The promising use of an emergency department observation unit to manage patients with opioid use disorder.

INTRODUCTION: The opioid epidemic continues to grow, placing a significant strain on Emergency Departments (EDs), resulting in patients presenting daily with opioid-related concerns including intoxication, withdrawal, infections, injury, and death. Consequently, in recent years many EDs, including our own, have utilized Emergency Department Observation Units (EDOU) to not only manage withdrawal and overdose, but also initiate long-term treatment. This study aims to evaluate the outcomes of patients with opioid use disorder (OUD) who were managed in our EDOU. METHODS: This was a retrospective

End-of-Life Decision Making in Internal Medicine: A Retrospective Cohort Study of Clinical Ethics Consultations.

BACKGROUND: Internal Medicine physicians frequently face complex ethical dilemmas in end-of-life (EOL) care, particularly when prognoses are uncertain and patients lack decisional capacity, surrogate representation, or clear care preferences. Clinical Ethics Consultations (CECs) can provide support, yet little empirical data describe how Internal Medicine teams utilize CEC services. OBJECTIVE: To characterize the clinical features, ethical themes, and recommendations of CECs requested by Internal Medicine services, particularly in the context of EOL care. DESIGN: Retrospective cohort study u

Factors influencing self-efficacy of healthcare professionals towards end-of-life care in acute care settings in Palestine: a cross-sectional study.

BACKGROUND: Caring for individuals at the end of life is a profound emotional and complex responsibility, especially within acute care settings where healthcare professionals must navigate the delicate balance between administering life-saving treatments and providing end-of-life care. This dual role can create considerable challenges for healthcare providers, many of whom may feel inadequately prepared or lack the confidence to offer the specialized care required in these situations. This study aimed to examine the factors influencing nurses' self-efficacy in delivering EOLC in Palestinian ac

Navigating emotions, communication, and pain during prehospital labour: a mixed-methods survey with emergency ambulance services.

INTRODUCTION: Despite their crucial role in prehospital obstetric emergencies, there is little research exploring emergency medical service (EMS) personnel's perspectives and attitudes on out-of-hospital births, including balancing the numerous needs of the patient. Our aims were to (1) identify how EMS demographics and professional experience related to their confidence and comfort in treating a patient in labour and (2) explore EMS' perspectives on managing emotional needs and administering analgesia in a labour and birth setting in New Zealand (NZ). METHODS: This mixed-methods survey invit

Health insurance literacy and health services access barriers in Niemann-Pick disease: the patient and caregiver voice.

BACKGROUND: Major challenges to health care access include low health insurance literacy, prohibitive costs, and insurance barriers. Niemann-Pick disease (NPD), comprising acid sphingomyelinase deficiency (ASMD) and Niemann-Pick type C (NPC), is a group of rare, autosomal recessive, highly heterogeneous, neurovisceral, life-threatening, relentlessly progressive lysosomal disorders. Patients experience debilitating systemic and neurological symptoms and substantial emotional and financial stress. Currently, these multifaceted disorders are managed symptomatically as there are no approved therap

Adverse Childhood Experiences and Trauma-Informed Care in Rehabilitation Clinical Practice.

Adverse childhood experiences (ACEs) contribute to physical, behavioral, and mental health issues throughout the lifespan and, because of their prevalence, constitute a significant public health issue. Practitioners in all health care disciplines need to be knowledgeable about ACEs and prepared to address them. ACEs can contribute to conditions that lead patients to seek rehabilitation care, and therefore a framework is needed that enables rehabilitation professionals to understand the effects of ACEs and how to discuss them with patients. This article summarizes ACE research and its clinical

Clinical and Genetic Characteristics of Pediatric Colorectal Cancer.

BACKGROUND: Compared to colorectal cancer (CRC) in adults, CRC in children is extremely rare. Although its incidence has increased recently, there is a lack of clinical research on the disease. Inherited cancer susceptibility syndromes (ICSS), a group of disorders in which patients are predisposed to susceptibility to a wide range of tumors as a result of pathogenic mutations in genes in their germ line, are an important cause of CRC in children. Delayed diagnosis due to atypical clinical presentation, as well as limited awareness of ICSS among doctors, contributes to poor outcomes in juvenile

Comparison and Evaluation of the Accuracy for Thoracic and Lumbar Pedicle Screw Fixation in Early-Onset Congenital Scoliosis Children.

BACKGROUND: Compared to adult scoliosis, correcting scoliosis in children often presents greater challenges. This is attributed to two key factors. Firstly, it involves accounting for the growth potential of children. Secondly, the thinner pedicles in children can complicate screw insertion, particularly when dealing with existing deformities. The utilization of intraoperative navigation technology offers a modest improvement in the precision of screw placement but does come with the drawback of increased radiation exposure. The aim of this study is to investigate and assess the accuracy of ma

Clinician Perspectives on Barriers and Facilitators to Abdominal Organ Transplant Access.

IMPORTANCE: Organ transplant is lifesaving for patients with end-stage organ disease; however, prominent disparities exist in waiting list placement among racial and ethnic minoritized and socially deprived populations. The process of determining transplant candidacy involves a multidisciplinary team of clinicians and staff who possess unique insight into the multilevel factors influencing equitable waiting list access. OBJECTIVE: To understand clinician and staff perspectives on barriers and facilitators to transplant access and inform targeted strategies for transplant center-based interven

Do patient-specific cutting guides for pelvic and sacral tumour resection provide tumour-free bone margins?

BACKGROUND: Achieving R0 margin, i.e., tumour-free margin, during conservative surgery for pelvic or sacral tumours is demanding. Small cohort studies suggest that patient-specific instrumentation (PSI) may hold promise, notably for bone margin accuracy. Objectives of this study were to determine whether PSI allowed R0 bone margins; R0 soft-tissue margins; and increased disease-free survival, without local recurrence or distant metastases, comparatively to previous studies of patients treated by free-hand or navigated surgery; and whether PSI positioning for the bone cuts was readily achieved.

Use of Tc99m-nanocolloid for sentinel nodes identification in cervical cancer.

BACKGROUND: The initial draining lymph node for a primary tumor is referred to as the "sentinel" node. Firstly adopted in the management of patients with cutaneous melanoma and breast cancer, it is now widely tested in cervical cancer. In patients with cervical cancer, lymph node status is the most important prognostic factor for survival. In patients with cervical cancer FIGO stage I and II pelvic lymph node metastases are expected in 0-16 and 24.5-31% and para-aortic lymph node metastases are expected in 0-22 and 11-19% of patients. The removal of pelvic and para-aortic lymph nodes is essent

Invisible patients: Reframing women informal caregivers' access to healthcare and support services as a gendered health systems issue.

Women informal caregivers are overrepresented in unpaid and informal caregiving roles yet remain underrecognized as patients and policy subjects within health and social care systems. Despite extensive evidence that caregiving affects women's own health and healthcare engagement, their access barriers are frequently attributed to individual help-seeking behaviors rather than to systemic and gendered conditions. This conceptual paper argues that women informal caregivers' access to healthcare and support services should be understood as a gendered health systems issue shaped by sociocultural ex

Qualitative Exploration of Fertility-Related Attitudes and Perspectives Among Young Adult Women Diagnosed With Breast Cancer at Periurban Cancer Center in Uganda.

PURPOSE: Understanding how women navigate fertility-related decisions after a breast cancer diagnosis is critical for patient-centered survivorship care in low-resource settings. This study explored young women's fertility-related perspectives and, importantly, how their experiences point to system-level gaps in oncofertility counseling and access to fertility preservation at a periurban cancer center in Uganda. METHODS: A qualitative phenomenologic study design was used. Twenty young adult women diagnosed with breast cancer were purposively selected from St Francis Hospital Nsambya, in Kampa
