Vitamin E concentrations in hospitalized adult horses and foals.

OBJECTIVE: To determine the prevalence of vitamin E deficiency in horses at admission and at the time of discharge. The hypothesis was that vitamin E concentrations would decrease during hospitalization. METHODS: A prospective cohort study was performed of client-owned horses admitted as patients or companions through the emergency service at a tertiary referral center. Exclusion criteria included vitamin E supplementation. Whole blood was collected into an additive-free tube at the time of admission for all horses and at discharge if the horse was hospitalized for a minimum of 5 days. Serum

Identification of the Potential Pharmacotherapeutic Risk in Hospitalized Geriatric Patients.

INTRODUCTION: Geriatric patients need carefully tailored pharmacotherapeutic approaches to minimize the risks of inappropriate treatment and hospitalizations. In Slovakia, there is a lack of studies documenting the medication use patterns in hospitalized geriatric patients. The primary outcome of our study was to identify potential pharmacotherapeutic risks in hospitalized geriatric patients. METHODS: We performed a retrospective analysis of the medical records from 122 patients (65 years and older) hospitalized in the geriatric department. Potential pharmacotherapeutic risk was assessed by i

Difficult Patient Behaviors in the Hospital Setting: A Narrative Review.

In the inpatient medical setting, difficult patient behaviors can complicate care delivery. When difficult behaviors are present, patients may experience worsened outcomes, and medical providers may experience distress. Most recommendations for such behaviors in the literature focus on outpatient care or inpatient psychiatric settings; recommendations for inpatient medical settings are limited. While difficult behaviors are commonly associated with personality disorders, patients without AQ6personality disorders may also exhibit these behaviors when acutely stressed, such as when they are expe

Living alone and provider behaviour in public and private hospitals.

Following COVID-19, hospitals in many OECD countries are under pressure to absorb backlogs accumulated due to the suspension of health services. Reductions in length of stay can generate capacity to treat patients and increase efficiency. Personal circumstances, such as living alone, can affect how long patients stay in hospital. We test whether such non-clinical factors affect care received by patients. Several countries are experiencing an increase in the number of elderly people who live alone. Patients who live alone may lack support at home leading to delayed discharges despite being clin

Non-Profit Hospital Governance, Conduct, and CEO Pay.

We investigate whether the membership of a non-profit hospital's CEO on its board of directors is associated with CEO pay and several measures of hospital performance, including price, operating margin, quality, and service to low-income patients. Although the consequences of CEO board membership for for-profit firms have been studied extensively, the consequences for non-profits in general, and non-profit hospitals in particular, have received less attention. Because most hospitals are non-profit and non-profit hospital prices have increased rapidly over the past 20 years, this gap is importa

The Oncology Drug Shortages and Its Impact on Community Hospitals.

The ongoing shortage of oncology drugs, particularly generic chemotherapies like platinum agents, has had a disproportionate impact on community and safety net hospitals in the United States and globally. These institutions, often serving rural and underserved populations, face significant challenges due to limited financial resources. This article examines the practical implications of these shortages through the lens of a community hospital, where creative solutions were employed to maximize limited resources where drug shortages were concerned. This article also highlights the emergence of

"He needs to die in the hospital": A caregiver's distress call.

The setting in which patients face the end of life (EOL) is shaped by complex, individualized needs beyond physical care. Mr. X, a man in his mid-50s with glioblastoma, endured treatment complications and progressive decline. Supportive care was engaged to manage symptoms and support his wife, Mrs. X, who was simultaneously the family's financial provider and caregiver for both her husband and their autistic adult son. As his condition worsened, she resisted hospice and code status changes, citing emotional, logistical, and financial barriers, while fiercely advocating for his wish to die in t

Patient Perception of University Hospitals' PCs for People Initiative.

The COVID-19 pandemic and post-pandemic world have highlighted the integral part that social connections play in one's mental and emotional well-being. The modern world had to suddenly adapt and accommodate an abundantly virtual one. Those who were unable to make these technological adaptations, many being low socioeconomic households, were facing many new obstacles, such as losing in-person jobs and being disconnected from their community and services by not having the technology to stay in contact, all while living through a global pandemic. University Hospitals understands and recognizes th

Stratifying Risk for Postpartum Depression at Time of Hospital Discharge.

OBJECTIVE: Postpartum depression (PPD) is a major contributor to postpartum morbidity and mortality. Beyond efforts at routine screening, risk stratification models could enable more targeted interventions in settings with limited resources. The authors sought to develop and estimate the performance of a generalizable risk stratification model for PPD in patients without a history of depression, using information collected as part of routine clinical care. METHODS: The authors conducted a retrospective cohort study of all individuals who delivered between 2017 and 2022 in one of two large aca

Impact of Private Equity Acquisition of Hospitals on Surgical Outcomes.

OBJECTIVE: To characterize outcomes of patients undergoing a major surgical procedure at private equity-acquired (PEA) versus non-acquired centers. BACKGROUND: Private-equity acquisitions of health centers can be followed by policy changes aimed at maximizing profit over quality of care. METHODS: Patients who underwent a major surgical procedure between 2016 and 2021 were identified using 100% Medicare Standard Analytic Files. Propensity score matching (PSM) created a well-balanced 1:1 matched cohort among patients treated at PEA and non-PEA hospitals; multivariable regression was used to as

Hybrid Hospital-at-Home Program in Singapore: Ethnographic Study.

BACKGROUND: Hospital-at-Home (HaH) programs deliver hospital-level care in the home setting, traditionally emphasizing high-touch, in-person clinical visits. In recent years, especially during the COVID-19 pandemic, HaH models have rapidly evolved to incorporate digital technologies, enabling remote consultations and monitoring as part of care delivery. While both in-person and remote HaH modalities have individually shown positive outcomes with patient experience and clinical safety, less is known about how hybrid models, combining both modalities, are experienced by patients and caregivers.
