Extra-foraminal prolapsed lumbar disc: a possible cause of recurrent sciatica in failed low-back surgery patient. Case report.

The present report describes the case of a 48 year-old woman who had undergone two separate lumbar laminectomies for a L4-L5 disc. She was admitted for a recurrent sciatica of 6-months duration, which failed to respond to conservative treatment. Myelogram on admission was negative. However CT showed a far laterally placed bulging L4-L5 disc. The patient was successfully reoperated on using a paramedian posterolateral approach on the side of the lesion. The rationale for this approach is discussed.

The causative mechanisms of mitral valve prolapse in progressive muscular dystrophy in reference to thorax and thoracic spine deformities and left ventricular dysfunction.

The causative mechanisms of mitral valve prolapse (MVP) were evaluated in 58 patients with progressive muscular dystrophy (PMD). Two possible causes, 1) left ventricular (LV) dysfunction and 2) thoracic spine and thorax deformities were assessed. Patients were classified into three groups by echocardiographic findings. Group 1: 31 patients without MVP, group 2: 11 patients with MVP confirmed only by M-mode echocardiogram, group 3: 16 patients with MVP confirmed by both two-dimensional and M-mode echocardiograms. LV functions evaluated by systolic time intervals and fractional shortening showed

Prolapsed lumbar disc treated with intramuscularly administered dexamethasonephosphate. A prospectively planned, double-blind, controlled clinical trial in 52 patients.

Prompted by previously results of systemic steroid treatment of symptoms of prolapsed lumbar disc, which appeared to be promising, dexamethasone phosphate was tested in a controlled, double-blind clinical study. 52 patients were included in the study: 25 received dexamethasone phosphate, 24 received placebo and 3 patients dropped out. Upon termination of the study the two groups were found to be comparable as to age, sex, duration of symptoms and degree of heavy work. We were not able to demonstrate any effect of dexamethasone phosphate on the following parameters pain, paraesthesia, paresismu

[Prolapse of the iris through spontaneous perforation of the cornea and pathological costal fracture after cortisone therapy for rheumatoid arthritis (author's transl)].

A case of spontaneous corneal ulcer with iris prolapse is reported in a patient suffering from rheumatoid arthritis. The concomitant presence of a pathological costal fracture, recognised to be steroid nature, suggests a correlation between the corneal manifestations and the corticosteroid therapy to be established.

The evaluation of the tests that can predict de novo stress urinary incontinence after advanced pelvic organ prolapse surgery: a prospective study.

BACKGROUND: De novo stress urinary incontinence (SUI) may reduce patient satisfaction following a pelvic organ prolapse (POP) surgery and may necessitate additional anti-incontinence surgery in the future. The aim of this study is to evaluate physical examination tests that can predict de novo SUI after POP surgery. METHODS: A total of 30 patients diagnosed with stage 3-4 POP who underwent POP surgery were included in this study. The results of the Q-tip test, reduced stress test, and urodynamic stress urinary incontinence test, which were performed before the surgery, and stress test results

Intense Coughing Following Caudal Epidural Steroid Injection for Lumbar and Lumbo-Sacral Disc Prolapse with Radiculopathy. A Case Report with Pathophysiological Insights.

Caudal epidural steroid injection (CESI) is a commonly performed intervention for chronic lumbar and lumbosacral radicular pain, generally considered safe with infrequent adverse effects. We report a rare case of sudden-onset, intense coughing immediately following ultrasound-guided CESI in a 32-year-old male with prolapsed intervertebral disks at the L4-L5 and L5-S1 levels. The episode lasted approximately four minutes and resolved spontaneously without neurological or hemodynamic compromise. Possible mechanisms include cephalad spread of the injectate, inadvertent intravascular injection or

Cost-effectiveness and patient-reported outcomes of combined versus isolated robotic rectal and pelvic organ prolapse repair: A multicentre prospective cohort study.

AIM: Rectal prolapse (RP) and pelvic organ prolapse (POP) frequently coexist, yet are often evaluated and treated separately. This study aimed to compare patient-reported outcomes and cost-effectiveness of combined versus isolated robotic prolapse repair. METHODS: We conducted a prospective multicentre cohort study across three tertiary pelvic floor centres. Women undergoing robotic rectal prolapse repair (rRP), robotic pelvic organ prolapse repair (rPOP) or combined robotic repair (rRP + rPOP) were included. Validated patient-reported outcome measures were collected at baseline and 12 months

A Reverse Saddle Mitral Annulus Shape Explains Apparent Systolic Mitral Annular Disjunction in Mitral Valve Prolapse: A Three-Dimensional Transesophageal Echocardiography Analysis.

AIMS: Mitral annular disjunction (MAD) is considered an abnormal anatomical/functional feature in patients with mitral valve prolapse (MVP). Recent evidence suggests that in most cases, systolic disjunction is only apparent (Pseudo-MAD) because of altered annular dynamics, with the leaflet hinge point remaining anchored to the annulus. The aim of this study was to analyze mitral annular shape and dynamic function in patients with MVP by three-dimensional (3D) transesophageal echocardiography (TEE). METHODS AND RESULTS: Patients with MVP and grade 3 or greater mitral regurgitation (MR) who und

Double sling with polyvinylidene fluoride implant: Evaluation of functional and anatomical outcomes of concomitant retropubic sling and anterior vaginal wall prolapse repair.

BACKGROUND: Several kinds of procedures have been introduced for surgical rectification of pelvic organ prolapse (POP) and/or stress urinary incontinence (SUI) using various synthetic meshes such as polypropylene (PP) material. Polyvinylidene fluoride (PVDF) meshes have been proven to have higher biocompatibility, lower morbidity, and also less inflammatory and fibrotic reactions in comparison with PP meshes. Here, we intend to report a 2-year follow-up report of patients who had undergone transvaginal surgery using PVDF meshes to rectify POP and concomitant SUI. MATERIALS AND METHODS: Betwee

Transvaginal bowel evisceration and gangrenous small-bowel obstruction after self-amputation of neglected uterovaginal prolapse: a case report and review of literature.

Neglected uterovaginal prolapse (UVP) can lead to life-threatening complications. We report a 45-year-old para 5 woman with a 10-year history of UVP who presented after 2 days of a protruding vaginal mass, bilious vomiting, fever, and abdominal pain following an attempt to cut the prolapsed tissue with a blade. On examination, she was tachycardic, hypotensive, actively bleeding from the prolapse, and showed signs of small-bowel evisceration. Emergency laparotomy identified a 4-cm anterior uterine defect with 60 cm gangrenous ileum herniated ~15 cm from the ileocecal junction; the necrotic segm

Modified Arteriovenous Loop Technique for Successful Antegrade Device Closure of Perimembranous Ventricular Septal Defects with Aortic Valve Prolapse-A Single-Center Experience.

In ventricular septal defects (VSDs) close to aortic valve, mostly surgical closure is preferred. Currently, transcatheter method of device closure is done in selective cases with satisfactory results. Perimembranous with outflow extension VSDs having mild aortic valve prolapse with trivial to mild aortic regurgitation (AR) were selected. VSD devices were deployed through antegrade route with our modification of standard arteriovenous (A-V) loop technique to prevent aortic valve compression and progression of AR. Among 55 cases of perimembranous VSDs having outlet extension with aortic valve p

Free Margin Running Suture Repair for Bileaflet Mitral Valve Prolapse in Patients with Left Ventricular Dysfunction: A Mid-term Follow-up Study.

OBJECTIVES: Repairing severe mitral regurgitation (MR) in patients with degenerative bileaflet prolapse and reduced left ventricular ejection fraction (LVrEF) is challenging. The Free Margin Running Suture (FMRS) technique offers a non-resectional approach, but mid-term data are limited. METHODS: We analysed 28 patients with bileaflet degenerative MR and LVrEF (≤40%) undergoing FMRS. Primary outcomes were mid-term survival and MR recurrence; secondary outcomes included LVEF, in-hospital complications, transmitral gradient, coaptation length, and mitral valve orifice area. RESULTS: Mean age w

A randomized, prospective, controlled study comparing uterine preserving laparoscopic lateral suspension with mesh versus laparoscopic sacrohysteropexy in the treatment of uterine prolapse.

OBJECTIVES: Pelvic organ prolapse (POP) significantly affects women's quality of life, occurring in 20-30% of females aged over 20 globally. With aging populations, demand for pelvic reconstructive surgery is rising. Patients seek anatomical restoration while preserving uterus and sexual function. Sacrohysteropexy is the gold standard for apical prolapse, but carries risks. Lateral suspension, offers safer apical and anterior correction especially for obese, sexually active women. Our prospective study compares laparoscopic sacrohysteropexy and lateral suspension objectively and subjectively.

Giant Morgagni hernia and aorto-pulmonary collaterals in a Loeys-Dietz patient undergoing surgery for aortic root aneurysm and mitral valve prolapse.

The case of a Loeys-Dietz syndrome patient undergoing mitral valve repair and composite aortic root and valve replacement is here described: preoperative CT scan unravelled a previously misdiagnosed Morgagni hernia (anterior diaphragmatic), containing omentum only, compressing the right ventricle. Intraoperatively, an abnormal oxygenated blood backflow into the left ventricle was observed, postoperatively found to be caused by major aorto-pulmonary collateral arteries. This is the 1st case of Morgagni hernia and systemic-pulmonary shunt ever reported associated with Loeys-Dietz syndrome. These
