sexual behavior among homosexual men observed after the start of the acquired immunodeficiency syndrome epidemic resulted in a change in herpes simplex virus (HSV) seroprevalence in this group over time. In a cross-sectional study, the prevalence of herpesvirus types 1 (HSV1) and 2 (HSV2) was determined at study entry in 1984-1985 and 1995-1997 among 532 young (aged < or = 30 years) homosexual men participating in the Amsterdam Cohort Studies on HIV/AIDS. Risk factors for the presence of HSV antibodies, including human immunodef

The epidemiology of HIV and AIDS in the United States.

The HIV/AIDS epidemic has slowed in growth and is approaching a leveling in incidence. However, the number of HIV-infected persons is high, and new infections continue to occur. The prevention of AIDS-related OIs and decreases in AIDS-related mortality are an encouraging sign that HIV-infected persons are living better and longer lives. However, if the improvements in clinical management and patient survival are not matched by reductions in HIV incidence, the population of HIV-infected persons will increase. The characteristics of persons with HIV infection are changing over time and must be m

Severe complications of perianal sepsis in patients with human immunodeficiency virus.

Fifty human immunodeficiency virus (HIV)-infected patients with perianal sepsis were studied. Seven (14 per cent) had serious septic complications, four patients with severe necrotizing gangrene, and three with abscesses in the mediastinum, liver and brain respectively. CD4+ lymphocyte counts were significantly lower in patients with severe septic complications as compared with those with uncomplicated perianal sepsis (P < 0.05). In patients with HIV presenting with rare (metastatic) abscesses, perianal sepsis must always be kept in mind as a possible focus. Although HIV-infected patients have

Condom use among gay/bisexual male substance abusers using the timeline follow-back method.

Sexual risk for HIV transmission under the influence of alcohol and/or other drugs is not simply a cause-effect relationship: not everyone who drinks or uses other drugs has unprotected sex. The purpose of this study is to explore differences between substance using gay/bisexual men who use condoms during anal sex from those who do not. These differences are identified by comparing men whose anal sex while under the influence of alcohol and/or drugs is consistently protected to men whose anal sex while under the influence of alcohol and/or drugs is consistently unprotected. Complications of varicella zoster virus reactivation in HIV-infected homosexual men.

OBJECT: To study the complication rate of varicella zoster virus (VZV) reactivation and the relationship between complications, presentation and localization of zoster and immune function in HIV disease. DESIGN AND METHODS: A total of 142 episodes of VZV reactivation in 113 out of 544 HIV-1-infected participants in the Amsterdam Cohort Study of homosexual men were studied. Persistent hyperkeratotic or necrotic skin lesions, post-herpetic neuralgia, other neurological events, ocular events and pneumonitis occurring within 6 months of the onset of the last episode of VZV reactivation were defin

Unusual clinical aspects of oral non-Hodgkin lymphomas in patients with HIV infection.

Patients with HIV infection are at greater risk of developing malignancies. We report two HIV-seropositive patients with primary oral manifestation of a B-cell non-Hodgkin lymphoma (NHL). Localisations of tumours were the palate, with an unusual bifocal origin, and the tongue which is rarely the primary site of NHL. Ulcerations and extensive tissue necrosis were observed. Histologically both cases were high grade malignant lymphomas, immunoblastic. Epstein-Barr virus DNA was detected with in situ hybridisation in 1 patient.

Comparative prevalence rates of sexually transmitted diseases in heterosexual and homosexual men.

This study compared prevalence rates of most common sexually transmitted diseases (STD) in heterosexual and homosexual men who made respectively 12,201 and 5324 visits to an STD clinic over 18 months. Overall, homosexual men were significantly (p < 0.001) more likely than heterosexual men to have gonorrhea (30.31% vs. 19.83%), early syphilis (1.08% vs. 0.34%) and anal warts (2.90% vs. 0.26%) but less likely to have nongonococcal urethritis (NGU) (14.63% vs. 36.40%, p < 0.001), herpes genitalis (0.93% vs. 3.65%, p < 0.001), pediculosis pubis (4.30% vs. 5.35%, p < 0.005), scabies (0.42% vs. 0.76

Herpes viruses and HIV-1 drug resistance mutations influence the virologic and immunologic milieu of the male genital tract.

OBJECTIVES: To further understand the role that chronic viral infections of the male genital tract play on HIV-1 dynamics and replication. DESIGN: Retrospective, observational study including 236 paired semen and blood samples collected from 115 recently HIV-1 infected antiretroviral naive men who have sex with men. METHODS: In this study, we evaluated the association of seminal HIV-1 shedding to coinfections with seven herpes viruses, blood plasma HIV-1 RNA levels, CD4 T-cell counts, presence of transmitted drug resistance mutations (DRMs) in HIV-1 pol, participants' age and stage of HIV-in

Hepatitis C testing and re-testing among people attending sexual health services in Australia, and hepatitis C incidence among people with human immunodeficiency virus: analysis of national sentinel surveillance data.

BACKGROUND: Direct acting antivirals are expected to drastically reduce the burden of hepatitis C virus (HCV) in people living with Human Immunodeficiency Virus (HIV). However, rates of HCV testing, re-testing and incident infection in this group remain uncertain in Australia. We assessed trends in HCV testing, re-testing and incident infection among HIV-positive individuals, and evaluated factors associated with HCV re-testing and incident infection. METHODS: The study population consisted of HIV-positive individuals who visited a sexual health service involved in the Australian Collaboratio

Comparison of Antimicrobial Susceptibility of Urogenital Neisseria gonorrhoeae Isolates Obtained From Women and Men.

BACKGROUND: The US system for gonococcal antimicrobial susceptibility surveillance monitors trends exclusively among men with urethral infection, the population from whom the yield of gonococcal culture is highest. Little is known about the susceptibility of female urogenital isolates, and it is unclear whether gonococcal susceptibility among men who report sex exclusively with women (MSW) is representative of susceptibility among women. METHODS: Using isolates collected during a recent treatment trial in 5 US cities, we performed a secondary analysis to compare antimicrobial susceptibilities

A traditional society in change encounters HIV/AIDS: knowledge, attitudes, and risk behavior among students in northwestern China.

China has to date in general only been moderately affected by the global HIV epidemic, but there are cities, particularly in Yunnan, with a high prevalence. This situation seems to be changing, however, with the risk of a rapidly growing epidemic. Our aim was to investigate the level of knowledge about HIV and AIDS and risk behavior among young people at Xinjiang Medical University, Urumqi, China. Xinjiang has a population of mixed minority groups and Han Chinese. The largest minority group is the Uygur (8.82 million), who are Sunni Muslims and speak a Turkish language. Questionnaires were han

HIV testing, and the care and treatment of HIV positive people in English prisons.

In a study of 452 ex-prisoners in England in 1990, 66 people reported that they were tested for HIV antibodies when last in prison. The circumstances under which many of those who were tested were difficult: 36% found it an unpleasant experience, 17% had not taken the test voluntarily and 55% said they received no counselling. Further information was gathered about the experiences of those who were HIV positive or assumed to be. Most were accommodated in a 'special location', not allowed to associate with other prisoners and denied access to work or recreational facilities. These data highligh

Evidence for a sexually transmitted cofactor for AIDS-related Kaposi's sarcoma in a cohort of homosexual men.

We examined factors associated with the subsequent development of AIDS-related Kaposi's sarcoma in a cohort of 353 homosexual men infected with human immunodeficiency virus (HIV). Cumulative incidence curves for the development of Kaposi's sarcoma and opportunistic infection were stratified over a wide range of variables at enrollment, including those related to demographics, sexual behavior, illicit drug use, and medical history. We found no strong associations between any of these variables and the development of opportunistic infection, but two were related to Kaposi's sarcoma: use of nitri

Class-specific antibody response to pneumococcal capsular polysaccharides in men infected with human immunodeficiency virus type 1.

We characterized the effect of infection with human immunodeficiency virus type 1 (HIV) on levels of total immunoglobulins and pneumococcal vaccine-specific immunoglobulins in 28 heterosexual and 25 homosexual men seronegative for HIV; 27 asymptomatic, seropositive homosexual men; and 21 patients with AIDS. Total serum IgG levels were increased in both HIV-seropositive groups compared with the HIV-seronegative men (P less than .001). Total IgM levels, however, were elevated only in the asymptomatic, HIV-seropositive men (P less than .08); total IgA levels were elevated only in the patients wit
