The Inter-Society Consensus for the Management of Peripheral Arterial Disease (TASC) guidelines were last updated in 2007 (TASC II) and represented the collaboration of international vascular specialties involved in the management of patients with peripheral arterial disease (PAD). Since the publication of TASC II, there have been innovations in endovascular revascularization strategies for patients with PAD. The intent of this publication is to provide a complete anatomic lower limb TASC lesion classification, including the infrapopliteal segment, and an updated literature review of new endov

With demands for an evidence-based approach to patient care, the management of facial fractures will come under increasing scrutiny because there is an overall deficiency in higher level clinical evidence. This article reviews the management of facial fractures, focusing on an evidence-based approach. It focuses on select areas of facial trauma in which there is controversy and presents randomized studies and meta-analysis to help define best practice. The article notes the many areas where the evidenced-based literature is weak and looks at the future of evidence-based facial trauma care.

Choledochal cyst is prevalent in female than male. Diagnosis and management of choledochal cyst is important in clinical practice, or else, patients may develop cholangiocarcinoma. Currently, complete resection is the standard method for the treatment of choledochal cyst. However, traditional open surgery is usually needed in these procedures, which may induce postoperative injuries. In this case, we present our experiences for the management of choledochal cyst through radical removal combined with Roux-en-Y anastomosis under laparoscope. Our procedures are minimally invasive and the outcomes

As a result of rapidly evolving technology, advances in the understanding of the pathogenesis of cervical neoplasia, and the introduction of TBS terminology, we stand at a crossroads in the screening and management of this disease. This bulletin reflects management considered appropriate at this time. A number of key issues as indicated above must be addressed before more specific guidelines can be incorporated into routine clinical practice. Until data from clinical trials evaluating these new methods are available, the gynecologist is best advised to manage patients using the guidelines in t

A series of 205 ruptured adult spleens, half of them due to personal assault, has been collected from several hospitals. In some of these cases a trial of non-operative management has become standard practice unless other abdominal injuries are suspected. One hundred and thirty-seven cases were managed this way initially, successfully so in 80%. Splenectomy was performed in 70 patients with eight deaths; three of them in the 27 instances where non-operative management was said to have failed. None of these eight deaths was due to operative delay.

A brief review of the development of the surgical management of infantile hypertrophic pyloric stenosis is made. The incidence, investigation, management and complications of cases treated at the Royal Hospital for Sick Children, Glasgow over a period of six decades is reported. The mortality fell from 59 per cent in 1925 to 0 per cent in 1975. Pyloromyotomy in infants with hypertrophic pyloric stenosis is shown to be a very effective treatment for this potentially lethal condition and must rank as one of the most cost-effective forms of treatment in medical practice.

Obstructive sleep apnea (OSA) is a common but frequently undiagnosed condition. Symptoms include loud snoring, nocturnal awakening, and daytime sleepiness. Treating OSA can meaningfully improve symptoms and achieve modest reductions in blood pressure. Although OSA care has traditionally been limited to sleep specialists, technologies are available that allow primary care clinicians to diagnose OSA and initiate treatment. Treatments include positional therapy, weight management, application of external positive airway pressure, mandibular advancement devices, and surgical interventions. Inpatie

Immunotherapy is transforming breast cancer management, but it also presents new challenges for surgical and reconstructive practice. This review summarizes current evidence on immune checkpoint inhibitors, perioperative safety, and immune-related adverse events, highlighting their impact on surgical timing, postoperative recovery, and reconstructive outcomes. We also discuss interactions between implants and the tumor immune microenvironment and emphasize the need for individualized perioperative management. Finally, we identify key research gaps, including long-term toxicity, real-world surg

Postural orthostatic tachycardia syndrome (POTS) is a disorder characterized by a sudden onset of orthostatic intolerance upon standing, which may be relieved when lying down. The most common complications associated with POTS are trauma or injury from falling or fainting. Pediatric patients with POTS may have comorbidities that interfere with their quality of life. Careful management of the disorder is essential to prevent injury. POTS management is often misunderstood, which may lead to misdiagnosis and invalidation by health care professionals. This article discusses how POTS affects pediat

UNLABELLED: A popliteal cyst, also referred to as a Baker's cyst, is among the most common cystic lesions encountered around the knee joint in orthopedic practice, resulting from distension of the gastrocnemius-semimembranosus bursa within the popliteal fossa and is typically filled with synovial fluid. Magnetic resonance imaging (MRI) is widely used to detect and characterize these cysts. Various invasive and non-invasive therapeutic strategies have been described for the management of Baker's cysts. This review summarizes the clinical presentation, diagnostic evaluation, and current manageme

Although lesions of the mandible and maxilla are relatively common findings on imaging examinations, they still pose a diagnostic challenge for radiologists in clinical practice. Many of these lesions share similar radiographic characteristics, making it difficult to differentiate between benign, infectious, and malignant entities, which can delay appropriate management. This article aims to illustrate the main imaging findings and present a systematic approach based on radiological patterns, such as attenuation type, margins, relationship with dental structures, and anatomical distribution, i

Most children receive emergency care by general emergency physicians and not in designated children's hospitals. There are unique considerations in the care of children that differ from the care of adults. Many management principles can be extrapolated from adult studies, but the unique pathophysiology of pediatric disease requires specialized attention and management updates. This article highlights ten impactful articles from the 2024 whose findings can improve the care of children in the Emergency Department (ED). These studies address pediatric cervical spine injury, resuscitation, infant

Stereotactic radiosurgery (SRS) is a non-invasive technique that is transforming the management of benign intracranial lesions through its precision and preservation of healthy tissues. It is effective for meningiomas, trigeminal neuralgia (TN), pituitary adenomas, vestibular schwannomas, and arteriovenous malformations. SRS ensures high tumor control rates, particularly for Grade I meningiomas and vestibular schwannomas. For refractory TN, it provides initial pain relief > 80 %. The advent of technologies such as PET-MRI, hypofractionation, and artificial intelligence is further improving tre

Major advances in neurocritical care and the modalities used to treat aneurysms have led to improvement in the outlook of patients with aneurysmal subarachnoid hemorrhage. Yet, several knowledge gaps remain widely open. Variability in practices stems from the lack of solid evidence to guide management, which recent guidelines from professional organizations aim to mitigate. In this article, the authors review some of these gaps in knowledge, highlight important messages from recent management guidelines, emphasize aspects of our practice that we consider particularly useful to optimize patient

Heart failure with reduced ejection fraction (HFrEF) is a commonly seen clinical entity in the family physician's practice. This clinical review focuses on the pharmacologic management of chronic HFrEF. Special attention is paid to the classification of heart failure and the newest recommendations from the American Heart Association concerning the use of guideline-directed medical therapy. β blockers, ACE inhibitors, ARBs, mineralocorticoid receptor antagonists are discussed in detail. The new emphasis on sacubitril-valsartan and SGLT2i's as therapies for HFrEF are reviewed, followed by a brie

Male external genitalia injury with testicular loss is relatively uncommon in civilian practice. Testicular amputation is usually found with military trauma and self-mutilation. However, they do occur while operating machines for domestic purposes. The standard protocols for the management of traumatic testicular amputation are lacking. The management is focused on resuscitation of the patient and restoration of testicular function with acceptable cosmesis. Here, we present a case of a 45-year-old man with traumatic right testicular amputation while he was using a thresher for rice cutting.
