The 6-min walk test (6MWT) is a commonly used test for the objective assessment of functional exercise capacity for the management of patients with moderate-to-severe pulmonary disease. Unlike pulmonary function testing, the 6MWT captures the often coexisting extrapulmonary manifestations of chronic respiratory disease, including cardiovascular disease, frailty, sarcopenia, and cancer. In contrast with cardiopulmonary exercise stress testing, this test does not require complex equipment or technical expertise. In this low complexity, safe test, the patient is asked to walk as far as possible a

A 12-year-old boy with Duchenne muscular dystrophy underwent posterior spinal fusion for progressive scoliosis. Preoperative evaluation was focused on respiratory function as well as cardiac function, which revealed markedly reduced respiratory reserve (FVC 0.77 l, %FVC 25.9%, FEV1.0 0.48 l, %FEV1.0 62%) and well-preserved biventricular function. A possible association between malignant hyperthermia and Duchenne muscular dystrophy has been documented. Thus anesthesia was administered without triggering agents. Propofol and fentanyl were used for induction and maintenance of anesthesia, and the

BACKGROUND: Congenital hypothyroidism (CH) is a common condition in both preterm and term infants characterized by either thyroid gland absence or hypofunctionality. The clinical association of refractory lactic acidosis and heart failure has rarely been observed in cases of pediatric patients with CH pathology. Here, we explored the etiological relationship between CH, heart failure, and refractory lactic acidosis to reflect the importance of thyroid function screening in neonates with heart disease.
CASE SUMMARY: A 33-day-old extremely premature female infant presented with tachypnea, respi

INTRODUCTION: There are no precise data about the effect of Aspergillus infection on lung function other than allergic bronchopulmonary aspergillosis (ABPA) in patients with cystic fibrosis (pwCF). Here, we aimed to determine clinical phenotypes caused by Aspergillus spp. using laboratory and immunologic parameters and to compare Aspergillus phenotypes in terms of pulmonary function tests (PFT) prospectively.
METHODS: Twenty-three pwCF who had Aspergillus isolation from respiratory cultures in the last year (case group) and 20 pwCF without Aspergillus isolation in sputum (control group) were

Congenital diaphragmatic hernia (CDH) is associated with pulmonary hypoplasia and pulmonary hypertension. The objective of this study was to assess pulmonary function and exercise capacity and its early determinants in children and adolescents born with high-risk CDH (CDH-associated respiratory distress within the first 24 h) and to explore the relationship of these findings with CDH severity. Of 159 patients born with high-risk CDH, 84 survived. Of the 69 eligible patients, 53 children (mean+/-SD age 11.9+/-3.5 yrs) underwent spirometry, lung volume measurements and maximal cardiopulmonary ex

OBJECTIVE: Phrenic nerve transfer has been used for treating lesions of the brachial plexus since 1970. Although, today, surgeons are more experienced with the technique, there are still widespread concerns about its effects on pulmonary function. This study was undertaken to evaluate the effectiveness and safety of this procedure.
METHODS: Fourteen patients with complete palsy of the upper limb were submitted to phrenic nerve transfer as part of a strategy for surgical reconstruction of their plexuses. Two patients were lost to follow-up, and 2 patients were followed for less than 2 years. O

BACKGROUND: Polymorphonuclear, neutrophil granulocytes (PMN) play a major role in the control of infections, and people who abuse alcohol are susceptible to infections. Resistance against infections ensues intracellularly following initial phagocytosis of microorganisms with the oxygen-dependent respiratory burst, the key enzyme of which is the respiratory burst oxidase, whereby oxygen radicals are produced for microbial destruction. To date there is insufficient information available in connection with the process of impaired defence against infection in patients suffering from alcohol depend

PURPOSE: Never-smokers account for a large proportion of subjects in general population studies on nontuberculous mycobacteria lung disease (NTM-LD). However, the influence of NTM infection on the lung function of never-smokers has not yet been evaluated. The aim of this study was to determine how NTM-LD impairs the lung function in never-smokers, and whether there are an association between successful NTM-LD treatment in radiologic outcomes and improvement in lung function of never-smokers with NTM-LD or not.
METHODS: We performed a retrospective study of patients (1) who have never smoked d

BACKGROUND: Chest x-ray (CXR) and computed tomography (CT) are commonly used for diagnosing bronchopulmonary dysplasia (BPD), though each modality has limitations. CXR has low sensitivity for early parenchymal changes, while CT involves radiation exposure. Lung ultrasound (LUS) has emerged as a promising, radiation-free alternative.
OBJECTIVE: To assess the diagnostic accuracy of LUS in BPD compared to CT and CXR, and to evaluate correlations between LUS scores and pulmonary function parameters, including spirometry, interrupter resistance technique (RINT), and nocturnal respiratory monitorin

BACKGROUND: Morbid obesity in adolescents impacts respiratory function, often leading to reduced lung volume and obstructive ventilatory defects. However, standard spirometric values frequently remain within normal ranges.
OBJECTIVES: We hypothesized that Lung Clearance Index (LCI) is a more sensitive marker for detecting airway dysfunction in adolescents with morbid obesity than conventional lung function tests.
METHODS: A prospective single-center cohort study evaluated adolescents with morbid obesity undergoing laparoscopic sleeve gastrectomy (LSG). Assessments included fractional exhaled

BACKGROUND: In this work we describe a breath emulator system, used to simulate temporal characteristics of exhaled carbon dioxide (CO2) concentration waveform versus time simulating how much CO2 is present at each phase of the human lung respiratory process. The system provides a method for testing capnometers incorporating fast response non-dispersive infrared (NDIR) CO2 gas sensing devices - in a clinical setting, capnography devices assess ventilation which is the CO2 movement in and out of the lungs. A mathematical model describing the waveform of the expired CO2 characteristic and influe

BACKGROUND: Amateur runners may benefit from combining respiratory muscle training (RMT) with resistance or aerobic modalities, but direct comparisons are scarce. This study compared different RMT-based combinations on pulmonary function, respiratory muscle strength, and whole-body exercise capacity.
METHODS: In this randomized four-arm trial, 48 amateur runners were allocated equally to stand-alone RMT, RMT plus upper-limb resistance (RMT + ULRT), RMT plus lower-limb resistance (RMT + LLRT), or RMT plus aerobic exercise (RMT + AET). All groups completed supervised sessions three times per we

Background: Amyotrophic lateral sclerosis (ALS) is a neuromuscular progressive disorder characterized by limb and bulbar muscle wasting and weakness. A total of 30% of patients present a bulbar onset, while 70% have a spinal outbreak. Respiratory involvement represents one of the worst prognostic factors, and its early identification is fundamental for the early starting of non-invasive ventilation and for the stratification of patients. Due to the lack of biomarkers of early respiratory impairment, we aimed to evaluate the role of chest dynamic MRI in ALS patients. Methods: We enrolled 15 ALS

BACKGROUND: Complaints of peripheral muscle weakness are quite common in patients with systemic sclerosis (SSc). It is likely that the muscle impairments may reduce the patients' exercise performance, which in turn may decrease their functional capacity and exert a direct impact on their quality of life.
OBJECTIVES: To assess the peripheral and respiratory muscle strength in individuals with SSc and to investigate their correlation with the 6-min walk distance (6 MWD) and quality of life measurements. Moreover, we aimed to characterize their nutritional status, pulmonary function, functional
