Preventing and managing surgical wound dehiscence.

Most surgical wounds heal uneventfully--the dehisced surgical incision is definitely the exception to the rule. However, management of a dehisced incision is a fairly common home care challenge that must be understood. This month's column answers the questions: Why do some incisions dehisce, and what, if anything, can be done to prevent dehiscence? Are there early warning signs to look for? How should a dehisced wound be managed?

Autonomic dysreflexia.

The purpose of this article is to help perinatal nurses understand how to more effectively care for women in labor who have spinal cord injuries (SCIs), especially those women who develop the complication of autonomic dysreflexia (AD). AD is a syndrome that can occur in patients with SCIs above the level of T6. Its symptoms include dangerous paroxysmal hypertension, resulting from abnormal interplay between the parasympathetic and sympathetic nervous systems. While this syndrome was not commonly encountered by perinatal nurses in childbirth settings in the past, the dramatic improvements in bo

Knowledge, attitudes, and practices of parents of asthmatic children in Cape Town.

To obtain information useful to asthma care in a relatively poor, high asthma prevalence population, a focus group study was undertaken with 72 parents of children with asthma or recurrent wheezing. There was a reasonable level of understanding of the causes of asthma, although variable acceptance of the diagnosis. Willingness to undertake home management of acute episodes and environmental control measures was high. Reported treatment was characterized by reliance on syrups, use of home remedies, resistance to inhaled therapy, and relatively low compliance with prescribed treatment. There was

Changes in conceptions of meaning, effects and treatment of amblyopia. A phenomenographic analysis of interview data from parents of amblyopic children.

The research reported is an experimental study on the effects of intensified education of parents of children with amblyopia on their understanding of the nature of the defect, its origins and treatment. Parents in the control group were exposed to the ordinary information provided at two departments of paediatric ophthalmology, whereas parents in the experimental group, attending the same clinics, were asked to read a booklet aiming at enhancing their understanding of amblyopia and its treatment. Experimental as well as control subjects were thereafter interviewed about their understanding of

Influence of Parental Alcohol Consumption on Adolescent Drinking: A Middle-Range Theory Based on Roy's Adaptation Model.

The objective of this study is to develop a middle-range theory describing how adolescents adapt to parental alcohol consumption from a nursing perspective, using Fawcett's conceptual-theoretical-empirical framework. The theory of adolescent adaptation to parental alcohol consumption explains how adolescents cope with parental problem drinking, considering personal, family, and social factors. Parental alcohol use is conceptualized as a focal stimulus, while adolescent age and sex, stress, and parenting practices act as contextual stimuli, influencing adaptation. The theory's six propositions

Evolving a Historic Theory to Address a Modern Threat: Infant Stress-Mediated Cardiac Arrest Following Cardiac Surgery.

Infants undergoing cardiac surgery are vulnerable to cardiac arrest and subsequent morbidity and mortality. While various risk factors for postoperative cardiac arrest have been identified, no comprehensive framework exists to explain the interrelationships among them. In this study, we first extended Selye's General Adaptation Syndrome to illustrate the mechanisms leading to postoperative cardiac arrest in this population. We then interviewed 17 expert clinicians to explore infant stress exposure and stress response across the clinical trajectory. Using the qualitative data, we revised and ev

Middle-Range Theory on Fatigue in Chronic Kidney Disease.

This study aimed to develop a middle-range theory (MRT) on the phenomenon of fatigue in individuals with chronic kidney disease. It was a methodological study, conducted through an integrative literature review in Scopus, SciELO, and PubMed databases, which included 41 studies. The theory was developed in five steps: defining the theoretical approach, identifying core concepts, establishing causal relationships supported by clinical practice, designing a representative pictogram, and constructing explanatory propositions. This MRT includes 5 essential attributes, 13 clinical antecedents, 9 emp

Patient presentations at large-scale events: a thematic analysis of the literature.

Music festivals, football matches and other sporting events, agricultural shows and state fairs, and religious festivals are large-scale events where people present with medical or care needs in the pre-hospital setting. The author undertook a literature review on patient presentations at large-scale events to provide insight to nurses working at such events about the presentations they are likely to encounter and the skills they need to manage them. This article discusses patient presentations from lower-limb injury to drug intoxication, influenza, heat stroke and cardiac arrest. The author e

Evolution of Extravascular ICD Implantation: Shift Toward Local Anesthesia and Nurse-Led Sedation Without Propofol.

We describe the first successful implantation of an extravascular implantable cardioverter-defibrillator (EV-ICD) performed under local anesthetic with nurse-led sedation, without the use of propofol or anesthetic support. The procedure was well tolerated in a carefully selected, motivated patient. Although this is a single case, the patient's experience and procedural metrics were comparable to those performed under general anesthesia. This approach may offer a safe and resource-efficient alternative for individuals for whom general anesthesia is high risk or unavailable. It marks a potential

So you think you are safe from litigation?

Mrs J is a 45-year-old mother of three teenagers who developed a leg ulcer after a minor injury because of underlying venous issues from years of standing as a hairdresser. Despite regular visits to her GP practice nurse, her ulcer worsened over three years. A turning point came when she joined a local Leg Club, where her ulcer healed rapidly. Mrs J later sought specialised treatment and pursued legal action against her GP for inadequate care, highlighting the critical need for proper training and adherence to guidelines in ulcer management.

Zuranolone: A Rapidly Acting Oral Neuroactive Steroid Antidepressant for the Treatment of Postpartum Depression.

Severe depression, anxiety, and mood dysregulation during pregnancy and postpartum are risk factors for the safety and health of mother and baby. Current best practice guidelines recommend screening for depressive and anxiety symptoms before and during pregnancy, in addition to the postpartum period. Rapid identification and treatment of depression has the potential to be life-saving and prevent deleterious consequences, including impaired mother-infant bonding, emotional dysregulation, and other developmental problems. The current article reviews the neurobiological mechanisms of postpartum d

Using Evidence to Prevent Risk Associated With Perioperative Pressure Injuries.

Pressure injuries (PI) are a significant concern for surgical patients due to prolonged immobility and potential exposure to other risk factors associated with procedures. PI prevention strategies should begin early in the patient's surgical encounter starting with preoperative assessment. Much literature has been published to discuss PI risk factors, assessment tools, and evidence-based prevention measures. The purpose of this article is to critically review current best evidence to holistically assess patient risk for Hospital Acquired Pressure Injuries (HAPI) and review current tools used f

British Gynaecological Cancer Society/British Association of Gynaecological Pathology consensus for germline and tumor testing for BRCA1/2 variants in ovarian cancer in the United Kingdom.

The British Gynecological Cancer Society and the British Association of Gynecological Pathologists established a multidisciplinary consensus group comprising experts in surgical gynecological oncology, medical oncology, genetics, and laboratory science, and clinical nurse specialists to identify the optimal pathways to BRCA germline and tumor testing in patients with ovarian cancer in routine clinical practice. In particular, the group explored models of consent, quality standards identified at pathology laboratories, and experience and data from pioneering cancer centers. The group liaised wi

A review of interventions to reduce pain in chronic wounds.

Reduction of chronic wound pain has the potential to improve patients' quality of life, expedite the healing process and, ultimately, relieve pressure on community services. Despite this, education on pain assessment in the management of chronic wounds is lacking. This literature review seeks to provide evidence-based recommendations to reduce chronic wound pain and inform the practice of community nurses. The results of a thematic analysis indicate that a honey dressing or native collagen matrix dressing and conditioning exercises for the lower leg can reduce pain, and nitroglycerin ointment

Faecal incontinence in palliative and end-of-life care.

Faecal incontinence can be a distressing and undignified experience, and is particularly a care delivery need for those at the end of life. There are various publications by national bodies including NHS England, the Department of Health, NHS Scotland and others, discussed in this article, to inform readers of the best way to care for someone experiencing faecal incontinence who is being looked after through palliative care in the community. The article will also discuss the application of recommended faecal collection devices and some of the associated risks. The devices discussed are: anal p

Late life depression: nursing actions that can help.

PURPOSE: This article explains the symptoms of late life depression (LLD) and discusses evidence-informed actions that nurses can implement to provide older adults with the help they need. CONCLUSIONS: Recognizing and addressing depression in older adults can enhance quality of life. PRACTICE IMPLICATIONS: People with LLD may not appear sad or express feelings of depression. Instead, they demonstrate loss of interest, frailty, cognitive impairment, suicidal ideation, unexplained somatic complaints, and loneliness. Documenting symptoms, screening, and assessing suicidal ideation are essential

[Long-term central venous catheter-related infections].

Long-term intravenous catheters (LTIVC) are standard practice for patients with chronic diseases such as cancer, digestive disease requiring total parenteral nutrition or end-stage renal disease. Even if they greatly improved patients' care, the use of LTIVC is also associated with microbial contamination and subsequent infection. These catheter-related infections are associated with morbidity, mortality and increased health-care costs. As patients carrying these LTIVC stay at home for their treatment (home parenteral nutrition for instance) or between cycles of treatment (antineoplastic chemo

Recognizing and responding to hyperglycaemic emergencies.

Diabetic ketoacidosis (DKA) and hyperglycaemic hyperosmolar state (HHS) are both diabetic emergencies associated with hyperglycaemia and can be fatal if healthcare professionals fail to recognize and intervene appropriately. While many students and qualified nurses may be able to recall common signs and symptoms related to DKA and HHS - for example polyuria, polydipsia and elevated blood sugars - understanding the physiological mechanisms behind abnormal observations and restoring homeostasis through appropriate management is far more complex. Health educators can play a significant role in co

An unusual case of subterfuge in the emergency department: covert administration of antipsychotic and anxiolytic medications to control an agitated patient.

Pediatric, nursing home, and institutionalized psychiatric patients frequently receive medications covertly or against their will. Surreptitious medicating of emergency department (ED) psychiatric patients may occur but has not been reported. We discuss competing ethical, therapeutic, and legal issues in potential conflict during the treatment of an acutely psychotic patient who had homicidal and suicidal ideation and presented to a busy, urban ED. The practice of covertly medicating may not be uncommon in EDs, but fear of professional censure probably inhibits open discussion and documentatio
