Impact of elexacaftor/tezacaftor/ivacaftor on the presence of bacterial and fungal pathogens in the lower respiratory tract of children with cystic fibrosis.

BACKGROUND: Highly effective CFTR modulator therapy, such as elexacaftor/tezacaftor/ivacaftor (ETI), has been linked to significant clinical improvements in people with cystic fibrosis (pwCF). However, its effect on the presence of respiratory pathogens in the lower respiratory tract of younger, mainly non-expectorant children with CF remains poorly understood, especially using lower airway sampling methods. We aimed to investigate changes in lower airway microbiology and clinical outcomes in children with CF aged 6-12 years before and after starting ETI. METHODS: We conducted a prospective o

Recurrent multiterritorial ischemic stroke with persistent moderate hypereosinophilia: A case report with focused literature review.

Persistent hypereosinophilia may be overlooked during stroke evaluation when conventional vascular risks coexist. We report the case of a man in his late 50s who had hypertension, type 2 diabetes, continued active smoking, and eight clinically apparent magnetic resonance imaging-confirmed strokes in multiple bilateral territories over 29 months. Before systemic glucocorticoid exposure, absolute eosinophil counts ranged from 1.87 to 3.23 × 109/L on repeated measurements. Bone marrow morphology and flow cytometry revealed eosinophil expansion without aberrancy, and common tested clonal drivers w

Isoflurane-Assisted Electroacupuncture in a Rat Model of Oligoasthenozoospermia.

Oligoasthenozoospermia is one of the most common forms of male reproductive dysfunction. In rodent studies, delivering electroacupuncture (EA) consistently across repeated sessions poses a technical challenge, particularly when combining abdominal midline and hind-limb acupoints, where stable electrode contact is critical. Zhongji (CV3), Guanyuan (CV4), Zusanli (ST36), and Sanyinjiao (SP6) were selected because these points are frequently used in acupuncture protocols for infertility and semen abnormalities, and the abdominal midline plus hind-limb configuration suits standardized electrode pa

Defining early carcinogenic markers for the prediction of the hazard potential of mineral fibres in an in vitro human 3D bronchial model.

This study aimed to identify early carcinogenic markers to predict the hazard potential of mineral fibres using a human 3D bronchial model. Traditional 2D cell models inadequately mimic lung complexity, prompting the use of more physiologically relevant 3D systems. We investigated the effects of crocidolite (CRO) and two size fractions of chrysotile (≤ and > 5 μm length: CHR S and CHR L) to elucidate early toxicological mechanisms and relate them to the IARC key characteristics (KCs) of carcinogens. Initial analyses (MTT, TEER, and histology) at 24-48 h showed a transient acute toxicity and ti

Mechanisms and therapeutic strategies of asthma: from bench to bedside.

Asthma is a heterogeneous chronic airway disease arising from a complex interplay of genetic susceptibility and environmental exposures. Key pathobiological features include dysregulated immune responses (particularly type 2 inflammation), structural airway remodeling, and mucus hypersecretion. Recent advances have illuminated mechanisms from epithelial "alarmin" cytokine release (TSLP, IL-33, IL-25) to downstream cellular networks involving Th2/Th17 lymphocytes, group 2 innate lymphoid cells (ILC2s), eosinophils, and mast cells. These pathways converge on bronchial hyperresponsiveness and air

Efficacy of Indacaterol vs Tiotropium in COPD patients due to biomass exposure in improving quality of life and reducing symptoms.

BACKGROUND: Few treatment trials have been tested on COPD patients associated with biomass smoke exposure (COPD-B). Patients with COPD-B improve hyperinflation 24 h after inhaling long-acting beta-agonist (LABA) and long-acting muscarinic antagonist (LAMA), and after six months of treatment, improve symptoms, quality of life, and exacerbations. However, the usefulness of LAMA or LABA for an extended period for improving quality of life and symptoms has not yet been demonstrated in the COPD-B phenotype. The primary aim of this trial was to compare tiotropium (TIO) with indacaterol (IND) in impr

Short-Term Effect of Air Pollution on Daily Hospital Visits for Anxiety Disorders in Southern China with Low Pollution Concentrations.

The global prevalence and burden of anxiety disorders (ADs) are increasing. However, findings on the acute effects of air pollution on ADs remain inconclusive. We evaluated the effects of short-term exposure to ambient air pollutants, including fine particulate matter (PM2.5), inhalable particulate matter (PM10), nitrogen dioxide (NO2), carbon monoxide (CO), sulfur dioxide (SO2), and ozone (O3), on daily hospital visits for ADs. A generalized additive model was used to perform a time-series analysis on data from a Southern China city's medical insurance system between 1 March 2021, and 31 July

Continuously producible aztreonam-loaded inhalable lipid nanoparticles for cystic fibrosis-associated Pseudomonas aeruginosa infections - Development and in-vitro characterization.

Cystic fibrosis (CF) is a genetic disorder affecting nearly 105,000 patients worldwide and is characterized by poor respiratory function due to accumulation of thick mucus in the lungs, which not just acts as a physical barrier, but also provides a breeding ground for bacterial infections. These infections can be controlled with the help of antibiotics which can be delivered directly into the lungs for amplifying the local anti-bacterial effect. More than 50 % of CF patients are associated with Pseudomonas aeruginosa infection in their lungs which requires antibiotics such as Aztreonam (AZT).

Current insights and assumptions on α-synuclein in Lewy body disease.

Lewy body disorders are heterogeneous neurological conditions defined by intracellular inclusions composed of misshapen α-synuclein protein aggregates. Although α-synuclein aggregates are only one component of inclusions and not strictly coupled to neurodegeneration, evidence suggests they seed the propagation of Lewy pathology within and across cells. Genetic mutations, genomic multiplications, and sequence polymorphisms of the gene encoding α-synuclein are also causally linked to Lewy body disease. In nonfamilial cases of Lewy body disease, the disease trigger remains unidentified but may ra

Club cell-specific telomere protection protein 1 (TPP1) protects against tobacco smoke-induced lung inflammation, xenobiotic metabolic dysregulation, and injurious responses.

Inhaling xenobiotics, such as tobacco smoke is a major risk factor for pulmonary diseases, e.g., COPD/emphysema, interstitial lung disease, and pre-invasive diseases. Shelterin complex or telosome provides telomeric end protection during replication. Telomere protection protein 1 (TPP1) is one of the main six subunits of the shelterin complex supporting the telomere stability and genomic integrity. Dysfunctional telomeres and shelterin complex are associated as a disease mechanism of tobacco smoke-induced pulmonary damage and disease processes. The airway epithelium is critical to maintaining

Gold Nanoparticles: Tunable Characteristics and Potential for Nasal Drug Delivery.

A general procedure to prepare gold nanourchins (GNUs) via a seed-mediated method was followed using dopamine hydrochloride as a reducing agent and silver nitrate salt (AgNO3) as a shape-directing agent. The novelty of this study comes from the successful incorporation of the prepared gold urchins as an aqueous suspension in a nasal pressurized metered dose inhaler (pMDI) formulation and the investigation of their potential for olfactory targeting for direct nose-to-brain drug delivery (NTBDD). The developed pMDI formulation was composed of 0.025% w/w GNUs, 2% w/w Milli-Q water, and 2% w/w EtO

Changes in mindfulness facets across yoga, CBT and stress education in individuals with generalized anxiety disorder.

BACKGROUND: Systemic corticosteroid use in acute respiratory failure has yielded uncertain benefits, partially because of off-target side effects. Inhaled corticosteroids (ICSs) confer localized antiinflammatory benefits and may protect adults with direct lung injury (DLI) from developing respiratory failure. To our knowledge, this relationship has not been studied in children. RESEARCH QUESTION: Do children with DLI who are prescribed ICSs before hospitalization have lower odds of progressing to respiratory failure? STUDY DESIGN AND METHODS: This retrospective, single-center cohort identifi

An Inhalable Hybrid Biomimetic Nanoplatform for Sequential Drug Release and Remodeling Lung Immune Homeostasis in Acute Lung Injury Treatment.

Interactions of lung macrophages and recruited neutrophils with the lung microenvironment continuously aggravate the dysregulation of lung inflammation in the pathogenesis of acute lung injury (ALI) or acute respiratory distress syndrome (ARDS). Either modulating macrophages or destroying neutrophil counts cannot guarantee a satisfactory outcome in ARDS treatment. Aimed at inhibiting the coordinated action of neutrophils and macrophages and modulating the hyper-inflammatory condition, an inhalable biomimetic sequential drug-releasing nanoplatform was developed for the combinatorial treatment o

Hormonal Contraceptives Are Associated With an Increase in Incidence of Asthma in Women.

BACKGROUND: Use of exogenous female sex hormones is associated with the development of asthma, but the question of whether the effect is protective or harmful remains unresolved. OBJECTIVE: To investigate whether initiation of hormonal contraceptive (HC) treatment was associated with development of asthma. METHODS: We performed a register-based, exposure-matched cohort study including women who initiated HC treatment of any kind between 10 and 40 years of age and compared the incidence of asthma with women who did not initiate HCs. Human Cerebral Perfusion, Oxygen Consumption, and Lactate Production in Response to Hypoxic Exposure.

Exposure to moderate hypoxia in humans leads to cerebral lactate production, which occurs even when the cerebral metabolic rate of oxygen (CMRO2) is unaffected. We searched for the mechanism of this lactate production by testing the hypothesis of upregulation of cerebral glycolysis mediated by hypoxic sensing. Describing the pathways counteracting brain hypoxia could help us understand brain diseases associated with hypoxia. A total of 65 subjects participated in this study: 30 subjects were exposed to poikilocapnic hypoxia, 14 were exposed to isocapnic hypoxia, and 21 were exposed to carbon m
