Ocular neuromyotonia. A clinical description of six patients.

We report the cases of six patients with ocular neuromyotonia, a disorder believed to result from episodic involuntary discharge of ocular motor nerves producing sustained and inappropriate contraction of their respective ocular muscles. Transient disturbances of ocular motility and diplopia result. Four patients had involvement of ocular muscles supplied by the third cranial nerve: one had presumed involvement of the superior oblique muscle, and one the lateral rectus muscle, suggesting abnormal discharge in the fourth and sixth cranial nerves, respectively. Four of six patients received prio

Pattern reversal visual evoked potentials. Studies in Charcot-Marie-Tooth hereditary neuropathy.

Pattern reversal visual evoked potentials (PRVEP) were studied in 25 patients from 17 separate families with Charcot-Marie-Tooth (CMT) syndrome. Twenty patients had classic CMT with depressed tendon reflexes, slow motor nerve conduction velocities, and autosomal dominant inheritance. Four patients demonstrated typical findings of CMT, but there was no documented family history. One patient had the axonal form of hereditary motor sensory neuropathy. Abnormally delayed latencies of the PRVEP were found in four patients (16% of the total), including the only patient with clinical optic atrophy. T

Chronic polyradiculoneuropathy of infancy. A report of three cases with familial incidence.

Two siblings and a third child exhibited a syndrome of progressive muscular weakness and wasting, closely resembling Werdnig-Hoffmann's disease. Autopsy of one of the siblings and the third child showed nearly total absence of myelin sheaths in the cranial and spinal nerve roots, relative preservation of axons, and normal neurons in the motor cranial nerve nuclei and anterior spinal gray matter. The mother of the siblings had bilateral pes cavus, and the father of the third child had a sensory-motor polyneuropathy dating to childhood, associated with pes cavus and scoliosis. The disorder in th

The Cognitive Functions Dementia Battery: A Novel Computerized Tool for Neurocognitive Disorders.

The Cognitive Functions Dementia (CFD) is a computerized battery to assess the cognitive domains included in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) criteria for neurocognitive disorders. We aimed at examining the psychometric properties and the clinical validity of the CFD for neurocognitive disorders. Psychometric characterization was conducted in healthy individuals, stratified according to age, sex, and education to represent a norming sample. Analyses included structural validity and measurement invariance, assessed through confirmatory factor analyses (CFA) and

Abnormal Cerebrospinal Fluid Cytology in Functional Movement Disorders.

OBJECTIVE: The role of inflammation and neuroimmune mechanisms, which have been documented in various neuropsychiatric disorders including the seizure subtype of functional neurological disorder, remains unclear in functional movement disorders (FMD). To explore these mechanisms, we analyzed selected inflammatory markers in cerebrospinal fluid (CSF) in patients with FMD. METHODS: We compared CSF markers in 26 patients with clinically established FMD (20 females; mean [SD] age = 43.3 [10.9], disease duration = 3.9 [3], range = 0.1-11 years; mean follow-up after lumbar puncture = 4.3 [2] years,

Optogenetically-inspired neuromodulation: Translating basic discoveries into therapeutic strategies.

Optogenetic tools allow for the selective activation, inhibition or modulation of genetically-defined neural circuits with incredible temporal precision. Over the past decade, application of these tools in preclinical models of psychiatric disease has advanced our understanding the neural circuit basis of maladaptive behaviors in these disorders. Despite their power as an investigational tool, optogenetics cannot yet be applied in the clinical for the treatment of neurological and psychiatric disorders. To date, deep brain stimulation (DBS) is the only clinical treatment that can be used to ac

Eye Movement Abnormalities in Multiple Sclerosis: Pathogenesis, Modeling, and Treatment.

Multiple sclerosis (MS) commonly causes eye movement abnormalities that may have a significant impact on patients' disability. Inflammatory demyelinating lesions, especially occurring in the posterior fossa, result in a wide range of disorders, spanning from acquired pendular nystagmus (APN) to internuclear ophthalmoplegia (INO), among the most common. As the control of eye movements is well understood in terms of anatomical substrate and underlying physiological network, studying ocular motor abnormalities in MS provides a unique opportunity to gain insights into mechanisms of disease. [Motility and functional gastrointestinal disorders].

This article discusses the studies on functional and motor gastrointestinal disorders presented at the 2014 Digestive Diseases Week conference that are of greatest interest to us. New data have been provided on the clinical importance of functional gastrointestinal disorders, with recent prevalence data for irritable bowel syndrome and fecal incontinence. We know more about the pathophysiological mechanisms of the various functional disorders, especially irritable bowel syndrome, which has had the largest number of studies. Thus, we have gained new data on microinflammation, genetics, microbio

Pharmacotherapeutics of Tourette syndrome and stereotypies in autism.

Tourette syndrome (TS) and stereotypy in autism spectrum disorders (ASDs) are 2 common movement disorders in childhood. The objective of this review was to summarize randomized controlled trials published over the past 5 years as an update of the current pharmacotherapeutic options for the treatment of tics, TS, and motor stereotypies in children with ASD. We searched MEDLINE (2005-May 2010) for randomized controlled trials of medications used for the treatment of these disorders. For the treatment of tics in TS, 2 trials suggest that levetiracetam is not effective, whereas 1 trial found that

CNTN6 Modulates Slit-Robo Signaling in Cortical GABAergic Interneuron Migration.

Contactin-6 (CNTN6) is a membrane protein that belongs to the contactin subgroup of the immunoglobulin (Ig) superfamily. Substantial evidence supports a significant role of its encoding gene, CNTN6, in neurodevelopment and psychiatric disorders, including autism spectrum disorders and schizophrenia. Further studies are required to clarify the role of CNTN6 in the etiology of these disorders. Here, we show that the first three Ig domains of the extracellular region of CNTN6 compete with Robo2 for binding to the leucine-rich repeat (LRR) domain of Slit2 and inhibit Slit-Robo signaling, thereby r

Chloride Homeostasis in Neuronal Disorders: Bridging Measurement to Therapy.

Neuronal chloride (Cl-) homeostasis is fundamental for brain function, with disruptions increasingly recognized as pathogenic across neurological disorders. This review synthesizes evidence from preclinical models and clinical studies, integrating electrophysiological measurements, molecular analyses, imaging with genetically encoded sensors like ClopHensor, and behavioral assays. Key findings demonstrate that Cl- dysregulation follows distinct patterns: (1) in epilepsy, KCC2 downregulation converts GABAergic inhibition to excitation, promoting seizures; (2) in Alzheimer's disease (AD) models,

Population-based assessment of neurodevelopmental and mental health diagnoses among pediatric patients with Turner Syndrome: A PEDSnet study.

Individuals with Turner syndrome (TS) are known to be at increased risk for neurodevelopmental disorders (NDD) and mental health (MH) conditions, but data from large, population-based pediatric samples remain limited. We examined the prevalence of NDD and MH diagnoses among youth with TS (N = 2,145) compared to matched female controls (N = 8,580) across six U.S. pediatric health systems. Odds ratios (OR) and 95% confidence intervals (CI) were calculated using generalized estimating equations. Youth with TS had significantly higher odds of an NDD diagnosis (24.2% vs. 11.9%; OR 2.37, 95% CI 2.11

[Algorithms for the diagnosis and treatment of cognitive impairment and dysphagia in stroke patients].

Stroke is a socially significant neurological disease, the second most common cause of disability and mortality. A wide range of neurological problems that occur after stroke: cognitive, motor, speech, and language disfunction, neuropsychiatric, swallowing disorders and others, complicate rehabilitation, impair social and everyday adaptation, and reduce the quality of life of patients and their caregivers. Cognitive impairment (CI) is one of the most significant and common complications of stroke. Stroke increases the risk of their development by 5-8 times. Dysphagia is also a common symptom o

Exosomes: A Cellular Communication Medium That Has Multiple Effects On Brain Diseases.

Exosomes, as membranous vesicles generated by multiple cell types and secreted to extracellular space, play a crucial role in a range of brain injury-related brain disorders by transporting diverse proteins, RNA, DNA fragments, and other functional substances. The nervous system's pathogenic mechanisms are complicated, involving pathological processes like as inflammation, apoptosis, oxidative stress, and autophagy, all of which result in blood-brain barrier damage, cognitive impairment, and even loss of normal motor function. Exosomes have been linked to the incidence and progression of brain

Cerebellar mutism syndrome: the importance of preoperative language assessment.

Children undergoing surgical removal of tumors in the posterior cranial fossa can encounter a varied and complex constellation of neurological symptoms, called cerebellar mutism, defined as a disturbance in the planning and programming of motor language with preserved understanding, behavioral disorders such as inattention, visual-spatial disorganization, personality change, as well as ataxia and dysmetria. In the last years, several groups have been trying to establish risk factors or even predictive scores in order to be able at least in part to predict the appearance of speech disorders bef
