Postoperative Findings Mimicking Duodenal Leak After Graham Patch Repair: Intact Repair With Retroperitoneal Abscess.

Perforated duodenal ulcers represent a surgical emergency requiring prompt operative intervention. The Graham omental patch is the most commonly performed procedure for emergent repair; however, postoperative findings suggestive of leakage remain a serious concern and are associated with significant morbidity and mortality. Traditionally, suspected postoperative leaks have prompted early surgical re-exploration due to the risk of peritonitis and sepsis. However, select patients with concerning postoperative findings may be managed successfully with appropriate drainage, decompression, and nutr

Yeokwisan Attenuates Reflux-Induced Esophageal Injury and Suppresses Macrophage Inflammatory Responses Associated with MAPK/PI3K-Akt and NF-κB/COX-2 Signaling.

Background/Objectives: Reflux esophagitis (RE) is a chronic inflammatory disorder caused by reflux of gastric contents into the esophagus, leading to mucosal injury and epithelial barrier dysfunction. Although proton pump inhibitors are widely used, persistent inflammation and oxidative stress remain important therapeutic challenges. Yeokwisan (YWS) is prescribed for gastroesophageal reflux disease and functional dyspepsia; however, its protective mechanisms against RE remain unclear. This study investigated the protective and anti-inflammatory effects of YWS. Methods: We integrated network ph

Impact of gastric length of myotomy during POEM on the incidence of gastroesophageal reflux: A randomized controlled trial.

BACKGROUND AND AIMS: Peroral endoscopic myotomy (POEM) is an effective treatment for achalasia cardia, but gastroesophageal reflux disease (GERD) remains a significant concern. The length of gastric myotomy may influence reflux outcomes, but high-quality data are lacking. This randomized controlled trial compared the incidence of GERD after short versus long gastric myotomy (SGM vs LGM) during POEM. METHODS: In this randomized trial, we compared SGM (gastric myotomy <3 cm) with LGM (gastric myotomy≥3 cm) during POEM in cases with type I or II achalasia. The primary endpoint was significant re

Omeprazole as a Novel Treatment Option for Giardiasis in Cats.

BACKGROUND: Giardiasis is a common protozoal infection in cats, often treated with nitroimidazole derivatives such as metronidazole and secnidazole. However, there are concerns about resistance to nitroimidazoles and their adverse effects. Recent in vitro studies suggest that omeprazole, a proton pump inhibitor, exhibits anti-giardial effects by inhibiting giardial triosephosphate isomerase. No clinical studies have evaluated its efficacy in naturally infected cats. OBJECTIVES: This study aimed to assess and compare the anti-giardial efficacy of omeprazole, metronidazole, and secnidazole in c

Endoscopic Prevalence of Barrett's Esophagus in a Predominantly African American and Hispanic Population.

Background and aim Barrett's esophagus (BE) is a well-known premalignant disorder. Investigators have shown that the prevalence of histologically confirmed BE varies among ethnic groups in the US. However, these studies were not performed at institutions that predominantly serve US minority populations. The aim of this study was to assess the prevalence of histologically confirmed BE at an institution that serves a predominantly African American (AA) and Hispanic American patient population. Methods The Harlem Hospital endoscopy and pathology databases were searched to identify all patients wh

Hyperbaric Oxygen Therapy for Late Radiation-Induced Duodenal Toxicity After Stereotactic Body Radiotherapy in a Patient with Cholangiocellular Carcinoma: A Unique Case Report.

Stereotactic body radiotherapy (SBRT) is an effective and increasingly utilized treatment modality for abdominal tumors, offering high rates of local control with generally acceptable toxicity profiles. Nevertheless, rare but severe late gastrointestinal complications, including radiation-induced ulceration, may occur and significantly impair patients' quality of life, as well as continuation of oncologic treatment. Hyperbaric oxygen therapy (HBOT) has shown potential benefit in the management of chronic radiation-induced tissue injury, although evidence regarding its role following SBRT remai

Gastric neoplasm with foveolar-cell differentiation in Helicobacter pylori-naïve patients.

The incidence of Helicobacter pylori (Hp)-naïve gastric neoplasms (HpNGNs) is increasing due to a growing Hp-naïve population and improved recognition. Among these, HpNGNs that predominantly exhibit foveolar-cell differentiation include foveolar-type gastric adenomas (FGA) and fundic gland polyps with dysplasia (FGPD). Traditionally, FGAs have been considered large, whitish, flat lesions (flat-type FGA), primarily associated with syndromic conditions, such as familial adenomatous polyposis (FAP) and gastric adenocarcinoma and proximal polyposis of the stomach (GAPPS), while sporadic cases are

Esomeprazole Potentiates the Cytotoxic Effects of Cisplatin in Gastric Carcinoma Cells.

Proton pump inhibitors (PPIs), including esomeprazole, impact the acidic tumor microenvironment, potentially influencing cancer cell behavior. By examining the combined effects of esomeprazole and cisplatin on SNU-1 gastric carcinoma cells, this study sought to elucidate the mechanisms through which esomeprazole enhances cisplatin's cytotoxicity, potentially allowing for effective treatment with reduced cisplatin dosages. SNU-1 cells were treated with varying doses of esomeprazole and cisplatin, alone and in combination. Cell viability was assessed using the XTT assay. Oxidative stress (TAS/TO

Yogic gut aeration techniques to inhibit Helicobacter pylori-induced acid peptic diseases: A pilot randomized controlled trial.

BACKGROUND: The most common cause of acid-peptic diseases (APDs) is Helicobacter Pylori (H. pylori) infection. Conventionally, proton-pump inhibitors (PPIs) are used to manage hyperacidity and dyspepsia. PPIs' persistent use can lead to bacterial resistance, side effects, poor quality of life (QoL), and increased economic burden. H. pylori, being microaerophilic, can be inhibited by gut aeration. Therefore, yogic gut aeration techniques (YGATs) may be a potential option to mitigate H. pylori-induced APDs and reduce medication doses and economic burden. OBJECTIVE: The study aimed to determine

Association between magnesium depletion score and Klotho levels among U.S. adults: Findings from NHANES 2007-2016.

BACKGROUND: Magnesium is a vital mineral that plays a crucial role in various physiological processes, and its imbalance has been associated with many chronic aging-related diseases. Traditional assessment methods are insufficient, and the Magnesium Depletion Score (MDS) has been proposed as a significant indicator. Klotho, a transmembrane protein, is involved in aging regulation and oxidative stress modulation. Despite the shared relevance of MDS and Klotho to aging, their relationship remains unclear. AIMS: This study aims to investigate the relationship between MDS and Klotho levels in U.S

Nonreceptor tyrosine kinase ABL1 regulates lysosomal acidification by phosphorylating the ATP6V1B2 subunit of the vacuolar-type H+-ATPase.

The vacuolar-type H+-ATPase (V-ATPase) is a proton pump responsible for controlling the intracellular and extracellular pH of cells. Its activity and assembly are tightly controlled by multiple pathways, of which phosphorylation-mediated regulation is poorly understood. In this report, we show that in response to starvation stimuli, the nonreceptor tyrosine kinase ABL1 directly interacts with ATP6V1B2, a subunit of the V1 domain of the V-ATPase, and phosphorylates ATP6V1B2 at Y68. Y68 phosphorylation in ATP6V1B2 facilitates the recruitment of the ATP6V1D subunit into the V1 subcomplex of V-ATP

Clinical Characteristics and Outcomes of Incomplete Cardia Reconstruction Cases Following Multiple Anti-reflux Mucosectomy/Anti-reflux Mucosal Ablation Treatments.

INTRODUCTION: Anti-reflux mucosectomy (ARMS) and anti-reflux mucosal ablation (ARMA) are novel endoscopic treatments for proton pump inhibitor or potassium-competitive acid blocker-refractory gastroesophageal reflux disease. These procedures induce scarring of the artificial ulcer at the gastric cardia, which tightens the enlarged cardiac opening. However, we encountered patients with unresolved symptoms due to insufficient cardiac shrinkage despite multiple ARMS/ARMA treatments. This study analyzed the frequency and characteristics of the refractory cases. METHODS: We retrospectively reviewe

Methyl-2,4-dihydroxybenzoate induces transcriptomic reprogramming in Fusarium oxysporum f. sp. cubense and defense gene responses in Kadali banana.

The emergence of fungicide resistance and environmental concerns with conventional chemicals necessitate the identification of novel antifungal compounds. Methyl-2,4-dihydroxybenzoate (MDHB), a hydroxybenzoate derivative, exhibits potent antifungal activity against Fusarium oxysporum f. sp. cubense, the causative agent of Panama disease in bananas. To uncover its molecular mechanism of action, we performed comprehensive transcriptomic profiling of Foc treated with 100 ppm MDHB using RNA sequencing technology. The analysis revealed 173 differentially expressed genes (DEGs), comprising 121 upreg

Efficacy and safety of anticoagulant treatment in elderly patients with venous thromboembolism beyond the first 3 to 6 months of therapy: A narrative review.

Extended anticoagulation remains a crucial consideration in patients with venous thromboembolism (VTE), especially in older adults. In this population, the two main challenges are the increased risk of bleeding and the underrepresentation in clinical trials. Current guidelines recommend indefinite anticoagulation for unprovoked VTE, except in patients at high bleeding risk, where the benefits and risks must be carefully balanced. In contrast, VTE triggered by a transient risk factor typically does not require prolonged treatment once the factor has resolved. For patients with chronic risk fact
