Insulin-lowering medications in polycystic ovary syndrome.

A growing body of evidence suggests that serum hyperinsulinemia contributes to the excess ovarian androgen secretion observed in women with PCOS. As a group, women with PCOS are hyperinsulinemic and insulin resistant when compared with weight-matched normal women, but not all PCOS subjects display clear metabolic defects. The small studies using insulin-sensitizing drugs have demonstrated conclusively that a reduction in serum insulin levels is associated with a reduction of ovarian androgen secretion in PCOS, providing further evidence that hyperinsulinemia contributes to hyperandrogenism by

The sixth Carlos E. Rubio Memorial Lecture. Prevention and treatment of variceal hemorrhage.

The medical treatment of portal hypertension has experienced a marked progress in the past decade due to the introduction of effective portal hypotensive therapy. This has been possible because of the better understanding of the pathophysiological mechanisms leading to portal hypertension. A major step forward was the introduction of beta-blockers for the prevention of bleeding and rebleeding from gastroesophageal varices. Effective therapy requires the reduction of the hepatic venous pressure gradient (HVPG) to 12 mmHg or below, or at least by 20% of baseline values. Unfortunately, this is on

Use of eptifibatide as a bridge antiplatelet agent for intrathecal drug delivery system placement.

Use of antiplatelet agents is becoming increasingly common, and their management may require new strategies if neuroaxial techniques are to be employed in patients who will not tolerate discontinuation of antiplatelet therapy. The patient was a 46-year-old man with a past medical history significant for coronary artery disease and who had undergone 14 stents. He developed stent thrombosis (ST) while on clopidogrel. Following the ST, he was subsequently placed on prasugrel. While on prasugrel, the patient presented for an intrathecal drug delivery system (IDDS) trial and placement due to severe

Persistent pulmonary hypertension of the newborn: experience in a single institution.

Persistent pulmonary hypertension of the newborn (PPHN) remains one of the most challenging situations in the neonatal intensive care unit, and it is associated with high mortality and morbidity. The optimal treatment for PPHN is controversial. We report our 9-year experience in the management of PPHN through a retrospective review of 29 neonates with persistent pulmonary hypertension. The diagnosis of PPHN is made by echocardiography and/or preductal and postductal oxygen tension difference. The treatment modalities include supportive medical care, vasodilator therapy, mechanical ventilation

New and emerging therapies for bone metastases in genitourinary cancers.

CONTEXT: Bone metastases are a common feature of advanced genitourinary malignancies and a prominent cause of morbidity and mortality. OBJECTIVE: The objective of this review is to discuss the incidence, pathophysiology, and management of bone metastases in the most prevalent genitourinary malignancies. EVIDENCE ACQUISITION: We reviewed the relevant medical literature, with a particular emphasis on prospective randomized controlled trials. Much of the relevant clinical trial data focus on prostate cancer (PCa). We provide a nonsystematic review and our perspective on the available data. EVI

Heart failure prevalence, medications, and outcomes in transcatheter aortic valve interventions: the real-world analysis.

BACKGROUND: Despite successful transcatheter aortic valve interventions (TAVIs), a residual risk of heart failure (HF) hospitalization persists. RESULTS: A total of 37 490 patients from Optum's de-identified Clinformatics Data Mart were included. HF diagnosis was recorded in 21 283 (56.9%) patients during a year before TAVI. One-/two-year risks for a composite of HF hospitalization and mortality were 15.6% [95% confidence interval (CI), 15.2-16.0]/25.4% (95% CI, 24.8-25.9). Post-TAVI, HF medication prescriptions remained low. MAJOR FINDINGS: HF burden among TAVI recipients is high and HF the

Traumatic Glaucoma Managed With Ab Interno Trabeculotomy (Trabectome®): A Case Report.

A 55-year-old patient sustained a bungee cord injury to his left eye, resulting in a traumatic cataract, vitreous prolapse into the anterior chamber, and extensive angle recession. Despite cataract surgery with pars plana vitrectomy and maximal medical therapy, intraocular pressure (IOP) remained elevated at 40 mmHg. The patient was diagnosed with mixed-mechanism traumatic glaucoma. Ab interno trabeculotomy with the Trabectome® system (MicroSurgical Technology, Redmond, WA, USA) successfully lowered IOP over 12 months, significantly reducing the medication burden. To the best of our knowledge,

Granulomatosis With Polyangiitis Presenting As Orbital Apex Syndrome.

Granulomatosis with polyangiitis (GPA) is a rare small-vessel vasculitis that typically presents with a triad of sinonasal, pulmonary, and renal symptoms. Here, we present the case of a 43-year-old female with a history of substance use disorder who presented with vision changes and worsening left eye pain over five days. Previous evaluations raised concerns about GPA versus cocaine-induced vasculitis, but diagnostic confirmation was hindered by a lack of medical follow-up. Prompt multidisciplinary intervention led to significant improvement following steroid therapy and IV antibiotics, and th

Appendiceal Schistosomiasis Presenting as an Appendiceal Mass.

BACKGROUND: Schistosomiasis has been noted to be one of the most widespread parasitic diseases in the world. It is commonest in developing countries and 85% of the infections have been reported in Sub-Saharan Africa. Presentation with an appendiceal mass has been a rarity among the cases reported thus far. CASE PRESENTATION: We report the case of a 23-year-old man who presented with an appendiceal mass that responded to conservative treatment and had interval appendicectomy with histological confirmation of schistosomal appendicitis. He received post-operative medical therapy for schistosomia

Migraine and Current Pharmacologic Management.

In light of the multiple pharmacologic alternatives for migraine management, the adverse effects associated with different treatment regimens, and their varying efficacy, it is vital to undertake ongoing evaluations and seek a tailored therapeutic strategy. This case report describes two patients who presented with classic migraine headaches, with the first responding to both abortive and prophylactic migraine medications and the second, with status migrainosus-like presentation, responding to the newer antimigraine agents. We also examined current pharmacological therapeutic options and exist

The Opioid Crisis in Missouri: A Call to Action for Physicians, Legislators, and Society.

Opioid overdose deaths in the United States increased sharply over the last decade leading the President to declare a national emergency. The neurobiology of opioid addiction is explored in conjunction with the historical events preceding the current epidemic. A patient-centric perspective is provided along with rationale for contemporary Medical Assisted Therapy (MAT) options to safely reduce overdose deaths and other preventable consequences of prescription misuse and heroin abuse. A multidisciplinary medical provider approach is essential, in addition to legislative efforts to reduce curren

Introduction: From pathogenesis to therapy, deep endometriosis remains a source of controversy.

Deep endometriosis remains a source of controversy. A number of theories may explain its pathogenesis and many arguments support the hypothesis that genetic or epigenetic changes are a prerequisite for development of lesions into deep endometriosis. Deep endometriosis is frequently responsible for pelvic pain, dysmenorrhea, and/or deep dyspareunia, but can also cause obstetrical complications. Diagnosis may be improved by high-quality imaging. Therapeutic approaches are a source of contention as well. In this issue's Views and Reviews, medical and surgical strategies are discussed, and it is e

Resolution of myelofibrosis-associated pulmonary arterial hypertension following allogeneic hematopoietic stem cell transplantation.

We present the case of a 62-year-old man with myelofibrosis-associated pulmonary arterial hypertension (PAH) who underwent allogeneic hematopoietic stem cell transplantation with subsequent resolution of disease and PAH. Right heart catheterization was used to guide PAH therapy before and after transplantation. Drug interactions, adverse effects, and renal insufficiency posed clinical challenges for the management of PAH-specific medications after transplantation. PAH improved soon after transplantation, and vasoactive medications were tapered off. Resolution of PAH was confirmed with repeat m

A practical overview of considerations for penile prosthesis placement.

Penile prostheses have remained the gold-standard therapy for medically refractory erectile dysfunction (ED) since their popularization. Advances in device design and surgical techniques have yielded improved rates of infection, satisfaction, and mechanical survival of devices. Operative techniques in penile prosthesis surgery include the use of adjunctive procedures (such as ventral phalloplasty and release of the suspensory ligament), management of penile fibrosis, and manoeuvres to correct Peyronie's-disease-related curvature. Complications include urethral and corporal perforation, crossov

Tracheal foreign bodies: are radiographs misleading?

OBJECTIVE: This study aimed to highlight the pitfalls of relying on radiographs in identifying potentially life-threatening upper airway foreign bodies in children. DESIGN: Case series. METHODS: A review of the medical records of 2 cases of upper tracheal foreign body was performed. RESULTS: Both patients presented with biphasic stridor refractory to medical therapy and chest radiographs that were normal. One patient had a history of witnessed aspiration while the other did not. Both patients were managed by endoscopic removal of foreign bodies in the operating room and experienced no long-

Fulminant myocarditis associated with novel H1N1 influenza A.

Myocarditis secondary to H1N1 influenza has been described in children, but only very rarely in adults. We describe a 36-year-old man with no significant medical history who presented with flu-like symptoms of 3-week duration. When he sought medical attention, he was already manifesting heart failure secondary to fulminant myocarditis, along with multiorgan failure. Despite aggressive management, including circulatory support with a catheter-based mechanical cardiac assist device (Impella 2.5 Cardiac Assist Device, Abiomed, Danvers, MA) as a bridge to cardiac transplant, and aggressive antivir

Ebstein malformation of the tricuspid valve: current concepts in management and outcomes.

Ebstein anomaly is a myopathy of the right ventricle that results in variable degrees of failure of delamination of the tricuspid valve leaflets from the underlying endocardium, leading to severe tricuspid valve regurgitation and some degree of right ventricular dysfunction. In neonates or infants who remain in congestive heart failure or profoundly cyanotic while receiving appropriate medical therapy, operation is required. Current strategies include biventricular or single-ventricle repair. In children and adults, medical management may be used, but most patients eventually require surgery.
