HIV-1 drug resistance before initiation or re-initiation of first-line antiretroviral therapy in low-income and middle-income countries: a systematic review and meta-regression analysis.

BACKGROUND: Pretreatment drug resistance in people initiating or re-initiating antiretroviral therapy (ART) containing non-nucleoside reverse transcriptase inhibitors (NNRTIs) might compromise HIV control in low-income and middle-income countries (LMICs). We aimed to assess the scale of this problem and whether it is associated with the intiation or re-initiation of ART in people who have had previous exposure to antiretroviral drugs. METHODS: This study was a systematic review and meta-regression analysis. We assessed regional prevalence of pretreatment drug resistance and risk of pretreatme

Tuberculosis incidence is high in HIV-infected African children but is reduced by co-trimoxazole and time on antiretroviral therapy.

BACKGROUND: There are few data on tuberculosis (TB) incidence in HIV-infected children on antiretroviral therapy (ART). Observational studies suggest co-trimoxazole prophylaxis may prevent TB, but there are no randomized data supporting this. The ARROW trial, which enrolled HIV-infected children initiating ART in Uganda and Zimbabwe and included randomized cessation of co-trimoxazole prophylaxis, provided an opportunity to estimate the incidence of TB over time, to explore potential risk factors for TB, and to evaluate the effect of stopping co-trimoxazole prophylaxis. METHODS: Of 1,206 child

Emergence of multiclass drug-resistance in HIV-2 in antiretroviral-treated individuals in Senegal: implications for HIV-2 treatment in resouce-limited West Africa.

BACKGROUND: The efficacy of various antiretroviral (ARV) therapy regimens for human immunodeficiency virus type 2 (HIV-2) infection remains unclear. HIV-2 is intrinsically resistant to the nonnucleoside reverse-transcriptase inhibitors and to enfuvirtide and may also be less susceptible than HIV-1 to some protease inhibitors (PIs). However, the mutations in HIV-2 that confer ARV resistance are not well characterized. METHODS: Twenty-three patients were studied as part of an ongoing prospective longitudinal cohort study of ARV therapy for HIV-2 infection in Senegal. Patients were treated with

Platelets, a typical source of error in real-time PCR quantification of mitochondrial DNA content in human peripheral blood cells.

Nucleoside analogues can induce toxic effects on mitochondria by inhibiting the human DNA polymerase-gamma. The clinically observed toxicities can range from slightly increased serum lactate levels to potentially severe and fatal lactic acidosis. A growing interest exists for detection of changes in mitochondrial (mt) DNA content in patients receiving antiretroviral therapy (HAART). Most studies use peripheral blood mononuclear cell (PBMC) fractions to investigate mt DNA content via Real-Time PCR in patients, not accounting platelets falsifying the mitochondrial (mt)DNA:nuclear (n)DNA-ratio. Cisplatin, fludarabine, and cytarabine: a novel, pharmacologically designed salvage therapy for patients with refractory, histologically aggressive or mantle cell non-Hodgkin's lymphoma.

BACKGROUND: Based on in vitro synergism, the combination of cytarabine (ara-C) and cisplatin is the basis of many salvage regimens for patients with aggressive non-Hodgkin lymphoma (NHL). However, patients with previously refractory disease are significantly less likely to respond, stimulating the search for novel salvage regimens. In vitro, fludarabine enhances the cytotoxicity of both ara-C and cisplatin, increasing ara-C incorporation into DNA and inhibiting repair of platinum/DNA adducts, suggesting that the combination of cisplatin, fludarabine, and ara-C (PFA) may have clinical utility.

Indinavir: a review of its use in the management of HIV infection.

Indinavir is a protease inhibitor used in the treatment of patients with HIV infection. Combination antiretroviral therapy with indinavir plus 2 nucleoside reverse transcriptase inhibitors (NRTIs) is associated with greater reductions in viral load, greater increases in CD4+ cell counts, and reduced morbidity and mortality when compared with 2 NRTIs alone. In the landmark clinical trial ACTG 320, the rate of progression to AIDS or death (primary end-point) among zidovudine-experienced patients treated with indinavir, zidovudine and lamivudine was approximately half that of patients who receive

In silico design and molecular docking evaluation of antisense oligonucleotides targeting IDO1 mRNA in the context of depression.

Depression is a prevalent mental disorder worldwide, often accompanied by severe complications that significantly impair patients' daily lives. Indoleamine-2,3-dioxygenase 1 (IDO1), a key metabolic enzyme in the kynurenine pathway, is considered a promising therapeutic target for depression treatment. Antisense oligonucleotides (ASOs) are small nucleic acid drugs characterized by high specificity, high potency, minimal drug-drug interactions, a prolonged duration of action, and a low risk of off-target activity, making them effective tools for targeting molecules previously considered undrugga

Fabrication, spectral characterisation and exploration of DNA/RNA binding affinity and biological potential of transition metal complexes incorporating isoniazid and aromatic nitrogen-donor ligands.

Four mixed-ligand transition metal(II) complexes, [ML(L1)Cl2] (where M = Co, Ni, Cu, and Zn; L = Schiff base derived from isoniazid and 3-nitrobenzaldehyde; L1 = 1,10-phenanthroline), were synthesised and characterised using elemental analysis, FT-IR, UV-Vis, 1H NMR, EPR (for Cu), and molar conductance measurements. Physicochemical and spectroscopic data suggested an octahedral geometry for all complexes. Antimicrobial assays revealed that the complexes exhibited greater activity than the free Schiff base ligand, with inhibition zones of 14-22 mm against Staphylococcus aureus and Escherichia c

Design, synthesis, and biological properties of novel metabolizable pH-responsive lipids for use in lipid nanoparticles for siRNA delivery and mRNA vaccination.

We developed pH-responsive lipids, LIPIDs-1 and LIPIDs-2, which feature an N,N-dimethylaminopropyl head and 2 hydrophobic tails connected by carbonate esters. These lipids are metabolizable owing to acetyl ester structures that can be cleaved by esterases, reducing their affinity for biological membranes after hydrolysis of the esters. We evaluated both lipids in vivo using siRNA-loaded lipid nanoparticles (LNPs) targeting Factor VII, which resulted in significant knockdown activity for up to 6 days. Toxicological studies of LNPs using LIPIDs-1 and LIPIDs-2 in rats indicated minimal liver toxi

USP53 regulates glycolytic reprogramming and accelerates Abdominal aortic aneurysm progression and rupture.

Abdominal aortic aneurysm (AAA) rupture remains a fatal vascular event with limited mechanistic understanding beyond diameter-based risk assessment. Emerging evidence suggests that metabolic reprogramming contributes to aneurysm progression, yet regulators linking metabolism to rupture remain poorly defined. An AngII/BAPN-induced mouse model generated intact (iAAA) and ruptured AAA (rAAA) tissues. Bulk RNA sequencing and untargeted metabolomics profiled transcriptomic and metabolic alterations across sham, iAAA, and rAAA groups. Candidate ubiquitin-specific proteases (USPs) were screened by in

Post-COVID increase in N-methyl-2-pyridone-5-carboxamide (Met2PY) and N-methyl-4-pyridone-3-carboxamide (Met4PY) associates with inflammatory and endothelial activation markers.

COVID-19 is associated with long-term vascular complications, but the underlying mechanisms remain incompletely understood. During infection, NAD+ homeostasis becomes dys-regulated, with excessive NAD+ consumption and enhanced catabolic flux through nicotinamide methylation pathways. This imbalance leads to NAD+ depletion accompanied by accumulation of pyridone metabolites, including N-methyl-2-pyridone-5-carboxamide (Met2PY) and N-methyl-4-pyridone-3-carboxamide (Met4PY). These derivatives have been linked to oxidative stress, endothelial dysfunction, and cardiovascular risk, yet their role i

"Aptamer-guided therapeutics and diagnostics for metabolic syndrome: Design, delivery, and translation of aptamer-based systems".

Metabolic syndrome (MetS) is a composite cardiometabolic condition defined by visceral obesity, dyslipidemia, hypertension, and insulin resistance, and it promotes the worldwide prevalence of type 2 diabetes and atherosclerotic cardiovascular disease. Nucleic-acid aptamers have developed as programmable recognition ligands that integrate antibody-like specificity with chemical synthesis, modular functionalization, low batch variability, and usually a low immunogenicity. Their translational significance in MetS is not solely in target binding, but in the capacity to establish disease-relevant a

Can the combination of 5-fluorouracil and gemcitabine increase the treatment efficacy of pancreatic and colon cancer?

Pancreatic ductal adenocarcinoma (PDAC) is intrinsically drug-resistant. Combinations of 5-fluorouracil (5-FU) with other cytostatics in FOLFIRINOX and gemcitabine (Gem) with nab-paclitaxel are first-line treatments. Colorectal cancer (CRC) is treated with FOLFOX or FOLFIRI, but in advanced disease patients become resistant for whom Gem has been considered. We investigated whether combining the key drugs, 5-FU and Gem, would be advantageous with different treatment schedules. We used CRC (human WiDr cells and Colon 26-A, a syngeneic mouse tumor) and a murine PDAC (PANC02 cells and tumors) as w

RNAseq-based meta-analyses revealed tumor suppressor-inducer fusion events in liver, oral, and ovarian cancer in the Indian population: a cancer cell surviving mechanism.

Cancer cell characteristics are determined by gene expression, influenced by genomic, epigenetic, and transcriptional modifications. Genomic rearrangements and transcriptional splicing can result in the formation of fusion genes. BCR-ABL1 is an established fusion gene employed as a biomarker in leukemia. A single gene can amalgamate with several other genes and may impact cellular fate. Ethnicity-specific variants of fusion genes have been identified, such as the TMPRSS2-ERG variation observed in prostate malignancies among African-American, Caucasian, and Japanese populations in research stud

Synergistic MgSiO3 nanocatalysis for the efficient synthesis of benzo[d]oxazol-2-yl phenylisoxazoles: structural elucidation, drug-likeness profiling, and target-oriented biomolecular interactions.

A new library of 2-(5-phenylisoxazole-3-yl)benzo[d]oxazole derivatives 3(a-f) was synthesized through a magnesium silicate nanocatalyst-assisted condensation of 2-aminophenol with a series of substituted 5-phenylisoxazole-3-carbaldehydes. The identity of the products was confirmed by FT-IR, 1H NMR, 13C NMR, and mass spectrometry. Their interaction with calf thymus DNA (CT-DNA) was systematically examined using absorption spectroscopy, viscosity assays, thermal denaturation experiments, and molecular docking simulations. The compounds exhibited binding constants in the 105-106 M-1 range, with 3
