Juvenile Idiopathic Arthritis.

Juvenile idiopathic arthritis is the most common chronic rheumatic disease of unknown aetiology in childhood and predominantly presents with peripheral arthritis. The disease is divided into several subgroups, according to demographic characteristics, clinical features, treatment modalities and disease prognosis. Systemic juvenile idiopathic arthritis, which is one of the most frequent disease subtypes, is characterized by recurrent fever and rash. Oligoarticular juvenile idiopathic arthritis, common among young female patients, is usually accompanied by anti-nuclear antibodie positivity and a

Dickkopf 1 protein circulating levels as a possible biomarker of functional disability and chronic damage in patients with rheumatoid arthritis.

Rheumatoid arthritis (RA) is an inflammatory disease characterized by joint destruction, deformity, lower functionality, and decrease in life expectancy. Wingless signaling pathway (Wnt) has been recently involved in bone homeostasis. Studies suggest that overexpression of the pathway inhibitors, like the Dickkopf 1 protein (DKK1), has been implicated in bone destruction. The objective of this study is to compare circulating levels of DKK1 in different groups of patients with disease activity (remission, low, moderate, high activity,) and functionality status. Three hundred seventy-nine patien

A highly selective inhibitor of I kappa B kinase, BMS-345541, blocks both joint inflammation and destruction in collagen-induced arthritis in mice.

OBJECTIVE: The transcription of several cytokines, cell adhesion molecules, and enzymes involved in the inflammatory and destructive mechanisms of rheumatoid arthritis is dependent on nuclear factor kappa B (NF-kappa B). Because I kappa B kinase (IKK) is critical in transducing the signal-inducible activation of NF-kappa B, we examined whether the highly selective and orally bioavailable IKK inhibitor BMS-345541 is efficacious against collagen-induced arthritis (CIA) in mice. METHODS: Arthritis in DBA/1LacJ male mice was induced by subcutaneous immunization with bovine type II collagen on day

Correlation between Wilkes staging and arthroscopic findings in TMJ internal derangements: A 20-year retrospective study.

PURPOSE: The Wilkes classification is widely used to guide diagnosis and surgical decision-making in temporomandibular joint (TMJ) internal derangements. However, its ability to predict true intra-articular pathology remains uncertain. The purpose of this study was to evaluate the agreement between the Wilkes clinical-radiologic classification and the Bronstein-Merrill arthroscopic staging system. MATERIALS AND METHODS: A retrospective observational study was conducted using the TMJ arthroscopy database of the Department of Oral and Maxillofacial Surgery at Hospital Clínico San Carlos (Madrid

[Strategy and discussion of hip joint preserving surgery treatments for adult osteonecrosis of the femoral head].

As a common disease in orthopedics, osteonecrosis of the femoral head (ONFH) frequently occurs among young and middle-aged people. Hip joint preserving surgery has attracted more and more attention due to the fact that artificial joint prosthesis has a certain useful life and the rate of hip joint revision is high and the higher risk of many complications. There are a large number of operative methods for preserving hip joint in clinical practice without definitive effectiveness. How to choose appropriate surgical methods at the right time is also a continuous understanding. The pathological p

Inter-observer reliability of three different radiographic scores for adult haemophilia.

To evaluate the inter-observer reliability of radiological assessment systems for haemophilic arthropathy, three senior orthopaedic surgeons with expertise in haemophilia independently evaluated a total of 527 joint radiographs of adult haemophilia patients, without any knowledge of the clinical data. This study was the largest study to evaluate the reliability of radiological assessment systems. As for the results, the Arnold-Hilgartner staging system showed moderate reliability (kappa value: κ = 0.44, P = 0.000), and the De Palma grading system and the Pettersson scoring system showed fair r

[Magnetic resonance arthrography indications and technique (I). Upper limb].

Magnetic resonance (MR) is the most effective imaging technique in the diagnosis of articular pathology. MR arthrography (MRA), with intra-articular injection of diluted gadolinium or physiological saline solution has become more common in recent years. The intra-articular fluid facilitates the detection of articular pathology by delineating the articular structures, separating adjacent anatomic structures, and filling potential spaces that originate in or communicate with the joint. MRA provides additional information about the integrity of the articular structures, especially cartilage, fibr

Eminectomy for the treatment of chronic subluxation and recurrent dislocation of the temporomandibular joint and a new method of patient evaluation.

We have performed this technique bilaterally on 39 patients with chronic subluxation and 13 patients with recurrent dislocation since 1985 and have had good results with patient satisfaction. These patients had significantly higher articular eminences than normal control individuals. Also, we have used echography as a new application of an old technique, to evaluate the patients objectively in the preoperative and postoperative periods.
