Luteinizing hormone releasing hormone agonist for contraception in breast feeding women.

During the period of lactation there is a need for a reliable method of contraception since the suppressive effects of lactation on ovulation decline as the duration of breastfeeding is decreased. The aim of this study was to establish that chronic treatment with a LHRH agonist would prevent ovulation throughout the period of lactation and to evaluate the effects of the treatment on estrogen production, bleeding patterns, and nursing practice. Starting 6 weeks postpartum, nine mothers took 300 micrograms LHRH agonist (buserelin), intranasally once daily for the remainder of the duration of bre

[Circadian variations in the content of plasma constituants in the brood mare].

Twenty-one circadian blood sample profiles were made in heavy brood mares during pregnancy, lactation or the dry, non-pregnant period. The mares were fed forage-rich diets containing different levels of energy and nitrogen (table 1). Each profile consisted of 7 samples taken at 8 a.m., 11 a.m., 3 p.m., 7 p.m., 11 p.m., 4 a.m. and 8 a.m. The animals were fed at 8:30 a.m. The eleven plasma components evaluated were glucose, non-esterified fatty acids, beta-hydroxybutyrate, acetate, alanine, insulin (energy metabolites), urea, total protein (nitrogen metabolites), calcium, inorganic phosphorus an

Modified model for end-stage liver disease improves short-term prognosis of hepatitis B virus-related acute-on-chronic liver failure.

AIM: To investigate whether the short-term prognosis of hepatitis B virus (HBV)-related acute-on-chronic liver failure (ACLF) could be improved by using a modified model for end-stage liver disease (MELD) including serum lactate. METHODS: This clinical study was conducted at the First Affiliated Hospital, Fujian Medicine University, China. From 2009 to 2015, 236 patients diagnosed with HBV-related ACLF at our center were recruited for this 3-month follow-up study. Demographic data and serum lactate levels were collected from the patients. The MELD scores with or without serum lactate levels f

A laboratory procedure for the induction of flashbacks.

The authors administered infusions of lactate intravenously to seven patients with a DSM-III diagnosis of posttraumatic stress disorder, six of whom also met DSM-III criteria for panic disorder. The lactate infusions resulted in flashbacks in all seven patients and panic attacks in six patients. The authors conclude that with further development intravenous lactate infusion may be used to study flashbacks and other dissociative phenomena and to determine the relationship between flashbacks and panic anxiety.

Postpartum Care for Patients With Diabetes: A Comprehensive and Whole-Life Approach.

Diabetes in pregnancy is associated with increased risk of cardiometabolic syndrome, mood disorders, intimate partner violence, and additional sequelae. These risks may persist beyond the pregnancy into the postpartum period. Comprehensive management and counseling require an understanding of the physiology of the puerperium, types of potential complications, and evidence behind management options. This review will discuss evidence-based immediate postpartum management, the impact of lactation on diabetes, and the long-term and transgenerational impact of diabetes in pregnancy.

Maternal and Early-Life Nutrition and Health.

Nutritional challenges prior to and during gestation, lactation, and early life are known to influence the lifelong health of the infant. In this editorial, I briefly discuss the 13 articles published in this Special Issue, "Maternal and Early-Life Nutrition and Health". This Special Issue discusses topics including maternal nutrition behaviors, maternal overnutrition/obesity, maternal iron deficiency, breastfeeding, and others. This issue paves the way to better understand perinatal nutrition and how it can impact maternal and offspring health.

Pregnancy and breastfeeding in hidradenitis suppurativa: A review of medication safety.

Hidradenitis suppurativa (HS) is a chronic inflammatory disease that disproportionately affects women of childbearing age. Pregnancy influences HS severity for many women, thus diligent continued management throughout pregnancy and postpartum may be required. Herein, we provide an updated review of pregnancy and lactation safety data for HS medications, including topical antiseptic washes, topical and systemic antibiotics, biologic and nonbiologic immunomodulators, immunosuppressants, adjunct medical therapies, and pain medications, to help guide risk-benefit discussions between providers and

Continuous heart murmur in a 26-year-old woman.

In this report, we describe a continuous murmur heard in a young woman at the beginning of her lactation period, which illustrates a typical example of 'Mammary Souffle', described one century ago. Colour Doppler and pulsed-wave mode echocardiogram allowed to precise, for the first time, the arterial nature of increased systolic and diastolic blood flow in tortuous branches of internal mammary artery, most probably responsible of the murmur, after ruling out any other origin of continuous murmur.

Breastfeeding in the woman with a compromised nervous system.

An increasing number of women with physical limitations are choosing to conceive and are entering the health care system for the care of their pregnancies. These women are faced with the choice of how to feed the child. Although research specific to lactation and nervous system disorders could not be found, this paper reviews selected articles from the literature on three central nervous system disorders: spinal cord injury, multiple sclerosis, and stroke. Potential effects on breastfeeding are discussed.

Epidemiology of metabolic disorders in the periparturient dairy cow.

The descriptive epidemiology and risk factors were reviewed for six clinical disorders: milk fever, downer cow syndrome, hypomagnesemic tetany, udder edema, left displaced abomasum, and ketosis. Data were included also from preliminary analyses of a data set from approximately 61,000 Finnish Ayrshire cows. A web of postulated associations among the metabolic disorders and other risks factors (previous lactation diseases and milk yield, calf factors, certain dry period nutritional factors, dystocia, retained placenta, and metritis) was diagrammed.

Malnutrition and behavior: the performance versus learning problem revisited.

At 100 days of age rats, whose dams suffered severe zinc deficiency and/or undernutrition throughout lactation, were tested in two learning tasks. One test was an original discrimination learning task and the other test was a more difficult reverse discrimination learning task. In the reverse learning test, performance differed significantly between rehabilitated malnourished rats and normal rats. Difference in motivation, and not learning ability, was the explanation for the differences in performance between the malnourished and normal rats.

Impact of Primary Tumor Resection on Type B Lactic Acidosis in a Case of Metastatic Colon Cancer.

Malignancies seldom lead to hyperlactatemia or lactic acidosis. The elimination of the primary tumor is anticipated to result in the amelioration of lactate levels in such situations. A patient with obstructing descending colon cancer was subjected to surgical intervention as their serum lactate levels reached 3.6 mmol/L. The tumor was removed, and the ischemic bowel proximal to it was excised as well. The patient demonstrated signs of recuperation; however, their serum lactate levels persisted at levels exceeding 6.5 mmol/L. Consequently, the patient was subjected to further investigation and

Chronic maxillary sinusitis. Energy metabolism in sinus mucosa and secretion.

The energy metabolism in maxillary sinus mucosa and secretion from 41 patients with, in most cases, recurrent or chronic sinusitis was studied and compared with that in mucosal samples from patients with no previous history of sinus abnormality or infection. In freeze-dried samples, glucose and lactate were measured by enzymatic assays and adenine nucleotides by HPLC. In chronic sinusitis the lactate concentration in the mucosa was increased, probably as a result of increased glycolysis or of impaired diffusion within the mucosa. The lactate concentrations in purulent and non-purulent secretio

D-Lactic acidosis after jejunoileal bypass: identification of organic anions by nuclear magnetic resonance spectroscopy.

A 40-year-old man with jejunoileal bypass developed a syndrome of bizarre behavior, slurred speech, ataxic gait, and inappropriate affect, associated with a metabolic acidosis characterized by an increase in the anion gap. Serum L-lactate level was normal, but high-resolution proton nuclear magnetic resonance spectrums of the patient's serum showed a high concentration of lactate. A diagnosis of D-lactic acidosis was confirmed by a specific enzymatic assay for D-lactate. The D-lactic acidosis was cleared using antibiotic therapy, suggesting that D-lactate is produced from fermentation of inges

The influence of exogenous oestradiol benzoate on the pituitary responsiveness to LHRH during the puerperium in women.

The FSH and LH compared to 25 micrograms LHRH was measured in lactating women and the results compared to a similar group in whom lactation was suppressed by administration of bromocriptine shortly after delivery. The response was also measured in both groups after "challenge" with oestradiol benzoate. In lactating women the FSH response to LHRH was diminished following the oestradiol challenge, as compared with that in unchallenged women. At the same time the LH responses were low in both circumstances, indicating a negative feedback effect of oestradiol. However, the oestradiol challenge res

[Vegetarian and vegan diets among pregnant and breastfeeding women].

The prevalence of vegetarian and vegan diets among pregnant women in Denmark is rising. This paper reviews the maternal and neonatal outcomes associated with such diets, highlighting considerations for supplementation and potential risks, including B12-vitamin deficiency. Recommendations include early dietary assessment, B12-supplementation, and monitoring of key nutrients such as protein and iron. Despite potential challenges, with proper guidance and supplementation, vegetarian and vegan diets can be compatible with a healthy pregnancy and sufficient lactation.

The Effect of Sexual Abuse and Prenatal Substance Use on Successful Breastfeeding.

Barriers to breastfeeding in women with substance use disorders (SUDs) often exist. Neonatal abstinence syndrome-related feeding difficulties, maternal SUD-related maladaptive behaviors, and psychological comorbidities can adversely affect breastfeeding. A neglected barrier that frequently occurs in women with SUDs is a history of sexual abuse. It is important that nurses and providers understand each maternal and/or infant factor that can affect the breastfeeding course to assist effectively with lactation support for these frequently misunderstood dyads.

Ankyloglossia its impact breastfeeding.

This article contrasts two very different experiences of one mother breastfeeding her two sons to demonstrate the potential impact of ankyloglossia on breastfeeding. When too restrictive, ankyloglossia, also known as tongue-tie, can cause the newborn to ineffectively suckle at the breast. Breastfeeding difficulties can occur, such as long feedings or damaged nipples. When nurses, lactation consultants and other providers recognize this situation, they can refer women for further care and treatment, which can ultimately lead to breastfeeding success.

Bilateral galactocele in a male infant: a rare cause of gynecomastia in childhood.

A galactocele is a rare benign breast lesion usually occurring in females during or following lactation. These lesions are a rare cause of breast enlargement in infants and children. In this article we present a 10 month-old boy who was admitted with a two-month history of bilateral progressive breast enlargement, and diagnosed as having galactocele. Our purpose was to emphasize the importance of galactocele as a benign condition in the differential diagnosis of gynecomastia in childhood.
