Impact of Pain Severity and Opioid Use on Health Care Resource Utilization and Costs Among Patients with Knee and Hip Osteoarthritis.

BACKGROUND: Osteoarthritis (OA) is the most common form of arthritis. The primary symptom of OA-pain-increases the disease burden by negatively affecting daily activities and quality of life. Opioids are often prescribed for treating pain in patients with OA but have questionable benefit-risk profiles. There is limited evidence on the economic impact of pain severity and opioid use among patients with OA. OBJECTIVES: To (a) evaluate the association of pain severity with health care resource utilization (HRU) and costs among patients with knee/hip OA and (b) characterize the association of opi

State variation in opioid treatment policies and opioid-related hospital readmissions.

BACKGROUND: State policy approaches designed to provide opioid treatment options have received significant attention in addressing the opioid epidemic in the United States. In particular, expanded availability of naloxone to reverse overdose, Good Samaritan laws intended to protect individuals who attempt to provide or obtain emergency services for someone experiencing an opioid overdose, and expanded coverage of medication-assisted treatment (MAT) for individuals with opioid abuse or dependence may help curtail hospital readmissions from opioids. The objective of this retrospective cohort stu

Association between insurance status and patient safety in the lumbar spine fusion population.

BACKGROUND CONTEXT: Lumbar fusion is a common and costly procedure in the United States. Reimbursement for surgical procedures is increasingly tied to care quality and patient safety as part of value-based reimbursement programs. The incidence of adverse quality events among lumbar fusion patients is unknown using the definition of care quality (patient safety indicators [PSI]) used by the Centers for Medicare and Medicaid Services (CMS). The association between insurance status and the incidence of PSI is similarly unknown in lumbar fusion patients. PURPOSE: This study sought to determine th

Ill, itinerant, and insured: the top 20 users of emergency departments in Baltimore city.

The purpose of this study was to document the clinical and demographic characteristics of the 20 most frequent users of emergency departments (EDs) in one urban area. We reviewed administrative records from three EDs and two agencies providing services to homeless people in Baltimore City. The top 20 users accounted for 2,079 visits at the three EDs. Their mean age was 48, and median age was 51. Nineteen patients visited at least 2 EDs, 18 were homeless, and 13 had some form of public insurance. The vast majority of visits (86%) were triaged as moderate or high acuity. The five most frequent d

Economic study on the impact of side effects in patients taking oxycodone controlled-release for noncancer pain.

BACKGROUND: Chronic pain is a prevalent condition in the United States. Musculoskeletal pain, including joint and back pain, is the most common type of chronic pain, and many patients with back pain have a neuropathic component. Pain has direct economic consequences. While oxycodone controlled-release (CR) is one of the most widely used oral long-acting opioids for pain, including pain with a neuropathic component, it is often associated with bothersome side effects, resulting in additional medical resource use (MRU) and costs. OBJECTIVE: To examine the impact on MRU and costs to payers of si

Variation in staging and treatment of local and regional breast cancer in the elderly.

BACKGROUND: Few studies of practice variation in the management of early breast cancer for elderly women have examined the process of care in depth. This study evaluated the effects of age and other factors on surgical staging techniques and treatment. METHODS: Virginia cancer registry data were linked with Medicare claims and 1990 census data. The sample included all newly diagnosed patients with pathologic confirmed local and regional breast cancer in 1985-1989 (n = 3,361). Analyses included descriptive univariate statistics and multiple logistic regression analysis for staging and treatmen

Contemporary US Practice Patterns Regarding Anticoagulation and LAA Closure in ICH Survivors With Atrial Fibrillation.

BACKGROUND: Optimal secondary stroke prevention remains uncertain for patients with intracerebral hemorrhage (ICH) and atrial fibrillation (AF). Anticoagulation reduces ischemic stroke in AF but increases hemorrhage. Left atrial appendage (LAA) occlusion is emerging as an alternative to anticoagulation, but few data exist on its safety in patients with ICH. Amid this uncertainty, little is known about US practice patterns in this scenario. METHODS: We performed a study using linked electronic health record, pharmacy, and insurance claims data on >11 million patients at 29 US healthcare organi

Incidence and prevalence of connective tissue disease (CTD)-associated interstitial lung disease in the United States.

OBJECTIVES: Interstitial lung disease (ILD) is a pulmonary manifestation of connective tissue diseases (CTDs). Limited data exist describing the epidemiology of CTD-ILD; therefore, this study aimed to estimate the incidence and prevalence of CTD-ILD in the US, overall and by CTD-ILD subtypes. METHODS: Patients with CTD-ILD and rheumatoid arthritis (RA), systemic sclerosis (SSc), systemic lupus erythematosus (SLE), idiopathic inflammatory myopathy (IIM), mixed CTD (MCTD), and Sjögren's disease (SjD) were identified from Optum® de-identified Electronic Health Record data set (Optum® EHR) and Ma

Initiation of Apixaban Versus Warfarin in Patients With Kidney Failure on Hemodialysis With Newly Diagnosed Atrial Fibrillation: A Target Trial Emulation.

RATIONALE & OBJECTIVE: Patients with kidney failure on hemodialysis (KFHD) and atrial fibrillation (AF) are at high risk of ischemic stroke. Oral anticoagulation (OAC) may reduce this risk but at the expense of excess bleeding. We emulated a randomized comparative effectiveness trial of apixaban versus warfarin initiation in patients with KFHD and newly diagnosed AF. STUDY DESIGN: Trial emulation embedded in the US Renal Data System. SETTING & PARTICIPANTS: Medicare-insured patients with KFHD and newly diagnosed AF without recent OAC use (2014-2019). EXPOSURES: Initiation of apixaban versus

A real-world evaluation of longitudinal healthcare expenses in a health system registry of type-2 diabetes mellitus and cardiovascular disease enabled by the 21st century cures act.

INTRODUCTION: Type 2 diabetes (T2D) is associated with substantial healthcare spending, but quantifying these expenses has been limited to cohorts of self-selected patients or assessments of insurance claims for major healthcare events. Leveraging the 21st Century Cures Act, which mandated reporting hospital-level service, we pursued a comprehensive evaluation of healthcare spending in a diverse cohort of individuals with T2D. METHODS: We designed a pragmatic, observational cohort study of patients with T2D seeking regular care (≥ 1 visit/2 years) across 5 hospitals and an outpatient network

Site-specific heterogeneity in treatment response among patients with metastatic bladder cancer: implications for oligometastatic disease-a retrospective cohort study.

BACKGROUND: Oligometastatic bladder cancer (OMBC) has emerged as a potential clinical entity in which selected patients may benefit from metastasis-directed local therapy in addition to systemic treatment. However, the definition of OMBC remains controversial, particularly regarding whether metastases at all anatomical sites should be considered within the oligometastatic spectrum. This study aimed to evaluate whether bladder cancer metastases at different anatomical sites respond differently to local treatment strategies and whether these metastatic sites are suitable for inclusion within the

Nationwide Analysis of Sociodemographic Disparities in Health-Related Quality of Life of Colon Cancer Survivors.

BACKGROUND: Health-related quality of life (HRQOL) measures patients' overall well-being and can help guide survivorship care. Evidence comparing individual patients' HRQOL pre- and post-diagnosis is limited due to the unpredictable nature of cancer onset. Our study aims to evaluate trends in HRQOL in elderly colon cancer patients and identify predictors of HRQOL change across two key transitions: (1) pre- to post-diagnosis, and (2) at diagnosis versus 2 years later, with the goal of identifying at-risk patients for a significant HRQOL decrease. METHODS: Utilizing the SEER-Medicare Health Out
