A randomised, placebo-controlled clinical trial with the α2/3/5 subunit selective GABAA positive allosteric modulator PF-06372865 in patients with chronic low back pain.

The effect of PF-06372865, a subtype-selective positive allosteric modulator of the γ-aminobutyric acid type A (GABAA) receptor, on chronic low back pain was investigated in a randomised, placebo- and active-controlled phase 2 clinical trial. The parallel treatment group trial consisted of a 1-week single-blind placebo run in the phase, followed by 4-week double-blind treatment. Patients were randomised to receive either PF-06372865, naproxen, or placebo twice a day for 4 weeks. The primary end point was the numerical rating score of low back pain intensity after 4 weeks of active treatment. S

What can large population-based birth cohort study ask about past, present and future of children with disorders of development, learning and behaviour?

A large cohort consisting of all children born to mothers from community provides 'natural' selection into different exposures and is a powerful resource for epidemiological research. A large population-based birth cohort with detailed systematic information already recorded, as part of longitudinal medical care, historical and current school data, detailed birth certificate data and all three resources available for every member of the birth cohort, are extremely rare. Our population-based birth cohort consists of all children born between 1976 and 2000 to mothers residing in Olmsted County,

Recurrent Lumbar Disc Herniation After Tubular Microdiscectomy: Analysis of Learning Curve Progression.

OBJECTIVE: Tubular microdiscectomy has become a staple technique among spine surgeons. Yet the associated learning curve, especially its later stages, has not been extensively studied. With studies reporting a higher rate of recurrent herniation using tubular microdiscectomy, surgeons' level of experience becomes of primary importance for the interpretation of such findings. We aimed to analyze possible improvements in the later stages of the learning curve and to identify factors independently associated with recurrent herniation. METHODS: A retrospective study was conducted using prospectiv

Treadmill exercise improves behavioral outcomes and spatial learning memory through up-regulation of reelin signaling pathway in autistic rats.

Autism is a complex neurodevelopmental disability with impairments of social interaction and communication, and repetitive behavior. Reelin is an extracellular glycoprotein that is essential for neuronal migration and brain development. Neuroprotective effects of exercise on various brain insults are well documented, however, the effects of exercise on autism in relation with reelin expression are not clarified. In the present study, we investigated the effects of treadmill exercise on the functional recovery and on the expressions of reelin and its downstream molecules, phosphatidylinositol-3

Convergent evidence for abnormal striatal synaptic plasticity in dystonia.

Dystonia is a functionally disabling movement disorder characterized by abnormal movements and postures. Although substantial recent progress has been made in identifying genetic factors, the pathophysiology of the disease remains a mystery. A provocative suggestion gaining broader acceptance is that some aspect of neural plasticity may be abnormal. There is also evidence that, at least in some forms of dystonia, sensorimotor "use" may be a contributing factor. Most empirical evidence of abnormal plasticity in dystonia comes from measures of sensorimotor cortical organization and physiology. H

Constraint-induced movement therapy (CIMT): pediatric applications.

The purpose of this article is to describe theoretical and research bases for constraint-induced movement therapy (CIMT), to discuss key features and variations in protocols currently in use with children, and to review the results of studies of efficacy. CIMT has been found to be an effective intervention for increasing functional use of the hemiparetic upper extremity in adults with chronic disability from stroke. CIMT developed out of behavioral research on the phenomenon of "learned nonuse" of an upper extremity, commonly observed following sensory and/or motor CNS injury, in which failure

Long-term cognitive impairment and myelination deficiency in a rat model of perinatal hypoxic-ischemic brain injury.

Although periventricular white matter injury is a leading cause of major neurologic disability in premature infants, the relationship between myelination deficiency and long-term cognitive dysfunction is not well understood. The purpose of this study was to investigate oligodendrocytes myelination and long-term spatial cognitive function in rats with perinatal hypoxia-ischemia (HI). Postnatal day 3 (P3) rats were subjected to right carotid artery ligation followed by 2.5 h of hypoxia (6% oxygen). Brain injury during the early and late phases was evaluated by immunostaining at P6 (72 h after th

Effects of cyclooxygenase (COX) inhibition on memory impairment and hippocampal damage in the early period of cerebral hypoperfusion in rats.

Chronic cerebral hypoperfusion is related to neurological disorders and contributes to a cognitive decline. Its experimental model in rats is permanent, bilateral common carotid artery occlusion. The cyclooxygenase (COX) system plays a pivotal role in the evolution of ischemic brain damage. Several COX inhibitors have proved to be neuroprotective in stroke models. We set out to characterize the effects of COX inhibitors in rats with permanent cerebral hypoperfusion. Some of the animals were exposed to two-vessel occlusion (n=72), while the others served as sham-operated controls (n=54). This w

Quantitative generation of diffusion-weighted imaging from non-contrast CT in acute ischemic stroke: a multi-scanner deep-learning model with external validation.

BACKGROUND: Acute ischemic stroke (AIS) is a leading cause of death and disability worldwide, where early diagnosis and treatment are crucial for effective management. Computed tomography (CT), particularly non-contrast CT, is widely used due to its speed and low cost; however, it has limited sensitivity and specificity. Although diffusion-weighted imaging (DWI) provides higher diagnostic accuracy, it is time-consuming and may delay treatment. This study aims to investigate the feasibility of using deep learning techniques to convert non-contrast CT images of patients with cerebral ischemic st

An innovative 2D optical navigation workflow for percutaneous pedicle screw fixation in thoracolumbar fractures: comparison with O-arm 3D navigation.

OBJECTIVE: Thoracolumbar fractures (TLFs) are common spinal injuries caused by high-energy trauma, falls, or osteoporosis. Traditional open surgery may involve substantial blood loss, tissue damage, and prolonged recovery. Percutaneous pedicle screw fixation (PPSF) is minimally invasive and can accelerate rehabilitation, but it often depends on repeated fluoroscopy, increasing radiation exposure and potential neurovascular risk. This study compared the clinical outcomes of two-dimensional (2D) optical navigation and three-dimensional (3D) navigation systems for PPSF in TLFs, focusing on operat

Pain Reprocessing Therapy for migraine: A case series.

Pain Reprocessing Therapy (PRT) is a behavioral intervention that aims to remediate learned threat associations underlying chronic pain. It has shown efficacy in treating chronic low back pain and was explicitly developed to address a broader range of centrally mediated pain conditions. The focus of PRT on learned threat associations dovetails with emerging neurobiological models of migraine, which highlight learned threat associations-nonharmful internal (e.g., hunger, fatigue) or external (e.g., weather changes) cues that are persistently misinterpreted as danger signals-as a modifiable cont

Unique behavioral strategies in visuomotor learning: Hope for the non-learner.

The existence of individual differences in motor learning capability is well known but the behaviors or strategies that contribute to this variability have been vastly understudied. What performance characteristics distinguish an expert level performer from individuals who experience little to no success, those labeled non-learners? We designed a rule-based visuomotor task which requires identification (discovery) and then exploitation of specific explicit and implicit task components that requires a specific movement pattern, the task rule, for goal achievement. When participants first attemp

Disproportionate Representation of Children of Color and Parents with Disabilities in the Child Welfare System: The Intersection of Race/Ethnicity, Immigration Status, and Disability.

An almost 5-year-old girl is referred to a developmental-behavioral clinician for developmental evaluation because of language and learning concerns. Her developmental screening in the primary pediatrics office showed scores concerning for delays in communication, social-emotional, gross, and fine motor domains. Her mother has concerns about her language. Her mother's primary language is Spanish, but the patient and her siblings speak primarily English. She speaks in short phrases and sentences with grammatical errors. Her mother understands approximately 75% of what she says, and strangers un

A Developmental and Sequenced One-to-One Educational Intervention for Autism Spectrum Disorder: A Randomized Single-Blind Controlled Trial.

INTRODUCTION: Individuals with autism spectrum disorder (ASD) who also exhibit severe-to-moderate ranges of intellectual disability (ID) still face many challenges (i.e., less evidence-based trials, less inclusion in school with peers). METHODS: We implemented a novel model called the "Developmental and Sequenced One-to-One Educational Intervention" (DS1-EI) in 5- to 9-year-old children with co-occurring ASD and ID. The treatment protocol was adapted for school implementation by designing it using an educational agenda. The intervention was based on intensity, regular assessments, updating ob

Neuropsychological sequelae of bacterial and viral meningitis.

Survivors of meningitis often complain about neurological and neuropsychological consequences. In this study, the extent of these sequelae was quantified and correlated to MRI findings. Neurological, neuropsychological and neuroradiological examinations were performed with adult patients younger than 70 years, 1-12 years after recovery from bacterial meningitis (BM; n = 59), or from viral meningitis (VM; n = 59). Patients with other potential causes for neuropsychological deficits (e.g. alcoholism) were carefully excluded. Patients were compared to 30 healthy subjects adjusted for age, gender

Measuring quality of life in theory and in practice: a dialogue between philosophical and psychological approaches.

Measuring quality of life is of concern to both philosophers and psychologists, yet the two disciplines typically approach the question in very different ways, ways so diverse that it may look as if they are engaged in such disparate activities that no dialogue between them is possible. In this paper we aim to construct the beginnings of a dialogue between the two disciplines which will show how they could serve each other and yet also show how, from the dialogue, difficult and previously unconsidered issues emerge for both sides.

Decoding the Anxiety Burden in Asia and Developing an XGBoost-Based Online Screening Tool Using Korean Data.

Anxiety disorders impose a substantial burden across Asia, yet comprehensive regional surveillance remains limited. In addition, the utility of the Health-Related Quality of Life Instrument with 8 Items (HINT-8) for anxiety screening in the Korean population has not been fully clarified. This study integrated data from Global Burden of Disease (GBD) 2021 and Korea National Health and Nutrition Examination Survey (KNHANES) to comprehensively assess the prevalence, incidence, and disability-adjusted life year (DALY) rates of Anxiety disorders across 48 Asian countries and territories from 1990 t

The role of metabolic health in neurostructural and cognitive alterations in bipolar disorders.

BACKGROUND: Bipolar disorders (BD) rank among the most disabling conditions, affecting millions worldwide. Cognitive impairment in BD is linked with brain changes and worse functional outcomes. Obesity and metabolic syndrome (MetSy) are overrepresented in BD and are associated with more pronounced brain structural and cognitive alterations in the general population. Here, we studied for the first time the contribution of metabolic dysfunction to both brain and cognitive alterations in BD. METHODS: We recruited 163 participants (76 individuals with BD, 87 controls). We used principal component

Unmasking the noise: aberrant cortical oscillations in visual snow syndrome.

BACKGROUND: Visual snow syndrome (VSS) is characterised by persistent visual static and disabling perceptual disturbances, yet its underlying neural mechanisms remain poorly understood and are frequently confounded by migraine comorbidity. METHODS: We examined resting-state cortical oscillatory activity in VSS (n = 30) using EEG with distributed source modelling. Absolute and normalised spectral power was quantified across cortical regions, and machine-learning classifiers were trained to identify diagnostic spectral signatures. RESULTS: Relative to healthy controls (HC, n = 47), VSS showed a
