Neuropsychological functioning and progression in Lafora disease.

OBJECTIVE: Lafora disease (LD) is a rare and severe form of progressive myoclonus epilepsy characterized by cognitive decline that ultimately leads to dementia. However, its cognitive features and adaptive functioning remain poorly understood. This study aimed to characterize the neuropsychological profile of LD and its progression. METHODS: In this monocentric cohort study, consecutive LD patients were administered a battery including cognitive measures (Leiter-3, Children's Color Trail Test [CCTT], Wechsler Adult Intelligence Scale-IV Vocabulary, and Visual-Motor Integration [VMI] test) and

Sex differences in dengue-related neurological complications: insights from the 2023 Taiwan outbreak.

Dengue is a prevalent vector-borne disease that threatens nearly half of the world's population. Although not traditionally classified as neurotropic, dengue can lead to a wide range of neurological manifestations beyond the conventional spectrum of disease severity. This study investigated neurological manifestations during the 2023 dengue outbreak in Tainan, Taiwan, with a focus on the observed male predominance in symptomatic and severe cases. In 2023, the national infectious disease reporting system recorded 21 513 dengue cases in Tainan, of whom 53% were male. Among these, 768 patients (5

The spearmint extract L-carvone as a euthanasia agent in sheep.

L-carvone is a natural monoterpene found in spearmint and other foods that can be administered intravenously to euthanize rats. We hypothesized intravenous L-carvone rapidly alters consciousness and produces death in sheep, resulting in tissue L-carvone concentrations that are commonly found in foods. Jugular catheters were placed in five adult Hampshire-Suffolk ewes, and a carotid arterial catheter was inadvertently placed in a sixth ewe. Animals were restrained in a seated position and instrumented with surface electroencephalographic (EEG) and electromyographic electrodes and a two-lead ele

Interleaved stimulation in globus pallidus internus deep brain stimulation in a case of adult-onset Huntington's disease.

BACKGROUND: Huntington’s disease (HD) is an autosomal dominant neurodegenerative disorder caused by pathological expansion of CAG repeats in the HTT gene. A higher CAG repeat count (≥ 55) is generally associated with an earlier age at onset, more rapid disease progression, and a less favorable prognosis. In recent years, deep brain stimulation (DBS) targeting the globus pallidus internus (GPi) has been explored as a therapeutic intervention for medically refractory chorea; however, the available clinical evidence is largely limited to small case series and case reports, and optimal programming

Case Report: Compound heterozygous KCTD7 variants in two siblings presenting with myoclonic epilepsy and ataxia.

OBJECTIVE: Biallelic variants in KCTD7 have been associated with progressive myoclonic epilepsy (PME), a rare autosomal recessive disorder characterized by early-onset epilepsy, cognitive decline, myoclonus, and ataxia. METHODS: Whole-exome sequencing was first performed in the elder sister to identify candidate variants, followed by in silico pathogenicity prediction. Sanger sequencing was then used to validate the variants in both parents and the younger brother. RESULTS: We report two siblings with progressive myoclonic epilepsy (PME) carrying compound heterozygous KCTD7 variants: c.334C

Exploring the role of apolipoprotein ε4 in progressive myoclonic epilepsy type 1.

OBJECTIVE: Progressive myoclonic epilepsy type 1 (EPM1) is a neurodegenerative disease caused by biallelic variants in the cystatin B (CSTB) gene. Despite a progressive course, phenotype severity varies among patients, even within families. We studied the potential role of APOE ε4 in modifying phenotypic diversity in EPM1, given its established association with neurodegeneration, particularly in Alzheimer's disease. METHODS: APOE genotypes were determined for 65 genetically verified EPM1 patients homozygous for the CSTB expansion mutation. The Unified Myoclonus Rating Scale (UMRS), Quality of

G protein-coupled receptor (GPCR) gene variants and human genetic disease.

Genetic variations in the genes encoding G protein-coupled receptors (GPCRs) can disrupt receptor structure and function, which can result in human genetic diseases. Disease-causing mutations have been reported in at least 55 GPCRs for more than 66 monogenic diseases in humans. The spectrum of pathogenic and likely pathogenic variants includes loss of function variants that decrease receptor signaling on one extreme and gain of function that may result in biased signaling or constitutive activity, originally modeled on prototypical rhodopsin GPCR variants identified in retinitis pigmentosa, on

Exploring individual fixel-based white matter abnormalities in epilepsy.

Diffusion MRI has provided insight into the widespread structural connectivity changes that characterize epilepsies. Although syndrome-specific white matter abnormalities have been demonstrated, studies to date have predominantly relied on statistical comparisons between patient and control groups. For diffusion MRI techniques to be of clinical value, they should be able to detect white matter microstructural changes in individual patients. In this study, we apply an individualized approach to a technique known as fixel-based analysis, to examine fibre-tract-specific abnormalities in individua

Long-term electro-clinical profile of sudden cardiac arrest survivors.

OBJECTIVE: Recent research has explored the use of continuous EEG (cEEG) monitoring for prognostication of spontaneous cardiac arrest (SCA). However, there is limited literature on the long-term (post-hospital discharge) electrographic findings among SCA survivors and their clinical correlates. Our study aims to fill this critical knowledge gap. METHODS: We retrospectively used our EEG database to identify adults (≥18 years) with SCA history who underwent an outpatient laboratory-based EEG between 01/01/2011 and 12/31/2018. After electronic medical records (EMR) review, patients with epilepsy

Pallidal degenerations and related disorders: an update.

Neurodegenerative disorders involving preferentially the globus pallidus, its efferet and afferent circuits and/or related neuronal systems are rare. They include a variety of both familial and sporadic progressive movement disorders, clinically manifesting as choreoathetosis, dystonia, Parkinsonism, akinesia or myoclonus, often associated with seizures, mental impairment and motor or cerebellar symptoms. Based on the involved neuronal systems, this heterogenous group has been classified into several subgroups: "pure" pallidal atrophy (PPA) and extended forms, pallidonigral and pallidonigrospi

Adrenal ganglioneuroma: Prognostic factors (Review).

Ganglioneuroma, a rare neural crest-derived tumor, exhibits a benign profile in contrast to other neuroblastic tumors (neuroblastoma/ganglioneuroblastoma). Ganglioneuromas can be found anywhere autonomic ganglia are located, mostly abdominal/pelvic sites followed by the adrenal glands (one-third of cases), mediastinum/thorax and cervical area. Affecting especially children more than 10 years of age, Ganglioneuroma is either asymptomatic or may cause local compressive effects; rarely inducing nonspecific abdominal complains or arterial hypertension related to oversecretion of epinephrine/norepi

Identification of intracerebral hemorrhage in the early-phase of MM1+2C-type sporadic Creutzfeldt-Jakob disease: A case report.

We report a case of early-phase sporadic Creutzfeldt-Jakob disease (sCJD) complicated by intracerebral hemorrhage (ICH), classified as MM1 + 2C-type based on autopsy. A 61-year-old Japanese man presented to our hospital with speaking difficulties including repeated usage of the same words. He was hospitalized on the seventh day after symptom onset, and diffusion-weighted images on magnetic resonance imaging showed hyperintense regions in the frontal cortex and caudate nucleus. On the 11th day after symptom onset, head computed tomography revealed ICH in the right occipital and parietal lobes.

Hot-spot KIF5A mutations cause familial ALS.

Heterozygous missense mutations in the N-terminal motor or coiled-coil domains of the kinesin family member 5A (KIF5A) gene cause monogenic spastic paraplegia (HSP10) and Charcot-Marie-Tooth disease type 2 (CMT2). Moreover, heterozygous de novo frame-shift mutations in the C-terminal domain of KIF5A are associated with neonatal intractable myoclonus, a neurodevelopmental syndrome. These findings, together with the observation that many of the disease genes associated with amyotrophic lateral sclerosis disrupt cytoskeletal function and intracellular transport, led us to hypothesize that mutatio
