Current advances in lesion-symptom mapping of the human cerebellum.

While high-resolution structural magnetic resonance imaging (MRI) combined with newer analysis methods has become a powerful tool in human cerebral lesion studies, comparatively few studies have used these advanced imaging techniques to study lesions of the human cerebellum and their associated symptoms. This review will summarize the methodology of MRI-based lesion-symptom mapping of the human cerebellum and discuss its potential for gaining insights into cerebellar function. The investigation of patients with defined focal lesions yields the greatest potential for obtaining meaningful correl

Measuring extent of ductal carcinoma in situ in breast excision specimens: a comparison of 4 methods.

CONTEXT: Measuring the extent of nonpalpable ductal carcinoma in situ (DCIS) in a breast specimen is challenging but important because it influences patient management. There is no standardized method for estimating the extent of DCIS, although serial sequential sampling with mammographic correlation is considered an accurate method. OBJECTIVE: To estimate the extent of DCIS using various methods and to compare these estimations with the extent as determined by the serial sequential sampling method. DESIGN: A total of 78 primary breast excisions with DCIS were retrospectively reviewed. All s

Functional magnetic resonance imaging (fMRI) in children sedated with propofol or midazolam.

Magnetic resonance imaging (MRI) requires patient immobility and children generally need to be sedated. The ideal sedative agent for functional MRI (fMRI) should only minimally hamper the neurophysiologic effect of the administered sensorial stimulation. This study compares the effect of propofol and midazolam on the fMRI auditory activation pattern in children. Fourteen children in the 3 to 7 year age group without neurologic or auditory deficits were randomly assigned to receive propofol or midazolam for sedation during auditory fMRI. Two patients in the midazolam group were excluded due to

Cortical and subcortical correlates of functional electrical stimulation of wrist extensor and flexor muscles revealed by fMRI.

The main scope of this study was to test the feasibility and reliability of FES in a MR-environment. Functional Electrical Stimulation (FES) is used in the rehabilitation therapy of patients after stroke or spinal cord injury to improve their motor abilities. Its principle lies in applying repeated electrical stimulation to the relevant nerves or muscles for eliciting either isometric or concentric contractions of the treated muscles. In this study we report cerebral activation patterns in healthy subjects undergoing fMRI during FES stimulation. We stimulated the wrist extensor and flexor musc

A preliminary functional magnetic resonance imaging study of prefrontal-amygdalar activation changes in adolescents with bipolar depression treated with lamotrigine.

OBJECTIVES: Hypotheses regarding mood dysregulation in bipolar disorder (BD) have centered on limbic overactivity with relative prefrontal underactivity during mood episodes. Therefore, we hypothesized that adolescents with bipolar depression successfully treated with lamotrigine would show decreases in amygdalar activation, and increases in prefrontal activation. METHODS: Eight adolescents with BD underwent functional magnetic resonance imaging (fMRI) at baseline and after eight weeks of lamotrigine treatment. Blocks of negatively and neutrally valenced emotional pictures were presented duri

Right-hemisphere reading in a case of developmental deep dyslexia.

The right-hemisphere hypothesis of deep dyslexia has received support from functional imaging studies of acquired deep dyslexia following damage to the left cerebral hemisphere, but no imaging studies of cases of developmental deep dyslexia, in which brain damage is not suspected, have been reported. In this paper, we report the first evidence of right hyperactivation in an adult case of developmental deep dyslexia. Hyperactivation was observed in the right inferior frontal cortex during functional magnetic resonance imaging (fMRI) of the oral reading of imageable content words and nonwords to

Visualization of vascular hemodynamics in a case of a large patent ductus arteriosus using flow sensitive 3D CMR at 3T.

Comprehensive flow velocity acquisition based on time-resolved three-dimensional phase-contrast CMR with three-directional velocity encoding was employed to assess arterial hemodynamics in a patient with a large patent ductus arteriosus with Eisenmenger's physiology. Computer-aided visualization of blood flow characteristics provided detailed information about temporal and spatial distribution of left and right ventricular outflow. Main findings included the depiction of the location and extent of two flow channels for systolic aortic filling, a relatively large amount of pulmonary artery to a

Skull base growth in children with Chiari malformation Type I.

OBJECT: The goal of this study was to establish whether children with Chiari malformation Type I (CM-I) have abnormal skull base geometry. METHODS: Distances and angles between skull base landmarks were measured on preoperative magnetic resonance images obtained in 30 children (age range 36-204 months) with symptomatic isolated CM-I; 16 of them (53%) had syringomyelia. Comparisons were made with 42 children of similar age who comprised the control group by using one-way analysis of variance. RESULTS: The angle formed by the crista galli (CG), dorsum sellae (DS), and foramen magnum (FM) was l

Demonstration of paramagnetic and diamagnetic cerebral lesions by using susceptibility weighted phase imaging (SWI).

Susceptibility-weighted MR imaging (SWI) has become a non-invasive diagnostic modality for functional MR imaging (fMRI) of the brain and also for the imaging of tumors, injuries, malformations or microhemorrhages. SWI often enables detection of otherwise subtle abnormalities or provides additional relevant information when combined with routine MR imaging. The purpose of this article is to illustrate the potential of SWI in the discrimination of paramagnetic and diamagnetic brain lesions in neuroradiological applications.

Reduced fractional anisotropy on diffusion tensor magnetic resonance imaging after hypoxic-ischemic encephalopathy.

OBJECTIVE: Apparent diffusion coefficients (ADC) that are measured by diffusion-weighted imaging are reduced in severe white matter (WM) and in some severe basal ganglia and thalamic (BGT) injury in infants who present with hypoxic-ischemic encephalopathy (HIE). However, ADC values may pseudonormalize or even be high during this time in some less severe but clinically significant injuries. We hypothesized that fractional anisotropy (FA), a measure of the directional diffusivity of water made using diffusion tensor imaging, may be abnormal in these less severe injuries; therefore, the objective

Prenatal sonographic features of hypospadia: two- and three-dimensional findings.

OBJECTIVE: To present our experience of applying three-dimensional ultrasonography in prenatal diagnosis of hypospadia. CASE REPORT: A 28-year-old woman, gravida 1, para 0, visited our hospital at 17 weeks' gestation. No gross anomaly was found on the scan at 21 weeks' gestation. Fetal growth was appropriate for gestational age on two-dimensional (2D) ultrasonography at 27 weeks' gestation, but an abnormally curved and shortened fetal penis was found incidentally. Three-dimensional (3D) imaging in surface-rendered mode was used to reconstruct the fetal penis. Multiplanar and surface-rendered

Automatic priming of semantically related words reduces activity in the fusiform gyrus.

We used rapid, event-related fMRI to identify the neural systems underlying object semantics. During scanning, subjects silently read rapidly presented word pairs (150 msec, SOA = 250 msec) that were either unrelated in meaning (ankle-carrot), semantically related (fork-cup), or identical (crow-crow). Activity in the left posterior region of the fusiform gyrus and left inferior frontal cortex was modulated by word-pair relationship. Semantically related pairs yielded less activity than unrelated pairs, but greater activity than identical pairs, mirroring the pattern of behavioral facilitation

Neuroanatomic deficits in congenital central hypoventilation syndrome.

Congenital Central Hypoventilation Syndrome (CCHS) patients exhibit compromised autonomic regulation, reduced breathing drive during sleep, diminished ventilatory responses to chemoreceptor stimulation, and diminished air hunger perception. The syndrome provides an opportunity to partition neural processes regulating breathing and cardiovascular action. No obvious lesions appear with conventional magnetic resonance imaging; however, T2 relaxometry procedures can detect reduced cell or fiber density or diminished myelination not found with routine evaluation. High-resolution T1, proton density,

The role of the dorsal stream for gesture production.

Skilled gestures require the integrity of the neural networks involved in storage, retrieval, and execution of motor programs. Premotor cortex and/or parietal cortex lesions frequently produce deficits during performance of gestures, transitive more than intransitive. The dorsal stream links object information with object action, suggesting that mechanical knowledge of tool use is stored focally in the brain. Using event-related fMRI, we explored activity during instructed-delay transitive and intransitive hand gestures. The comparison between planning-preparation and execution of gestures dem

Uterine peristalsis: comparison of transvaginal ultrasound and two different sequences of cine MR imaging.

PURPOSE: To compare uterine peristalsis as seen on two different magnetic resonance (MR) imaging sequences and transvaginal ultrasound (TVUS), so as to better determine the best method for evaluating uterine peristalsis. MATERIALS AND METHODS: Eleven women in the periovulatory phase of the menstrual cycle underwent TVUS and cine MR imaging within a time period of three hours. Findings on cine MR images obtained with a serial 60 half-Fourier acquisition single-shot turbo spin-echo (HASTE) sequence and a 300 true fast imaging with steady-state precession (FISP) sequence were compared with TVUS.

High-resolution diffusion tensor imaging of the brain stem at 3 T.

Diffusion tensor imaging with 1.8-mm isotropic resolution was performed to delineate structures of the brain stem. High-resolution single-shot imaging was achieved by the combination of a high-field magnet (3T) and the SENSitivity Encoding (or SENSE) parallel imaging technique. Various structures in the brain stem, such as the inferior olivary nuclei, deep cerebellar nuclei, some cranial nerves, and white matter tracts were identified, which have been difficult to appreciate by conventional MR techniques.

Elastin distribution in the myometrial and vascular smooth muscle of the human uterus.

Magnetic resonance imaging and transvaginal ultrasonography in women of reproductive age suggest that the myometrium consists of inner and outer layers. It was hypothesized that these structural and functional differences in the myometrium might be associated with a variation in elastin distribution. Fifty-one hysterectomy specimens representing all phases of the normal menstrual cycle were studied by immunocytochemistry, orcein staining and image analysis. Elastin was present within the outer myometrial smooth muscle, but was less widely distributed in the inner smooth muscle. Immunoreactivit
