Low-molecular-weight heparin for hip fracture patients treated with osteosynthesis: should thromboprophylaxis start before or after surgery? An observational study of 45,913 hip fractures reported to the Norwegian Hip Fracture Register.

Background and purpose - Controversies exist regarding thromboprophylaxis in orthopedic surgery. We studied whether the thromboprophylaxis in hip fracture patients treated with osteosynthesis should start preoperatively or postoperatively. Data were extracted from the nationwide Norwegian Hip Fracture Register (NHFR). The risks of postoperative deaths, reoperations, and intraoperative bleeding were studied within 6 months after surgery. Patients and methods - After each operation for hip fracture in Norway the surgeon reports information on the patient, the fracture, and the operation to the N

Incidence and Risk Factors for Posterior Cruciate Ligament Avulsion during Cruciate Retaining Total Knee Arthroplasty.

During cruciate retaining (CR) total knee arthroplasty (TKA), the posterior cruciate ligament (PCL) may avulse at its insertion. The incidence of PCL avulsion fracture has not been previously studied. The aim of this study is to report on the incidence and clinical significance of intraoperative PCL avulsion during primary CR TKA and to identify potential risk factors. Our institutional joint registry was retrospectively reviewed for PCL avulsion occurring during CR TKA implanted between April 2008 and April 2016. Patient demographics, preoperative range of motion (ROM), complications, and rev

A faster and simpler way of operation for Meckel's diverticulum: basal ligation combined with intraoperative frozen section.

BACKGROUND: The key step in Meckel's diverticulectomy (MD) is to achieve complete resection of MD along with the ectopic epithelium. Currently main treatment methods for Meckel's diverticulum are either intestinal resection and anastomosis or wedge resection. Here we introduced a new method to treat MD. The goal of this study was to investigate the clinical effects and advantages of a new operation method for Meckel's diverticulum: basal ligation combined with intraoperative frozen section. METHODS: 262 cases of Meckel's diverticulum were resected with simple basal ligation operation. Intraop

Carotid endarterectomy with adjunctive cephalad carotid stenting: Complementary, not competitive, techniques.

OBJECTIVE: Stenosis of the cephalad internal carotid artery (ICA) can present a challenge, making it difficult to obtain a technically satisfying distal end point during endarterectomy. Surgical revision of distal defects can be difficult and yield unsatisfactory results. The purpose of this review is to evaluate the efficacy of intraoperative carotid stenting as an adjunct to endarterectomy to salvage technical defects identified at the cephalad ICA endarterectomy site. METHODS: Between January 2001 and February 2008, 14 patients were found to have technical defects located at the cephalad I

Scrotal reconstruction using rapid intraoperative tissue expansion: a preliminary report.

PURPOSE: Extensive scrotal skin loss can pose a difficult management problem. In most cases delayed scrotal approximation is feasible due to the redundancy of scrotal skin. However, large defects usually require reconstruction using flaps or grafts. We present a novel technique of scrotal closure using rapid intraoperative tissue expansion to manage select extensive partial scrotal defects. MATERIALS AND METHODS: Between January 1996 and April 2006, 3 patients underwent delayed closure using rapid intraoperative tissue expansion for approximately 75% scrotal skin loss. Intraoperatively the re

Direct clipping of basilar trunk aneurysms using temporary balloon occlusion.

In the surgical treatment of basilar trunk aneurysms, there is still considerable technical difficulty in gaining both proximal artery control and a sufficient operative field. The authors describe their experience in five patients with basilar trunk aneurysms treated using temporary balloon occlusion and intraoperative digital subtraction angiography. With the patient under general anesthesia, a heparinized angiography catheter was guided into the dominant vertebral artery by means of the Seldinger technique. A silicone balloon catheter was introduced coaxially through the angiography cathete

Low-impact laparoscopic sleeve gastrectomy: A preliminary study of feasibility, safety and early outcomes using a single-incision low-pressure pneumoperitoneum approach.

INTRODUCTION: Patients with severe obesity are particularly vulnerable to the physiological effects of pneumoperitoneum during laparoscopic metabolic bariatric surgery (MBS). Low-pressure laparoscopy may reduce cardiopulmonary stress and postoperative pain; however, its feasibility in MBS remains debated. This study aimed to evaluate the feasibility and early outcomes of a low-impact laparoscopic sleeve gastrectomy (LIL-SG) technique, combining single-incision access with low-pressure pneumoperitoneum in patients with severe obesity. METHODS: Data from consecutive patients undergoing LIL-SG w

Comparison of Erector Spinae Plane Block and Intravenous Lidocaine in Opioid-Free Anesthesia for Laparoscopic Sleeve Gastrectomy: A Randomized Controlled Trial.

INTRODUCTION: Opioid-free anesthesia (OFA) protocols are increasingly adopted in metabolic and bariatric surgery to minimize opioid-related complications. Both erector spinae plane block (ESPB) and intravenous (IV) lidocaine have demonstrated analgesic efficacy in various surgical settings, but direct comparisons in an OFA framework for metabolic and bariatric procedures remain limited. METHODS: We conducted a prospective, randomized controlled trial comparing ESPB with IV lidocaine in adults with obesity (BMI ≥ 35 kg/m²) undergoing laparoscopic sleeve gastrectomy under OFA. Patients were rand

Clinical application of cannulated screws tension band and cerclage fixation for displaced patellar fractures based on fracture morphological classification.

INTRODUCTION: Numerous surgical strategies are available for displaced patellar fractures, yet a universally accepted treatment standard remains lacking. Internal fixation constructed on the tension-band principle constitutes the primary treatment modality for such injuries. This study aimed to evaluate the clinical outcomes of two cannulated screw tension-band fixation techniques combined with cerclage fixation for displaced patellar fractures. METHODS: A retrospective study was conducted on patients with displaced patellar fractures. The patients were divided into the group I treated with m

Hair Removal with 755 nm Alexandrite Laser via Subcutaneous Route in Stage-III Ear Reconstruction by Expansion.

BACKGROUND: Congenital microtia reconstruction often introduces unwanted hair onto the new auricle. Laser hair removal can address this issue, but conventional external treatments require multiple sessions, increasing the overall treatment time, discomfort, and cost. Preclinical studies suggest that delivering laser energy from the flap's underside may improve efficiency by directly targeting hair follicles. We evaluated an intraoperative subcutaneous laser depilation approach applied during stage-III surgery to improve hair removal in this setting. METHODS: A prospective cohort study include
