Demyelinating inflammatory neuropathies, including Guillain-Barré syndrome.

The highly complex and multiple mechanisms responsible for the development of demyelinating neuropathies are reviewed, in particular Guillain-Barré syndrome and its variant Miller Fisher syndrome, chronic inflammatory demyelinating neuropathy, multifocal motor neuropathy, anti-myelin-associated glycoprotein neuropathy, as well as experimental models. Recent investigations into the role of auto antibodies against myelin proteins, or glycolipids have given insights into the pathogenesis of demyelinating inflammatory neuropathies.

Isolated mucocele in an Onodi cell.

A rare case of retrobulbar optic neuropathy caused by an isolated mucocele in an Onodi cell is presented. A 63-year-old man suffering from left recurrent optic neuropathy had shown improved visual acuity with steroid treatment at the age of 48 and 56 years. Impaired visual acuity and blurred vision in the left eye appeared again, and a marked inferior decrease in the left visual field was noted. Although axial computed tomography (CT) showed no abnormal findings, magnetic resonance imaging (MRI) revealed a small squamous-shaped lesion (17 x 17 x 7.3 mm) in the left posterior ethmoidal air cell

[Neuralgic amyotrophy or an isolated lesion of the anterior interosseal nerve?].

In this article we describe five patients with acute or subacute weakness of flexor pollicis longus and flexor digitorum profundus. A possible diagnosis of an isolated lesion (entrapment) of the anterior interosseus nerve was considered. However, clinical and neurophysiological findings suggested a diagnosis of neuralgic amyotrophy. Three patients experienced acute shoulder pain at the onset. Sensory loss at the base of the thumb was observed in two patients, and two patients were affected bilaterally. All patients had EMG signs of involvement outside the anterior interosseus nerve innervation

Parametric and non-parametric tests for the overall comparison of several treatments to a control when treatment is expected to increase variability.

We consider the problem of making an overall comparison of several treatments to a control where experimental units are randomly assigned to either the 'control' group which receives no treatment or to one of k-1 'treatment' groups. We assume that the effect of the treatments is, if anything, a location shift possibly accompanied by an increase in scale relative to that of the control group. The ANOVA F test loses considerable power in such circumstances. A modification of the ANOVA F test has been proposed which uses the variance estimate from the controls in place of the usual pooled varianc

Combined Sever's release of the shoulder and osteotomy of the humerus for Erb's palsy.

In children with Erb's palsy who do not recover fully, the most common residual deformity is internal rotation-adduction contracture of the shoulder. Surgical correction of this contracture is described in this paper in 12 consecutive patients by combined Sever's release of the shoulder and osteotomy of the humerus. After an average follow-up of 3 years, the average gain of active external rotation of the shoulder was 32 degrees and average gain of active abduction was 61 degrees.

Ganglionitis in paraneoplastic subacute sensory neuronopathy: a morphologic study.

A 69-year-old woman presented with subacute sensory neuropathy and autonomic dysfunction of 9 months' duration, associated with high serum titers of anti-Hu antibodies. A small cell carcinoma of the lung was diagnosed by biopsy. She died after cardiorespiratory arrest. At autopsy, spinal and autonomic ganglia showed subacute inflammation with diffuse endoneurial T-cell, B-cell, and plasma cell infiltration. The cytoplasm and nuclei of some ganglion neurons displayed IgG immunocytochemical positivity. CD8+ T cells were tightly attached to, and indented the cell surface of, IgG-positive and IgG-

Participation of cryoglobulinaemia in the severe peripheral neuropathies of primary Sjögren's syndrome.

A peripheral neuropathy is encountered in 10 to 50% of patients with primary Sjögren's syndrome (pSS). The pathogenesis of this complication remains unknown, but severe cases of peripheral neuropathies seem to result from a vasculitis. Since about 20% of pSS are associated with a cryoglobulinaemia, we initiated a retrospective study to assess the participation of cryoglobulinaemia in the severe peripheral neuropathies (SPN) of pSS. We found 6 cases of SPN (presence of pain and/or muscle weakness) in a group of 115 pSS (5%): 3 polyneuropathies and 3 mononeuritis multiplex. In all cases, the inv

Diffuse unilateral subacute neuroretinitis. Morphometric, serologic, and epidemiologic support for Baylisascaris as a causative agent.

PURPOSE: Several nematodes have been postulated as etiologic agents in diffuse unilateral subacute neuroretinitis (DUSN), but the cause of this condition remains uncertain. The authors report the first case of DUSN from the western United States (northern California), along with morphometric, serologic, and epidemiologic evidence supporting Baylisascaris procyonis as its cause. METHODS: One patient was examined and evaluated for disc edema and transient obscurations of vision. A diagnosis of DUSN was confirmed when a motile nematode was identified within the substance of the patient's retina.

Toxoplasmosis neuroretinitis.

BACKGROUND: Neuroretinitis is a distinct clinical entity consisting of moderate to severe visual loss, optic nerve head edema, macular exudate in a stellate pattern, and variable vitreous inflammation. Although the etiology is usually postviral or idiopathic, an acute infectious cause occasionally is demonstrated. METHODS: Five juvenile or young adult patients with neuroretinitis are presented with serologic evidence of Toxoplasma gondii infection. Four of the five patients were treated with systemic antibiotics and corticosteroids; one patient was not treated. RESULTS: With a mean follow-up

Nonvasculitic, steroid-responsive mononeuritis multiplex.

We report two patients with mononeuritis multiplex, both of whom had focal inflammation of the perineurium and endoneurium on sural nerve biopsy without necrosis of blood vessel walls, histologic evidence of lymphoid malignancy, or mycobacterial infection. The predominant early sensory symptoms were asymmetric pain and paresthesias; subsequently, muscle weakness developed. Electrophysiologic studies showed an asymmetric sensorimotor axon loss radiculoneuropathy with denervation of limb and paraspinal muscles. Spinal fluid protein was elevated in one patient. There was no cause or underlying sy

[Varying clinical pictures among young children with influenza virus type A infections].

The clinical course of Influenza type A virus infections in 47 hospitalized children aged 0-9 years was assessed retrospectively. The infection was diagnosed by demonstration of the virus in the nasopharyngeal secretion during the acute phase of the illness. Out of 21 Influenza A strains in which the subtype was determined, one was found to be H1- and 20 were H3-subtype. Lower respiratory tract disease was the main diagnosis in 21 children, 16 of whom had pneumonia; 14 of these patients were under three years of age. Gastro-intestinal symptoms occurred in 40% of the children over three years o

Mononeuritis multiplex in a child with cutaneous polyarteritis.

A 14-year-old boy presented with a febrile illness associated with arthritis. Shortly later he developed mononeuritis multiplex. After certain typical skin lesions had developed after two months, the diagnosis cutaneous polyarteritis could be made. The diagnostic features of this benign disease, which may involve peripheral nerves, are discussed.

Perimyositis with perineuritis and myofiber type grouping in the eosinophilia myalgia syndrome associated with tryptophan ingestion.

Skeletal muscle biopsies from five patients with severe myalgias, peripheral eosinophilia, and a recent history of L-tryptophan ingestion were analyzed. Perimysial inflammation, predominantly mononuclear with variable numbers of eosinophils was seen (in five of five patients), which was perineurial (in three of five) and perivascular (in five of five) in location. Grouping of the myofiber types was identified by enzyme histochemistry in two of four patients; fresh muscle for histochemical studies was unavailable from one patient. An occasional degenerating myofiber was seen in only one patient

Giant cell arteritis. A case with unusual neurologic manifestations and a normal sedimentation rate.

Giant cell (temporal) arteritis is a serious inflammatory condition that can lead to blindness, stroke, or other adverse sequelae if not properly treated. An elevated erythrocyte sedimentation rate has traditionally been emphasized as a criterion for making this diagnosis. Delays in diagnosis and unnecessary testing may occur when a patient presents with a normal erythrocyte sedimentation rate and a clinical history consistent with this condition. We describe a patient with giant cell arteritis who presented with a normal erythrocyte sedimentation rate and who subsequently developed devastatin

[Qualitative and quantitative diagnostic possibilities of papillary colorimetry].

Papillary colorimetry is a qualitative and quantitative diagnostic method that defines the displacement of the temporal and nasal color ranges on the x;y plane of the German Industrial Standard (DIN) color chart. Starting with normal temporal and nasal papillary stain values, there are, for qualitative diagnosis, four different directions in which the color range can shift from normal values on the DIN color chart, i.e., in connection with simple optic atrophies, optic atrophies due to vascular conditions, hyperemic papillae, and deviations of the color range under vasodilative therapy. Mononeuritis multiplex in incomplete Behçet's disease: a case report and the review of the literature.

A case of Behçet's disease with peripheral nervous system involvement is described. A 58-year-old female with a 16-year history of Behçet's disease was admitted to our hospital because of numbness in multiple areas in both the upper and lower extremities. A biopsy of the sural nerve revealed degeneration of the axons and a capillary lesion. This finding was compatible with mononeuritis multiplex. This case of Behçet's disease with biopsy-proved mononeuritis multiplex is a very rare one.
