Trimeric superoxide dismutase 1 antibody as a universal biomarker for ALS.

Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disease that leads to the loss of motor neurons, resulting in paralysis and death. Currently, there are no specific biomarkers available for diagnosing ALS. As a result, diagnosis currently relies on excluding other conditions, which forces patients to endure months or even years of uncertainty. The absence of a specific, reliable diagnostic tool has hindered both early intervention and therapeutic progress. Here we develop a novel synthetic antibody that can detect a toxic form of a known protein linked to ALS. This trimeric ass

What matters more? Common or specific factors in cognitive behavioral therapy for OCD: Therapeutic alliance and expectations as predictors of treatment outcome.

CBT for obsessive-compulsive disorder (OCD) is a strong challenge to the contention that common factors explain most of the variance in outcomes in all therapies and all disorders, given that the treatment is focused and placebo response is low. In this study, the relative contributions of expectancy and therapeutic alliance as predictors of outcome in the treatment of OCD are examined and compared to the contribution of specific treatment effects. One hundred and eight patients with OCD were randomly assigned to two forms of CBT: exposure and response prevention (EX/RP) or stress management t

Patient-Specific Parameters Associated With Traction in Primary and Revision Hip Arthroscopic Surgery.

BACKGROUND: Distraction of the hip joint is a necessary step during hip arthroscopic surgery. The force of traction needed to distract the hip is not routinely measured, and little is known about which patient factors may influence this force. PURPOSE: To quantify the force of traction required for adequate distraction of the hip during arthroscopic surgery and explore the relationship between hip joint stiffness and patient-specific demographics, flexibility, and anatomy. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A total of 101 patients (61 female) undergoing primary hip ar

Multiview Laryngoscopic Image Fusion for Patient-Level Stratification of Vocal Fold Lesions.

Malignant or high-grade vocal fold disease is usually judged across repeated laryngoscopic views at different observation distances and fields of view, whereas most artificial intelligence systems remain frame-based. We evaluated whether patient-level fusion of multiview laryngoscopic image sets could improve benign versus malignant/high-risk stratification of vocal fold lesions. In this retrospective multicenter study of 628 patients, internal data from the Cancer Hospital and Institute of the Chinese Academy of Medical Sciences collected between 2008 and 2025 were split into training (n = 38

Obesity-specific improvement of lung cancer outcomes and immunotherapy efficacy with metformin.

BACKGROUND: Preclinical cancer studies ascribe promising anticancer properties to metformin. Yet, clinical findings vary, casting uncertainty on its therapeutic value for non-small cell lung cancer (NSCLC) patients. We hypothesized that metformin could benefit obese and overweight patients with NSCLC. METHODS: We retrospectively analyzed 2 clinical cohorts and employed complementary mouse models to test our hypothesis. One cohort included NSCLC patients with overweight body mass index (≥25 kg/m2, n = 511) and nonoverweight body mass index (<25 kg/m2, n = 232) who underwent lobectomy, evaluati

Gaps in access to pulmonary hypertension care and opportunities for improvement: a multi-site qualitative study.

BACKGROUND: Pulmonary hypertension (PH) is a progressive disease leading to right heart failure and early mortality. Early recognition of the disease and timely initiation of PH-specific therapies for qualifying PH subgroups are crucial for improving patient outcomes. Yet delays in diagnosis and treatment of PH persist. We aimed to explore patient and provider perspectives on access to and timeliness of PH care across the patient's entire health journey from symptom onset through follow-up care. METHODS: We conducted a multi-site qualitative study at three expert PH centers in the United Stat

A quantitative review of finite element-based biomechanics of lumbar decompression surgery.

Lumbar decompression surgeries are commonly performed in the USA to treat pain from spinal stenosis, often with little to no biomechanical evidence to evaluate the risks and benefits of a given surgery. Finite element models of lumbar spinal decompression surgeries attempt to elucidate the biomechanical benefits and risks of these procedures. Each published finite element model uses a unique subset of lumbar decompression surgeries, a unique human lumbar spine, and unique model inputs. Thus, drawing conclusions about biomechanical changes and biomechanical complications due to surgical variati

Prediction Model for Failure of Nonoperative Management of Uncomplicated Appendicitis in Adults.

BACKGROUND: Prediction of failure of nonoperative management (NOM) in uncomplicated appendicitis (UA) is difficult. This study aimed to establish a new prediction model for NOM failure in UA. METHODS: We included 141 adults with UA who received NOM as initial treatment. NOM failure was defined as conversion to operation during hospitalization. Independent predictors of NOM failure were identified using logistic regression analysis. A prediction model was established based on these independent predictors. Receiver operating characteristic (ROC) curve analysis and the Hosmer-Lemeshow test were

Unique Challenges of Shared Decision-Making in Colorectal Surgery for the Elderly Patient.

Older adults require disproportionately more operations and suffer more complications than their younger counterparts, making a thoughtful approach to shared decision-making imperative. This article reviews key considerations in the decision to pursue operative intervention for older adults. First, we explore the "Fix It" model and the importance of acknowledging trade-offs, with a specific focus on quality of life. This is followed by a discussion of how surgical recovery affects physical function, bowel function, and cognition. We describe strategies for shared decision-making in the older p

The three-dimensional shape symmetry of the lunate and its implications.

We studied the three-dimensional (3-D) shape variations and symmetry of the lunate to evaluate whether a contralateral shape-based approach to design patient-specific implants for treatment of Kienböck's disease is accurate. A 3-D statistical shape model of the lunate was built using the computed tomography scans of 54 lunate pairs and shape symmetry was evaluated based on an intraclass correlation analysis. The lunate shape was not bilaterally symmetrical in (1) the angle scaphoid surface - radius-ulna surface, (2) the dorsal side and the length of the side adjacent to the triquetrum, (3) the

Detection of Barrett's oesophagus through exhaled breath using an electronic nose device.

Timely detection of oesophageal adenocarcinoma (OAC) and even more so its precursor Barrett's oesophagus (BO) could contribute to decrease OAC incidence and mortality. An accurate, minimally-invasive screening method for BO for widespread use is currently not available. In a proof-of-principle study in 402 patients, we developed and cross-validated a BO prediction model using volatile organic compounds (VOCs) analysis with an electronic nose device. This electronic nose was able to distinguish between patients with and without BO with good diagnostic accuracy (sensitivity 91% specificity 74%)

Development, Validation and Comparison of Artificial Neural Network and Logistic Regression Models Predicting Eosinophilic Chronic Rhinosinusitis With Nasal Polyps.

PURPOSE: Chronic rhinosinusitis with nasal polyps (CRSwNP) can be classified into eosinophilic CRSwNP (eCRSwNP) and non-eosinophilic CRSwNP (non-eCRSwNP) by tissue biopsy, which is difficult to perform preoperatively. Clinical biomarkers have predictive value for the classification of CRSwNP. We aimed to evaluate the application of artificial neural network (ANN) modeling in distinguishing different endotypes of CRSwNP based on clinical biomarkers. METHODS: Clinical parameters were collected from 109 CRSwNP patients, and their predictive ability was analyzed. ANN and logistic regression (LR)

A machine learning algorithm for the detection of paroxysmal nocturnal haemoglobinuria (PNH) in UK primary care electronic health records.

BACKGROUND: Paroxysmal Nocturnal Haemoglobinuria (PNH) is an ultra-rare, acquired disorder that is challenging to diagnose due to varied symptoms, heterogeneous patient presentations, and lack of awareness among healthcare professionals. This leads to frequent misdiagnosis and delays in diagnosis. This study evaluated the feasibility of a machine learning model to identify undiagnosed PNH patients using structured electronic health records. METHODS: The study used data from the Optimum Patient Care Research Database, which contains electronic health records from general practitioner (GP) prac

Association between no-shows to scheduled clinic appointments and 30-day risk of overdose in patients prescribed methadone for opioid use disorder.

BACKGROUND: The introduction of synthetic opiates and non-opiate sedatives into the illicit drug market has increased overdose risk for individuals who use opiates and other drugs. The ongoing risk of overdose for patients receiving methadone as a medication for opioid use disorder in the context of this more potent and less predictable drug supply is not well characterized. Additionally, little research has explored whether commonly available clinical data (including data available even in low resource settings) can predict near-term acute overdose in patients prescribed methadone for opioid
